The choice of an adequate antithrombotic regimen for atrial fibrillation patients undergoing emergent or elective percutaneous coronary intervention (PCI) should be based on the ischemic event and on the risk of bleeding. Recent randomized controlled trials have consistently demonstrated that dual antithrombotic therapeutic regimens, using non-vitamin K anticoagulants and clopidogrel, are superior to triple or dual therapy with warfarin and aspirin. This report incorporates findings of recent notable studies to provide concrete, clinically useful details and recommendations for bleeding risk assessment and optimal antithrombotic therapeutic strategies after PCI. In addition, we introduce guidelines for antithrombotic management after structural heart disease intervention. (Korean J Med 2019;94:330-342)

Infection still remains a major cause of morbidity and mortality in systemic lupus erythematosus (SLE). Patients with SLE are well known to have an increased risk of various opportunistic infections, which can be fatal. Central nervous system (CNS) infections such as meningitis are rare complications to SLE. On occasion, nonspecific neurologic manifestations of infectious meningitis in SLE patients can be confused with neuropsychiatric lupus. Listeria monocytogenes is a less-commonly identified organism causing meningitis in SLE patients. Here, we describe a case of Listeria monocytogenes meningitis presenting with bilateral abducens nerve (sixth cranial nerve) palsy in a patient with SLE, who was successfully treated with systemic antibiotics.

This report incorporates recent authoritative studies to provide detailed recommendations for managing atrial fibrillation in specific clinical settings. The principles of the Atrial fibrillation Better Care (ABC) pathway apply in these clinical settings. In addition, specific considerations are discussed for each of these conditions and populations. HMR 패키지디자인 속 푸드 스타일링이 소비자태도와 재구매의도에 미치는 영향

본 연구에서는 소비자들이 HMR을 접했을 때 패키지디자인 속 푸드 스타일링 연출이 소비자태도와 재구매의도에 미치는 영향과 푸드 스타일링 연출 적용의 여부가 HMR 패키지사진의 소구력과 설득력에 어떠한 영향을 미치는지 알아보고, 보다 효과적인 표현의 장치로써 푸드 스타일링의 역할과 마케팅적 시사점을 제시하는데 그 목적을 두었으며 이상의 연구결과를 요약하면 소비자들이 HMR을 접했을 때 패키지디자인 속 푸드 스타일링 연출이 소비자태도와 재구매의도에 상호 유의미한 관계를 가지고 있는 것으로 나타났다. 즉, 푸드 스타일링은 식품산업의 다양한 분야에서 시각적인 설득력이 있는 커뮤니케이션 도구이며, 효과적인 마케팅 도구로써도 활용되어 질 수 있다고 사료된다. 또한 본 연구의 결과로, 푸드 스타일링이 HMR 제품뿐만 아니라 식품산업 전반에 있어 시각적 효과를 배가시키는 판매 촉진적 역할과 마케팅적 가치를 지님을 인식하고 HMR 제품 뿐 아니라 식생활, 외식산업과 관련된 다양한 제품의 시각적 커뮤니케이션 도구로써도 적절하게 활용되어 시너지 효과를 발휘할 수 있기를 기대해 본다. This study aims to investigate how the dramatic composition of food styling found in the package images affects perceived value, consumer attitude and repurchase intention when the consumer first encounters HMR and whether the presence of dramatic composition of food styling affects the appealing and persuasive power of the HMR package image. This study also aims to suggest roles for food styling as a mechanism for expression with greater effectiveness and propose its marketing implications. Furthermore, another objective of study is to suggest a direction in which food styling can be utilized as a visual transmission mechanism that is faster and more appealing than language in food package marketing. From these research results, it was revealed that food styling in package images has mutually significant relationships with perceived value, consumer attitude and repurchase intention when the consumer encounters HMR. Therefore, the study believes that food styling may be utilized as a visually persuasive communication tool as well as an effective marketing tool in numerous areas of the food industry. In addition, it is anticipated that the sales promoting strategic mechanism and marketing value of food styling, which multiplies the visual effects for HMR products and the entire food industry, will be acknowledged and that food styling will create a synergic effect when used adequately as a visual communication tool for various products related to diet, through the results of the study.

키즈카페의 서비스 품질과 고객만족, 충성도 간의 관계

키즈카페의 시장이 커지게 되면서 업체 간의 경쟁이 심화됨에 따라 서비스품질이 중요하게 부각되고 있다. 따라서 본 연구의 목적은 키즈카페의 서비스품질과 고객만족, 충성도 간의 관계를 검증하는데 있다. 선행연구를 바탕으로 이론적 고찰 및 설문지를 작성하여, 판단표본추출 방법으로 영유아 자녀가 있으면서 키즈카페를 방문 하였던 부모들을 대상으로 설문조사를 실시하였다. 설문지는 2019년 8월 15일부터 8월 23일까지 배포 및 수거하였다. 설문조사에서는 총 405부를 배포하고, 전량 회수하였으며, 이 중에서 데이터로 사용할 수 없다고 판단한 설문지 29부를 제외하고 총 376부를 통계분석에 사용하였다. 가설 1, “서비스품질은 고객만족에 정(+)의 영향을 미칠 것이다.”라는 가설은 부분 채택되었다. 다만, 서비스품질 요인 중 결과품질은 고객만족에 정(+)의 영향을 미치지 않는 것으로 나타났다. 가설 2, “고객만족은 충성도에 정(+)의 영향을 미칠 것이다.”라는 가설은 채택되었다. 가설 3, “서비스 품질은 고객만족을 매개로 충성도에 정(+)의 영향을 미칠 것이다.”라는 가설은 부분 채택되었다. 다만, 서비스품질 요인 중 결과품질은 고객만족을 매개로 하여 충성도에 정(+)의 영향을 미치지 않는 것으로 나타났다. 본 연구는 키즈카페의 경영자에게 차별적 경영과 경쟁력 제고를 위한 방향을 제시함으로써, 서비스의 질을 향상시키기 위한 마케팅 전략을 구축하는데 유용한 자료를 제공하였다. As the market for kids cafes grows, the quality of service has become important in competition between companies. Therefore, the purpose of this study is to verify the relationship between the service quality and customer satisfaction and loyalty of the Kids Cafe. Theoretical consideration and questionnaire were prepared based on prior research. As a method of sampling judgment, a survey was conducted on parents who had infants and children and visited kids cafes. The questionnaire was distributed and collected from August 15, 2019 to August 23, 2019. In the survey, a total of 405 copies were distributed, and 376 of them were used for statistical analysis, except for 29 of them that were determined not to be available as data. For Hypothesis 1, the hypothesis that "service quality will have a positive effect on customer satisfaction" was partially adopted. Out of the service quality factors, the result quality was rejected. For Hypothesis 2, the hypothesis that "customer satisfaction will have a positive effect on loyalty" was adopted. For Hypothesis 3, the hypothesis that "service quality will have a positive effect on loyalty by means of customer satisfaction" was partially adopted. Out of the service quality factors, the result quality was rejected. This study provided useful data for building marketing strategies to improve the quality of service by presenting a direction for discriminative management and enhancing competitiveness to the managers of kids Cafe.

삼산화비소 치료 후 만성 신부전으로 진행된 급성전골수구백혈병 1례

급성전골수구백혈병 환자에서 발생하는 신부전은 대부분 급성 신부전의 형태로 가 역적인 것으로 알려져 있으며 만성 신부전으로 진행하는 경우는 드문 것으로 알려져 있고 국내에서는 보고된 바 없다. 저자들은 ATRA 와 삼산화비소 치료 후 만성 신부전으로 진행한 1례를 경험하였기에 문헌고찰 과 함께 보고하는 바이다. The standard remission induction chemotherapy of acute promyelocytic leukemia (APL) is all-trans-retinoic acid (ATRA) combined with anthracycline. Recently, arsenic trioxide therapy has been represented significant response in induction therapy for relapsed or high-risk APL patients. ATRA and arsenic trioxide are usually well tolerable, but some major side effects can be observed, such as APL differentiation syndrome (APLDS). Renal failure in APL may be presented as a part of DIC or as a side effect of ATRA or arsenic trioxide. But, renal dysfunction is usually reversible and chronic kidney failure associated with APLDS has been rarely reported. We report a patient with APL who developed chronic kidney failure after ATRA and arsenic trioxide therapy.

증례 : 호흡기 ; 임산부에서 발생한 원발성 폐암 1예

A 36-year-old primivida, at 29 weeks of pregnancy with no smoking history, was admitted to the hospital complaining of cough for 3 days. Chest X-rays revealed atelectasis of the right upper lobe, and a transbronchial Lung biopsy confirmed primary Lung adenocarcinoma. After consulting with obstetricians and neonatologists, we wanted to deliver the child and treat the mother with chemoradiotherapy. But as she was adamantly opposed to treatment until fetal Lung maturation was complete, we planned to delay the birth until 34 weeks, deliver the baby by caesarian section, and then treat the mother. However, maternal hypoxia and fetal distress resulted in an emergency delivery at 30 weeks. After delivery, we treated the mother`s brain metastases with radiation therapy and systemic cisplatin-pemetrexed, but she deteriorated and expired 95 days after the diagnosis. Lung cancer during pregnancy is a rare disease and raises many medical and ethical issues in deciding the best course of therapy. We describe our clinical approach and review the potentially challenging features of managing a pregnant patient with Lung cancer. (Korean J Med 2012;82:85-89)

Non-vitamin K antagonist oral anticoagulants (NOACs) are alternatives to vitamin K antagonists to prevent stroke in patients with non-valvular atrial fibrillation (AF) and have emerged as the preferred choice. The use of NOACs is rapidly increasing in Korea after coverage by insurance since 2015. However, the rate of prescribing anticoagulants in Korean patients with AF remains low compared to other countries. Most of the NOAC anticoagulant prescriptions are issued at hospitals. As the prevalence rate of AF in Korea is expected to increase rapidly with the increase in the elderly population, the need to prescribe NOACs in primary care clinics will also increase. Therefore, The Korean Heart Rhythm Society organized the Korean Atrial Fibrillation Management Guideline Committee and analyzed all available studies based on the 2018 European Heart Rhythm Association Practical Guide on the use of NOACs for managing AF, as well as studies on Korean patients. The authors would like to introduce practical guidelines for NOAC prescriptions in Korean patients with AF. (Korean J Med 2019;94:17-39)
