치유농업 가이드라인 작업치료적 접근

Care farm recently has been introduced as social farm and green care centered on Europe, and has been developing rapidly in the last 20 years. Although it is being introduced gradually in Korea, it is still only at the beginning level, and it does not provide a guide for systematic farm owners' model and operation of the Care farm. In Care farm, advanced countries are making it possible to operate a more common care farm by providing a systematic national guide. By reviewing the literature of these guidelines in advanced countries, the contents are organized to help compose the guidelines for domestic care farm. In Canada, Ireland, the Netherlands, Norway and Finland, the contents of the guidelines for care farm by country can be broadly divided into general matters, how to start a care farm project for farm owners, understanding of target audience, farm organization system, farm management, and farm life. As the demand for care farms increases in Korea as well, guidelines for farm owners should be disseminated to facilitate access to care farms. It is also considered a good way to make a guide by combining it with the medical field, and it seems that it is necessary for the farmer to understand the medical field to some extent.

치매환자의 치유농장 프로그램 이용 후 환자보호자의 경험에 대한 질적 사례연구

In this study, we conducted a care farm program that is currently being operated as a pilot in a care farm in Korea, interviewed the experiences and feelings of dementia patients who experienced this as a patient care-giver, and confirmed the strengths and weaknesses of the care farm program. Accordingly, we investigated the points for improvement and the characteristics of the program in the care farm program and operation. In particular, opinions are gathered on the use of a care farm in the early stages of dementia, accessibility issues from residence to the care farm, continuity of various programs and programs, and development of seasonal content. In order to develop a program related to care agriculture for dementia patients by using healing agriculture in future care farms, it is necessary to consider factors such as use problems, accessibility, diversity, continuity, and seasonality of early dementia patients.

성인기 지적장애인과 주부양자의 특성, 사회적지지, 가족기능, 재활의 필요성 사이에 관련성

Purpose : The purpose of this study was to examine the relationship between characteristics of caregivers and adults with intellectual disability, and social support, family function, and rehabilitation needs in caregivers. Methods : 98 pairs of adults with intellectual disability and caregivers participated in this study. The researchers examined the general characteristics of adults with intellectual disability and caregivers. The evaluation included the level of activities of daily living, the ability to communicate, the health status of the adults with intellectual disability, and the family income, the health status, the utility and the need for rehabilitation, the social support (multidimensional scaled perceived social support, MSPSS), family function (adaptation, partnership, growth, affection, resolve, APGAR index) of caregivers were measured. The collected data were analyzed to relationship the characteristics of people with intellectual disability, and social support, family function, rehabilitation needs of caregivers using regression and correlation analysis. Results : Rehabilitation needs were significantly correlated with age of people with intellectual disability (p<.01), and subjective health status of caregivers (p<.05). Education level of caregivers affected social support significantly (R2=.058, p=.021). Communication ability of people with intellectual disability affected family function (R2=.071, p=.01). Social support of caregivers had a significant influence on family function (R2=.488, p<.001). Conclusion : These findings suggest that the barriers to system of community rehabilitation should be lowered, and the authors discussed the results of present investigation. 본 연구는 성인기 지적장애인과 주부양자의 특성, 사회적지지, 가족기능, 재활의 필요성 사이의 연관성을 알아보고자 하였다. 본 연구에 성인기 지적장애인과 주부양자 98쌍이 참여하였다. 조사자는 지적장애인과 주부양자의 일반적 특성에 대해서 평가하였다. 측정은 지적장애인의 일상생활수준, 의사소통능력, 주관적 건강상태, 그리고 주부양자의 가구소득, 주관적 건강상태, 사회적지지, 가족기능, 재활의 필요성과 이용여부에 대해 이루어졌다. 수집한 자료를 이용하여 상관분석과 회귀분석을 통해 변수 사이의 상관성을 분석하였다. 재활의 필요성은 지적장애인의 나이(p<.01), 주부양자의 주관적 건강상태(p<.05)와 유의한 상관이 있었다. 주부양자의 교육수준은 사회적지지에 유의한 영향을 미치는 요인이었다 (R2=.058, p=.021). 지적장애인의 의사소통능력은 주부양자의 가족기능에 영향을 미치는 요인이었다 (R2=.071, p=.01). 주부양자의 사회적지지는 가족기능에 유의하게 영향을 미쳤다 (R2=.488, p<.001). 이러한 결과는 지역사회 재활체계에 대한 장벽을 낮추어야 한다는 것을 시사하고 저자들은 현재의 결과에 대해서 논의하였다.

무릎관절 치환술 환자를 위한 작업치료 임상 가이드라인 개발

목적 : 본 연구는 무릎관절 치환술 환자를 위한 작업치료 임상 가이드라인을 체계적으로 개발하고자 하였다. 전 세계적으로 무릎관절 치환술을 받은 환자들의 회복과 지역사회 복귀에 있어 작업치료의 역할이 매우 중요하다는 문헌이 보고되고 있으나, 우리나라에서는 관절 치환술 환자를 위한 작업치료가 거의 제공되지 않는다는 현실이 있다. 본 연구에서는 무릎관절 치환술 환자를 위한 근거기반 작업치료 중재를 확인하고 이것을 가이드라인을 통해 제안하고자 한다. 연구방법 : 가이드라인 개발 방법은 영국 작업치료사협회의 「Practice guideline development manual(4th edition)」에서 소개한 절차를 따랐다. Patient/Population, Intervention, Comparison and Outcomes(PICO) 임상 질문 설정은 이해관계자의 의견을 수렴하여 한국의 보건·의료제도를 고려하여 설정하고 이를 바탕으로 체계적 고찰을 진행하였다. 데이터베이스에서 총 7,241건의 논문을 추출하였으며 논문의 질 평가 및 비평적 읽기를 통해 최종 66편의 근거 논문을 선정하였다. 논문의 중재를 바탕으로 가이드라인 내용을 구성하고 권고 강도는 Grading of Recommendations, Assessment, Development, and Evaluations(GRADE) approach 체계를 활용하여 [권고]와 [제안]으로 구분하였다. 결과 : 무릎관절 치환술 환자를 위한 작업치료 임상 가이드라인 권고 내용은 ‘환자 중심’과 ‘서비스 제공 중심’으로 범주화하였다. ‘환자 중심’범주에는 ‘일상생활 활동기능 향상’, ‘정신건강의 안정(불안, 우울 등)’, ‘환자 만족도 증가’, ‘지역사회복귀 향상’을, ‘서비스 제공 중심’범주에는 ‘작업치료사를 포함한 다학제 재활팀 구성’과 ‘작업치료 를 통한 병원 입원 기간 단축’을 위한 내용을 포함하였다. 결론 : 무릎관절 치환술 환자를 위한 작업치료 임상 가이드라인은 수술환자들의 안정적인 지역사회 복귀와 입원 기간 단축에 기여할 것이다. 또한 정형외과 작업치료사 역량 강화를 위한 교육자료로 활용될 수 있으며 의료비 용 감소를 위한 정책 마련에 근거 자료가 될 것이다. Objective: This study is aimed at the development of evidence-based occupational therapy guidelines for patients undergoing a total knee replacement (TKR) in Korea. Although occupational therapy plays a critical role in their recovery and return to the community, it is not widely provided in Korea. Therefore, this study proposes effective evidence-based interventions for occupational therapy in TKR patients through the development of practice guidelines. Method: We developed the guidelines following the methods in the Practice guideline development manual (4th edition) of the British OT Association. We collected the opinions of health professionals during the initial PICO question stage and selected 66 articles after extracting 7,241 articles from various databases. The recommendations were graded as either a [recommendation] or [suggestion] using the GRADE approach. Results: As the result of this study, recommendations were categorized into patient-centered and service-centered. The patient-centered recommendations include improving the activities of daily living, stabilizing mental health, increasing the level of patient satisfaction, and improving the return to the local community. The service-centered recommendations include the composition of a multidisciplinary rehabilitation team that includes occupational therapists and reducing the length of hospital stay through occupational therapy. Conclusions: The guidelines will contribute to a stable early return to the community and a shortening of the hospitalization period for TKR patients, strengthen the competency of orthopedic occupational therapists, and serve as a basis for preparing policies to decrease medical costs for TKR patients.
