        Highly Sensitive and Novel Point-of-Care System, aQcare Chlamydia TRF Kit for Detecting Chlamydia trachomatis by Using Europium (Eu) (III) Chelated Nanoparticles

        Background: The bacterium Chlamydia trachomatis is one of the leading causes of sexually transmitted diseases worldwide. Since no simple and effective tool exists to diagnose C. trachomatis infections, we evaluated a novel point-of-care (POC) test, aQcare Chlamydia TRF kit, which uses europium-chelated nanoparticles and a time-resolved fluorescence reader. Methods: The test performance was evaluated by comparing the results obtained using the novel POC testing kit with those obtained using a nucleic acid amplification test (NAAT), using 114 NAAT-positive and 327 NAAT-negative samples. Results: The cut-off value of the novel test was 20.8 with a detection limit of 0.27 ng/mL. No interference or cross-reactivity was observed. Diagnostic accuracy showed an overall sensitivity of 93.0% (106/114), specificity of 96.3% (315/327), positive predictive value (PPV) of 89.8% (106/118), and negative predictive value (NPV) of 97.5% (315/323). The sensitivity of the novel test was much higher than that of currently available POC tests. Furthermore, the relative ease and short turnaround time (30 min) of this assay enables C. trachomatis-infected individuals to be treated without a diagnostic delay. Conclusions: This simple and novel test is a potential tool to screen a larger population, especially those in areas with limited resources.

        5% DMSO와 자연냉동보존제를 이용한 말초혈액조혈모세포 채집액의 냉동보존

        Background: Cryopreservation of hematopoietic stem cells has become an important process due to the therapeutic protocol, which includes stem cell transplantation after chemotherapy, for many hematological malignancies. The conventional medium contains 10% dimethyl sulfoxide (DMSO) as a cryoprotectant, but this has been reported to be related with many complications. We analyzed the usefulness of trehalose, catalase and zVAD-fmk for cryopreservation along with using a reduced concentration of DMSO to 5%. Methods: Peripheral blood stem cells were frozen in 10% DMSO as a control and also in 5% DMSO with trehalose and catalase. After 3 weeks of storage in a liquid nitrogen tank, the viability of the thawed hematopoietic stem cells was measured using Trypan blue staining and 7-AAD analysis via conducting flow cytometry. The colony forming potential was assessed using methylcellulose culture. We measured the viability of cells in 5% DMSO medium with or without addition of 30 uM zVAD-fmk right after thawing, and we also did this 6 and 24 hours after incubation. Results: Cryopreserved cells in 5% DMSO with trehalose and catalase showed similar survival (50.42%)compared with the control (49.78%). The viability of cells that were also treated with added zVAD-fmk showed a better result (13.12%) than without it (5.5%) after 24 hours of incubation. Colony forming assay showed similar colony formation in 5% DMSO with the natural cryoprotectants. Conclusion: According to the results, lowering the DMSO concentration to 5% is significant and we can expect better cell viability and prevent many side effects of high dose DMSO when adding natural cryprotectants in the cryopreservation medium or by adding caspase-inhibitor right after thawing.

        중학생들의 지구과학 용어에 대한 용이도와 실제 이해 정도 분석

        본 연구에서는 지구과학 개념에 대한 학생들의 이해 정도를 파악하기 위해 중학생들을 대상으로 중학교 과학 교과서에 제시된 지구과학 용어 전체에 대해 느껴지는 쉬운 정도(용이도)와 알고 있는 것을 조사했다. 이를 위해 우선 중학교 “과학” 1, 2, 3학년 교과서에서 고딕체로 제시된 용어를 추출해, 물리, 화학, 생물, 지구과학 별로 용어를 구분했다. 여기서 추출된 지구과학 용어 198개를 다시 지질, 천문, 대기, 해양 영역별로 구분해 중학교 1학년 남학생 107명, 2학년 100명, 3학년 114명을 대상으로 용어 쉬움의 정도와 실제 정답율을 분석했다. 연구 결과는 다음과 같다. 첫째, 학생들은 해양 용어를 가장 쉽게, 천문 용어를 가장 어렵게 느꼈고, 지질 용어를 가장 잘 이해하고 있었고 천문 용어에 대한 이해도가 가장 낮았다. 둘째, 상급 학년 학생들은 아래 학년 학생들보다 지구과학 용어에 대해 더 잘 이해하고 있었지만 용어에서 느껴지는 쉬움의 정도에는 차이가 없었 다. 이러한 경향은 지질, 천문, 대기, 해양 등의 모든 세부 영역에서 비슷하게 나타났다. 셋째, 중학 생들이 느끼는 지구과학 용어 쉬움의 정도와 실제로 알고 있는 정도와의 상관관계는 .381로 낮은 편이었고 특히 해양 용어에서 그러한 경향이 더욱 두드러졌다. 마지막으로, 학생들의 오답을 분석 한 결과 일상생활 속 익숙한 용어로 인한 오답, 용어에서 느껴지는 어감에 따른 혼동, 다른 용어와 의 혼동 등의 유형으로 나눌 수 있었다. 본 연구 결과는 학생들에게 지구과학 용어에 대한 교수 학 습에 시사점을 줄 것으로 기대된다.

        형질세포골수종 환자에서 고농도 항암 화학요법과 조혈모세포이식 후 발생한 면역글로불린 동형의 변화

        A malignant plasma cell clone usually produces a single abnormal monoclonal protein with a constant isotype. However, switching of paraprotein isotype has been reported to be a transient phenomenon associated with the recovery of B-cell function, and, in some cases, the switching might be misinterpreted as relapse. In August 2008, we encountered a case of a 59-year-old man with proteinuria and high IgG level (5.6 g/dL), κ free light chain level of 2,660 mg/L, reversed A/G ratio (0.51), and multiple osteolytic lesions. Plasma cells, which accounted for 57% of all the nucleated cells, in bone marrow aspirates were positive for κ immunostaining. Serum protein electrophoresis showed one M-spike, concentration of 4.87 g/dL in the β region. Immunofixation electrophoresis revealed the peak as an IgG-κ monoclonal protein; therefore, a diagnosis of plasma cell myeloma was made. Complete remission was achieved after chemotherapy, and autologous peripheral stem cell collection was performed. In March 2009, the patient underwent high-dose chemotherapy and autologous peripheral stem cell transplantation support. After 2 months, serum protein electrophoresis showed 2 M-spikes in the γ region with positive IgM-λ, IgG-λ, and IgG-κ, and these bands persisted. The electrophoretic mobility of the IgG-κ protein was different from that of the original disease protein, and bone marrow results were the same as the previous ones. Although immunoglobulin isotype switch is known to have a benign course, it always requires careful monitoring because, in rare cases, true clonal switching may occur.

        생분해성 PLA-나노바이오실리카 복합재 사출성형 최적화

        화학적 플라스틱 제품은 다양한 환경문제를 발생시키며, 소각 시 폐기물로 인한 탄소발생 증가로 인하여 생분해성 고분자와 같은 자연분해가 가능한 생분해성플라스틱 제품에 대한 관심이 증가하고 있다. 식물기반 PLA(poly lactic acid)는 낮은 유리전이 온도로 저온성형특성이 좋지 않아 압출방식을 이용한 일회용기 제품생산 등 매우 한정적으로 사용된다. 본 연구에서는 사출성형 공정을 이용한 생분해성 폴리머를 상용화를 위하여 바이오첨가제에 의한 PLA 폴리머 물성변화 및 사출성형 최적화 조건을 탐색하였다. 생분해성 PLA 복합재에 나노실리카를 첨가하여 PLA의 취성특성을 개선하고 수축률을 조정하였으며, 최적 사출성형을 위한 사출온도, 기계적 압력을 재설정하였다. 5wt% 나노실 리카를 첨가한 PLA 복합재는 순수 PLA에 비해 용융 흐름이 15% 감소하였으나, 인장 강도는 13% 높아지고, 굴곡 강도는 15.9% 상승하며, 충격 강도는 18.2% 향상되고, 경도는 40% 증가하여, 180-190도의 온도에서도 사출 성형이 가능한특성을 보였다. 사출조건 설정을 위하여 나노복합재의 성형 시뮬레이션 해석 프로그램 ‘Moldflow’를 이용한 공정조건설정과 러너와 핀게이트에 따른 제품 사출 성형 최적화 분석을 진행하였다. 나노실리카를 첨가하여 PLA 특성을 조절 하고 이에 따른 사출성형 공정을 최적화 함으로써 다양한 형태의 제품을 생산할 수 있을 것으로 전망한다. Plant-based biodegradable polylactic acid (PLA) polymers have limited applications, such as in the production of disposable products using the extrusion method. This restricted usability is due to their low glass transition temperature and poor low-temperature molding properties. In the present study, we investigated the changes in PLA polymer properties induced by bioadditives and optimized the conditions of extrusion molding to commercialize biodegradable polymers using an injection molding process. Compared to pure PLA, the constructed PLA composite with nanosilica showed a 15% reduction in melt flow; however, the tensile strength increased by 13%, flexural strength increased by 15.9%, impact strength improved by 18.2%, and hardness increased by 40%, enabling injection molding even at temperatures of 180-190 °C. The mold flow of the nanocomposite and optimization analysis of product injection molding were conducted using a runner and fingate.

        말라리아의 이례적인 소견: 사람의 말초혈액에서 관찰된 말라리아 기생충의 편모방출 숫생식자

        Exflagellation of the malaria parasite microgametocyte usually occurs in the gut cavity of Anopheles mosquitoes following an infective blood meal. Exflagellation is a very rare event in human blood. Due to its rarity,the appearance of this structure in a peripheral blood smear will easily create a diagnostic dilemma. We report a case of malaria with exflagellated microgametes in human blood that was initially mistaken for a double infection of Plasmodium and another blood flagellate. The patient was a 29-year-old Parkistani man presenting with fluctuating fever accompanied by chills and fatigue for 4 days. Initial peripheral blood smear examination showed a number of Plasmodium ring forms, trophozoites, and gametocytes. Additionally, several filamentous structures resembling blood flagellates were seen. With these features, an initial diagnostic impression of combined infection of malaria and blood flagellate was made. Later, we determined that these structures resembling blood flagellates were exflagellated microgametes of malarial parasite. Therefore, the knowledge that exflagellation may appear in human blood with Plasmodium species infection and being more familiar with differentiation of the morphologic features of other species infection can prevent further possible misinterpretation.

        Clinical chemistry values in elderly Korean people: single institutional study

        Laboratory values change with age and interpreting laboratory results from elderly people using the reference intervals for younger adults may not be appropriate. The authors investigated the distribution patterns of routine chemistry values from elderly people to determine whether current reference intervals are also valid for elderly people. A total of 1,215 persons older than 65 years and 1,827 healthy adults below 65 years of age were evaluated. Blood samples were collected after an overnight fast and analyzed for chemistry tests. Computing the central 95th percentile showed that the total protein, albumin, ALP, LD, creatinine, uric acid, triglyceride,HDL-cholesterol, and electrolytes of elderly people were within the standard reference intervals used in our laboratory. For AST and ALT, the upper range of the central 95th percentile in the elderly population was found to be outside the common reference interval. However, the central 90th percentile values of AST and ALT were compatible with the common reference intervals. GGT, BUN, total cholesterol, LDL-cholesterol, and glucose showed higher values than the upper limits of the reference intervals. For common clinical chemistry tests, the common reference values in general should be applicable to elderly people, even though some parameters showed wider distributions in the elderly.
