        근골격계 질환에서 진단의학검사의 활용

        Objectives : It is difficult to make accurate diagnosis of musculoskeletal disease because of its multiple, subjective and non-specific symptoms. It is possible to reduce errors of differential diagnosis through detailed history taking and physical examination in parallel with laboratory tests based on clinical decision. Methods : Korean and foreign on-line databases(Pubmed, Cochran Library, NDSL, KISS and OASIS) were researched for articles discussing laboratory tests in musculoskeletal diseases. Results : Laboratory tests could be applied usefully for various musculoskeletal diseases, In this review, available laboratory components in these musculoskeletal diseases are summarized, and then significance and usefulness of disease-specific laboratory examination are described. Conclusions : When examining musculoskeletal patients, it needs to accurate differential diagnosis by full interview and physical examination, to select required tests by understanding laboratory tests thoroughly, and to judge the prognosis precisely.

      • 구강 내 장치를 이용한 한의 임상연구 고찰

        Objectives: The purpose of this review is to analyse the trends of clinical studies using oral appliance in Korean Medicine. Methods: We reviewed Korean Medicine papers by searching Korean web databases 'Korea Traditional Knowledge Portal (KTKP)', 'Korea Medical Information Portal (OASIS)', 'Academic Research Information Service (RISS)', 'Scientific and Technological Information Integration Service (NDSL)'. We classified the papers by the year of publishment, the title of journals, the type of study and contents. Results: 1. Clinical studies using oral appliance have been increasingly published since 2011. 2. Most of the studies were published in the Journal of TMJ Balancing Medicine. 3. 33 research papers were divided in to 6 original articles, 27 case reports. But almost presented a low level of evidence. 4. In the content of the studies, oral appliance were used for various diseases such as nervous system, musculoskeletal system and visceral system. Conclusions: We expect that various studies and clinical application will be actively carried out with the development of technology related to oral appliance.

        발목 골절 수술 후 한방 재활치료 프로토콜 적용의 임상적 효과: 증례 보고

        The objective of this study was to propose a rehabilitation protocol for ankle fracture in Korean Medicine and to report its effectiveness. Four patients who had undergone ankle fracture by car accident were treated using the rehabilitation protocol in Korean Medicine. We estimated the outcome evaluating physical examination findings of the ankle joint, numeric rating scale, Olerud-Molander score, and walking state. After the treatment, we observed that ankle movement and muscular strength were improved. Moreover numeric rating scale, Olerud-Molander score, and walking state of patients were improved. A rehabilitation in Korean Medicine can be effectively used for ankle fracture patients. The limitation of this study was the insufficient number of cases. Further studies are needed to design a rehabilitation protocol in Korean Medicine.

        원위 요골 골절에 대한 한방 재활치료 프로토콜 적용의 임상적 효과: 증례 보고

        The objective of this study was to propose a rehabilitation protocol for distal radius fracture in Korean Medicine and to report its effectiveness. Three patients who had undergone distal radius fracture were treated using the rehabilitation protocol in Korean Medicine. We estimated the outcome evaluating physical examination findings of the wrist joint, numeric rating scale and quick the Disabilities of the Arm, Shoulder and Hand (DASH) score. After the treatment, we observed that wrist movement, numeric rating scale and quick DASH score were improved. A rehabilitation in Korean Medicine can be effectively used for distal radius fracture patients. The limitation of this study was the insufficient number of cases. Further studies are needed to design a rehabilitation protocol in Korean Medicine. (J Korean Med Rehabil 2018;28(3):97-106)

        요족형 족저근막염 환자에 대한 도침요법과 근막 추나요법 병행 치료의 임상적 효과: 증례 보고

        Objectives : This case study aimed to investigate the effect of acupotomy and fascia chuna therapy on talipes cavus-type plantar fasciitis. Methods : We classified the foot arch type using a podoscope and estimated the out- come by evaluating the numeric rating scale, pain disability index, and EuroQol-5-di- mension measurement. Results : After treatment, heel pain decreased and the quality-of-life score improved. Conclusions : This study suggests that treatment with acupotomy and muscle en- ergy techniques based on the foot arch type may be effective for plantar fasciitis. A limitation of this study is the small number of cases. Further clinical studies are required

        고관절 전치환술 후 한방 재활치료에 대한 증례 보고

        The objective of this study was to propose a rehabilitation protocol for total hip replacement in Korean medicine and to report its effectiveness. Three patients who had undergone total hip replacement were treated using the rehabilitation protocol in Korean medicine. We evaluated the outcome evaluating physical examination findings of the hip joint, numeric rating scale, pain disability index, Harris hip score, and walking state. After the treatment, we observed that hip movement and muscular strength improved. Moreover numeric rating scale, pain disability index, Harris hip score, and walking state of patients improved. A rehabilitation in Korean medicine can be effectively used for total hip replacement patients. The limitation of this study was the insufficient number of cases.         대퇴부 괴사성 근막염의 외과적 수술 후 한방치료에 대한 증례 보고

        Objectives : The aim of the present study was to report the clinical progress of a patient who underwent surgical treatment of necrotizing fasciitis. Methods : During the admission period, The patient was treated with acupuncture, cupping, Korean Medicine, Chuna therapy every day and observed by Numeric Rating Scale(NRS), Pain Disability Index(PDI), Beck Depression Inventory(BDI), and assessment about range of motion(ROM) and manual muscle test(MMT) for hip & knee joint. Results : After treated by using above treatment, NRS, PDI, BDI and ROM, MMT were improved. Conclusions : Korean Medical treatment are estimated to be good for patients after surgical treatment of necrotizing fasciitis. But more cases are required to prove the effectiveness of the treatment.

        근막통증증후군의 한의표준임상진료지침 개발 프로토콜

        Objectives This study aimed to develop a Korean medicine (KM) clinical practice guideline for myofascial pain syndrome (MPS) via the collaboration of clinical and methodological experts. Methods The study will include an initial survey to establish a common understanding of MPS. To develop the clinical guideline, a multidisciplinary development group was formed. The group will finalize the clinical questions based on a preliminary draft. The GRADE methodology is going to be applied to determine the level of evidence and grading of the recommendations. Finally, approval from the relevant medical societies will be obtained. Results A protocol for developing a KM clinical guideline for MPS was presented. Before finalizing the clinical key questions, a literature search was conducted according to the protocol, and a draft of 19 clinical key questions was established. Conclusions An evidence-based KM clinical guideline for MPS is expected to contribute to the management of MPS. This may also serve as a reference for the development of other KM clinical practice guidelines in the future. (J Korean Med Rehabil 2023; 33(4):203-213)

        노인등록연구 사례를 통한 임상평가지표 선정 과정 및 검증된 설문도구 승인 경험의 공유

        Objectives: Underpinned by the context of a Korean traditional medicine cohort study on healthy aging, this research primarily aims to guide the selection of Clinical Outcome Assessments (COAs) for elderly healthy aging patient registry research, offering insights into the selection process; and secondly, to streamline the resource-intensive process of obtaining permissions for validated COAs, benefiting future traditional Korean medicine clinical researchers. Methods : In this study, we identified outcomes through a review of previous studies, followed by a process involving expert consultations to select the final outcomes. Subsequently, for the selected outcomes that were Clinical Outcome Assessments (COAs) developed tools, we searched in commercial databases to confirm the availability of Korean versions and the necessity of obtaining permissions. Finally, we obtained permissions for their utilization and, when needed, acquired the original instrument questionnaire through payment. Results: Through a literature review of existing observational studies, a total of 57 outcomes were selected, with 19 of them identified as COA instruments. Upon verifying usage permissions for these 19 instruments, it was found that 17 required author-specific permissions, and among these, 2 needed a purchase as they were commercially available. Conclusion: This study provides a detailed overview of outcome selection and permission acquisition for elderly patient registry research. It underscores the importance of Clinical Outcome Assessment (COA) tools and the rigorous approval process, aiming to enhance research reliability. Continuous verification of COA information is essential, and future research should explore Core Outcome Set (COS) development through consensus-building approaches like Delphi studies.

        Development of an ultrasound-imaging procedure and acquisition of ultrasound images of acupuncture points for safety and accuracy of needle insertion

        Background: Acupuncture is a relatively safe, commonly used “alternative” medical treatment for various symptoms. However, adverse effects can occur, including trauma, pneumothorax, and central-nervous-system injury. Our objective was to develop a reliable and practical procedure for ultrasound imaging of acupuncture points to improve safety during needling, and to acquire ultrasound images of several (44) acupuncture points, especially those in high-risk areas, according to an in-house standard operating procedure. Methods: We created the standard operating procedure for ultrasound imaging for acupuncture, and collected ultrasound images of acupuncture points in clinical trials. Results: Ultrasound images for 44 acupuncture points considered as high-risk points were collected from 85 healthy people who were classified by body-mass index, and high-quality, clear representative images of all 44 points were obtained. Conclusion: These baseline images could be helpful for understanding the anatomy under the skin at acupuncture points, which would allow for an enhanced safety and more accurate needling.
