        Aggressive NK Cell Leukemia with RAB29-NUCKS1 Gene Rearrangement: A Case Report

        Aggressive natural killer (NK) cell leukemia (ANKL) is a rare form of leukemia that may be accompanied by various chromosomal abnormalities such as del(6)(q21q25) or del(11q). Here, we describe a case of a patient with ANKL and an RAB29-NUCKS1 rearrangement that has never been described before. An RNA fusion panel test found a gene fusion between exon 5 of RAB29 and exon 2 of NUCKS1 at chromosome 1q32.1. Additionally, reverse-transcription polymerase chain reaction and Sanger sequencing confirmed cryptic RAB29-NUCKS1 fusion. RAB29 encodes a protein that regulates exocytic and endocytic pathways. NUCKS1 encodes a chromatin-associated protein involved in DNA damage responses. Further studies will be necessary to understand the molecular pathogenesis of ANKL related to an RAB29-NUCKS1 rearrangement.

        아동의 말초정맥주사 관리를 위한 근거기반 간호 프로토콜 개발

        Purpose: The purpose of this study was to develop an evidence-based nursing protocol for peripheral intravenous catheter management in hospitalized children. Methods: The preliminary protocol of this study was developed based on domestic and foreign guidelines, research, a survey and an analysis of medical records in a university hospital. It is a methodological research to develop evidence-based nursing protocol of peripheral intravenous catheter management verified by content validity by a group of specialists and users. Results: The specialists’ verification of validity in the preparatory protocol had a CVI level of 0.94 and the propriety and conveniency of users had an average of 3.0±0.52. The final evidence-based nursing protocol was composed of 5 areas (education, dressing and fixation, maintenance and replacement, observation and record, and coping to complication) with 46 specific recommendations. Conclusion: Based on domestic and foreign research and guidelines verified by specialists and users, the findings in this study provide a simple, applicable and evidence-based nursing protocol for peripheral intravenous catheter management in hospitalized children. By the clinical application of this protocol, nurse tasks in managing peripheral intravenous catheter can be performed with more scientific evidence and be standardized.

        Purpose: The aim of this study was to compare the effect of training and implementation of a pediatric peripheral intravenous infusion protocol on the nurses' performance and infusion complication rate. Methods: A quasiexperimental study using a posttest comparison of a one group pretest-time series design was utilized. This method was developed to measure nursing knowledge and performance based on a pediatric peripheral intravenous infusion protocol. To calculate the occurrence rate of complications, the number of complications were divided into the number of intravenous insertions. Results: It was shown that the training developed by the study team in 2015 contributed to nurses' increased knowledge, as well as improved job performance. Furthermore, the implementation of the protocol revealed that the number of peripheral intravenous insertions, an invasive procedure, decreased. Additionally, the nursing workload related to pediatric intravenous infusions was reduced. Conclusion: This protocol may contribute to the improved scientific quality and efficiency of nursing management of peripheral intravenous infusions.

        어린이병원 간호사의 업무 스트레스와 공감만족, 공감피로, 소진 간의 관계

        Purpose: Nurses experience burnout related to various factors. For this descriptive research job stress, compassion satisfaction, and compassion fatigue were examined as to their relationship to burnout in nurses from children's hospital. Methods: The participants were 305 nurses working in children's hospital. Self-report questionnaires were used to measure job stress, compassion satisfaction, compassion fatigue and burnout. Results: Nurses in children's hospital experienced a greater than moderate degree of job stress, compassion satisfaction, compassion fatigue and burnout, whereas differences existed according to general characteristics. Job stress, compassion fatigue and burnout showed a significant positive correlation and results of compassion fatigue and burnout were similar. Also, job stress, compassion satisfaction and compassion fatigue were associated with burnout in nurses working in children's hospital. Conclusion: Findings indicate that as longer work experience is accompanied by higher job stress and burnout, it is necessary to develop intervention programs to reduce burnout among career nurses exposed to greater job stress in children's hospital.

        Clinical Usefulness of the QMAC-dRAST System for AmpC β-lactamase-Producing Enterobacterales

        Background: Rapid antimicrobial susceptibility testing (RAST) is important for theappropriate treatment of bloodstream infections. The QMAC-dRAST system (QuantaMatrixInc., Korea) can directly perform RAST using positive blood culture samples with microscopicimaging. This study aimed to evaluate the performance of the QMAC-dRAST system for AmpC-β-lactamase-producing Enterobacterales. Methods: Eighty isolates (20 Morganella morganii, 20 Serratia marcescens, 10 Klebsiellaaerogenes, 10 Enterobacter cloacae, and 20 Citrobacter freundii) and 14 antimicrobial agentswere included in the antimicrobial susceptibility testing (AST). The performance of theQMAC-dRAST system was evaluated by simulating the clinical blood culturing process. Weconducted a comparative evaluation of the QMAC-dRAST and Vitek 2 systems (bioMérieux Inc.,France). Broth microdilution tests were performed as the reference method to resolve anydiscrepancies in the AST results between the two systems. Results: For 20 M. morganii and 20 S. marcescens, the categorical agreement (CA) betweenthe QMAC-dRAST and Vitek 2 systems increased from 55.4% to 83.8% after AST algorithmoptimization. Moreover, the discrepancy rates decreased as follows: from 19.1% to 5.4% verymajor errors (VME), from 38.3% to 4.3% major errors (ME), and from 14.6% to 12.1% minorerrors (mE) for the QMAC-dRAST system compared to the Vitek 2 system. For all 80 testedisolates, the QMAC-dRAST system showed 93.0% CA, 1.7% VME, 2.3% ME, and 4.9% mE. Conclusion: The QMAC-dRAST system was comparable to the Vitek 2 system after ASTalgorithm optimization for AmpC β-lactamase-producers, which are major pathogens andrequire time to express the enzyme. However, further modifications of the AST algorithm arestill warranted.

        Performance Evaluation of QMAC-dRAST Using Consecutive Positive Clinical Blood Culture Samples

        Background: Bacteremia is life-threatening to patients, with a case fatality rate of 30–40%. The QMAC-dRAST system (QuantaMatrix, Republic of Korea), which is based on time-lapse imaging technology, can generate antimicrobial susceptibility testing (AST) results earlier than conventional equipment. Here, we evaluated the performance of QMAC-dRAST for positive blood culture samples. Methods: In total, 204 isolates were collected from positive blood cultures, with 104 Gram-positive cocci (GPC) and 100 Gram-negative rods (GNR), including Staphylococcus spp., Enterococcus spp., Enterobacteriaceae, Pseudomonas aeruginosa, and Acinetobacter spp. Before and after improvement of the AST algorithm, 67 GPC isolates and 72 GNR isolates were tested and their results were compared. In the final test, 37 GPC and 28 GNR were added, and the agreement rates between QMAC-dRAST and Vitek 2 were analyzed using 204 samples. To resolve discrepant AST results between two systems, broth microdilution tests were performed. Results: In the first and second tests with 67 GPC and 72 GNR, the categorical agreement (CA) between QMAC-dRAST and Vitek 2 was increased from 92.7% and 96.2% to 94.3% and 96.5% by updating the AST algorithm, respectively. For all 204 samples, the agreement rates were 94.5% and 95.4% for CA; 4.8% and 0.6% for very major errors; 2.5% and 2.0% for major errors; and 2.1% and 3.0% for minor errors. Conclusions: The QMAC-dRAST system has reliable performance and the advantage of faster AST reporting than conventional methods. This system will demonstrate more acceptable agreement rates with conventional AST systems in the future by improving AST algorithms.

        중증도 분류 간호사에 의한 응급환자 중증도 분류 신뢰도 측정 연구: Emergency Severity Index Version 4를 중심으로

        Purpose: The aim of this study was to measure the inter-rater reliability of Emergency severity index (ESI) version 4 among triage nurse. Methods: This study was carried out from August 11, 2010 to September 7, 2010 in a regional emergency department. Data collection was done by ten triage nurses who trained ESI v.4. Two research nurses and ten triage nurses scored the ESI version 4 to the patients as references, independently. We calculated the weighted kappa between the triage nurses and research nurses to evaluate the consistency of the ESI v.4. Results: A total of 233 patients were enrolled in this study. Classification of ESI level was as follows - level 1 (0.4%), level 2 (21.0%), level 3 (67.8%), level 4 (9.4%), and level 5 (1.3%). Inter-rater reliability by weighted kappa was 0.79 (95% Confidence Interval= 0.74-0.83) and agreement rate was 87.1%. Under-triage rate by triage nurse was 6.0% and over-triage rate was 6.9%. Conclusion: For this study, inter-rater reliability was measured good level between triage nurses and research nurses in Korean single ED.
