        의사 대상 불면증 치료 현황 조사 연구

        목 적 : 본 연구는 1차 및 2차, 3차 의료기관에 종사하는 의사 102명을 대상으로 설문조사를 통해 불면증 환자의 치료 실태를 조사하고자 하였다. 방 법 : 총 102명의 정신건강의학과 및 타과 전공 의사들이 설문에 참여하였고, 분석에 포함된 설문조사 대상자는 100명이었다. 결 과 : 실제 치료 장면에서 가장 자주 사용되고 있는 치료는 약물학적 치료였으며, 수면위생 교육과 인지행동 치료 등의 비약물학적 치료도 시행이 되고 있는 것으로 나타났다. 그러나 비약물학적 치료를 시행하더라도 그 효과에 대한 만족도는 높지 않았다. 의사가 보기에 불면증 환자들의 비약물학적 치료에 대한 관심은 일정 수준 있는 것으로 나타났으나 이에 대한 식견이나 의식 수준은 아직 미흡한 것으로 보고되었고, 치료의 실행도 충분히 적절하게 이루어지고 있지는 않은 것으로 보고되었다. 본 연구에서는 추가적으로 의료진이 생각하는 불면증 환자의 적정 진료를 위해 요구되는 점에 대해서도 탐색하였다. 결 론 : 본 연구는, 치료적 개입에 대한 보다 구체적인 정보를 획득하지는 못했다는 점, 환자를 대상으로도 설문을 실시하지는 못했다는 점 등의 제한점을 지니고 있으나, 그럼에도 불구하고 불면증 치료의 개선과 변화를 위해 필요한 현황 조사를 면밀히 실시했다는 점에서 그 의의가 있다. 결론적으로 다수의 의사들이 불면증의 비약물학적 치료가 중요하다고 생각하고 있었으나 진료 현실에서 적용하기 힘든 부분이 있다고 보고하였다. 본 연구 결과는 불면증에 대한 적정 치료를 위해 비약물학적 치료를 위한 진료 여건 개선 및 의사대상 교육이 필요함을 제시하였다. Objectives: The present study investigated current practices of insomnia treatment among Korean doctors in clinical settings. Methods: A total of 100 doctors participated in the present study and filled out a series of survey questions regarding their treatment of insomnia patients. Results: The results revealed that the primary type of insomnia treatment was pharmacological and that the most popular medication was zolpidem. The majority of doctors reported that they also utilized non-pharmacological treatments such as sleep hygiene education and cognitive-behavioral therapy. However, these treatments tended to result in low satisfaction. In addition, the doctors perceived that patients largely preferred pharmacological treatments to non-pharmacological ones and did not have sufficient knowledge of non-pharmacological treatments. Conclusion: Many doctors believed that non-pharmacological treatments for insomnia were important, but reported that they were difficult to implement in practice. The results of this study suggest that improved medical conditions for non-pharmacological treatments and education of physicians are necessary to appropriately treat insomnia.

        Investigation of blade failure in a gas turbine

        A gas turbine abruptly tripped to a stop during its daily morning start-up period. Out of a total of 81 blades in the first row only one blade was broken at its root. Prior to this accident, there were three blade failure accidents in the same plant during the last 10 years. First,the fracture surface of the troubled blade was investigated. Stress analysis of the blade showed that the maximum stress occurred due to the pressure profiles developed during operation. Modal analysis for one blade and the assembly of blades was performed and Campbell diagram and Interference diagram were drawn to check the dynamic characteristics of the blades. The vibration of the turbine was measured using accelerometers during the operation condition. The result shows that the fatigue fracture of the blade was originated during transient events internal to the combustion chamber which was close to the resonance condition of the assembled blades.

      • 고속전철 Catenary/Pantograph계의 동적응답에 대한 수치해석

        To increase the velocity of high speed train, electric power collection through pantograph from trolley wire of should be guaranted. As the velocity increase, there are lots of possibility of separation between pantograph and trolley wire due to the mutual dynamic behaviors. However, the complexity of actual Catenary system makes almost impossible to find dynamic responses analytically. In this respect a numerical method is developed to calculate the dynamic responses of Catenary/Pantograph system using central difference method. And the dynamic characteristics of Catenary/Pantograph are investigated as the design parameter variation.

      • 회전축계 위치제어를 위한 비선형 마찰 보상

        Friction effects in rotor system with a brush type D. C. servo motor is explored for the precise position control based on experiments. The success of friction compensation depends on the accurate measurement of real friction torque. Experimental results have clearly shown that both static and dynamic friction have nonsymmetric characteristics. Estimated friction torque was used to linearize the system by feedforward compensator. In practice, overcompensation may cause another stability problem due to sampling time, gain, and modeling. Therefore, a compensation ratio was introduced for the elimination of the instability.

      • 회전기계의 부분 접촉에 따른 마멸현상에 대한 실험적 연구

        Rubbing phenomenon occurs when the whirling orbit of a rotor is greater than the allowable clearance between the rotor and stator of a rotating machinery. The contact and friction during rubbing makes the phenomenon complex; subharmonics, superharmonics, distorted orbit, and high vibration level. The several partial rubbing phenomena of superharmonics, subharmonics and jump phenomenon including the complicated orbits are demonstrated for the partial rub using RK-4 rotor kit in this study. The orbit patterns are calculated using directional FRF method to show forward or backward components during rubbing. In addition, 1/2 X subharmonics of partial rubbing, i. e., one impact per two revolutions during heavy rubbing, is demonstrated experimentally and verified through numerical analysis.

      • 모드해석에서의 경계조건 영향

        Modal analysis is a fundamental procedure to analyze the dynamic characteristics of structures or machines. Modal analysis is composed of theoretical modal analysis using finite element method and experimental modal analysis. The discrepancy between these two makes modal analysis difficult and ususally comes from misunderstandings on boundary conditions of the structure. Boundary condition effect in modal analysis is investigated theorectically and experimentally in this study for a rod and a beam. The result shows that boundary condition affects on lower modes, but can be ignored for higher modes. Boundary condition can be modeled by a spring and a damper having finite values. Using this spring and damper model, the boundary conditions of a theoretical modal analysis model can be corrected by comparing the results of experimental modal analysis.

      • 달크로즈 피아노 즉흥연주를 활용한 불규칙박자 학습 지도방안 연구

        현대 피아노교육에서 소개되고 있는 레퍼토리를 살펴보면 다양한 박자로 작곡된 작 품이 점차 늘어나고 있다. 그러나 피아노 교육 현장에서는 교사들이 사용하는 레퍼토리 가 한정되어 있고, 작품에 대한 지식과 효과적인 교수법적 접근에 대한 연구가 부족하 여 다양한 악곡에 대한 균형 잡힌 피아노 레슨을 시도하기가 어려운 것이 사실이다. 본 논문에서는 달크로즈 교수법의 즉흥연주 영역을 적용하여 불규칙박자에 대한 학생 의 흥미를 이끌어낼 수 있는 창의적인 피아노 레슨 방안을 모색하였다. 학습의 주제로는 불규칙박자 중 비교적 쉽게 접근할 수 있는 5/4박자를 선정하였고, 달크로즈 교수법의 원리가 적용된 즉흥연주 방법을 통해 5/4박자에 대한 경험을 충분하게 쌓은 후, 이를 소 재로 학생 스스로 새로운 작품을 작곡할 수 있도록 하는 방안을 제시하였다. 이를 통해 학생의 창의성, 음악성 향상과 함께 불규칙박자로 작곡된 악곡에 대한 관심을 높여주고, 교사들에게는 불규칙박자를 지도하는 하나의 방법을 제시함으로써 작품 속에서 나타나 는 어려운 음악이론들을 쉽고 효과적으로 지도할 수 있는 학습방법을 제안한다. Works that composed with various meters are increasing among the repatoire introduced in modern piano education. But, the repatoire that used by teachers in the field of education are limited. It is difficult to piano lesson about various works because piano teachers knowledge and effective teaching methods is not sufficient. In this paper, improvisation of Dalcroze Method was applied to students for a piano lesson to interest in unequal meters music. Subject of study was chosen as an unequal meter, 5/4 meter that is commonly found in unequal meters music. Students were fully experience the 5 beats music rhythm through improvisation using the principle of Dalcroze Method. Then, utilizing these experience, students will be able to compose their own contemporary music. This study suggests that how to teach unaccustomed theories of contemporary music effectively by present one of the possibility for teaching the unequal meter and enhance the creativity of students and their interest in unequal meters music.

      • 간극내 외팔보 운동의 실험해석

        Many mechenical systems experience intermittent motion of contact and separation due to the existence of clearances which may cause from manufacturing tolerances, wear, or backlash. The complex nonlinear dynamic responses within such systems can produce proper functioning or loss of performance as like wear, fretting, fatigue, as well as high noise level. In this study, the dynamic responses of a cantilever beam motion in clearance are measured with a simplified experimental set-up and the experimental results are compared with those of numerical harmonic balance method and those of numerical intergration. The results show that the cantilever motion in clearance can be modeled as a piecewise-linear oscillator, which characterizes nonlinear system. As a result, the nonlinear responses of jump phenomenon and higher harmonics are investigated experimentally and analytically. Also, the effects of the variation on system parameter were studied.

      • 고속 전철 Catenary/Pantograph계의 동특성 해석

        The vibrations in Catenary/Pantograph system should be avoided to assure the power collection in High-Speed train. Contacts between pantograph and trolley wire depend on various design parameters. To select good design parameters, analytical and numerical simulation were performed for a simplified model. The dynamic characteristics of tensioned wire subject to moving force was calculated using Laplace transform. And, a simulation program was developed to calculate the displacement of trolley wire using Euler method. The numerical results showed good agreements between two methods.
