      • 간호실무 : 여자 청소년의 체질량지수에 따른 체중조절행위 변화단계, 변화과정과 의사결정균형 비교

        Purpose: The purpose of this study was to compare variables related to weight control between normal weight and overweight/obese female adolescents. Methods: This study is a supplementary analysis using a total of 293 female adolescents in Seoul. Data were collected through self-report questionnaires on the topics of stages of change, processes of change, and the decisional balance pertaining to weight control. Body weight and height were also measured. Results: There was a significant difference in the stages of change between the normal weight and overweight/obese groups. Of 12 processes of change, 9 processes were significantly higher in the overweight/obese group than in the normal weight groups. Also, female adolescents who were overweight or obese had significantly higher pros and eating efficacy scores comparing to those in the normal weight group. Conclusion: The findings of this study suggest that overweight/obese female adolescents, especially those in the action stage need supports to continue their weight control behaviors. In addition, a weight control program for female adolescents should emphasize their participation in physical activities in addition to their dietary control efforts.

      • 특수병동 아동 간호사의 신체활동과 피로

        Purpose: This study was designed to explore the levels of physical activity and fatigue among nurses and to identify the relationship between these variables. Methods: Participants were 89 nurses working at an intensive care unit and an operating room in a children’s hospital. Data were collected using self-reported questionnaires including the International Physical Activity Questionnaire and Brief Fatigue Inventory. Results: Most of the nurses’ physical activity was work related, and there was a significant difference according to the working unit. Nurses with higher work stress and lower job satisfaction showed higher levels of fatigue than their counterparts did. Work-related physical activity and the interference of fatigue with relationships were related positively, whereas transport and leisure time physical activity were negatively related to usual fatigue and the interference of fatigue with life enjoyment. Conclusion: The levels of physical activity and fatigue of nurses were higher than those observed among other populations. The appropriate level of physical activity for these nurses needs to be investigated carefully. Also, in the intervention for nurses' fatigue, their level of work-related and leisure-time physical activity should be considered separately, and their work environment and psychological satisfaction level should be included.

        천식아동 간호 시뮬레이션 학습의 효과

        Purpose: The purpose of this study was to examine the effects of simulation-based learning on the knowledge about and performance confidence in the nursing care of children with asthma. Methods: A one-group pre- and post- study design was used. A total of 70 nursing students participated in the study. In groups of five, the participants experienced simulation-based learning during their child health nursing practicum. The given scenario was about nursing care for children with asthma. The simulation learning was provided for 3 hours, and included a group discussion before the simulation and a debriefing. Results: After the simulation, knowledge about and performance confidence in the nursing care of children with asthma significantly increased. The nursing students reported a high level of satisfaction with the simulation-based learning. There was a significant positive correlation between knowledge and performance confidence. Conclusion: Simulation-based learning was effective for nursing students in the nursing care of children with asthma. Our results suggest utilizing this new way of learning to strengthen the clinical experience of child health nursing in nursing students.

      • 유연성 운동프로그램이 노인의 관절각도, 혈압 및 자기효능감에 미치는 효과

        Purpose: The purpose of this study was to develop and evaluate an exercise program for elderly community-dwelling adults. The program was intended to improve joint flexibility and self-efficacy and to reduce blood pressure. Methods: A quasi-experimental, nonequivalent control group pre-post test design was used to evaluate the program. Twenty-six subjects were enrolled in the experimental group, and twenty-seven subjects were enrolled in the control group. The program was carried out between March 6 and May 4, 2006. The experimental group subjects participated in the exercise program once a week for eight weeks and received a phone call weekly to encourage self-exercise at home. Results: After program participation, the experimental group subjects showed significantly higher flexibility in the right shoulder joint and higher self-efficacy than subjects in the control group. Systolic and diastolic blood pressures were also significantly lower in the experimental group subjects. However, there were no significant differences in knee joint flexibility between the two groups after the intervention. Conclusion: The eight-week exercise program was effective in decreasing blood pressure and improving joint flexibility and self-efficacy in older adults. This study suggests that this exercise program could be utilized as an effective independent nursing intervention modality in elderly persons.

        퇴원 후 미숙아의 수유 유형과 영향요인

        Purpose: To investigate factors that may affect the method of feeding among preterm infants at 4 weeks after discharge. Methods: This study included 222 mother-infant dyads born before a gestational age of 37 weeks. The feeding method and general medical characteristics of the participants were assessed at 4 weeks after discharge using a structured questionnaire. Multinomial logistic regression analysis was used to examine which factors were associated with breastfeeding at home. Results: Of the 222 infants who qualified for the study, 71 (32.9%) continued to receive breastmilk at 4 weeks post-discharge. Multinomial logistic regression analysis showed that breastfeeding at 4 weeks post-discharge was associated with higher breastfeeding self-efficacy, vaginal delivery (experience), direct breastfeeding in the neonatal intensive care unit (NICU), gestational age between 30 and 34 weeks, and breastmilk consumption in the NICU. The following factors were associated with mixed feeding at 4 weeks post-discharge: being employed, having higher breastfeeding self-efficacy, and direct breastfeeding in the NICU. Conclusion: NICU nurses should provide opportunities for direct breastfeeding during hospitalization and support breastfeeding to enhance breastfeeding self-efficacy. These factors may help to ensure the continuation of breastfeeding after discharge. Moreover, factors that affect breastfeeding should be considered when providing interventions.

        중증 선천성 심장병 청소년의 건강관리 경험

        중증 선천성 심장병은 환자의 삶의 질을 높이기 위해 지속적이고 전인적인 건강관리가 필요한 만성질환이다. 본 연구는 한국 중증 선천성 심장병 청소년의 건강관리 경험을 탐색하기 위하여 시도되었으며, 질적 연구방법을 적용하여 중고등학생 9명을 대상으로 심층 면담을 수행하였다. 수집된 자료를 분석한 결과, 4개의 범주와 12개의 하위 범주가 도출되었다. 중증 선천성 심장병 청소년들은 자신의 건강상태와 건강관리의 필요성에 대한 이해가 다소 부족하였다. 그러나 심장 기능, 자아정체성, 사회적 지지의 영향을 받으며 부분적인 건강관리를 실천하고 있었다. 중증 선천성 심장병 청소년들은 신체적인 제한이나 주변의 차별적인 시선에 영향을 받기도 하였지만, 질병정보 관리, 또래와의 동질성 추구, 자신의 질병에 대한 긍정적인 수용을 통해 정상화를 이루려고 노력하였다. 건강관리 프로그램에 대한 요구로는 질병지식과 정신건강 내용을 선호하였고, 청소년캠프와 스마트폰 애플리케이션을 이용한 전달방식을 선호하였다. 본 연구결과를 바탕으로 중증 선천성 심장병 청소년의 건강한 성인기로의 전환을 도울 수 있는 건강관리 중재가 필요할 것이며, 체계적이고 장기적인 건강관리를 위한 정책적 방향에 대한 논의가 필요할 것으로 고려된다. Severe congenital heart disease (CHD) is a chronic disease requiring continuous and holistic health management to improve patients’ quality of life. This study explored the health management experiences of adolescents with severe CHDs in Korea. On the basis of in-depth interviews with nine youths, four categories and 12 subcategories were identified. The participants lacked a full understanding of their health status and the need for appropriate health management for their health status. Nonetheless, they practiced health management to some extent, in ways that were affected by heart function, self-identity, and social support. Although adolescents with severe CHDs were affected by physical limitations and the discriminatory gaze around them, they tried to achieve normalcy through managing information, seeking homogeneity with peers, and fostering positive acceptance about their disease. Regarding demands for health management programs, disease knowledge and mental health content were preferred, and the preferred delivery methods were self-help group camps and smartphone applications. Based on the results of this study, we suggest a health management intervention to promote a healthy transition to adulthood for adolescents with severe CHDs, and that policy-makers should consider measures that would enable a successful transition to adulthood in the future of youth with severe CHDs.

      • 빈곤계층 아동 양육자의 우울, 건강 상태와 양육 스트레스

        This is a descriptive study to identify the relationships of depression, health status, and parenting stress of caregivers of children in poverty. Methods: The participants were 42 caregivers of preschool or school age children enrolled for the Dream Start Program in a city of Kyeonggi province, which is a program of the Korean Ministry of Health & Welfare offering services for growth and development of low-income children. Data were collected from November, 2009 to February, 2010 by using a questionnaire. Results: The vast majority of the participants were mothers (n=33, 78.6%), and less than a half were married (n=16, 38.1%). Depression of the participants was at the moderate level (18.60±10.13). They perceived their health was fair (81.10±18.97). Their parenting stress was high (93.45±20.06). Their parenting stress was significantly correlated with depression (r=.57, p<.001) and perceived health status (r=.49, p=.001), which indicates their parenting stress was high as their depression was high or they perceived they were not healthy. Conclusion: The study results suggest to provide the caregivers of children in poverty with a well- developed program to manage their depression and parenting stress and ultimately to improve their mental health.

        중학생의 자유학기제 경험

        본 연구는 2016년 중학교 자유학기제의 전면도입을 앞 둔 시점에서, 실질적 자유학기제의 주체라고 할 수 있는 중학생의 자유학기제 경험을 탐색하기 위하여 시도되었다. 질적 연구방법을 적용하여 의도적 표집을 통해 자유학기제 시범학교 중학생 8명을 대상으로 심층면담을 수행하였다. 수집된 자료를 분석한 결과, 10개의 주제가 도출되었다. 각 주제는 다음과 같다: 울며 겨자먹기로 수행평가에서 고군분투하기, 시험에 대한 부담감, 진로체험의 아쉬움, 깊이 있는 우정, 차별과 존중의 양면성, 초등과 중등 사이에서 휴식과 적응의 기회, 생각 없이 흘러간 찰나같은 아쉬움, 자유로움과 압박의 공존, 친구들이 있는 따뜻한 공간, 생색내기식 리허설에 끌려다님. 연구결과, 자유학기제 학교생활에 대한 양가감정과 함께, 특히 친구관계에서의 특별한 우정에 대한 경험이 가장 우세하게 떠올랐다. 자유학기제 경험의 전반에서 협력위주의 우정이 지배적 범주로 떠오른 것은 경쟁위주의 현 학교 현실에 대한 긍정적 대안으로서 자유학기제의 성공가능성을 시사해준다. 중학생을 대상으로 자유학기제 경험의 의미를 확인한 본 연구결과를 토대로 향후 자유학기제의 성공적 시행을 위한 정책적 방향을 논의할 수 있을 것으로 기대한다. Free Learning Semester, the new education policy of middle schools in Korea, will be completely implemented in 2016. The purpose of this study was to explore the school experiences of middle-school students in the Free Learning Semester through qualitative approach. In-depth interview was conducted with 8 middle-school students from the purposive sampling. As a result of the analysis, 10 essential themes were identified. Themes were as follows: Struggling from the performance evaluation with biting the bullet, Great pressure on final examination that is only once a year, Inconvenience of career education experience, Higher level of friendship’, Double face of discrimination and respect, Chance to adaptation between elementary and middle-school, Regret about fleeting time without thought, Ambivalence of freedom and suppression, Warm place with peer, Being pushed rehearsal wearing others cloth. In conclusion, the nature of lived experience of middle-school students in the Free Learning Semester is based on ambivalence of various aspects, but is predominant in higher level of friendship. It suggested that the Free Learning Semester can be a positive alternative to existing education system that made students to compete each other. It is expected to be able to discuss the future policy direction using the result of this study which explored the nature of lived experience related to the Free Learning Semester.

        아동간호 임상프리셉터십 경험 유무에 따른 간호학생의 전문직 자아개념과 임상수행능력 자신감, 실습 만족도 비교

        Purpose: The purpose of this study was to compare professional self-concept, clinical nursing competence, and practicum satisfaction between nursing students with and without pediatric nursing clinical preceptorship. Methods: This study used a comparative research design. A total of 85 junior nursing students participated in the study, and data from 76 students were analyzed using SPSS WIN 20.0. Among them, 44 students experienced pediatric nursing clinical preceptorship. Results: The students with clinical preceptorship showed a significantly higher score in nursing skill related items of professional self-concept and the confidence in knowledge of injection preparation and administration compared to the counterpart students. Practicum satisfaction was also significantly higher in the students with clinical preceptorship than in the other students. There were significant positive correlations among professional self-concept, performance confidence, and practicum satisfaction. Conclusion: The study findings suggest that clinical preceptorship is an effective educational method to teach clinical practicum of pediatric nursing. The use of clinical preceptorship needs to be considered to motivate nursing students actively involved into the clinical practicum. The roles of clinical preceptors should be clearly identified in clinical education.

        당뇨병성 족부궤양 환자의 치료형태 및 가정간호 연계

        Purpose: The purpose of this study was to investigate treatment types and the status of referral to home care services for patients with diabetic foot. Methods: A retrospective survey was conducted by reviewing medical records from January to December in 2008 at a university hospital. The subjects were 76 patients at the age of 20 years or older who were admitted, had home care services, or received outpatient care for diabetic foot. The data were analyzed using descriptive statistics. Results: Among the total of 9,317 patients diagnosed with diabetes, 5.03% (n=469) had diabetic foot. Admission (81.6%) was the most frequently used treatment type followed by outpatient care only (7.9%) and hemodialysis only (10.5%). Of the 76 admission cases, 44.9% received post-discharge care at outpatient clinics, 20.5% had both outpatient and home care services, and 16.7% were transferred to other hospitals. Readmission rate after discharge was 15.6% for one year. Conclusion: This study suggests referral to home care services should be encouraged to provide effective follow-up care to patients with diabetic foot after discharge from a hospital.
