Purpose: To evaluate the efficacy and safety of acellular human dermis (AlloDerm) as a spacer graft in the correction of eyelid retraction. Methods: We used acellular human dermis (AlloDerm) as a spacer in 8 patients with eyelid retraction. Two patients had upper eyelid retraction and 6 had lower eyelid retraction. The causes of the former were congenital (1) and ptosis operation (1), and of the latter were strabismus surgery (1), necrotizing blepharitis (1), blow-out fracture (1), anophthalmic socket contracture (1), cicatricial entropion (1), and lower lid blepharoplasty (1). The follow-up period ranged from 5 to 14 months (mean 9 months). Results: Surgical results in 7 eyelids were satisfactory. There was mild overcorrection in one eyelid with congenital upper eyelid retraction. There were no other complications such as exposure, infection, or rejection. Conclusions: AlloDerm was easy to manipulate, closely approximated the eyelid tarsus in contour and thickness, and appeared to be nontoxic and nonallergenic. We suggest AlloDerm as the spacer material of choice for the treatment of eyelid retraction.

Purpose: To investigate the clinical features of orbital and ocular adnexal malignant lymphoma and the effectiveness of radiotherapy for its treatment. Methods: We retrospectively reviewed the medical records of 54 eyes in 44 patients who were diagnosed with orbital and ocular adnexal malignant lymphoma and who underwent radiotherapy. Results: The main symptoms of malignant lymphoma patients were conjunctival mass, lid mass, lid swelling, and exophthalmos. Histopathologically, MALT-lymphoma was the most common in 39 patients (88.6%). In all 54 eyes of the 44 patients, the mass either decreased or disappeared completely after radiotherapy. The mean follow-up period was 31 months, ranging from 4 to 68 months. There was no recurrence of malignant lymphoma in the orbit or ocular adnexa during this period. However, there were three cases where the tumor recurred in other systemic organs. Complications of radiotherapy included mild punctate epithelial erosion, xerophthalmia, cataract, and retinopathy. Conclusions: Localized radiotherapy is an effective and safe way to treat malignant lymphoma of the orbit and the ocular adnexa. In order to decrease complications, the use of lens shielding and optimized radiation amount should be considered.

We suggest AlloDerm as the spacer material of choice for the treatment of eyelid retraction. 목적 : AlloDerm을 spacer로 이용하여 안검후퇴교정술을 시행하고 수술결과 및 AlloDerm의 안전성을 알아보고자 하였다 대상과 방법 : 상안검후퇴 2명(2안), 하안검후퇴 6명(6안), 총 8명(8안)에 대해 spacer로 AlloDerm을 이식하였다. 상안검후퇴의 원인은 선천성 1안, 안검하수교정술 1안이었고, 하안검후퇴의 원인은 사시수술 1안, 괴사성안검염 1안, 안와하벽골절정복술 1안, 무안구성socket위축 1안, 반흔성내반 1안, 하안검성형술 1안이었으며 관찰기간은 평균 9개월(5~14개월)이었다. 결과 : 7안에서 안검교정상태는 만족스러운 결과를 얻었으며, 선천성 상안검후퇴 1안에서 약간의 과교정이 관찰되었다. AlloDerm 이식편은 수축이 적고 잘 유지되었으며 노출이나 염증 및 거부반응 등의 합병증은 발생하지 않았다. 결론 : AlloDerm은 다루기 쉽고, 검판과 비교시 형태, 두께 등에서 유사하며, 독성 및 거부 반응이 거의 없는 장점이 있다. 안검후퇴교정술에서 spacer가 필요한 경우 AlloDerm이 적절한 재료 중의 하나로 사용될 수 있을 것으로 생각된다.

영상처리를 이용한 관절가동범위 측정의 자동화 및 휴대용 시스템

고니오미터란 인체관절의 운동범위를 측정하고 평가하기 위해 사용하는 측정도구이다. 이 측정은 운동 처방 전후의 평가와 변화를 모니터링 하는데 사용된다. 관절가동범위는 측정 시 전문적 지식이 필요하며, 수동으로 측정하는 기존방식 때문에 시행오류가 불가피하다. 시행오류를 줄이기 위해서는 숙련된 동일한 평가자가 동일한 자세에서 동일한 기술로 동일한 시점에 측정하도록 권해진다. 본 논문에서는 영상처리를 이용하여 관절가동범위 측정의 과정을 자동화하고 휴대용 시스템으로 간소화 시키는 연구를 진행하였다.

본 연구는 부산지역의 주요기업 변화를 살펴보기 위해 매출액 기준 전국 상위 1,000대 기업에 속하는 부산기업을 조사하여 비교분석을 실시하였다. 분석결과 2005년에 전국 1,000대 기업 중 46개 기업이 부산에 소재하였는데, 2019년에는 12 개가 줄어든 34개 기업만이 전국 1,000대 기업에 포함되었다. 또한, 전국 상위 1,000대 기업의 매출액 기준으로 부산지역 기업의 매출액 비중은 2005년 1.6%에서 2019년 1.4%로 줄어든 것으 로 나타났다. 이는 수도권 집중현상이 점차 강화되고 있기 때문이다. 한편, 부산지역 매출액 기준 상위 10대 기업은 많은 변화가 있었는데, 2005년 상위 10대 기업 중 2019년에도 상위 10대 기 업에 포함된 기업은 ㈜한진중공업, 르노삼성자동차(주), ㈜부산은행, ㈜서원유통 등 4개뿐인 것 으로 나타났다. 통계적 인과 관계는 확정할 수 없으나 지역 대기업의 쇠퇴 및 유출과 지역의 쇠퇴가 동시에 이루어지고 있어 지역 경제의 침체와 대기업의 쇠퇴 및 유출이 상당한 관계가 있음을 유추해 볼 수 있다. 특히 2011년 이후 지역 주력 제조업 기업들이 1000대 기업 진입에 실패하였다는 점을 주목해 볼 필요가 있다. 지역 제조업 대기업의 이탈 또는 쇠퇴는 지역 생산성 하락에 영향을 미 쳤을 가능성이 높고, 낮아진 지역 산업 생산성이 지역 경제성장률 둔화에도 영향을 미칠 가능성 이 있다. 따라서 지역의 경제활력 저하 문제를 제고하기 위해서는 지역 산업구조 개편 문제와 생 산성 향상 방안에 대한 고민이 필요하다.

아래눈꺼풀에 발생한 원발성 피부 역형성 대세포 림프종 1예

Purpose: Primary cutaneous anaplastic large cell lymphoma is rarely encountered in the lower eyelids. We report a patient with primary cutaneous anaplastic large cell lymphoma arising from the lower eyelid. Methods: A 39-year-old man presented with a relatively fast growing mass on the center of his left lower eyelid for one month. The mass did not respond to local injection of triamcinolone at a local clinic. The lesion appeared as a solitary reddish nodule with ulceration, was non-tender, round, crusted, and measured 13 mm×11 mm×5 mm. Well- developed superficial vessels were found on the surface of the nodule. An incisional biopsy was performed. Results: Histologic examination revealed that the bulk of the infiltrate was in the papillary and reticular dermis. Tumor cells had abundant, well-defined cytoplasm and pleomorphic nuclei with multiple nucleoli. The majority of the neoplastic cells showed immunoreactivity for CD 30 (Ki-1) along the cell membrane. A histopathological diagnosis of primary cutaneous anaplastic large cell lymphoma was made. Conclusions: Most cases of primary cutaneous anaplastic large cell lymphoma arise from the body and extremities. However, since primary cutaneous anaplastic large cell lymphoma may occur in the eyelid, it should be differentiated from nodular and relatively fast growing inflammatory tumors despite local steroid treatment.
