        시대적으로 바라본 마음과 몸의 수사학 : (편)두통의 사례

        마음과 몸의 관계는 오랜 논의에도 불구하고 설득력 있는 답을 구하지 못하고 있다. 한편, 서구 의학이 유물론적 패러다임을 받아들이면서, 기질적 이상이 없는 증상들은 "의학적으로 설명되지 않는 증상"이라 하여 변방에 머물게 된다. 이러한 증상을 이해하고자 전문가들은 마음과 몸의 관계를 바탕으로 소위 정신신체의학이라는 해석의 틀을 내놓았다. 이 해석의 틀은 의사소통 방식뿐 아니라, 환자들의 건강추구 행위 및 증상을 경험하는 양식도 변화시켰다. 시대의 필요나 새로운 과학발견에 의해 해석의 틀은 변화되어 왔으며, 어떤 때는 마음이 어떤 때는 몸이 강조되었다. 특히 치료법이 부재할 때에는 마음이 강조되면서 환자의 인격이 비난 받거나, 환자의 책임이 더 강조되었다. 반면 약물치료가 등장한 후에는 마음을 강조할 필요성이 줄어들면서 환자의 책임 역시 면제되었다. 본 논고에서는 마음과 몸의 관계를 중심으로 해석의 틀이 어떻게 시대에 따라 변화했는지를, 두통과 편두통의 사례를 통해 살펴보고자 한다. 이를 통해 해석의 틀이 어떻게 증상을 경험하는 양상을 변화시켰으며, 그때마다 책임 소재가 어떻게 달라졌는지 고찰할 것이다. 이러한 통시적 고찰은, 전문가로 하여금 그들이 만들어내는 해석의 틀이 얼마나 큰 영향력을 가지는 지, 그것이 얼마나 시대 상황과 밀접한 관계를 맺는지를 고찰할 기회를 마련할 것이다. The relationship of mind and body has stimulated extensive discussion for a long time. However, answers are ambigous and not forthcoming yet. Meanwhile, after the western medicine had embraced materialistic paradigm, conditions which cannot be substantiated by organic change were pushed out to periphery under the banner of "Medically Unexplained Symptoms". Medical experts endeavored to understand these mysterious illnesses based on the mind-body relationship and provided a frame of interpretation called Psychosomatic Medicine. This frame of interpretation had influenced not only the communication practice but also the health-seeking behavior and even the subjective experience of patients regarding their illnesses. The frame of interpretation had been drastically changed many times keeping pace with the socio-economic situation and the new scientific discoveries. Accordingly, body or mind was given differential importance by medical experts. For instance, when treatment modalities were lacking, mind was given excessive importance and patients were blamed for their unconscious motives, characterological weaknesses and for their lack of responsibility. In contrast, after the discovery of effective drug, mind was given no more attention and patients were no longer blamed for their contribution to suffering. In this paper, the historical change of the frame of interpretation for understanding migraine and headache was presented as an example. By this, it can be demonstrated how the frame of interpretation has modified the subjective experience of patients, and how the patients' responsibilities were viewed differently. This observation can help to realize the enormous influence of the frame of interpretation provided by medical experts.

        유리선량계를 이용한 투과선량 기반 환자선량 평가 시스템 개발을 위한 가능성 연구

        방사선치료는 수술, 항암치료와 함께 암의 3대 치료방법으로써 많은 암환자들이 방사선치료를 받게 된다. 최대한 많은 방사선을 암에 집중시키고 최대한 적은 방사선을 주변 정상 조직에 가해주기 위해 치료 전 치료계획을 철저히 세우고 품질 관리를 시행하지만 방사선치료가 잘못 시행되어 의도치 않은 방사선이 환자에게 전달되는 의료사고가 발생하기도 한다. 이를 해결하기 위해 환자 내부의 선량을 검증하기 위한 방법을 투과선량 측정을 통한 환자 내부선량의 역추정 방법이 제시되고 있다. 본 연구에서 제시한 투과선량을 이용한 환자선량 계산 방법을 거리역자승법칙, 심부선량백분율, scatter factor를 이용한 방법으로써 실제 환자 선량 평가 가능성에 대해 균질한 물등가 팬텀을 이용한 연구이다. 투과선량에 대한 이온함과 유리선량계의 교정 결과 유리선량계의 신호값이 이온함으로 측정한 선량값에 비해 6 MV에서 0.824, 10 MV에서 0.736배인 것으로 나타났고 scatter factor는 평균적으로 1.4정도인 것으로 확인되었다. 심부선량백분율 데이터를 사용하기 위해 Mayneord F factor를 적용하였으며 위의 정보들을 이용하여 균질한 팬텀에서 알고리즘을 검증한 결과 최대 오차 약 1.65%로 계산이 정확하게 실시됨을 확인하였다. As radiation therapy is one of three major cancer treatment methods, many cancer patients get radiation therapy. To exposure as much radiation to cancer while normal tissues near tumor get little radiation, medical physicists make a radiotherapy plan treatment and perform quality assurance before patient treatment. Despite these efforts, unintended medical accidents can occur by some errors. In order to solve the problem, patient internal dose reconstruction methods by measuring transit dose are suggested. As feasibility study for development of patient dose verification system, inverse square law, percentage depth dose and scatter factor are used to calculate dose in the water-equivalent homogeneous phantom. As a calibration results of ionization chamber and glass dosimeter to transit radiation, signals of glass dosimeter are 0.824 times at 6 MV and 0.736 times at 10 MV compared to dose measured by ionization chamber. Average scatter factor is 1.4 and Mayneord F factor was used to apply percentage depth dose data. When we verified the algorithm using the water-equivalent homogeneous phantom, maximum error was 1.65%.

        가속도계를 이용한 사장교의 지진거동 계측시스템 개발에 대한 연구

        In this study, a structural health monitoring system for cable-stayed bridges is developed. In the system, condition assessment of the structure is performed based on measured records from seismic accelerometers. Response indices are defined to monitor structural safety and serviceability and derived from the measured acceleration data. The derivation process of the indices is structured to follow the transformation from the raw data to the outcome. The process includes noise filtering, baseline correction, numerical integration, and calculation of relative differences. The system is packed as a condition assessment program, which consists of four major processes of the structural health evaluation: (i) format conversion of the raw data, (ii) noise filtering, (iii) generation of response indices, and (iv) condition evaluation. An example set of limit states is presented to evaluate the structural condition of the test-bed and cable-stayed bridge.

        진동계측자료 기반 안전성평가 시스템을 활용한 건축물의 비틀림 분석 방법 개발

        In this study, algorithms for analyzing the torsion of buildings under earthquake excitation are developed. The algorithm and formulations to account for the torsional angle are verified by analyzing the seismic acceleration time history data. The method was applied to the reference buildings to examine their operation and usability. The reference application demonstrated that the noise-canceling scheme successfully overcame various obstacles in the field measurements. The developed method is expected to be used as a tool to support a loss assessment system for determining the direction and priority of disaster response in the event of an earthquake.

        Poly(trimethylene terephthalate)/Cellulose 교편물의 전처리 및 염색에 관한 연구

        The effect of mercerizing, presetting and dyeing of single jersey poly(trimethylene terephthalate) (PTT)/cellulose knitted fabrics were carried out. Pre-treating, dyeing, and finishing conditions were optimized to make uniform PTT/Cellulose knitted fabrics. The optimum conditions of the PTT/Cellulose knitted fabrics for mercerization were 22.5 wt% NaOH, $20^{\circ}C$ and 23 m/min of tenter speed. A cold-pad-batch (CPB) dyeing method could minimize process problems of high tension and rubbing in comparison with an ordinary exhaust dyeing method. Fastness was the best when the fabric was dyed at temperature of $105^{\circ}C$ and for 40 min. We resolved a tailing problem of the CPB process by controlling tenter speed and temperature.

        ■Background Overall prevalence of visual field loss after stroke was estimated at 31%, ranged from 5.5- 57%. Among them, homonymous hemianopia is the most common symptom of post-stroke visual field loss and leads to a decrease in the quality of life and therapeutic effect. However, standard treatment has not been established. ■Case report A 48-year-old male with right side homonymous hemianopia by a left occipital-parietal lobe hemorrhage was treated with Qu-Ji-Du-Huang-Wan, acupuncture and electroacupuncture for 20 days. We used confrontation visual field exam, automated perimetry and visual analog scale for evaluating symptoms. Following treatment, confrontation visual field exam and the subjective discomfort was improved. ■Conclusions The present case report suggests that combined Korean medicine treatment might be an effective treatment of post-stroke homonymous hemianopia. A number of follow up studies should be conducted to clarify the effectiveness of treatment.

        부직포의 균제도 평가에 관한 연구(I)

        Image analysis method was applied to evaluate the evenness of various nonwoven fabrics. Binder-bonded nonwoven fabrics were used for this research. Images of nonwoven fabrics were scanned and stored into 256 gray scale system. At first, the effects of scanning condition on evaluating the evenness of areal density ($g/m^2$) of nonwoven fabrics were investigated. A coefficient of variation of gray values was used as an evaluation factor. In previous evaluation, the variation means the variance of weight in unit area. In this study, however, it means the variances of gray values obtained from the pixels on the fabric images. This research verified that the coefficient of variation of gray values was proper factor to evaluate the evenness of areal density of nonwoven fabrics.

        한국의 방사선치료 기기 품질관리 현황에 관한 설문 결과

        방사선치료는 수술, 항암치료와 함께 암의 3대 치료방법으로 많은 암환자들이 방사선치료를 받게 된다. 방사선은 눈에 보이지 않아 방사선 치료기계에서 나가는 방사선의 질과 양을 확인하기 쉽지 않고 산란되는 특성 때문에 다른 장기에 2차암이 유도할 수 있고 최근 치료기술의 발달로 치료 방사선에 대한 보다 정밀한 검증이 필요하게 되었다. 따라서 방사선 치료기기에 대한 품질관리를 철저히 해야만 한다. 국제원자력기구, 미국의학물리학회 등 해외에서는 보고서들을 제시하여 각 지역적인 차원에서 방사선 치료기기의 품질관리에 대한 권고를 제시한다. 하지만 국내는 외국에 비해 규모가 작아 각 병원에서 국외의 권고 중 일부를 선택적으로 사용한다. 국내 병원을 대상으로만 한 방사선치료의 품질관리 권고가 존재하지 않지만 국외의 권고들이 갱신되고 있으며 향후 국내 품질관리 권고를 제작하기 위해 국내 품질관리의 현황 역시 조사 및 갱신 될 필요성이 있으므로 본 연구에서는 방사선치료기에 대한 품질관리 설문지를 제작하여 국내 품질관리 경향에 대한 조사를 실시하였다. 초본으로 제작된 설문지를 국내 5개 기관의 의학물리학자에게 배포하여 설문지의 부족한 점을 파악하고 보충하여 최종 설문지를 제작한 후 이를 전국 72개 병원의 방사선 종양학과에 배포하여 조사를 실시하였다. 72개 병원 중 37개 병원에서 요청에 응답해주었으며 이들 중 97.3%의 병원이 선형가속기를 보유하고 있으며 40.54%, 18.92%, 24.32%의 병원이 각각 근접치료장치, 토모치료장치, 방사선수술장치를 보유하고 있는 것으로 나타났다. 이 밖에 품질관리 인력 및 장치, 국내의 환자 품질관리 현황, 각 치료기별 품질관리 현황에 대한 설문이 실시되었으며 이 결과는 향후 표준 품질관리 절차서가 제작될 경우 사용되기에 좋은 자료가 될 것이다. As radiation therapy is one of three major cancer treatment methods, many cancer patients get radiation therapy. Because of the invisible and scattering characteristics of radiation, it is impossible to identify the quality and the amount of radiation and secondary cancer could be induced by scattered radiation. Because of advanced technique of radiation therapy and the reasons mentioned above, quality assurance of radiotherapy machine should be performed completely. International organizations such as International Atomic Energy Agency (IAEA), American Association of Physicists in Medicine (AAPM) suggest report of quality assurance to recommend united method of radiotherapy machine quality assurance. Domestic society of medical physics, however, is too small to make such a report, domestic hospitals selectively choose some of contents in global suggestions. As there are no suggestions for domestic hospitals and global suggestions are being updated, we did a survey about quality assurance for radiotherapy machine. The questionnaire is composed of possession of radiotherapy machine, items performed for quality assurance and manpower, etc. 37 of 72 hospitals answered to survey. These results could be used for making domestic standard quality assurance procedure.

        Spindle Speed Optimization of a Ring Spinning Machine for Better Surface Irregularity and Hairiness of Yarn and Fabric

        Producing yarn from natural fibers without creating irregularities in structure or having fibers protruding from the surface, remains the goal of spinners. This is a problem, as structural irregularities such as hairiness affect subsequent fabric manufacturing processes and the aesthetics of the final fabric. This work therefore focused on investigating the effects of varying the spindle speed of a ring spinning frame on the structure of yarn (i.e., its surface regularity and hairiness), its strength and the surface pilling of fabric made from such yarn with a view to optimizing the spindle speed. For this, yarns with counts of 20, 25, and 30 tex were produced at six different spindle speeds ranging from 11,000 to 21,000 rpm with an interval of 2,000 rpm. All other parameters were kept constant, including the draft for a particular count, the type and weight of the traveler, and the diameter of the ring. The results obtained revealed that as the spindle speed was increased to 17,000 rpm, the yarn structure became more regular and less hairy, thereby becoming stronger. Beyond 17,000 rpm, however, both the regularity and strength decreased, with the hairiness continuing to increase with increasing spindle speed. Consequently, the surface pilling of the fabric was found to be optimized when made from yarns produced at a spindle speed of 17,000 rpm.
