        포공영(蒲公英)이 MC/9 mast cell에서의 Th2 cytokine 발현에 미치는 영향

        1. Objective : The purpose of this study is to investigate the effects of TAM (Taraxacum mongolicum) on Th2 cytokine production in MC/9 mast cells. 2. Methods : The effects of TAM was analyzed by ELISA and Real-time PCR in MC/9 mast cells. Levels of IL-5, IL-13 were measured using enzyme-linked immunosorbent assays(ELISA). mRNA levels of IL-4, IL-5, IL-6, IL-13 were analyzed with Real-time PCR. 3. Results : 1) TAM inhibited the IL-4 production significantly in comparison to PI-control group at concentration of $50{\mu}g/ml$, $100{\mu}g/ml$, $200{\mu}g/ml$. 2) TAM inhibited the IL-13 production significantly in comparison to PI-control group at concentration of $50{\mu}g/ml$, $100{\mu}g/ml$, $200{\mu}g/ml$. 3) TAM inhibited the IL-4 mRNA expression significantly in comparison to PI-control group at concentration of $100{\mu}g/ml$. 4) TAM inhibited the IL-5 mRNA expression significantly in comparison to PI-control group at concentration of $50{\mu}g/ml$, $100{\mu}g/ml$. 5) TAM inhibited the IL-6 mRNA expression significantly in comparison to PI-control group at concentration of $100{\mu}g/ml$. 6) TAM inhibited the IL-13 mRNA expression significantly in comparison to PI-control group at concentration of $100{\mu}g/ml$. 4. Conclusions : These results indicate that TAM (Taraxacum mongolicum) has the effect of decreasing the Th2 cytokine production in the MC/9 mast cell.

        만성 중심장액맥락망막병증에서 절반 에너지를 적용한 광역학치료의 효과

        Purpose: To evaluate the clinical results of half-fluence photodynamic therapy (PDT) using verteporfin in patients with chronic central serous chorioretinopathy (CSC). Methods: The author reviewed the charts of 20 patients (22 eyes) who underwent PDT for chronic central serous chorioretinopathy. All 22 eyes were treated using an irradiance of 300 mW over 83 second (25 J/cm2). Postoperative evaluation included improvements in visual acuity, complications, changes in central macular thickness with the optical coherence tomogram (OCT), and recurrence. Results: The best-corrected visual acuity improved significantly from 0.50±0.32 to 0.12±0.19 logMAR at 3 months (p<0.01), and the mean central macular thickness decreased from 377.7±86.3 μm to 166.2±29.3 μm at 3 months (p<0.01). Exudative serous macular detachments were resolved completely in all eyes within 3 months. There was no recurrence after PDT during the follow-up period. Conclusions: PDT using half fluence appeared to be a beneficial treatment option for patients with chronic CSC. Further studies are needed to verify treatment safety as well as the time and rate of recurrences. 목적: 만성 중심장액맥락망막병증 환자에서 레이저 강도를 반으로 감소시켜 시행한 광역학치료의 효과에 대해 알아보고자 하였다. 대상과 방법: 2007년 8월부터 2008년 10월까지 만성 중심장액맥락망막병증으로 진료하였던 환자 중 절반 에너지를 적용한 광역학치 료를 받고 3개월 이상 경과 관찰이 가능하였던 환자 20명 22안을 대상으로 치료 후 시력, OCT 상의 변화, 합병증 및 재발 여부 등을 후향적으로 분석하였다. 결과: 초진 시 평균 최대교정시력은 LogMAR 단위로 0.50±0.32이었고, 1개월과 3개월 후 각각 0.27±0.27, 0.12±0.19로 유의하게 향상된 결과를 보였다(p<0.01). OCT로 측정한 황반 중심 두께는 치료 전 평균 377.7±86.3 μm에서 치료 후 1개월과 3개월 후 각각 179.0± 45.3 μm, 166.2±29.3 μm로 유의하게 감소하였고(p<0.01), 형광안저촬영에서도 모두 누출이 소실되었다. 결론: 만성 중심장액맥락망막병증에서 레이저 에너지 세기를 반으로 줄인 광역학치료는 단기간 동안 감각신경망막박리의 감소와 시력의 향상에 효과적임을 알 수 있으나 장기적 예후에 대해서는 앞으로 더 연구되어야 할 것으로 생각된다.

        약물로 반응하지 않아 섬유주절제술을 시행한 급성 폐쇄각녹내장 환자의 수술성공률 분석

        Purpose: To evaluate the outcomes of trabeculectomy performed on eyes with medically uncontrolled acute angle closure glaucoma (AACG). Methods: The authors reviewed 31 eyes of 30 acute primary angle-closure glaucoma patients who had undergone trabeculectomy. The eyes were divided into two groups: eyes which did not respond to medical and/or laser treatment (Group A, 16 eyes) and those which initially responded to medical and/or laser treatment and later had an intraocular pressure (IOP) increase (Group B, 15 eyes). Complete success was defined as a final IOP below 21 mmHg without medication, and qualified success was defined as a final IOP below 21 mmHg with medication. Patients whose postoperative IOP was 22 mmHg or greater on at least two serial measurements or who required additional glaucoma surgery were classified as failures. Results: The mean follow-up was 43.1±32.3 months. The five-year qualified success rate was 56.3% in Group A and 100% in Group B (log-rank test, p=0.02). The preoperative IOP was significantly higher in Group A (38.0±11.5 mmHg) than in Group B (21.8±12.2 mmHg) (p <0.01). Factors found not to be significant included age, gender, peripheral anterior synechiae, a glaucomatous optic disc, and visual field damage. The postoperative complications were similar between the two groups (p >0.05). Conclusions: The medically uncontrolled AACG appears to have an adverse effect on the outcome of trabeculectomy. Success of trabeculectomy in AACG seems to be inversely related with elevated preoperative intraocular pressure. 목적: 약물로 안압이 조절되지 않는 급성폐쇄각녹내장의 섬유주절제술의 성공률을 알아보고자 하였다. 대상과 방법: 약물이나 레이저 치료로 안압이 조절되지 않아 수술을 받은 급성폐쇄각녹내장 16안(A군)과 안압이 조절되었다가 다시 상승하여 수술을 받은 급성폐쇄각녹내장 15안(B군)을 후향적으로 분석하였다. 완전성공은 약물사용 없이 안압이 21 mmHg 이하일 때, 부분성공은 약물사용으로 안압이 21 mmHg 이하일 때, 실패는 약물사용으로도 2회 연속 안압이 22 mmHg 이상, 또는 추가 수술이 필요한 경우로 정의하였다. 결과: 5년간 부분 성공률은 A군 56.3%, B군 100%로 B군이 높은 성공률을 보였다(p=0.02). 수술 직전 안압은 A군 38.0±11.5 mmHg, B군 21.8±12.2 mmHg로 A군이 높았다(p <0.01). 나이, 성별, 주변홍채앞유착 정도, 녹내장성 시신경 변화/시야결손 등의 수술 전 인자와 술 후 합병증은 차이가 없었다. 결론: 급성폐쇄각녹내장에서 섬유주절제술은 약물이나 레이저 치료에 반응하지 않는 경우가 안압이 조절되었다가 다시 상승하여 수술 을 받은 경우보다 성공률이 낮았고, 수술 직전의 안압이 수술 성공률의 관련 인자로 분석되었다.

        Inhibitory effects of Taraxacum mongolicum with phreatic water on melanin synthesis

        Background: Recently, people have begun showing heightened interest in skin whitening. Melanin is an important factor that determines skin color. The purpose of this study is to investigate the inhibitory effect of Taraxacum mongolicum (TAM) with phreatic water (PW) from Dogo Hot Springs on melanin synthesis. Methods: We assessed the inhibitory effects of TAM on melanin synthesis in B16F10 mouse melanoma cells. The mRNA levels of tyrosinase related protein (TRP)-1, TRP-2, tyrosinase, MITF, ERK, and PKA protein were analyzed with reverse transcription polymerase chain reaction and Western blot analysis. We also assessed the inhibitory effects of TAM with PW on melanin synthesis in HRM-2 melanin-possessing hairless mice. After UVB irradiation, differences in melanin were analyzed with an image analysis software between the left dorsal skin (untreated part) and the right dorsal skin (treated part). The mRNA levels of TRP-1, TRP-2, and matrix metalloproteinase (MMP)-9 were analyzed with real-time quantitative polymerase chain reaction. The dorsal skins were analyzed with histological test by hematoxylin and eosin staining. Results: TAM inhibited the TRP-1, TRP-2, tyrosinase, MITF mRNA gene expression, and PKA protein expression on the concentration-dependent B16F10 cell. Moreover, TAM increased the ERK mRNA gene expression in the B16F10 cell. After UVB irradiation, TAM with PW increased the differences in melanin between the left dorsal skin (untreated part) and the right dorsal skin (treated part) in HRM-2 mice. TAM with PW inhibited the TRP-1, TRP-2, and MMP-9 mRNA gene expression in HRM-2 mice. TAM with PW decreased the epidermal thickness, around the cell deformation, keratinization, and infiltration in HRM-2 mice. Conclusion: These results indicate that TAM with PW has the inhibitory effect of decreasing the melanin synthesis.

        혈관성 치매 환자 치험 3례

        1. Objectives: This study is about three vascular dementia patients with disorientation, memory impairment and cognitive disorder. In this study, we evaluated the effects of Sasang constitutional treatment and suggested using the Mini Mental Status Examination - Korea (MMSE-K), Global Deterioration Scale (GDS). 2. Methods: These three patients were treated by Sasang constitutional herbal medications based on "Donguisusebowon". Acupuncture and west medications were limited as much as possible. We evaluated the symptoms through the Mini Mental Status Examination - Korea (MMSE-K), GDS (Global Deterioration Scale). 3. Results and Conclusions: Disorientation, memory impairment and cognitive disorder that were these three patients chief complaints were improved. This case study shows that Sasang constitutional herbal medications are an effective treatment for vascular dementia patients.
