        외인성 췌장부전은 췌장에서 분비되는 소화효소의 부족한 합성 및 분비에 기인한다. 대부분의 개체에서 보이는 임상증상은 체중감소, 다량의 연변, 지방변이다. 혈청 고양이 유사트립신 면역활성 농도검사는 외인성 췌장부전 진단에 있어 매우 유용한 검사법이다. 외인성 췌장부전 환자의 치료는 췌장 소화효소의 공급이다. 대부분의 외인성 췌장부전 환자에서 심각한 혈청 코발라민의 감소와 동반되기 때문에 코발라민의 투여는 반드시 필요하다. 마지막으로 개에 비해 흔하게 확인되진 않지만, 만성적인 연변 및 체중감소를 보이는 환자에서는 진단에 있어 외인성 췌장부전에 대한 고려가 필요하다.

        보은지역 흑색셰일 분포지역에서의 암석-토양-식물계내 잠재적 독성원소들의 분산과 이동

        This study had three purposes: (1) to investigate the enrichment levels and dispersion patterns of potentially toxic elements in the rock-soil-plant system; (2) to evaluate the uptake ratios of heavy metals from soils into plants and (3) to assess the chemical speciation of heavy metals in soils. Rock, surface soil and plant samples were collected in the Boeun area underlain by black shales of the Okchon Zone. These samples were analyzed for multi-elements using INAA, ICP-AES and AAS. The maximum abundance of U in black shales is 16 mg/kg and radioactivity counts up to 300 cpm. In particular, Mo, V, Ba, Cd, Pb and U are enriched in black shales. Most of soils derived from black shales show high concentrations of U, As, Mo, Ba, Cu, Cd, Pb, Zn and mean concentrations of As and Mo in soils (20 mg/kg of As and 6.6 mg/kg of Mo) are higher than the permissible level suggested by Kloke (1979). Enrichment index values of soils are calculated and higher than 1.0 in the black shale area with the highest value of 6.4. Mean concentration of Cd in plants is higher than those of Cu, Pb and Zn. The concentration of Cd in plant species decreases in the order of Chinese cabbage > red pepper > soybean=sesame > rice stalk > com > rice grain. The biological absorption coefficients (BAC) in plants are in the order of Cd > Zn=Cu > Pb, which suggests that Cd is more bioavailable to plants than Cu, Pb and Zn. From the results of sequential extraction analysis of soils, relatively high proportion of Cu, Pb and Zn are present as residual fractions whereas that of Cd as non-residual fractions. Cadmuim occurs predominantly as exchangeable/water-acid soluble phase in soils, and Cd is more mobile and bioavailable than Cu, Pb and Zn.

        폐금속광산지역 중금속의 위해성 평가

        본 연구에서는 폐금속광산 지역에서 중금속이 인체에 미치는 위해영향을 정량적으로 평가하기 위하여 3개 금속광산 주변 지역에서 농경지 토양, 농작물 및 지하수를 채취하여 화학분석을 실시하였다. 이들 폐금속광산 주변에 야적된 광산폐기물(광미)은 바람이나 집중 강우에 의해 광미댐 하부에 있는 농경지나 하천으로 유입되어 주변 환경을 오염시킬 가능성이 크다. 따라서 폐금속광산에서의 인체노출경로는 농사활동을 통한 토양의 섭취, 지하수(식수)의 섭취, 쌀의 섭취, 농사활동에 의한 토양의 피부접촉 등 4가지로 파악할 수 있었다. 각 노출경로별 비발암성위해도 평가 결과, 모든 광산에서 쌀의 섭취를 통한 중금속의 독성위해도가 가장 높게 나타났다. 광산별 4가지의 인체노출경로를 총합한 HI 지수가 3개 광산 모두 9.0 이상을 나타내었으며, 특히 B 광산이 HI 29.2로 중금속에 의한 독성위해도가 가장 높은 것으로 판명되었다. 비소에 대한 발암위해도 평가 결과, 3개의 폐금속광산 지역에서 쌀 섭취의 노출경로를 통한 비소에 의해 암이 발생할 확률이 천명중의 1명 이상으로 높게 나타났다. 이는 환경부 토양오염 위해성평가지침(안)과 미국 EPA에서 제시한 허용가능한 초과발암위해도보다도 크므로 이들 지역 주민들이 비소에 의해 오염된 농작물(쌀)을 지속적으로 장기간 섭취하게 된다면 비소가 건강에 미치는 위해영향이 크다고 판단된다. An environmental survey from three abandoned metal mine areas was undertaken on to assess the risk of adverse health effects on human exposure to heavy metals influenced by past mining activities. Tailings contained high concentrations of heavy metals may have a impact on soils and waters around the tailing piles. In order to perform the human risk assessment, chemical analysis data of soils, rice grains and waters for As, Cd, Cu and Pb have been used. The HQ values for heavy metals via the rice consumption were significantly higher compared with other exposure pathways in all metal mine areas. The resulting HI values in three mine areas were higher than 9.0, and their toxic risk due to rice ingestion was strong in these mine areas. The cancer risk of being exposed to As by the rice consumption from the A, B and C mine areas was $5.1\;{\times}\;10^{-3}$, $6.8\;{\times}\;10^{-3}$ and $3.1\;{\times}\;10^{-3}$, respectively. The As cancer risk via the exposure pathway of rice ingestion from these mine areas exceeds the acceptable risk of 1 in 10,000 set for regulatory purposes. Thus, the daily intakes of rice by the local residents from these mine areas can pose a potential health threat if exposed by long-term As exposure.

        항암화학요법과의 순차적, 그리고 병행적 요법으로서의 aRVS 투여로 장기 생존과 좋은 삶의 질을 유지한 전이성 비소세포폐암 증례보고

        This report documents a case in which the administration of an herbal product, an extract of the lacquer tree, Rhus verniciflua Stokes, as sequential and concurrent treatment with chemotherapy was associated with a long term survival and good quality of life in a patient with metastatic non-small cell lung cancer(NSCLC). A 51-year-old Korean female was referred to the $M{\cdot}{\mu}$ Integrative Cancer Center, East-West Neo Medical centrer, Kyung Hee University for stage IV, metastatic NSCLC. She was treated with aRVS alone for 19 months and then received 1st line paclitaxel/carboplatin combined with aRVS, 2nd line gefitinib, and 3rd line pemetrexed. The number of cycles of pemetrexed administered was seventeen. aRVS was restarted as the 13th pemetrexed was administered. Pemetrexed with aRVS is currently ongoing. This patient has been alive for 41 months, and has been keeping a good performance status so far. We suggest aRVS as sequential and concurrent treatment with chemotherapy is an effective alternative treatment strategy.

      • MMR 백신 부작용 발생 실태 - 보건소 전화 조사를 통한 전향적 연구 -

        Purpose : Adverse events(AE) associated with vaccination have not been systematically monitored in Korea. But since 1994, several deaths after inactivated mouse brain derived Japanese encephalitis vaccine injection arouse the safety problem of immunization in the public, and a evaluation of vaccination program including the effectiveness and safety problem had been started. We have been using MMR vaccine containing Japanese mumps vaccine strains, which are known to be associated with the high rate of adverse events including aseptic meningitis, for the last decade. Methods : We conducted a prospective study of vaccine adverse events associated with MMR vaccine through a tracking system using telephone calls. We followed up 7,594 cases of MMR vaccinees. Results : Reported adverse events included fever, couvulsion, parotitis, and aseptic meningitis. Nine cases of parotitis and seven cases of aseptic meningitis were recognized during follow up period. The incidence of both parotitis and aeptic meningitis was $85.1/10^5$, for Urabe mumps strain. For Hoshino strain, the incidence of parotitis and aseptic meningitis was $158.0/10^5$ and $94.8/10^5$, respectively. Conclusions : The incidence of adverse events was to be a higher than that of natural infection in Korea and was comparable to the results of studies done in other countries. 목 적 : 일본산 볼거리 백신주를 포함한 MMR 백신의 부작용을 알아보기 위하여 전화 조사라는 전향적인 방법을 통해 백신 부작용의 양상과 빈도, 부작용이 백신주 별로 차이가 있는지 알아보고자 하였다. 방 법 : 보건소 전화 조사를 통한 전향적 방법으로, 접종 후 2주, 4주에 백신 접종자들에게 전화로 부작용 여부를 조사하고 증상이 있을 때에 PCR이나 배양 검사를 통해 확인하였다. 결 과 : 발열 반응은 2차 조사에서 Urabe주 3.9%, Hoshino주 4.6%, Rubini주 0.3%로 백신주에 따라 유의한 차이를 보였다. 경련은 5례로 0.07%였으나 Rubini주 접종자 수가 적어 비교가 어려웠고 이하선염은 8례로 0.12%였으며 백신주간에 차이는 없었으나 1997년의 자연 발생률보다 높았다. 뇌수막염이 의심되었던 7례 중 1례에서 검사가 가능하였고 뇌척수액에서 볼거리 바이러스를 확인하였다. MMR 백신 접종자에서 임상 증상 기준으로 진단된 무균성 뇌수막염 발생률은 1998년 바이러스성 뇌막염 환자 발생률 보다 높았다. 결 론 : 전향적 연구 결과 MMR 백신 접종 후의 부작용으로는 무균성 뇌막염, 이하선염, 경련, 발열 등이 조사되었고 현재 사용되고 있는 일본산 MMR 백신에 함유되어 있는 볼거리주는 외국에서의 연구결과와 동일한 이하선염 및 무균성 뇌막염을 일으킬 생물학적 가능성을 지니고 있으므로 국내에서 MMR 백신 접종 후 상당수에서 이하선염이나 무균성 뇌막염 환자가 발생하였을 가능성이 있을 것으로 사료되나 부작용 발생 보고 체계를 갖추고 있지 않은 관계로 실제 부작용 발생 실태를 파악하지 못하고 있는 것으로 사료된다.

        전이성 유두상 갑상선암 환자를 대상으로 투여한 알러젠 제거 옻나무 추출물의 종양 소퇴 1례

        Papillary thyroid carcinoma is the most common type of thyroid malignancy. It has better prognosis and rarer incidence of distant metastasis than other types of thyroid malignancy. However, once distant metastasis happens, its result will be bad. They mostly spread to the lungs and bones. and rarely to the brain. We report a patient with a papillary thyroid carcinoma metastasis to the lymph nodes. the ribs. the brain, and the lungs. The patient underwent total thyroidectomy, radioiodine ablation, rib partial resection, and lung wedge resection. However, upon progress of metastasis. the patient insisted on being treated with traditional Korean medical treatment using allergen-removed Rhus Verniciflua Stokes (A-RVS) instead of the conventional treatment including chemotherapy. The size of the tumor partly decreased, and the patient showed no side effects. This case report brings forth the importance of a thorough study in papillary thyroid carcinomas and their metastasis from the traditional Korean medical point of view, along with the long-term effect of allergen-removed Rhus Verniciflua Stokes (A-RVS).

        충주지역 흑색셰일 분포지역에서의 잠재적 독성원소들의 분산과 부화

        This study had three purposes: (1) to investigate dispersion and enrichment level of potentially toxic elements; (2) to identify uranium-bearing minerals in black shales; and (3) to assess the chemical speciation of heavy metals in soils and sediments. Rock, surface soil and stream sediment samples were collected in the Chungjoo area covered with black shales in Korea. These samples were analyzed for multi-elements using INAA and ICP-AES. The maximum abundance of U in black shales is 56 ppm and radioactivity counts up to 240CPM. Molybdenum, V, Ba, Cu, and Pb are enriched in black shales and most of soils show high concentrations of U, Mo, Ba, Cu, Pb and Zn. Concentrations of potentially toxic elements decrease in the order of mountain soil > farmland soil > paddy soil. Enrichment index of soils and sediments are calculated and higher than 1.0 in the black shale area with the highest value of 6.1. In order to identify U-bearing minerals, electron probe micro analysis was applied, and uraninite and brannerite in black shale were found. Uraninite grains are closely associated with monazite or pyrite with the size of $2{\mu}m$ to $10{\mu}m$ in diameter whereas brannerite occurs as $50{\mu}m$ euhedral grains. With the results of sequential extraction scheme, residual fractions of Cu, Pb and Zn in soils are mainly derived from weathering of black shale but Cu, Pb and Zn in sediments are present as non-residual fractions. Lead is predominantly present as oxidizable phase in soils whereas Zn is in exchageable/water-acid soluble phase in sediments.
