항암제 사용 환아에서 수두에 대한 Acyclovir 치료 효과

수두는 면역 기능이 저하된 소아에서는 전신성 감염과 사망까지 초래하는 질환으로 알려져 있다. 최근에 Aryclovir가 효과적인 약물로 사용되고 있다. 본원 소아과에서 면역 기능이 저하된 9명의 환아를 대상으로 Acyclovir를 사용하여 만족할만한 결과를 얻었기에 이를 문헌 고찰과 함께 보고하는 바이다. Primary varicella infection has usually a benign clinical course in the normal healthy population, but in the immunocompromised patients it may frequently progress to visceral dissemination and even to death. Since acyclovir is recently known to be an effective antiviral agent to chickenpox, we performed a clinical study to evaluate the effect of acyclovir therapy on the chickenpox in the immunocompromised patients. Nice immunocompromised patients wish chickenpox were treated wish intravenous administration of acyclovir 30-60 mg/kg daily for five to eight days. Eight patients entered to the study within 72 hours after onset of skin lesion. One of them had abnormal liver function at entry and another one had pneumonia as a complication during treatment. Only one patient of nine, to whom acyclovir treatment was delayed until five darts after onset of skin lesion, died of extensive dissemination of the disease. The duration of vesicle formation was shorter among acyclovir recepients on the first day of onset than on the third day of onset(p<0.05), but no difference was found on the duration of fever between two groups(p>0.2). No clinical or laboratory adverse reactions were found during acyclovir therapy. These data suggest that acyclovir is effective on the treatment of chickenpox in the immunocompromised patients when it is given early in the course of the infection.

A case of a 9-year-old boy was presented with clinical and laboratory findings strongly suggestive of acute poststreptococcal glomerulonephritis but with no abnormalities in the routine urinalysis which were performed repeatedly during his hospitalization and follow up course. Diagnosis was confirmed by light, immunofluorescent and electron microscopic findings of the renal tissue obtained by percutaneous renal biopsy. A brief review of related literatures was also presented.
