예지성 있는 대구치부 임플란트을 위해서는 Crestal bone Stabliity를 위해 임플란트 주위에 biologic width라 할 수 있는 적절한 두께의 연조직이 필요하다. 그러기 위해선 기존에 우리가 적절하다고 혹은 어쩔 수 없는 한계라고 생각했던 깊이보다 좀 더 치조골 하방으로 깊이 심어야 하는 경우가 많은 것 같다. 이번 투고에서는 case report와 함께 관련된 surgical technique에 대해서 소개하면서 구치부 임플란트의 수직적 위치에 대해서 생각보고자 한다. For predictable posterior implants, appropriate soft tissue thickness, called the biologic width, is required around the implant for crestal bone stability. In order to do so, it seems that there are many cases where the implant should be positioned deeper than the depth that we previously thought was appropriate or inevitable limit. I would like to share my clinical experience about the vertical position of the posterior implant with the case reports and the related surgical technique.
KTX-산천 살사장치 서포트 브래킷 설계변경을 통한 강도 개선

살사장치 서포트 브래킷은 KTX-산천 선두 동력대차의 액슬박스에 조립되는 부품으로 매우 가혹한 환경을 견디어야 하는 철도차량 부품 중 하나이다. 기존에 사용되었던 브래킷(용접구조 타입)은 운행 도중 용접부에서 크랙이 발생하여 설계 변경이 요구되었다. 가혹한 환경 및 제작상의 오류를 최소화 할 수 있도록 설계 단계에서부터의 재검토를 수행하였으며, 수차례의 시행착오를 거치며 최적설계안을 도출하였다. 설계 변경 과정에서 중량 최소화를 위하여 재질 변경을 고려하였으며, 형상 및 재질, 중량 등을 모두 고려한 최적 설계안을 완성하였다. 도출된 설계안은 해석적 검토를 통하여 강도 개선 효과를 확인하였으며, IEC 61373 의 "Category 3" 에 해당하는 진동 시험 조건을 만족함을 시험을 통해 입증하였다. 변경된 설계안을 적용하여 제작된 살사장치 서포트 브래킷은 현재 KTX-산천 차량 5개 편성에 조립되어 성능을 검증 중에 있다. Support Bracket for Sanding Device is one of the parts of trainsets endure the most harsh environment. The bracket (welded structure that was used in the existing type) cracked in the weld zone during operation and design changes were required. From the design stage of the review was performed to minimize the harsh environment and production errors, and derive the optimum design proposal, subject to a number of trial-and-error. Were considered a material change, design changes in order to minimize weight in the process, considering both the shape and material, the weight of the optimal design proposal was completed. The derived design was determined the effect of the strength improvement through the review, proved the vibration test ‘Category 3’ corresponding to the IEC 61373. Support Brackets manufactured with the derived design are monitoring ongoing verification after application for 5 trainsets of KTX-Sancheon.

Purpose: The treatment of gingival recessions is needed to reduce root sensitivity and improve esthetical satisfaction. Several surgical techniques have been proposed to achieve these goals. The use of connective tissue grafts has made esthetic root coverage a predictable procedure. Numerous clinical studies have represented that using connective tissue grafts to cover exposed root surface showed high success rates. This is a case report which demonstrates the technique to obtain root coverage of a buccal recession defect. Materials and Methods: A 35-year-old patient with a high level of oral hygiene was selected for the study. This patient had one Class I Miller recession defect in the mandible. A coronally advanced flap in combination with the connective tissue graft was chosen for the treatment. After surgery, the patient was told to visit the hospital once a week for his oral management and professional prophylaxis. The depth of initial recession was 4.0 mm. Result: After three months, it reduced to 0.0 mm, and the average recession reduction was 4.0 mm. The average root coverage was 100%. Conclusion: The connective tissue graft is both effective and predictable way to produce root coverage in increasing the width of CAL and KT of various adjacent gingival recessions.
