S-268 : 위장관기질종양 환자에서 imatinib 사용 중 발생한 폐결핵 1예

서론: Imatinib은 BCR-ABL tyrosine kinase gene의 선택적 억제제로 현재 만성골수성백혈병(chronic myeloid leukemia, CML)과 c-kit mutation이 양성인 위장관기질종양(gastrointestinal stromal tumor, GIST)에서 수술적 절제가 불가능하거나 전이가 동반되어 있는 경우에서 표준 치료제이다. Imatinib은 T-세포매개성면역반응(T-cell mediated immune responese)을 저해하여 기회감염을 증가시키는 것으로 알려져 있다. 해외에서는 imatinib을 투여받는 환자에서 복막결핵 및 폐결핵이 발생한 증례가 보고되어 있으나 국내에는 아직 보고된 바가 없다. 이에 저자들은 위장관기질종양 환자에서 imatinib 사용 중 발생한 폐결핵 1예를 보고하는 바이다. 증례: 51세 남자 환자는 30년갑의 흡연력이 있으며, 5년전 위에 발생한 위장관기질종양으로 쐐기절제술(wedge resection)을 시행받았다. 경과 관찰 중에 16개월 전 발생한 제주위동통(periumbilical pain)으로 복부전산화단층촬영 및 양전자방출단층촬영을 시행하였고 좌측 횡경막하공간과 췌장미부를 침범한 다발성 위장관기질종양 재발이 의심되어 조직검사를 시행하였다. 조직검사상 위장관기질종양의 재발이 확인되어, 16개월 전부터 imatinib 400 mg을 하루 1회씩 지속적으로 복용하고 있었다. 4개월 전부터는 발열, 오한, 기침, 야간발한 및 체중감소 같은 증상은 없었으나 미색의 가래가 증가하여 단순흉부촬영 및 고해상컴퓨터단층촬영을 시행하였고 좌측상엽에 폐렴소견이 확인되어 항생제 투여를 시작하였다. 객담 항산균도말검사에서는 음성이었으나 4주 뒤 객담 항산균배양검사에서 결핵균(mycobacterium tuberculosis)이 확인되었다. Rifampicin은 CYP3A4 유도 약물로 imatinib과 병용시 혈중 imatinib의 농도를 저하시킬 수 있어서 moxifloxacin으로 대체하여 isoniazid, ethambutol, pyrazinamide와 함께 병용하여 항결핵제를 복용중이고 경과 관찰 중이다. 결론: Imatinib은 최근 연구결과에 의하면 T-세포매개성면역반응을 저해하여 면역저하를 유발할 수 있는 것으로 알려져 있다. 특히 결핵 유병률이 높은 국내에서는 imatinib을 투여받는 위장관기질종양 환자뿐만 아니라 만성골수성백혈병 환자에서도 결핵감염 가능성을 항상 염두해두어야 하겠다.

Podostroma cornu-damae is a rare species of fungus belonging to the Hyocreaceae family. Its fruit body is highly toxic, as it contains trichothecene mycotoxins. Unfortunately,it highly resembles Ganoderma lucidum and Cordyceps,well-known health foods; this can lead to poisoning. We experienced such a case of a 42-year old man who received mushroom poisoning by injesting Podostroma cornu-damae. The patient was presented with severe pancytopenia and infection. The patient recovered without any complications after conservative care, antibiotics therapy, and granulocyte colony stimulating factor administration. The most common complications of podostroma cornu-damae intoxication were reported pancytopenia, infection, disseminated intravascular coagulation, acute renal failure, etc. It is important to provide enough fluid therapy, use of antibiotics to infection and granulocyte colony stimulating factor administration.

A separator plays an important role to achieve good battery performance with stable cycle life, high energy density, high rate capability, and safety in lithium ion batteries (LIBs). Currently, polyolefin microporous membranes are commonly used as the separator in commercial LIBs due to their good chemical stability and high mechanical strength. However, some of their intrinsic properties, such as low porosity and poor wettability with polar liquid electrolyte, which affect cell resistance, energy density, and rate capability of LIB, should be improved. One of the effective approaches to overcome these limitations is to fabricate a separator using a thermally stable polymer by electrospinning technique. In this work, we have prepared polyacrylonitrile (PAN) nanofibrous membranes by electrospinning technique and those were converted to partially oxidized PAN (oxy-PAN) by heating at 230℃ in air for different time periods. Thus prepared oxy-PAN membranes showed good electrolyte wettability, high ionic conductivity, uniform tensile strength, excellent thermal stability and stable cycle performance in LIBs.
이중사출 성형을 위한 저온 경화 액상실리콘고무 (LSR)의 경화 거동 분석

In multi-material injection molding, since two or more materials with different process conditions are used, it is essential to maximize process efficiency by operating the cooling or heating system to a minimum. In this study, Liquid silicone rubber (LSR) that can be cured at a low temperature suitable for the multi-material injection molding was selected and the cure behavior according to the process conditions was analyzed through differential scanning calorimetry (DSC). Dynamic measurement results of DSC with different heating rate were obtained, and through this, the total heat of reaction when the LSR was completely cured was calculated. Isothermal measurement results of DSC were derived for 60 minutes at each temperature from 80 ° C to 110 ° C at 10 ° C intervals, and the final degree of cure at each temperature was calculated based on the total heat of reaction identified from the Dynamic DSC measurement results. As the result, it was found that when the temperature is lowered, the curing start time and the time required for the curing reaction increase, but at a temperature of 90 ° C or higher, LSR can secure a degree of cure of 80% or more. However, at 80 ° C., it was found that not only had a relatively low degree of curing of about 60%, but also significantly increased the curing start time. In addition, in the case of 110 ° C, the parameters were derived from experimental result using the Kamal kinetic model.
S-RIM을 이용한 Glass Fiber Chopped Strand Mat 강화 p-DCPD 복합재료 제작 및 수치해석을 통한 공정 시간 예측

Dicyclopentadiene is a low viscosity resin which forms a poly-dicyclopentadiene rapidly through ring opening metathesis polymerization (ROMP). This poly-dicyclopentadiene has outstanding properties of low-temperature, water and impact resistances. Due to these advantages, military and offshore structures try to apply the DCPD composites by using liquid composite molding process. In this study, 14%, 38% volume fraction fiber glass strand mat reinforced p-DCPD composites processed by structural reaction injection molding (S-RIM) which has resin-catalsyt mixing head and glass fiber preform in the mold. Additionally, S-RIM numerical analysis was conducted to predict the process time depending on fiber volume fraction and mold temperature. The process time is shorter when it has the lower fiber volume fraction or the higher mold temperature. At higher mold temperature, it is necessary to set the maximum mold temperature considering the resin curing time.

로봇암의 회전 토크를 높이기 위해 사용되는 감속기는 모터, 모션 제어기와 함께 산업용 협동 로봇의 3 대 핵심 부품으로써 로봇의 성능을 결정하는 하나의 중요한 요소이다. 따라서 감속기의 신뢰성은 로봇의 수명을 결정하는 핵심 요소이며 전세계적으로 유명한 선진사 제품의 감속기는 30,000 시간 이상의 내구성을 보증한다. 본 연구에서는 선진사의 감속기 내구 수명을 분석하여 향후 국산 제품의 내구 수명을 결정하는 각 파라미터의 기준을 제시하기 위해 가속계수를 적용한 벤치 테스트를 통해 감속기 표면에서 발생하는 온도와 시험 종료 후 초기 감속비 유지 여부를 분석하였다. 가속계수는 Weibull분포에 기반한 평균 무고장 시간을 산출하고, 하모닉 드라이브의 허용 토크를 고려하여 산출되었으며 시험 결과 감속비의 오차는 5% 미만, 표면 온도는 안정구간에서 60℃를 유지하였다. Harmonic reducer used to enhance the torque of robot arm is one of the core part applied to collaborative industrial robot, and therefore the reliability of this part is crucial to determine the robot performance. In this study, reduction ratio and surface temperature was suggested as parameters to define the functional failure in reliability perspective. The harmonic reducer produced by leading company was studied experimentally by accelerated life testing technique for analyzing the suggested parameters. Acceleration factor was obtained by mean time between failure (MTBF) based on Weibull distribution with consideration of allowable torque for harmonic drive. It was noted that the discrepancy of the reduction ratio is less than 5% and the temperature is maintained at 60℃ in the steady state. Consequently, this result can be used in new reducer development as reliability criteria.
