        한국인 정상 ABO 및 아형의 ABO 대립유전자의 전체염기서열분석

        Background:Complete sequencing, except for intron 1, of the ABO allele in some populations has been reported. However, so far, one report on complete sequencing of the ABO gene in three Korean families with normal ABO phenotypes has been published. This study aimed to establish a reference database of common ABO alleles in Koreans. Methods:Screening of common ABO alleles, including homozygote form, was performed by direct sequencing of exons 6 and 7 and by real-time PCR using displacing probes in 95 healthy donors. Genomic DNA from the common ABO group (n=8) and some ABO subgroups (n=7) was used in complete sequencing (except for intron 1) of the ABO allele. Results:The sequences of B101/B101 (n=1), O01/O01 (n=1) were identical with the corresponding sequences registered in Genebank. A102 and A105 had a common point mutation, 1142 C>T in intron 4. A102 (n=3/11) and O02 (n=3/3), selected by sequencing of exons 6 and 7, were reclassified into A105 and O65 by whole genomic sequencing, respectively. Analytic results for ABO subgroups were as follows: B3, B101/O01 (n=3) and B101/O02 (n=1); A1B3, A102/B101 (n=1) and A105/B101 (n=1); Ax, A102/O01 (n=1). Conclusion:We established a reference database of common ABO alleles in Koreans and found that the molecular basis of introns of ABO alleles in the Korean population differs from that reported in previous studies of other populations.

        Seroepidemiology of Toxocariasis and Its Clinical Implications in Gwangju and Jeonnam-province, Korea

        We investigated the seroepidemiological, clinical, and laboratory characteristics of patients suspected to have toxocariasis in Gwangju and Jeonnam-province, Korea. In total, 228 specimens were analyzed for anti-Toxocara canis IgG at two university hospitals from 2010 to 2012. The overall seropositive rate was 67.1%, and the seropositive rates among the eosinophilic and non-eosinophilic groups were 76.1% (105/138) and 53.3% (48/90), respectively. Risk factors for eosinophilia and toxocariasis were male sex (odds ratios [OR]=2.632 and 3.477, respectively) and a history of ingesting raw meat (OR=2.884 and 3.274, respectively), especially raw cow liver (OR=2.089 and 10.038, respectively). T. canis seropositivity (OR=5.807, P=0.004) and a history of consuming raw cow liver (OR=2.766, P=0.052) were risk factors for organ involvement. The anti-T. canis IgG level showed weakly positive correlations with eosinophil counts (r=0.234, P<0.001) and the duration of eosinophilia (r=0.155, P=0.019). Although limited to the regions of Gwangju and Jeonnam-province, this study supports the opinion that toxocariasis is a reasonable focus as a cause of eosinophilia and that it is also associated with organ involvement.

        Molecular Identification of Schizophyllum commune as a Cause of Allergic Fungal Sinusitis

        Schizophyllum commune, a basidiomycetous fungus, rarely causes disease in humans.We report a rare case of allergic fungal sinusitis caused by S. commune in a 14-yr-old girl. The patient presented with nasal obstruction and a purulent nasal discharge. Materials obtained during endoscopic surgery of the frontal recess revealed allergic mucin and a few fungal hyphae. A potato dextrose agar (PDA) culture from the allergic mucin yielded a rapidly growing white woolly mold. Although no distinctive features including hyphae bearing spicules or a clamp connection were present, the case isolate disclosed compatible mycological features including growth at 37˚C, susceptibility to cycloheximide, and production of a tart and disagreeable smell. S. commune was confirmed by sequence analysis of the internal transcribed spacer region and D1/D2 regions of the 26S ribosomal DNA. We believe this is the first report of allergic fungal sinusitis caused by S. commune in Korea. Moreover, this report highlights the value of gene sequencing as an identification tool for non-sporulating isolates of S. commune.

        Evaluation of Modified Formalin-Ether Concentration Method Using Para Tube in Clinical Settings

        Conventional formalin-ether concentration method is a gold standard for the diagnosis of parasite infection. However, it may be time-consuming and laborious. We aimed to reveal the clinical usefulness of a modified formalin-ether concentration method using the Para Tube (KS Corporation, Korea) compared with the conventional method. A total of 117 fresh, unpreserved fecal samples composed to 90 negative controls and 27 positive controls with ova of Diphyllobothrium latum/D. nihonkaiense, ova of Clonorchis sinensis and cysts of Giardia lamblia were used in this study. Both methods showed comparable correct identification rate (87.2% for the Para Tube vs. 86.3% for the conventional method).When five samples were examined at once, the Para Tube method reduced the procedure time compared with the conventional method (19 min 58 sec vs. 23 min 18 sec, P=0.0286). We concluded that the modified formalin-ether concentration method using the Para Tube is a rapid, simple, and reliable fecal concentration method for clinical use.

        Cis-AB 가계에서 발견된 새로운 O02 변이형 - 한 가족 내 3 증례 보고

        In preoperative ABO typing, we observed an ABO discrepancy in a 61 year-old patient with A2B3 phenotype. Standard serologic tests for the ABO blood group phenotypes and ABO gene direct sequencing for the exons 6 and 7 were done as part of the patient’s family study. We found that the patient’s genotype was identical to cis-AB01/O02, except for a 703 G>A polymorphism at exon 7. An allele-separation by cloning and subsequent sequencing of the heterozygote was carried out. We identified a novel O02 variant allele characterized by a 703 G>A polymorphism in the patient and in her 2 daughters. (Korean J Blood Transfus 2010;21:148-153) 저자들은 61세 여자 환자의 ABO 불일치를 해결하는 과정에서 혈청학적으로 cis-AB형을 의심하고 실시한 ABO 유전자의 액손 6과 7 분석에서액손 7의 703 G>A만을 제외하고는 cis-AB01/O02와 일치한 소견을 보여, 가계도 조사, 가족들ABO 혈액형의 혈청학적 분석, 유전형 분석 그리고 환자에 대해서는 추가로 클로닝을 실시하여cis-AB01 대립유전자와 O02 대립유전자를 분리한 후 다시 직접염기서열분석을 통해 703 G>A 변이가 새로운 O02 변이형에서 기원하였음을 증명하여 보고하는 바이다.

        새로운 국산 V-Tube 진공채혈관과 BD Vacutainer의 비교평가

        Background: Vacuum tubes are widely used in clinical laboratories for routine tests. We compared a newly developed V-tube (AB Medical, Korea)and BD tubes (BD, USA) in common clinical assays, i.e., hematological, chemical, and immunological tests. Methods: In total, 100 volunteers comprising 79 patients and 21 healthy volunteers were recruited and peripheral blood samples were collected with 2 brands of EDTA tubes and serum-separating tubes (SSTs). EDTA-tube samples were evaluated using 16 routine hematological tests. The SST samples were analyzed for 32 routine chemical parameters and 3 thyroid hormones. The results were statistically analyzed using the paired ttest and Bland-Altman plot. In addition, the stability of each analyte in 2 brands of vacutainers was evaluated. The results of the hematological tests at t=0 hr were compared with those at t=72±2 hr, and the results of the chemical parameters and thyroid hormones at t=0 hr were compared with those at t=72±2 hr and t=168±2 hr for each tube. Results: Paired t-test analysis revealed that the test results of 16 routine hematological parameters, 32 routine chemical parameters, and 3 thyroid hormones showed clinically allowable differences between the 2 brands of vacuum tubes (t =0 hr). The results obtained when using V-tubes showed a statistically significant correlation with those obtained when using BD tubes. The stability of each analyte was similar in both vacuum tubes. Except for 10 parameters (white blood cell count, mean corpuscular volume, basophils [ %], mean corpuscular hemoglobin concentration,monocytes [ %], phospholipid, sodium, potassium, chloride, and free T4), all parameters showed significant but clinically allowable differences with regard to storage duration. Conclusions: The newly developed V-tube vacutainers provide a suitable alternative to BD tubes in common clinical laboratories. 배경: 진공채혈관은 임상검사에서 널리 쓰이고 있다. 본 연구자들은 최근 개발된 V-tube와 BD Vacutainer 진공채혈관을 일반혈액검사, 일반화학검사 및 면역학적 검사를 대상으로 비교 평가하였다. 방법: 총 100명의 성인(건강인 21명, 환자 79명)을 대상으로 본 연구를 시행하였다. 말초혈액을 ethylene diamine tetra acetic acid(EDTA) tube와 serum separator tube (SST) 각각에 채혈하여 두 진공채혈관을 비교하였다. EDTA tube로는 일반혈액검사 16종을, SST로는 일반화학 32종 및 갑상선 기능검사 3종에 대하여 검사를 시행하였다. 두 진공채혈관의 검사 결과는 쌍체비교법과 Bland-Altmanplot을 이용해 비교하였다. 안정성 검사를 위해서 두 진공채혈관 각각의 채혈당일 검사결과를 기준으로 하여 일반혈액검사는 채혈 후3일째의 검사치의 변화를, 일반화학 및 갑상선기능검사는 채혈 후3일, 7일째의 검사치의 변화를 검사 항목별로 분석하였다. 결과: 당일 검체로 시행한 두 진공채혈관의 측정값 간 차이는 일반혈액검사 16종, 일반화학검사 32종, 갑상선기능검사 3종 모두에서높은 일치도를 보였고, 임상적으로 허용 가능한 범위 내에 속하였다. 또한 두 진공채혈관의 측정값 간에는 통계학적으로 유의한 강한 양의 상관관계를 보였다. 보관 기간에 따른 검사 안정성 비교에서 두 진공채혈관 모두 유사한 정도의 안정성을 보였다. 당일 검체와 3일째 검체의 비교 시 WBC, MCV, Basophil은 두 채혈관 모두에서 임상적으로 유의한 차이를 보였고, BD 진공채혈관에서는MCHC, Monocyte에서 임상적으로 유의한 차이를 보였다. 일반화학검사와 갑상선 기능검사의 경우 의 경우 당일 검체와 보관 검체(3일, 7일)와의 결과 비교 시 두 가지 진공채혈관 모두에서 대부분의 항목의 결과가 통계적으로 유의한 차이가 있음을 알 수 있었다. 하지만 PL, Na, K, Cl, free T4 항목을 제외한 대부분은 임상적으로허용할 만한 오차 범위 내의 결과를 보였다. 결론: 임상검사실에서 흔히 시행하는 검사들에 대해 새로 개발된V-tube를 BD Vacutainer 진공채혈관과 비교 평가한 결과 국산 Vtube진공채혈관이 수입제품을 대체할 수 있을 정도의 품질과 성능을 보유한 것으로 평가되었다.

        Taenia saginata infection incidentally detected during workup for lymphoma from an 8-year-old boy in Korea: a case report

        Human taeniasis is presumed to have almost disappeared from Korea. Recently, weincidentally detected a Taenia saginata infection in an 8-year-old boy undergoing lymphomadiagnosis. The patient had been suffering for 4 months from intensifying snoring andobstructive sleep apnea. A neck computed tomography scan revealed a nasopharyngealmass, and malignant B-cell lymphoma was supported by punch biopsy. On day 6 of thelymphoma workup period, the patient experienced anal itching, and two proglottids weredetected in his stool. The patient had experienced four or five similar episodes within thepast 2 years. He self-reported a history of raw beef and fish consumption and no history oftraveling abroad. Laboratory findings revealed mild eosinophilia (eosinophil count: 791/μL). Two proglottids exhibited movement and possessed more than 15 branched uterinestructures. Long segments approximately 84 cm in length were expelled after praziquanteltreatment. Sequencing of the cytochrome oxidase 1 gene confirmed T. saginata, ruling outrelated Taenia species. After treatment, no proglottids or ova were detected in his stool, andthe patient finally started chemotherapy for lymphoma. This case highlights the importanceof timely diagnosis of hidden taeniasis in low-frequency endemic regions.

        PCRㆍ직접염기서열분석법에 의한 ABO 유전형검사 6년 경험

        Background: ABO genotyping is essential for resolving ABO grouping discrepancy and for determinating ABO subgroups. Most clinical samples, including suspected inherited subgroups and acquired variant phenotypes, can be determined by PCR-sequencing of exons 6 and 7 in the ABO gene. Here, we describe our six years' experience performing ABO genotyping by PCR-direct sequencing. Methods: We conducted a retrospective investigation of serological and genotypical data from 205 samples (158patients and 47 of their family members) of patients who were referred to the Molecular Genetics Laboratory at Chonnam National University Hwasun Hospital for ABO genotyping between January 2007 and July 2012. ABO genotyping was performed on all samples with PCR-direct sequencing of exons 6 and 7 in the ABO gene; the standard serologic tests were also performed. Results: The frequency of phenotypes consistent with their genotypes was 70.8% (112/158 cases) and the A2B3phenotype with the cis-AB01 allele was the most common (31.0%, 49 cases) among them. The frequency of phenotypes inconsistent with their genotypes was 29.1% (46/158 cases) and the A1B3 phenotype was the most frequently recovered case (5.1%, 8 cases). Family study showed differential phenotype expression depending on the co-inherited ABO allele in five families with the B306, cis-AB01, Ael02, Aw14, or B305 allele and also showed a typical inheritance of a chimera with A102/B101/O04. Conclusion: We propose that ABO genotyping using PCR-direct sequencing is useful for the resolution of ABO discrepancies and for the investigation of ABO subgroups based on six years' experience. In addition, family study for analysis of phenotypic patterns of ABO subgroups is also crucial to ABO genotyping. 배경: ABO 유전형 검사는 ABO 불일치의 해결및 ABO 아형의 결정에 있어서 매우 중요하다. 대부분 ABO 아형 및 변이 표현형을 보이는 임상검체에서 그 원인은 ABO 유전자의 엑손 6과 7의PCR증폭 및 직접염기서열분석을 통해 결정될 수있다. 본 저자들은 PCR증폭 및 직접염기서열분석을 통한 ABO 유전형 검사를 시행한 6년 간의경험을 소개하고자 한다. 방법: 본 연구는 2007년 1월부터 2012년 7월 사이에 화순전남대학교병원에 의뢰된 205 검체(환자 158명, 가족 47명의 검체)의 ABO 유전자검사결과를 후향적인 분석을 통하여 실시하였다. ABO 유전형 검사는 ABO 유전자의 엑손 6과 7에 대한PCR 증폭 후 직접염기서열분석을 통해 실시하였다. 표준혈청학적 검사 또한 시행되었다. 결과: ABO 유전형 검사가 의뢰된 총 158 검체중 112건(70.8%)은 ‘표현형과 유전형이 일치한 경우’였으며, cis-AB01 대립유전자를 동반하는 A2B3형이 49건(31.0%)으로 가장 흔했다. 표현형과 유전형이 일치하지 않은 경우는 158건 중 46건(29.1%)이었으며, 그들 중에서는 A1B3형이 8건(5.1%)으로 가장 높은 빈도를 보였다. B306, cis-AB01,Ael02, Aw14 그리고 B305 대립유전자를 가진 5발단자의 가계조사에서 동반되는 ABO 대립유전자에 따라 가족들이 다른 표현형을 보임을 보여주었고, A102/B101/O04를 갖는 키메라는 전형적인 키메라 유전양상을 보여주었다. 결론: 저자들은 ABO 유전형 검사를 시행한 6년 간의 경험을 통해 ABO 불일치의 해결과 ABO 아형의 감별에 있어서 PCR-직접염기서열분석법이 유용함을 밝히는 바이다. 또한 다양한 ABO 아형의 표현 양상을 분석할 수 있는 가계조사가 함께 이뤄져야 할 것이다.
