        슬관절 반월판 열상 진단에서의 축상 자기공명영상의 유용성

        Purpose: To evaluate the role of standard axial MR images for the diagnosis of meniscal tears of the knee. Materials and Methods: Forty-five patients with a prior MRI examination that underwent arthroscopic surgery of the knee due to clinical impression of a meniscal tear were included in the study group. The sequence for meniscal evaluation was an axial fat-saturated proton density-weighted image with a 4 mm slice thickness. Axial MR images were independently reviewed by two radiologists and were compared with findings of arthroscopy. The sensitivity, specificity, positive predictive value, negative predictive value and accuracy of the axial MR scan for the diagnosis of the meniscal tear were calculated. Results: A total 90 menisci of 45 patients were evaluated in the axial MR scans. Forty-two patients had meniscal tears, and two of the patients had tears in both menisci; thus, a total of44 meniscal tears were found by arthroscopy. For meniscal tears, the sensitivity of the axial plane was 76.2%, the specificity was 89.1% and the accuracy was 81.1%. False negative meniscal tears were seen in 12 cases and false positive meniscal tears were seen in 5 cases on the axial MR images. Conclusion: In standard knee MRI examinations, axial images may be valuable for the detection and localization of meniscal tears. 목적: 일상적인 슬관절 자기공명영상에 포함된 축상영상의 반월판 열상 진단에 대한 정확도를 알아보고자 한다. 대상과 방법 : 임상적으로 슬관절의 반월판 열상을 의심하여 관절경 시술을 시행한 환자 중 수술 전 슬관절 자기공명영상 촬영을 시행한 환자 45명을 대상으로 하였다. 축상영상은 지방억제 양성자밀도강조영상을 사용해 얻었으며 절편두께와 간격은 모두 4 mm이었다. 총 45명 환자의 내측, 외측 반월판 90개의 자기공명영상을 2명의 영상의학과 의사가 후향적으로 분석하였으며 반월판 열상의 진단은 각 슬관절 자기공명영상의 축상영상만으로 진단을 하였다. 관절경에서의 반월판 열상 소견을 기준으로 하여 축상영상에서 판독한 반월판 열상의 존재 여부와 열상의 위치가 일치한 경우에 대한 민감도, 특이도, 양성 예측도, 음성 예측도, 정확도를 계산하였다. 결과: 총 45명의 환자에서 총 90개의 반월판을 관절경 시술로 확인하였다. 동시에 양측 반월판 열상을 앓고 있었던 2명을 포함하여 반월판 열상 환자는 모두 42명으로 반월판 열상은 모두 44예였다. 총 44개의 반월판 열상 중 축상영상에서의 민감도는 76.2%였고 특이도는 89.1%, 정확도는 81.1%였다. 축상영상에서 위음성과 위양성은 각각 12개와 5개였다. 결론: 일상적인 슬관절 자기공명영상에서 축상영상은 반월판 열상의 존재 여부와 침범된 위치를 판정하는데 도움이 된다.

        Malignant Extrarenal Rhabdoid Tumor in Adults: Three Case Reports Originating from the Ileum, Adrenal Gland and Uterus

        Malignant extrarenal rhabdoid tumor (MERT) is a very aggressive tumor that is extremely rare in adults. Herein, we introduce three cases of MERT in adults that originated in the ileum, adrenal gland, and uterine endometrium. The MERTs in the ileum and adrenal gland showed non-aggressive features and a good prognosis, while the MERT in the uterine presented with aggressive features and distant metastasis.

        Solitary Fibrous Tumor of the Ischiorectal Fossa : CT and MRI Findings

        Solitary fibrous tumor (SFT) is a rare neoplasm, which is usually presented as a pleural based mass, but can also occur in unusual locations based on its mesenchymal origin. However, the radiologic features of SFT occurred in the ischiorectal fossa have been rarely reported. In this case, we describe the MRI findings in a case of a SFT involving the ischiorectal fossa of a 36-year-old man. The tumor appeared as homogeneous iso-signal intensity relative to the adjacent muscle on T1 weighted images, a mixed high signal intensity on the T2 weighted images, and heterogeneous enhancement following the administration of the contrast material.

        Pancreatic Adenosquamous Cell Carcinoma with Solitary Liver Metastasis Showing Different Imaging Features

        Among exocrine pancreatic tumors, adenosquamous carcinoma is a rare, aggressive subtype with a poor prognosis and a high potential for metastases compared with its more conventional glandular counterpart, adenocarcinoma of the pancreas. We herein describe the imaging findings of pancreatic adenosquamous cell carcinoma with solitary liver metastasis showing different imaging features and also review the previous literature to recognize characteristic imaging features of pancreatic adenosquamous cell carcinoma.

        An Innovative Ultrasound Technique for Evaluation of Tumor Vascularity in Breast Cancers: Superb Micro-Vascular Imaging

        Tumor vascularity is an important indicator for differential diagnosis, tumor growth, and prognosis. Superb micro-vascular imaging (SMI) is an innovative ultrasound technique for vascular examination that uses a multidimensional filter to eliminate clutter and preserve extremely low-velocity flows. Theoretically, SMI could depict more vessels and more detailed vascular morphology, due to the increased sensitivity of slow blood flow. Here, we report the early experience of using SMI in 21 breast cancer patients. We evaluated tumor vascular features in breast cancer and compared SMI and conventional color or power Doppler imaging. SMI was superior to color or power Doppler imaging in detecting tumor vessels, the details of vessel morphology, and both peripheral and central vascular distribution. In conclusion, SMI is a promising ultrasound technique for evaluating microvascular information of breast cancers.

        Target Balloon-Assisted Antegrade and Retrograde Approach for Recanalization of Thrombosed Fem-Pop Bypass Graft Using the Outback Catheter

        The subintimal arterial flossing with antegrade-retrograde intervention (SAFARI) technique is reportedly effective in severe peripheral vascular disease that cannot be treated with standard endovascular techniques including subintimal angioplasty. In this report, we used a target balloon with the Outback catheter to recanalize a thrombosed bypass graft that could not be treated successfully with SAFARI.
