        Pedicle Screw-based Dynamic Stabilization with a Hinged Screw Head System in the Treatment of Lumbar Degenerative Disorders

        Objective: Lumbar dynamic stabilization systems have been developed for the treatment of lumbar degenerative diseases. However, the clinical results of dynamic stabilization systems are still not well established. The purpose of this study is to assess the clinical and radiologic outcomes of lumbar dynamic stabilization using a hinged screw head system(CosmicⓇ). Methods: Eighteen patients were followed for more than 1 year after dynamic stabilization with the Cosmic system. The following parameters were reviewed: age, sex, visual analog scale (VAS) for the leg and back, the Oswestry Disability Index (ODI), complications related to the operation, range of motion (ROM) of the adjacent segments, implantation segment and the whole lumbar spine. We compared the preoperative ODI, VAS, and ROM of the adjacent segments, implantation segment and the whole lumbar spine with the 1-year postoperative follow-up results. Results: The mean preoperative ODI and VAS scores were significantly decreased after the operation (p<0.05). ROM at both the cranial and caudal adjacent segments did not change significantly postoperatively (p>0.05). The mean preoperative ROM of the implantation segment was decreased postoperatively. However, the mean ROM of the whole lumbar spine did not change significantly postoperatively (p>0.05). Conclusion: According to our preliminary results, lumbar dynamic stabilization using a hinged screw head system did not influence adjacent segment motion, and had improved clinical outcomes for a 1-year postoperative follow-up period.       • 추간판 탈출의 기질 흡수에 관한 혈관 신생과 대식구의 역할

        Purposes: This study is performed to establish the histologic characteristics of matrix resorption in herniated intervertebral disc and to evaluate the role of neovascularization and macrophage infiltration, in especially concerning symptom duration and radiologic type of disc herniation. Materials & methods: Total seventy-two cases of herniated disc including protrusion 26, extrusion 24, and sequestration 22 cases were selected. The scales of matrix resorption, and vascularization were devided into 4 groups according to the extent and grade of each histologic changes. Through immunohistochemical staining (LSAB method) with monoclonal antibody CD34 (Neomarker, USA) and CD68 (DAKO, Denmark) in serial sections of paraffin embedded tissues, the expression of CD34 and CD68 in herniated discs was evaluated. Results: The incidence of matrix resorption and cell degeneration was 54.2%, which was markedly increased in prolonged (more than 1 month) symptom duration group and protruded or sequestrated type. Neovascularization, highlighted with CD 34 expression tends to be random and marginal, CD68 expressed macrophages were noted in well-vascularized region. Microvessel density (MVD) calculated by CD34 expression and CD68 expression in cases of matrix resorption were also pronounced in prolonged symptom duration group and protruded or sequestrated type. Conclusion: The authors confirm an important role of angiogenesis and macrophages in matrix resorption of disc herniation, especially in clinically prolonged and more severely herniated cases.

        Motion Preserving Surgery in Two Cases of Brown-Sequard Syndrome Caused by Herniated Cervical Discs

        We described two cases of Brown-Sequard syndrome caused by cervical disc herniation that were treated by cervical artificial disc replacement. Two patients presented Brown-Sequard syndrome. Computed tomography scanning and magnetic resonance imaging showed a massive soft disc herniation compressing the spinal cord. We performed a total discectomy to remove the ruptured disc particle and replace it with an artificial disc. The neurologic symptoms of two patients were resolved after the surgery. We suggest that cervical arthroplasty can also be used to treat more severe forms of cervical myelopathy such as Brown-Sequard syndrome.

        The Common Peroneal Nerve Injuries

        The dorsiflexion function of the peroneal nerve is important for human walking. Peroneal nerve damage has various etiologies, and the classification of the mechanism depends on the energy of the force responsible for injury. Peroneal nerve damage caused by low-energy mechanisms (entrapment, compression, and compartment) is not uncommon in neurosurgery, and accurate diagnosis and treatment of these conditions necessitate an understanding of the anatomical characteristics of the peroneal nerve, clinical symptoms, and prognosis. In addition to recent conventional diagnostic methods, diagnosis using magnetic resonance imaging and the surgical method of external neurolysis and decompression of the fibular tunnel in PICPNe patients will be described.

        친환경 식품 포장 정책에 대한 20대 소비자의 인식 연구:환경 인식 정도를 중심으로

        This study identified consumers' perception, interest, and intention to practice eco-friendly activities related to food packaging materials depending on the level of environmental perceptions and was intended to present the direction of sustainable activities of government and companies. For consumers in their 20s nationwide, the research was conducted using an online survey method for a month. It was analyzed by classifying into groups with high and low interest in environmental protection. As a result, the group with high environmental perceptions had significantly more women than men, and the group with high environmental perceptions acquired information on eco-friendliness through accessible media such as the Internet, TV, and YouTube. There was a significant difference in interest and perceptions of eco-friendly policy activities between two groups, and the group with high environmental perceptions had a more positive perception about eco-friendly activities of both government and businesses than the groups with low environmental perceptions. In the evaluation of policy promotion activities conducted by the government in relation to eco-friendly food packaging, the group with high perceptions of all items except diversity showed a positive evaluation. Therefore, in order to increase the use of eco-friendly packaging materials by consumers in their 20s, the government and companies needs to continue to make efforts to induce interest in the environment.

        Radiologic Determination of Corpus Callosum Injury in Patients with Mild Traumatic Brain Injury and Associated Clinical Characteristics

        Objective : To investigate the incidence of corpus callosum injury (CCI) in patients with mild traumatic brain injury (TBI) using brain MRI. We also performed a review of the clinical characteristics associated with this injury. Methods : A total of 356 patients in the study were diagnosed with TBI, with 94 patients classified as having mild TBI. We included patients with mild TBI for further evaluation if they had normal findings via brain computed tomography (CT) scans and also underwent brain MRI in the acute phase following trauma. As assessed by brain MRI, CCI was defined as a high-signal lesion in T2 sagittal images and a corresponding low-signal lesion as determined by axial gradient echo (GRE) imaging. Based on these criteria, we divided patients into two groups for further analysis : Group I (TBI patients with CCI) and Group II (TBI patients without CCI). Results : A total of 56 patients were enrolled in this study (including 16 patients in Group I and 40 patients in Group II). Analysis of clinical symptoms revealed a significant difference in headache severity between groups. Over 50% of patients in Group I experienced prolonged neurological symptoms including dizziness and gait disturbance and were more common in Group I than Group II (dizziness : 37 and 12% in Groups I and II, respectively; gait disturbance : 12 and 0% in Groups I and II, respectively). Conclusion : The incidence of CCI in patients with mild TBI was approximately 29%. We suggest that brain MRI is a useful method to reveal the cause of persistent symptoms and predict clinical prognosis.

        Double Fascicular Nerve Transfer to Restore Elbow Flexion Following Neuralgic Amyotrophy: A Case Report

        Double fascicular nerve transfer (DFNT) involves transferring fascicles from the median and ulnar nerves to the musculocutaneous nerve to restore elbow flexion. We performed this procedure on a patient who was diagnosed with neuralgic amyotrophy (NA) 2 years previously. We aimed to investigate the short-term outcomes and feasibility of DFNT applied to non-traumatic neuropathy. A 63-year-old, right-handed man presented to us with persistent right elbow flexion weakness 24 months after receiving the diagnosis of NA. On examination, biceps strength was 0/5, and shoulder abduction strength was 3/5. Magnetic resonance neurography demonstrated muscle atrophy in the upper trunk. DFNT was considered to restore elbow flexion. The donor and recipient fascicles were then coapted (flexor digitorum superficialis to biceps brachii, flexor carpi ulnaris to brachialis). Postoperatively, paresthesia was reported in the area of ulnar innervation, and the patient complained of hand clumsiness. From postoperative day 10 onward, biceps muscle contracture was identified and on the 14th day, grade 3 elbow flexion strength was observed. We suggest that DFNT may be a safe and effective method to restore elbow flexion in patients with NA.

        A Novel Association between Lysyl Oxidase Gene Polymorphism and Intracranial Aneurysm in Koreans

        Purpose: Lysyl oxidase (LOX) controls the cross-linking and maturation of elastin and collagen fibers. In this study, we investigated the association between LOX gene polymorphisms and intracranial aneurysm (IA) formation in a homogeneous Korean population. Materials and Methods: This cross-sectional study involved 80 age-sex matched patients with IA and controls. Fisher’s exact test was performed to analyze allelic associations between ten single nucleotide polymorphisms (SNPs) and IA, including 41 rupturedand 39 unruptured cases. Haplotype-specific associations were analyzed using the omnibus test estimating asymptotic chi-square statistics. Results: Of ten SNPs, three SNPs (rs2303656, rs3900446, and rs763497) were significantly associated with IA (p<0.01). The C allele of rs3900446 was significantly related to increased IA risk with a significant threshold [odds ratio (OR)=20.15, p=4.8×10-5]. Meanwhile,the A allele of rs2303656 showed a preventive effect against IA formation (p=8.2×10-4). Seventeen of 247 haplotype structuresshowed a suggestive association with IA (asymptotic p<0.001). Of ten SNP haplotype combinations, the CG combination of rs3900446 and rs763497 reached Bonferroni-adjusted significant threshold in IA patients (minor haplotype frequency=0.113, asymptoticp=1.3×10-5). However, there was no association between aneurysm rupture and the LOX gene. Conclusion: This preliminary study indicated that LOX gene polymorphisms, such as rs2303656, rs3900446, and rs763497, may play crucial roles in IA formation in the Korean population. Our novel findings need to be validated in a large-scale independent population.

      • Emerging Variant Glioma: Glioblastoma with a Primitive Neuro-Ectodermal Tumor(PNET) Component

        A 81-year old male with multiple mass lesion of the right fronto-temporo-parietal lobe and right frontal lobe was admitted to our hospital due to hemiparesis. Neuroimaging studies showed ring-enhancing lesions in right fronto-temporo-parietal lobe and right frontal lobe on magnetic resonance imaging (MRI). Tumor tissue evaluated to be consistent with glioblastoma radiologically was subjected to total excision. Using a morphological and immunophenotypic approach, the predominant component of the tumor was found to bear the properties of classic glioblastoma. The other component was composed of undifferentiated areas that exhibiting small cell morphology and diffuse neuronal immunophenotype. The case was diagnosed as ‘Glioblastoma with primitive neuroectodermal tumor (PNET) component’. The patient who had been performed postoperative radiotherapy, showed shrinkage of non-operative lesion and no sign of recurrence and neurological deficit during 12 months follow-up. We report a rare case of glioblastoma with a PNET component that has a relatively favorable outcome following surgery with radiotherapy and then review previous reports.
