        한방소아과학회지에 출판된 임상연구논문에 대한 분석

        Objectives The purpose of this study is to provide evidence of traditional Korean medicine treatment in children and adolescents by reviewing previous studies published in the journal of pediatrics of Korean medicine. Methods We have reviewed clinical studies of children and adolescents in the journal of pediatrics of Korean medicine from 2008 to 2017. Results Eighty-seven clinical studies of children and adolescents were reviewed. Total number of patients were 1644 in the studies. Among these, top 5 most commonly reported diseases in children and adolescents were atopic dermatitis, growth, autism, juvenile idiopathic arthritis and obesity. The most common intervention to treat those was taking herbal medicine. Most of the clinical studies showed effectiveness of this approach in treating children and adolescents. Conclusions More double-blinded randomized controlled clinical trials of Korean medicine interventions treating children and adolescents are needed to establish evidence-based treatment.

        眞武湯이 MIA 유도 골관절염 흰쥐 모델에 미치는 영향

        Objectives The object of this study was to investigate the antioxidative and antiinflammatory effects of Jinmu-tang extract (JMT) on the Monosodium iodoacetate (MIA)-induced rat osteoarthritis. Methods To investigate the antioxidant capacities of JMT, we measured the total polyphenol and flavonoid, and 2,2-diphenyl-1-picrylhydrazyl (DPPH) and 2,2’-Azino-bis(3-ethyl- benzothiazoline-6-sulfonic acid) (ABTS) radical scavenging activity. To evaluate the antioxidative and antiinflammatory effects of JMT, the rats were divided into 5 groups (n=8). Normal group was not induced by MIA and treated at all (N), control group was induced by MIA and not treated at all (Con), positive control group was induced by MIA and orally administered indomethacin 5 mg/kg (Indo) and experimental groups were induced by MIA and orally administered JMT 100 mg/kg (JMT100) and JMT 200 mg/kg (JMT200) for 4 weeks. The changes of anti-type II collagen antibody in serum, heme oxygenase-1 (HO-1), phosphorylated inhibitor of κBα (p-IκBα), cyclooxygenase-2 (COX-2), inducible nitric oxide synthase (iNOS) and tumor necrosis factor alpha (TNF-α) in knee joint tissue and histopathological observation (Hematoxylin & Eosin and Safranin-O stain) were measured. Results Total polyphenol and flavonoid levels of JMT were 26.90±0.33 mg/g and 6.02±0.34 mg/g. IC50 of L-ascorbic acid and JMT of DPPH radical scavenging activity were 1.35±0.07 μg/ml and 52.95±0.97 μg/ml. IC50 of L-ascorbic acid and JMT of ABTS radical scavenging activity were 3.18±0.02 μg/ml and 91.49±1.74 μg/ml. In serum, the anti-type II collagen antibody levels of JMT100 and JMT200 groups were decreased significantly. In knee joint tissue, the HO-1 level of JMT200 was increased significantly. The p-IκBα and TNF-α levels of JMT200 were decreased significantly. The COX-2 and iNOS levels of JMT groups were decreased significantly. In histopathological observation, in comparison with Con, synovial tissue, cartilage and proteoglycan of JMT100 and JMT200 were well preserved. Conclusions According to the results, It is considered that JMT has antioxidant and antiinflammatory effects for MIA-induced rat osteoarthritis, so it could be applied to osteoarthritis treatment.         Objectives : The purpose of this study is to report the effects of oriental medicine on 69 years old patient with postherpetic neuralgia. Methods : The patient was treated by oriental medical treatments such as herbal medication, Bee venom pharmacopuncture, acupuncture and so on. Results : After the treatment, Visual Analog Scale(VAS) scores and Inconvenience degree(ICD) were decreased. Conclusions : This study shows that the oriental medicine is effective on postherpetic neuralgia.

        당귀사역탕(當歸四逆湯)이 MIA로 유발된 골관절염 흰쥐에 미치는 영향

        Objectives The purpose of this study was to know the effects of Danggwisayeok-tang (Dangguisinitang) extract (DGSYT) on monosodium iodoacetate (MIA)-induced rat osteoarthritis. Methods For this purpose, rats were divided into 5 groups. Normal group was not injected with MIA and orally administered any medication. Control group was injected with MIA and not orally administered any medication. DGSYT100 group was injected with MIA and orally administered 100 mg/kg of DGSYT. DGSYT300 group was injected with MIA and orally administered 300 mg/kg of DGSYT. JoinsT group was injected with MIA and orally administered 20 mg/kg of Joins tablet. DGSYT100 and DGSYT300 groups were orally administered DGSYT during a week before and 3 weeks after based on the day MIA injected. The changes of hepatotoxicity, nephrotoxicity, relative hind paw weight distribution, cytokine in serum, cytokine messenger ribonucleic acid (mRNA) in joint tissue and histopathological observation (Hematoxylin & Eosin and Safranin-O staining) were measured. Results Alanine aminotransferase (ALT) levels of DGSYT100, DGSYT300 and JoinsT groups were increased significantly, but these results were within normal range. Aspartate aminotransferase (AST) and creatinine levels of all groups were not changed significantly. In the change of relative hind paw weight distribution, DGSYT300 and JoinsT groups were decreased significantly 14 and 21 days after MIA injected. Interleukin-1β (IL-1β) and Interleukin-6 (IL-6), Leukotriene B4 and Osteocalcin levels of DGSYT300 and JoinsT groups were decreased significantly. In measurement of IL-1β and nitric oxide synthase-Ⅱ mRNA relative quantitative of control, DGSYT100, DGSYT300 and JoinsT groups were decreased significantly. In measurement of TNF-α, IL-6 and Cyclooxygenase-2 mRNA relative quantitative of control, DGSYT300 and JoinsT groups was decreased significantly. In histopathological observation of knee, synovial tissue, cartilage and proteoglycan of DGSYT100, DGSYT300 and JoinsT were well preserved compared with control group. Conclusions According to the results, DGSYT has anti-inflammation and pain relief effects. So it should be suppressed progression of arthritis in MIA-induced osteoarthritis rat.         요추굴곡신연기법과 좌위흉요추교정법을 이용한 청소년기 특발성 척추측만증 환자 치험 2례

        Objectives : The purpose of this study was to report clinical effects of Lumbar flexion-distraction manipulation and Sitting position thoracic and lumbar vertebra adjustment manipulation on patient with Juvenile idiopathic scoliosis Methods : The patients have been treated with the above two Chuna manual manipulations. We measured Cobb's angle, VNRS before and after treatment Results : After treatment, we confirmed these transition : In first case, Cobb's angle of T-spine changed from 27 to 23 : Cobb's angle of L-spine changed from 17 to 10 : VNRS changed from 6 to 2. In second case Cobb's angle of T-spine changed from 30 to 21 : Cobb's angle of L-spine changed from 16 to 16 : VNRS chagned from 6 to 3 Conclusion : The result suggests that these two Chuna manual manipulations were effective treatment on patients with Juvenile idiopathic scoliosis.

        경추성 현훈에 대한 추나요법의 치료 효과 : 체계적 문헌고찰

        Objectives : To evaluate the evidence for effectiveness of Chuna manual therapy, for cervicogenic dizziness Methods : We conducted a search across 6 electronic databases to find all randomized controlled clinical trials(RCTs) that discuss Chuna manual therapy till September 2018. Results : 7 RCTs were selected for our inclusion criteria. The systematic review showed positive results for using Chuna manual therapy, for treating cervicogenic dizziness. Conclusions : Our systematic review found that effectiveness Chuna manual therapy for cervicogenic dizziness. However, our systematic review had several limited evidence. We recommend an additional research to test the effectiveness of Chuna manual therapy.

        부자탕 추출물이 골관절염 동물 모델에 미치는 영향

        Objectives The present study was designed to find out the therapeutic effects and possible underlying mechanism of Buja-tang, a herbal complex formula on experimental monosodium iodoacetate (MIA)-induced osteoarthritis. Methods Osteoarthritis models were created via intra-joint injection of MIA (50 μL with 80 mg/mL) in rats. Rats were divided into five groups and each group consisted of seven. Normal group was not injected MIA and did a normal diet. Control group injected MIA and received distilled water. Indo injected MIA and oral administration of 5 mg/kg of indomethacin. BJTL injected MIA and oral administration of 100 mg/kg of Buja-tang. BJTH injected MIA and oral administration of 200 mg/kg of Buja-tang. We analyzed weight-bearing ability of hind paws, oxidative stress related factor, antioxidant protein, inflammatory protein, inflammatory messenger and cytokine in joint tissue. Pathological observation of knee cartilage tissue structures was also performed with hematoxylin & eosin and safranin-O chromosomes. Results Weight-bearing ability of hind paws showed a tendency to reduce pain. The incidence of nicotinamide adenine dinucleotide phosphate oxidase and p22phox in articular tissue was significantly reduced, and the incidence of nuclear factor-erythroid 2-related factor 2 and heme oxygenase-1 and superoxide dismutases was significantly increased. The incidence of phosphorylated inhibitor of κBα, nuclear factor- kappa B p65, inducible nitric oxide synthase, cyclooxygenase-2, tumor necrosis factor alpha, interleukin (IL)-6, and IL-1β decreased significantly. In pathological observation, cartilage tissue damaged by MIAs in biopsy has significantly recovered from Buja-tang administration. Conclusions Buja-tang has anti-inflammation, antioxidation and pain relief effects. So this is thought to inhibit the progress of osteoarthritis in rat caused by the MIA.         족부 골절수술 후 한방 재활치료의 효과: 증례보고

        The objective of this study is to propose postoperative rehabilitation for foot fracture in Korean Medicine and to report its effectiveness. There were three patients who were got foot fracture surgery received korean medicine with acupuncture, herbal medicine, cupping and exercise treatment. Patients were evaluated numeric rating scale (NRS), range of motion (ROM), American Orthopaedic Foot and Ankle Society (AOFAS) ankle hindfoot score, foot function index (FFI) and EuroQol-5 dimension (EQ-5D) index. Case 1 and 2 were improved NRS from 6 to 3. Case 3 was improved NRS from 5 to 2. These cases were improved ROM in all directions, AOFAS score, FFI, EQ-5D and walking status. This study suggest that Korean medicine rehabilitation could be effective for patients who have received foot fracture operation. (J Korean Med Rehabil 2019;29(3):165-172)

        산약의 운동수행능력 증진 및 항피로 효과

        Objectives In this study, we analyzed the effect of Dioscoreae Rhizoma on exercise practice ability and blood fatigue variable factor in the forced swimming test. Methods Male mice were divided into four group. Mice were administrated with saline, Discorae Rhizoma extracted by water (DRW), Discorae Rhizoma extracted by 30% ethanol (DRE), red ginseng (RG), orally once per day, for 1 week. Exercise performance was evaluated by exhaustive swimming time and weights. Glycogen contents of liver and soleus muscle were measured at the end of the experiment. In addition, the biochemical parameters such as asparate aminotransferase (AST), alanine aminotransferase (ALT), blood urea nitrogen, creatine, lactate dehydrogenase (LDH), ammonia, triglycerides (TG), total cholesterol (TC) and free fatty acid (FFA) were analyzed. Results Both DRW and DRE showed an improvement of swimming endurance compared with control group. Levels of ALT, ammonia, TG, TC and FFA were significantly decreased in both DRW and DRE compared with control group. Levels of AST, Creatine were significantly reduced in DRE group compared with control group. Moreover LDH showed only a tendency to increase in the DRW and DRE groups. Conclusions In summary, the present study showed that Dioscoreae Rhizoma improved in physical fatigue and exercise practice ability and DRE improved the superior than those of DRW.       • 한방복합치료로 호전된 요추추간판 탈출증 3례

        The purpose of this study was to prove the effectiveness of Korean medical combined treatment for lumbar herniated intervertebral disc(L-HIVD) patients who were admitted to Korean medical hospital. We used acupuncture, herbal medicine, moxa, cupping and chuna treatment. These patients evaluated with visual analog scale(VAS), european quality of life 5 dimension(EQ5D), oswestry disability index(ODI) and pain free walking distance(PFWD). All of these 3 cases of patients showed the improvement of VAS, EQ5D, ODI, PFDW. The results of this study suggest the effectiveness of Korean medical combined treatment for the patients with L-spine disc herniation.
