Purpose: We report a case of ophthalmic artery occlusion with third nerve paresis in the left eye due to acute occlusion of the left ICA. Case summary: A 37-year-old man visited our emergency room with “black out” in the left eye, headache, and nausea. The corrected visual acuity was 20/25 in the right eye, and hand motion in the left eye. In the left eye, a relative afferent papillary defect was noted, with an intraocular pressure of six mmHg. Twenty prisms of exotropia in the primary position was observed, and ocular motor examination revealed limitations of supraduction, infraduction, and adduction in the left eye, suggesting third nerve palsy of the left eye. Fundus examination revealed a pale retina in the macula of the left eye. Brain MRI demonstrated multifocal faint low densities in the left caudate nucleus as well as the frontal and parietal lobes. CT angiography and four-vessel angiography demonstrated complete occlusion in the proximal part of the left internal carotid artery ICA. After five months the corrected visual acuity of the left eye improved to 20/200. The 20 prism diopters of exotropia persisted, and ocular motor examination revealed only a mild limitation of adduction in the left eye. J Korean Ophthalmol Soc 2009;50(4):624-629
단안 선천 상사근마비 환자에서 하사근절제술, 후전술, 전치술의 효과 비교

Purpose: To compare the clinical outcomes of myectomy, recession, and anterior transposition (AT) of the inferior oblique muscle in congenital unilateral superior oblique palsy. Methods: A total of 40 patients who were followed for longer than three months postoperatively and, who underwent inferior oblique myectomy, recession, or AT were evaluated retrospectively. Results: We compared the result of inferior oblique myectomy, recession, and AT in congenital unilateral superior oblique palsy. The decreased amounts of inferior oblique overaction (IOOA) were 2.24±0.67, 2.00±0.76, 3.00±1.15 for myectomy, recession, and AT, respectively. Improvement of superior oblique underaction was 0.80±0.66, 0.86±0.83, 1.00±0.58, and the changes in hyperdeviation were 10.96PD, 8.25PD, 14.86PD for myectomy, recession, and AT, respectively. There were no significant differences among the surgical methods. The proportion of patients who showed IOOA in the contralateral eye after operation were 20.0% in myectomy, 25.0% in recession, and 57.1% in AT. Conclusions: The efficacies of myectomy, recession, and AT of inferior oblique muscle in the congenital unilateral superior oblique palsies showed no significant differences among methods. The AT group tended to show a larger effect than did the other methods, along with a high development percentage of contralateral eye IOOA. Based on these results, AT of the IO shoud be selectively chosen in more severe cases. J Korean Ophthalmol Soc 2010;51(1):76-80

A small room air-conditioner or a chiller for constant temperature bath usually uses a constant speed compressor. The constant speed compressor is operated with on/off control for variable capacities. However, the on/off control has several disadvantages such as energy loss and low compressor efficiency. To avoid these problems, a bypass system can be suggested for continuous operation. In this study, a dual bypass system having a hot gas bypass system and a liquid bypass system was adopted in a heat pump. The performances of the hot gas bypass system and the liquid bypass system in the heat pump were investigated experimentally with the variation of the expansion valve opening.
열펌프의 난방운전시 핫가스 바이패스에 따른 성능 특성에 관한 실험적 연구

When the suction pressure of compressor decreases under its limit, the compression ratio is increased causing the malfunctions of compressor. As the method to decrease the compression ratio, hot gas bypass system is usually adopted in heat pump system. In the hot gas bypass system, the discharged gas from the compressor is bypassed into the compressor suction, which causes the increase of suction pressure and the decrease of compression ratio. In this study, the characteristics and performances of the hot gas bypass system in heat pump was investigated experimentally with a variation of the bypass flow rate ratio. With the increase of the bypass rate ratio, the compressor suction pressure was increased, even though the total capacity and COP was decreased. From the analysis of the experimental results, the optimum pressure control algorithm was suggested in this study.

본 논문에서는 하천구역 내에서의 경작활동으로 인한 비점오염원 유출특성을 파악하기 위하여 팔당상수원보호구역의 경기도 광주시에 위치한 G지역(논, 비닐하우스)과 남양주시의 S지역(노지 밭)을 선정하여 강우 시 현장조사를 수행하였다. 담수기능을 갖는 논은 강우강도에 따라 유출유량이 완만한 증감을 나타내어 제내지에 위치한 논의 유출특성과 유사한 현상을 보여주었다. 그러나 비닐하우스와 노지 밭 모두 강우강도에 비례하여 유출유량이 급격히 증가하는 현상을 보여주었다. 논, 비닐하우스 및 노지 밭의 BOD 평균 유출농도는 각각 2.0 mg/L, 2.8 mg/L, 7.9 mg/L로서 노지 밭의 유출농도가 높게 나타났으며, 강우사상에 따른 토양의 유출에 기인하여 SS, T-N 및 T-P 유출농도도 노지 밭이 가장 높은 것으로 조사되었다. 경작지별 배출부하량을 산정한 결과, 논의 T-N 배출부하량이 1,793.9 kg/year으로 밭에 비해 높게 나타났으며, 노지 밭의 SS 배출부하량은 69,704 kg/year로 전체 배출부하량의 70% 이상을 차지하는 것으로 산정되었다. The field study was carried out to investigate runoff characteristics of non-point source (NPS) by cultivation activity in river district. Two sites were selected mainly as G region (paddy field, green house), located in Kwangju, Gyeonggi Province, and S region (ordinary field), located in Namyangju, Gyeonggi Province, those belong to Paldang reservoir watershed. The paddy field with water storage capacity showed a slow increase in runoff as rainfall intensity increases. Meanwhile, both green house and ordinary filed revealed a rapid increase. The average BOD runoff concentration of paddy field, green house, and ordinary field was 2.0 mg/L, 2.8 mg/L, 7 9 mg/L, respectively. It indicates that ordinary field shows the highest value in BOD, SS, T-N, and T-P concentration due to the soil loss during rainfall. As a result of runoff load estimation according to the field, a T-N runoff load of paddy field was 1,793.9 kg/year, higher than that of ordinary field. It was estimated that a SS runoff load of ordinary field was 69,704 kg/year and accounts for more than 70% of overall runoff load.
