저자들은 가톨릭 의대 보속 성 바오로 병원에서 1979년 1월부터 1981년 12월까지 3년간 발생한 난관붙임술기왕력이 있는 환자에서 17예의 자궁외임신과 1예의정상 자궁내 쌍태임신을 경험하였기에 문헌고찰과 함께 보고하는 바이며 최근 인구증가의 대응책으로 권장되고 있는 난관붙임술이 자궁의임신을 일으킬 수 있는원인이 된다는 점을 고려하여 이러한 난관붙임술은 반드시 숙련된 전문가에 의해 시술되어야 하며 또한 어떠한 방법을 사용하더라도 실패의 위험성이 내포되므로 시술 후 최소한 2년간의 관찰이 필요함을 주지해야할 것으로 여겨진다. In a consecutive study of 139 women of ectopic pregnancy confirmed by histologic examination from Jan.1,1979 to Dec. 31, 1981, at the Dept. of Obst. and Gynec., St Paul Hospital, Catholic Medical College,17 women were found to have had pre- ⅵous history of tubal sterilization, and intrauterine twin pregnancy occured in one patient. We had reⅵewed the failure cases of tubal sterilization surgery and the results were as follows: 1. In 14 cases of these failure cases the type of tubal sterilization was laparos- copic eletrical coagulation of the tube, in 3cases, was by Falope ring, and one case by Pomeroy procedure. 2. ln our series 7 out of 18 cases could be considered as the failure resulting from recanalization of the tube. The cause of sterilization failure were in order of frequency, recanalization(7 cases), technical error(6 cases),fistula formation(5 ca- ses). ln one intrauterine twin pregnant woman who were delivered by abdomen, the type of sterilization was laparoscopic one and recanalization of the tube was confirmed histologically. 3. Pregnancy after tubal sterilization occured in half of the patients(50%) within 2 years, with the shortest interval beimg 3 month and the longest 69 months. 4. ln all of the cases, the site of tubal sterilization was isthmic portion of the tube and it is significant that in 13 out of 17 ectopic pregnancy was located in the distal tubal segment. The location of ectoplc pregnancy were in order of frequency, 12 patients in the ampullar, 3 patients in the cornual,1 patient in the isthmic por- tion and another patient in the fimbrial portion of the tube. 5. The marked increase in the incidence of ectopic pregancy occuring after a previous sterilization procedure is of particular significance and the percentage of ectopic pregnancy in our study was 12.2%, This suggests that the increasing impor- tance of previous sterilization as an etiologic factor in ectopic pregnancy.

본 저자들은 31세 임산부에서 진행된 부분포상기태가 있으면서 쌍태아 임신을 경험하였기에 간단한 문헌고찰과 함께 보고하는 바이다. Ultrasonography has become the preferred noninvasive diagnostic imaging modality that enables early confirmation of gestational trophoblastic disease. Twin pregnancy with partial hydatidiform mole and coexisting fetuses was lived at a later gestational age, and had higher preevacuation beta-hCG levels. When disease is not metastatic, chemotherapy is started for a rising or persistantly elevated beta-hCG level. Teh common complications of hydatidiform mole, hemorrhage and infection, decreased in incidence recently due to early diagnosis and evacuation of hydatidiform molar tissue. We recently experienced one case of advanced twin pregnancy with partial hydatidiform mole and coexisting fetuses.

최근 저자들은 경관이 50%이상 소실되고 2-5cm정도 개대되었으며 양막이 질강내로 탈출된 2명의 진행성 자궁경관 무력증 산모에서 복부를 통한 양수제거로 양막내압을 줄이고 부풀린 foley catheter로 양막을 자궁강 안으로 밀어올린 후 McDonald 술식으로 응급봉축술을 시행 하여 임신기간을 충분히 연장하고 생존아를 분만시킨 경험하였기에 문헌적 고찰과 함께 보고하는 바이다. Emergency cerclage may be defined as that performed in the setting of advanced cervical dilation with bulging or "hourglass" membranes. Two patients with bulging or hourglass membranes and marked cervical dilation in the second trimester were treated with emergency cerclage after transabdominal amniocentesis. pregnancy was prolonged for a sufficient time to deliver viable fetuses in both patients. the lack of significant maternal morbidity combined with the results for the fetus-infant supports further efforts in this area.

저자들은 최근 34세된 가정주부가 자궁근종의 임상적 진단하에 입원하여, 외과적수술로 적출된 자궁부속기의 병리조직검사에서 광인대에 전이된 내림프 기질성근증으로 판명된 1례를 경험하였기에 문헌적 고찰과 함께 보고하는 바이다. Endolymphatic stromal myosis is a low-grade endometrial stromal sarcoma with an indolent course that has been reported occasionally to recur and metastasize, usually after long period of time. The authors recently experienced a case of the endolymphatic stromal myosis with metastases to the broad ligament pathologically, in 34 years old multiparous woman who treated with total abdominal hysterectomy and left salpingo-oophorectomy, and then treated with progestin (Megace) postoperatively. A case of the endolymphatic stromal myosis with metastases to the broad ligament is reported with the brief review of the literature.

When the diameter of an endometrial cyst exceeds 15 cm, malignancy must be suspected. A case of large endometrial cyst of ovary is presented and a brief review of the literature is made.
