        신규간호사 교육체계에 대한 실태조사: 프리셉터십 운영을 중심으로

        The purpose of this study was to identify the current status of the new graduate nurses' education system, to establish guidelines for the education of new graduate nurses, and to provide dates for the improvement of the education system. Eighty-nine hospitals participated in the survey from July to August 2018. The questionnaires consisted of 24 items including general characteristics, new graduate nurses' education system, operation of nursing education teams, and preceptorship programs. The average duration of education/training for new graduate nurses was 57.3 days, and 26.6%-58.5% of general hospitals, hospitals, and long-term care hospitals had less than 30 days of new graduate nurses education/training, which is shorter than the education/training period of tertiary hospitals. There were new nursing education guidelines in 88.8% of all medical institutions and 58.6% of them had nursing education teams. Most of the personnel in charge of nursing education were not dedicated to education. A total of 87.6% of the all participating medical institutions had preceptorship programs, while 23.1%-33.3% of hospitals and long-term care hospitals did not. The breakdown of preceptorship programs showed 68.4% as "preceptor and preceptee are in charge of one team, and preceptor is teaching preceptee," and 19.7% were "preceptor and preceptee are in charge of each team, and preceptor is teaching preceptee at the same time." These results show that standardization and management of new graduate nurses' education programs is necessary. For the standardization and management of new graduate nurses' education programs, joint efforts of nurses' associations and support from the government should be encouraged.

        간호학 임상실습교육의 현황과 발전방안

        The quantitative expansion of nursing schools has necessitated the qualitative improvement of nursing education, which requires the development of nursing education for clinical practice. To identify strategies for strengthening the educational capacity of clinical fields and nursing schools, this study first examined the current status of nursing education for clinical practice, and then proposed several prospective directions for education. Nursing clinical practice-related studies from several Korean and international electronic databases were reviewed. Insufficient training hospitals and lack of qualified clinical training instructors were the main problems found within nursing educational resources for clinical practice, while the simple practice contents based on observation and inadequate evaluations were the main problems found in nursing educational management for clinical practice. This study suggests better standards and educational accountability for training hospitals and programs to nurture human resources for clinical practice, as well as a variety of training methods to integrate practical training courses and the expansion of formation evaluation. Based on these results, it is necessary to establish governance for nursing education for clinical practice and clarify the role and standards of each practitioner, strengthen the educational role of the hospital, and improve the system. In addition, introducing various types of education methods and strengthening evaluation standards are needed in order to enhance nursing education for clinical practice.

        간호대학생의 학업탄력성과 대인관계유능성이 행복감에 미치는 영향

        The purpose of this study was to investigate the effects of academic resilience and interpersonal competence on the happiness of nursing students. A descriptive design was used to study 158 nursing students in nursing school through convenience sampling. IBM SPSS ver. 23.0 (IBM Corp., Armonk, NY, USA) was used to analyze data for descriptive statistics, t-test, analysis of variance, Tukey post hoc test, Pearson's correlations, and multiple regressions. The results showed that the level of nursing students' happiness was lower than that of college students in other majors, but academic resilience and interpersonal competence among nursing students were relatively high. Nursing students' happiness was positively correlated with academic resilience and interpersonal competence. The factors affecting happiness were academic resilience (${\beta}=0.459$, p<0.001), satisfaction with major (${\beta}=0.272$, p=0.006), perceived subjective health status (${\beta}=0.223$, p=0.013), and interpersonal competence (${\beta}=0.185$, p=0.003). Explained variance for happiness was 56.4%, and academic resilience was the most influential factor in the happiness of nursing students. Nursing students are caring human beings and should be able to pursue a happy life; therefore, it is necessary to improve satisfaction levels with the nursing major, guide students to excel in their academic achievement, prepare health education programs, and design curriculum that inspires confidence in interpersonal relationships.

      • 정신건강복지법 개정 전후 응급입원 환자의 특성, 입원유지율 및형태, 재입원율 비교

        목 적 : 2017년 개정된 ‘정신건강증진 및 정신질환자 복지서비스 지원에 관한 법률’ 시행 전후 응급입원 환자군의 특성, 입원유지비율, 입원 형태, 재입원율의 변화를 알아보고자 연구를 수행하였다. 방 법 : 2016년, 2018년 각 6개월 동안 서울특별시 은평병원에 응급입원한 환자를 대상으로 445건의 의무기록을 후향적으로 검토하였으며, 2016년에 입원한 224명과 2018년에 입원한 221명의 특성, 입원유지율 및 형태, 재입원율을 통계적으로 비교하였다. 결 과 : 정신건강복지법 개정 전후 두 군간 사회인구학적, 임상적 변인은 차이가 없었다. 응급입원에 이어 입원이 유지된 비율에는 두 군간 차이가 없었으나, 물질 사용 장애군을 제외 시 입원유지율이 감소하였다. 입원이 유지된 군에서 자의 입원의 비율은 증가하였다. 또한 응급입원 후 퇴원한 환자 중 1개월 및 3개월 이내의 재입원율은 증가하였다. 결 론 : 응급입원에 이어 입원을 지속하는 환자 중 자발적 입원의 비율이 늘었으며, 이는 정신건강복지법의 개정 목적에 부합하는 변화라고 할 수 있겠다. 하지만 일부 환자 집단에서는 입원을 지속적으로 유지하는 것이 어려워졌으며, 응급입원 퇴원 후 단시일 내의 재입 원율이 증가는 변화도 관찰되었다. 이러한 사항을 고려하여 정신건강복지법의 보완과 개선이 고려되어야 할 것이다. Objectives : The research was conducted to compare the characteristics, rate and types of admission continuation, and rate of readmission in psychiatric emergency hospitalization patients before and after the revision of Mental Health Promotion and Welfare Act. Methods : The medical records of the 445 patients who underwent psychiatric emergency hospitalization at Seoul Metropolitan City Eunpyeong Hospital in 2016 and 2018 were reviewed retrospectively. The clinical and sociodemograhpic characteristics, rate and types of admission continuation, and rate of readmission were compared between 224 patients who admitted in 2016 and 221 patients who admitted in 2018. To compare the characteristics of the two groups, t-test and chi-square test were performed. Results : There is no difference in clinical and sociodemographic characteristics between 2016 and 2018 group. There was no difference in the rate of admission continuation before and after the revision of act, while the rate significantly decreased from 61.9% to 50.0% when patients with substance use disorder were excluded from overall group. Among those who maintained hospitalization, the proportion of voluntary admission increased from 5.5% to 13.1%. The rate of readmission after discharge from the emergency hospitalization within 1 and 3 month period increased after the revision of the act. Conclusion : This study showed that the proportion of voluntary admission increased in admission continuation, and this change is in accordance with the purpose of the revision of the Mental Health Promotion and Welfare Act. On the other hand, the rate of admission continuation decreased in a subgroup of patients, and the readmission rate after discharge from the emergency hospitalization increased. Current findings suggest that the Mental Health Promotion and Welfare Act needs further complementation and improvement.

        하이퍼텍스트 구조를 통한 현대 공공도서관의 공간특성 연구 - 커뮤니티 활동 분석을 중심으로 -

        This study analyzes the spatial characteristics of modern public libraries that have changed non-linearly and non-hierarchically after the introduction of digital media through architectural expressions of hypertext structure characteristics as non-linear, multi-center, interaction, and borderless. The non-linear spatial composition appears in the combination of the atrium and the vertical movement, and the multi-center spatial composition is formed by the program composition through the location of the atrium and the void space, and the non-hierarchical division of the program through the furniture layout in an open plane. Interaction takes place through the transparency of building materials, visual and auditory communication in open spaces, changes in use according to user actions, and linking with different programs. Borderless appear as behavioral differences in the public-private domain, cognitive boundaries due to level differences, and ambiguity from gradient changes of slope. The open space where community activities were mainly performed reduces the gap between program participants and non-participants with undivided openness. It was confirmed that even visitors of the library could voluntarily participate in the community program or have an indirect experience. Through this study, it was possible to understand that the community is formed and space is utilized in a new way in the public library by the structural characteristics of hypertext.

        간호사가 경험한 임상 교육의 어려움

        Purpose: Clinical nurses play an important role in guiding the practice of new nurses and nursing students. Therefore, it is necessary to study the difficulties and the environmental aspects faced by nurses during the clinical education for the quality improvement of clinical education. Methods: Focus group interview was conducted with 15 clinical nurses and analyzed according to the qualitative content analysis method of Elo and Kyngäs. Results: Five themes were identified; "Lack of education competency and teaching efficacy as a clinical nurse educator", "environment that cannot educate enough", "lack of proper guidelines for education", "no manpower dedicated to clinical nursing education", " Lack of mutual exchange between university and clinical practice’ Conclusion: Based on the results of this study, it is necessary to make efforts to improve the education environment for nursing education.

        중소병원 간호 · 간병통합서비스 병동 근무 간호사의 업무 관련 특성과 수면의 질

        Purpose: The purpose of this study was to examine the sleep quality of nurses in the comprehensive nursing care units of small-medium sized hospitals, and general and work-related characteristics associated with sleep quality of nurses. Methods: Participants were 312 nurses in comprehensive nursing care units of eight small and medium hospitals. The data were collected between June and August, 2018. A survey questionnaire including, sleep quality, general, and work-related characteristics of nurses was used. The data were analyzed using SPSS/WIN 23.0 program with descriptive statistics, independent t-test, and x² test. Logistic regression analyses were used to identify factors associated with sleep quality. Results: The average score for sleep quality was 8.58±3.18 (0~21). Using a cut-off point of 8.5, 51.9% were poor sleepers. Sleep quality was associated with age (Odds Ratio [OR]=0.93, 95% Confidence Interval [CI]=0.88~0.99). Conclusion: Age was identified as a factor associated with sleep quality among nurses in the comprehensive nursing care units at small and medium sized hospitals. Researchers and nurse administrators need to develop and provide strategies to improve young nurses’ sleep quality.

        간호대학생의 인수인계 자기효능감에 영향을 미치는 요인

        본 연구의 목적은 간호대학생의 인수인계 자기효능감에 영향을 미치는 요인을 분석하고 그 결과를 토대로 간호교육 전략을 모색하는 것이다. 조사대상은 임상실습경험이 있는 간호대학 233명이며, 인수인계 자기효능감 측정을 위해서는 Perceived self-efficacy of hand-off reporting 도구를 사용하였고, 포괄적 대인 의사소통능력척도와 임상수행능력 측정도구를 사용하였다. 수집된 자료의 분석은 SPSS 프로그램을 이용하여 t-test, ANOVA, 다중회귀분석을 이용하여 분석하였다. 연구결과, 대상 자의 65.7%가 인수인계 경험이 없다고 응답하였고, 73.4%가 인수인계에 대해 두려움이 있다고 응답하였으며, 대상자의 인수인계 자기효능감 정도는 평균 33.41점으로 중간 정도였다. 일반적 특성에 따른 인수인계 자기효능감의 차이 분석에서는 임상실습경험이 네 학기 이상인 경우, 실제상황에서 인수인계 경험이 있는 경우, 인수인계두려움이 없는 경우 인수인계 자기효능감이 높았으며, 다중회귀분석 결과, 인수인계자기효능감에 영향을 미치는 요인은 임상수행능력(β=.271, p<.001)과 모의상황에서의 인수인계 경험(β=.204, p<.001)과 실제상황에서의 인수인계 경험(β=.361, p<.001)인 것으로 나타났다. 결론적으로 인수인계 자기효능감 증진을 위해서는 임상수행능력이 기반이 되어야 하며, 인수인계를 해보는 경험이 중요한 것을 알 수 있으므로,실습교육 시 이러한 요인을 고려하여야 하겠다. The purpose of this study was to identify the factors affecting the handoff self-efficacy of nursing college students in order to explore teaching strategies for clinical nursing education. The subjects of this study were 233 nursing college students who had experience in clinical practice. Perceived self-efficacy of hand-off reporting tool was used to measure self-efficacy on the handoff. The collected data were analyzed using t-test, ANOVA and multiple regression analysis using SPSS program. As a result of the study, 65.7% of the subjects answered that they had no experience of handoff, and 73.4% answered that they were afraid of handoff. The mean score of handoff self-efficacy was 33.41. In the analysis of the difference of handoff self -efficacy according to the general characteristics, when the experience of clinical practice was more than four semesters, when there was experience of handoff in actual situation, Factors influencing handoff of self-efficacy include clinical competence (β=.271, p<.001) and experience of handoff in simulations (β= .204, p<.001), experience of handoff in real clinical settings (β=.361, p<.001). In conclusion, these findings showed that clinical competence and experience of handoff were important factors to improve the handoff self-efficacy. Therefore it should be taken into consideration in the clinical practice education.

        노인의 성별에 따른 인지기능과 영양관리상태의 차이

        Purpose: The purpose of this study was to identify the gender differences in the cognitive function and nutritional status among elderly people living in the community. Methods: This study used data from the 2014 National Survey on the Elderly and focused on 10,054 respondents who answered that they did not have dementia. The Korean version of Mini-Mental State Examination for Dementia Screening and the Nutrition Screening Initiative were used. IBM SPSS Statistics 23.0 was used to conduct statistical analyses. Results: The cognitive function score was 25.30 for men, which was higher than for women. The percentage of subjects who showed cognitive decline was 25.5% for males and 50.2% for females, which was statistically significant (p<.001). There were statistically significant differences in the nutritional status between males and females (p<.001). There was a statistically significant negative correlation between the cognitive function and nutritional status in both men and women (p<.001, p<.001). Conclusion: These results showed that the intervention to maintain cognitive functioning should be provided to elderly women with a low education level or high number of chronic diseases and medication, and an intervention to prevent the cognitive decline of the elderly should include nutritional management for health and function maintenance.
