        근육뼈대계 환자보고식 결과 측정과 KCF의 연결: 물리치료 중심으로

        PURPOSE: The present study was conducted to improve the understanding of the Korean Standard Functional, Disability, and Health Classification (KCF) and its ease of use in the clinical domain of the musculoskeletal system by comparing, analyzing, and linking the KCF codes with items from patient-reported outcome measures (PROMs), which are currently mainly used to evaluate patients with neck, shoulder, waist, and knee pain. METHODS: The items of the most widely used PROMs, the Neck Disability Index (NDI), Disabilities of the Arm, Shoulder, and Hand (DASH) scores, Oswestry Disability Index (ODI), Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC), and the codes of the KCF were linked by two experts according to the linking rules. RESULTS: The concordance between the items of each of the PROMs and the KCF code linked by the two experts was NDI,86.4%86.4%, DASH 83.3%, ODI 92.0%, and WOMAC 80.7%. The NDI, DASH, and WOMAC indexes were found to comprise items corresponding to physical function, activity, and participation areas, and were linked to 22, 43, and 31 KCF codes, respectively. In addition to these two areas, the ODI included items related to environmental factors and was linked to 25 codes (duplicated codes are treated as one). CONCLUSION: This research can be used by adding the KCF code to the questions of the currently used evaluation tool. This coding can be easily applied and will contribute to the easy understanding of the KCF.

      • PNF 매트 운동이 뇌졸중 환자의 균형수행력 향상에 미치는 효과

        Purpose : The purpose of this study was to investigate the effect of mat activities in PNF on improvement of balance performance in stroke patients. Methods : Included 12 persons with stroke who were living in the community. They participated in exercise class for 60minutes duration three times a week for 6 weeks. Mat activities in PNF program was modified PNF patterns and techniques on various posture. Balance indexes were measured at pre-treatment, post-treatment and follow-up by K.A.T.3000. Results : Balance index scores had larger improvements after 6 weekends of treatment(p<.05) than pre-treatment, also follow-up test(p<.05). Conclusion : The result of this study showed that PNF mat activities intervention can improve balance performance in disabled persons after stroke.

      • 감각 자극과 치료적 환경이 외상성 뇌손상 흰쥐의 BDNF 발현에 미치는 영향

        Purpose : The purpose of this study was to test the effect of balance training for proprioceptive and vestibular sensory stimulation and therapeutic environment on expression of BDNF after traumatic brain injury in the rat. Subject : Twelve Sprague-Dawley rats were randomly assigned into group I and group II. After traumatic brain injury, group I was housed in standard cage for 7 days. Group II was housed in therapeutic cage after balance training for 7 days. Method : Traumatic brain injury was induced by weight drop model and after operation they were housed in individual standard cages for 24 hours. After 7th day, the rats were sacrificed and cryostat coronal sections were processed individually in goat polyclonal anti-BDNF antibody. The morphologic characteristics and the BDNF expression were investigated in injured hemisphere section from immunohistochemistry using light microscope. Result : Immunohistochemical response of BDNF in lateral nucleus, purkinje cell layer, superior vestibular nucleus and pontine nucleus appeared very higher in group II than in group I Conclusion : The present result revealed that simultaneously application of balance training for proprioceptive and vestibular sensory stimulation input and therapeutic environment in traumatic brain injured rats is enhance expression of BDNF and it is facilitates neural plasticity.

        수중재활운동이 뇌졸중 환자의 신체기능과 폐기능에 미치는 영향

        Purpose: This study examined the effect of aquatic exercise on the improvement in physical and pulmonary function after stroke. Methods: Fourteen candidates, who had experienced stroke, were enrolled in this study. The program was carried out three times weekly, 1 hour per session and for 10 consecutive weeks. At pre-treatment and post-treatment, the subjects were tested with a 10 m and 100 m timed gait test, a timed get up and go test, a functional reach test, the difference in thoracic girth at inspiration and expiration, and breaths per minute. The forced vital capacity (FVC) and forced expiratory volume in one second (FEV1) were measured using a spirometer. Results: After ten weekends of an aquatic exercise program, the subjects showed a significant difference in all the test results except for the FEV1 (p<0.05). Conclusion: Intervention with this aquatic exercise program can improve the physical and pulmonary function in people who have had a stroke.

      • 고유감각과 전정감각 업력이 외상성 뇌손상 쥐의BDNF 발현에 미치는 영향

        Purpose : The pu:rposes of this study were to test the effect of proprioceptive and vestibular sensory input on expression of BDNF after traumatic brain injury in the rat. Subject : The control group was sacrificed at 24 hours after traumatic brain lnjury. The experimental group 1 was housed in standard cage for 7 days. The experimental group II was housed in standard cage after intervention to proprioceptive and vestibular sensory(balance training) for 7 days. Method : Traumatic brain injury was induced by weight drop model and after operation they were housed in individual standard cages for 24 hours. After 7th day, rats were sacrificed and cryostat coronal sections were processed individually in goat polyclonal anti-BDNF antibody. The morphologic characteristics and the BDNF expression were investigated in injured hemisphere section and contralateral brain section from immunohistochemistry using light microscope. Result : The results of this experiment were as follows: 1. In control group, cell bodies in lateral nucleus of cerebellum, superior vestibular nucleus, purkinje cell layer of cerebellum and pontine nucleus changed morphologically. 2. The expression of BDNF in contralateral hemisphere of group II were revealed. 3. On 7th day after operation, imrnunohistochemical response of BDNF in lateral nucleus, superior vestibular nucleus, purkinje cell layer and pontine nucleus appeared in group ll. Conclusion : The present results revealed that intervention to proprioceptive and vestibular sensory input is enhance expression of BDNF and it is useful in neuronal reorganization improvement after traumatic brain injury. .

        ICF core-set를 이용한 뇌졸중 환자의 기능수행 분석

        Purpose: The purpose of this study was to investigate level of functioning in patients with stroke using Modified Bathel Index (MBI), World Health Organization Disability Assessment Schedule 2.0 (WHODAS 2.0), and ICF core-set for stroke. Methods: Sixty-four patients with stroke were recruited for this study from nine medical institutes. The ICF core-set for stroke, WHODAS 2.0, and MBI were used to collect subjects' functional levels. ICF core-set was employed here as a standard frame to observe multi-dimension of functioning, that is physiological bodily function, activity and participation (AP) in daily life, and current environmental factors (EF) in patients with stroke. WHODAS 2.0 and MBI were also used in order to have a specific functioning level for subjects. The linkage of each item in WHODAS 2.0 and MBI into the ICF core-set for stroke was examined. Pearson correlation coefficient was used for analysis of their relationships. Results: Functioning level of participants showed moderate resulting from MBI and WHODAS 2.0 ($73.48{\pm}22.27$ and $35.55{\pm}12.53$, respectively). Strong relationship was observed between ICF core-set and WHODAS 2.0, and with MBI. Each item of disability scales was obtained its linkage into ICF in the domain of AP. However, lack of correlation between MBI and ICF in the domain of EF was found due to absence of related factors. Conclusion: MBI was found to be linked mainly into ICF in the domain of AP and to have limited linkage into EF. Therefore, it should be suggested that the ICF concept frame should be used as a multi-dimensional approach to patients with stroke.

        Korean Cultural Adaptation of WHODAS 2.0 (36-Item Version): Reliability and Linking to ICF

        Purpose: This study was to conduct a Korean cultural adaptation of the WHO disability assessment schedule (WHODAS) 2.0: 36-item version. Methods: An internationally standardized process of translation and cultural adaptation of an instrument was used to develop a Korean version of WHODAS 2.0: 36-item version. Linking each item into the international classification of functioning, disability and health (ICF) was also conducted in order to ensure the concepts in the translated instrument were compatible with ICF. All translated versions of the instrument, linking results and feedback from participants were used for the final adaptation of the Korean version of the instrument. The Korean version of the instrument was assessed twice on different occasions to examine inter- and intra-rater reliability, and the intra-class correlation coefficient (ICC) was calculated. Results: Twelve participants were involved in the translation and linking process. Ninety-five volunteers were invited to participate to examine the reliability of the instrument. Fifty participants completed the self-rated version of the instrument and 45 finished the interviewer version. The Korean WHODAS 2.0: 36-item version was found to have excellent reliability: self-rated version and interviewer version reliability coefficients were ICC=0.92 and ICC=0.94, respectively. Thirty-four items of the translated instrument were to be linked to ICF categories. Some adaptation was made; details and a familiar example were added to help respondents answer the questions. Conclusion: The study results show that the adaptation of the 36-item version of WHODAS 2.0 to Korean was successful and the instrument is ready for use in testing its psychometric properties.

        Mapping Items of Functioning Questionnaires into the International Classification of Functioning, Disability and Health: Stroke

        Purpose: The aim of the study was to investigate items of commonly used questionnaires that measure functioning status of persons with stroke and map to the International Classification of Functioning, Disability and Health (ICF). Methods: Eighty-six patients with stroke were recruited from 12 medical institutes for the study. Each item of the Modified Bathel Index (MBI), Stroke Impact Scale (SIS), Mini Mental Status Evaluation (MMSE) and SF-36 were examined and compared its concept with the ICF. Concept linking was performed by 10 health professionals independently. A field test was performed to assess its correlation between those of scales and their linked ICF category sets. Results: It was found that 11 items in MBI was linked to 14 ICF categories, whereas 27 items of MMSE had 10 categories of ICF linked. 60 items of SIS were to be linked with 35 ICF categories. Agreement between professionals in linking was found to be high: 97.5% for MBI items, 78.0%, 78.0%, and 74.8% for MMSE, SIS, and SF-36 respectively. Strong relationship was observed between measurement scales and linked ICF code sets (r=-0.76 for SIS, r=-0.78 for MBI, r=-0.47 for MMSE) whereas there was no relationship was found between SF-36 and its ICF code set (r=-0.06) from the field test. Conclusion: It was found that items of SIS, MMSE and MBI may be linked to ICF categories. Those of linking concept between clinical tools and the ICF could be helpful for clinical data standardization.
