        Effects of Knee Malalignment on Static and Dynamic Postural Stability

        Purpose: This study was conducted in order to determine the effects of knee malalignment including genu varum, valgum, and recurvatumon static and dynamic postural stability. Methods: A total of 80 subjects were enrolled in this study. Subjects who showed over 3 cm in the distance between the knees wereclassified as the genu varum group, and subjects who showed over 3 cm in the distance between the ankles were classified as the genuvalgum group. Subjects who showed over 1 cm in the distance between the patella and a table in prone position were classified as thegenu recurvatum group. Static and dynamic stability were measured as overall, anterioposterior, and mediolateral balance index using aBiodex Balance System. Results: This study showed that knee alignment affected static and dynamic postural stability. In particular, there were significant differencesin the mediolateral stability index among genu varum, valgum group, and the other groups, but no differences in overall and anteriolateralstability index. Significant differences in the anterioposterior stability index were observed between genu recurvatum and theother groups, however, there were no differences in overall and mediolateral stability index. Conclusion: The findings were that knee malalignment affects postural stability toward a specific direction. Treatment to improve posturalstability for treatment of knee malalignment or to prevent falling or injuries is needed and postural stability toward a specific directionaccording to the knee alignment conditions should be considered.

        Pelvic, Hip, and Knee Kinematics of Stair Climbing in People with Genu Varum

        Purpose: This study examined the effects of the lower limb alignment on the pelvis, hip, and knee kinematics in people with genu varum during stair walking. Methods: Forty subjects were enrolled in this study. People who had intercondylar distance ≥4cm were classified in the genu varum group, and people who had intercondylar distance <4cm and intermalleolar distance <4cm were placed in the control group. 3D motion analysis was used to collect the pelvis, hip, and knee kinematic data while subjects were walking stairs with three steps. Results: During stair ascent, the genu varum group had decreased pelvic lateral tilt and hip adduction at the early stance phase and decreased pelvic lateral tilt at the swing phase compared to the control group. At the same time, they had decreased minimal hip adduction ROM at the early stance and decreased maximum pelvic lateral tilt ROM and minimum hip rotation ROM at the swing phase. During stair descent, the genu varum group had decreased pelvic lateral tilt at the early stance and decreased pelvic lateral tilt and pelvic rotation at the swing phase. In addition, they had decreased pelvic frontal ROM during single limb support and increased knee sagittal ROM during the whole gait cycle. Conclusion: This study suggests that a genu varum deformity could affect the pelvis, hip and knee kinematics. In addition, the biomechanical risk factors that could result in the articular impairments by the excessive loads from lower limb malalignment were identified.

        The Effect of a Proximal and Distal Tibiofibular Joint Manipulation on Dorsiflexion and Balance in Individuals with a History of Lateral Ankle Sprain

        Purpose: This study aimed to evaluate the changes in dorsiflexion and balance following proximal and distal tibiofibular joint manipulation in individuals with a history of lateral ankle sprain (LAS). Methods: Fifteen participants with a history of unilateral LAS, exhibiting a restriction in ankle dorsiflexion were included in this study. LAS ankle received a manipulation to the proximal and distal tibiofibular joint, while the opposite control ankle received no manipulation intervention. The outcome measures included ankle dorsiflexion and balance. Ankle dorsiflexion was measured using weight-bearing lunge test. Static and dynamic balances were measured using the overall, anterioposterior, and mediolateral balance index via the biodex balance system. Measurements were obtained prior to and following manipulation. Results: This study showed that ankle dorsiflexion and dynamic balance were improved following the manipulation compared to those prior to the manipulation (p<0.05). There was no significant change in static balance (p>0.05). Conclusion: The joint manipulation technique applied to the ankle of those with a history of LAS appears to improve ankle dorsiflexion and dynamic balance. This suggest that a manipulation to the proximal and distal tibiofibular joint could be provided as preliminary data regarding the prophylactic effects of recurrent LAS.

        긴장성 두통에 대한 두개경부 운동의 효과

        Purpose: This study was done to assess the effect of changes in forward head posture (FHP), neck mobility and headache clinical parameters on episodic tension-type headache (ETTH) and chronic tension-type headache (CTTH) who did craniocervial exercises. Methods: Twelve CTTH subjects and twelve ETTH subjects were studied. Side-view pictures of subjects were taken in both sitting and standing positions, in order to assess FHP by measuring the craniovertebral angle. Cervical range of motion (CROM) was employed to measure cervical mobility. A headache diary was kept to assess headache intensity, frequency, and duration. All subjects did three types of craniocervical exercise over 8 weeks. Measurements were done at pre-treatment, and at 4 and 8 weeks post-treatment. Results: Forward head posture and headache-related clinical parameters showed a significant improvement after craniocervical exercise (p<0.05). Flexion/extension and left/right rotation of CROM was significantly increased after the intervention (p<0.05), whereas changes in left/right bending did not reach statistical significance. Conclusion: This study indicates that craniocervical exercise may be effective in the management of tension-type headache.

        Poor posture of the neck and head long been recognized as a factor contributing to the onset and perpetuation of pain in the head and neck region. The purposes of the study were to evaluate the changes in forward head posture and pressure pain threshold in tension type headache group and control group. Forward head posture were measured to craniovertebral angle and cranial rotation angle. Craniovertebral angle was smaller in tension type headache group(p<0.05), and cranial rotation angle was larger in tension type headache group(p<0.05). Pressure pain threshold was similar between tension type headache group and control group with the exception of right suboccipitalis and left temporalis(p<0.05).

        정적 및 동적 균형 수행력에 대한 임산부 여성의 변화

        Purpose: This study examined whether or not changes in the static and dynamic balance performance occur during pregnancy. Methods: Fourteen pregnant women and fourteen non-pregnant women volunteered to participate in the study. All subjects were tested for their balance performance on the Chattecx Balance System (Chattanooga Group, Inc., USA) under a two platform condition: stable platform and dynamic platform(forward-backward sliding and tilting). The Chattecx Balance System was measured using the postural sway index, anterior-posterior sway index, and medial-lateral sway index. Results: There was a significant difference in the postural sway index and anterior posterior sway index in the static and dynamic platform condition between the pregnant and non-pregnant women (p<0.05). However, there was no difference in the medial-lateral sway index. Conclusion: The postural sway index and anterior-posterior sway index decreases during pregnancy.

        예측된 그리고 예측되진 않은 갑작스런 상지로의 부하 적용시 요통 환자와 정상인의 압력 중심 이동 및 근활성 개시에 미치는 영향

        Purpose: This study was to compare the effect of center of pressure(COP) displacement and muscle activation onset during expected and unexpected sudden limb loading in subjects with low back pain and healthy control subjects. Most studies of COP displacement and muscle activation onset on postural task focused on sudden trunk loading or gross limb movements. Investigation of the COP displacement and muscle activation onset during expected and unexpected sudden upper limb loading deserves similar attention. Methods: For this study, 14 subjects with low back pain and 12 healthy control subjects are participated. Force plate and surface EMG measures were used to determine COP displacement and muscle activation onset under expected and unexpected sudden upper limb loading. Results: COP displacement and muscle activation onset under unexpected sudden upper limb loading were similar in subjects with low back pain and healthy control subjects. However, COP displacement and muscle activation onset under expected sudden upper limb loading were shortened in healthy control subject but not among the subjects with low back pain. Conclusion: The results provide evidence for impaired feed-forward control in subjects with low back pain.

        The Effect of Postural Stability on Genu Varum in Young Adults

        Purpose: Malalignment of the lower limbs may increases the difficulty of maintaining equilibrium. The purpose of this study was to study the effects of genu varum and poor posture in the sagittal plane on postural stability. Methods: We had 27 subjects with varus and 27 normal subjects participate in this study. Subjects for whom the distance between the medial epicondyles in the knee joint was more than 3 cm were classified as varus group, and subjects for whom the distance was less than 3 cm were classified as normal group. The measurements of static and dynamic stability were used overall stability index (OSI), anterioposterior stability index (APSI), and mediolateral stability index (MLSI) using a Biodex balance system. Results: When measuring the static stability index, there were significant differences in the mediolateral stability index between the varus and control groups. When measuring the dynamic stability index, there were significant differences in the overall, anteriorposterior, and mediolateral stability index between the varus and control groups. These results demonstrated that genu varum affects mediolateral movement in static stability, and overall, anterioposterior and mediolateral movements in dynamic stability. Conclusion: As genu varum affects static and dynamic stability in young adults, it increases the risk of injuries or falls. Exercise and surgery are required for realigning the genu varum. Future studies about postural stability in young children and elderly people who have a risk of falls due to lower postural control ability, are needed, as well as in young adults.
