Purpose: To describe the morphological features of a prominent ectasia of the cornea after laser in situ keratomileusis (LASIK). Methods: The morphology of the ectatic corneas were examined using corneal topography, optical microscopy and transmission electron microcopy in 2 cases who had undergone penetrating keratoplasty due to a poor visual acuity induced by progressive corneal ectasia after LASIK. Results: On a topographic examination, the apex of the corneal surface was observed within the central 3 mm zone and the smallest thickness was 116 ㎛ and 271 ㎛ in each case. The histological examination showed that the epithelial layer had become thinner and detached easily. The Bowman’s membrane was broken down and folded. An irregular arrangement of the stromal lamellae with the fibroblastic keratocytes was found. Scar tissue was observed between the epithelium and the Bowman’s layer in the central region. In contrast, the corneal endothelium was intact and no abnormality was found in both cases. Conclusions: The morphologic examination of 2 cases with corneal ectasia revealed a forward protrusion of both the anterior and posterior corneal surface. In addition epithelial detachment, breakage and folding of the Bowman’s membrane and irregular lamellae were found. The 2 cases had greatly thinned and protruding corneas, yet there was no abnormality in the corneal endothelium.
이산화탄소 레이저의 우식 억제 효과에 대한 레이저형광측정 평가

연구의 목적은 이산화탄소 레이저의 유치 법랑질 탈회 억제 및 재경화 효과를 레이저형광측정법을 사용하여 평가하는 것이었다. 사람 유치의 건전 평활면으로부터 법랑질 시편을 제작하여, 코카콜라에 담가 24시간 탈회시키는 전후에 정촛점 연속파 또는 탈촛점 펄스파, 3W 또는 6W로 4초간 직경 약 2 mm의 조사점에 이산화탄소 레이저를 조사하고, Diagnodent 측정을 통하여 탈회억제효과와 재경화 효과를 평가하였다. 탈회된 법랑질에 대한 정촛점 연속파 또는 탈촛점 펄스파 조사의 재경화 효과는 두 군 사이에 유의한 차이가 없었으며 모두 탈회 전과 유의한 차이가 없는 수준으로 감소하였다. 건전 법랑질에 대한 3W 또는 6W 탈촛점 펄스파 조사의 탈회 억제 효과에서 3W군은 탈회 후의 측정치가 유의하게 증가하였고(P<0.05), 6W군은 탈회 후 유의한 증가가 없었다. 탈회된 법랑질에 대한 3W또는 6W 탈촛점 펄스파 조사의 재경화 효과에서 3W군은 레이저 조사 후 유의하게 감소하였으나 탈회 전보다 유의하게 컸고(P<0.05), 6W군은 레이저 조사 후 유의하게 감소하였고(P<0.05) 탈회 전과 유의한 차이가 없었다(P>0.05). 유치 법랑질의 색은 6W로 탈회 후 조사하였을 때 갈색 내지 검은색으로 변색되었고 탈회 전 조사하였을 때 갈색으로 변색되었으며, 탈회 전과 후에 3W로 조사하였을 때에는 색의 변화가 없었다. The purpose of study was to evaluate the effects of the $CO_2$ laser irradiation on demineralization inhibition and rehardening of human primary tooth enamel by laser fluoresecence measurement. Enamel specimens were made from the human primary teeth. The center spots of the specimens about 2 mm diameter were irradiated by $CO_2$ laser at the conditions of focused continuous or defocused pulsed, 3 or 6 W, for 4 seconds, before or after the demineralization by Coca-Cola for 24 hours at $37^{\circ}C$. The Diagnodent was used to measure the degree of demineralization and rehardening. There was no significant difference between focused continuous and defocused pulsed irradiation. 6W irradiation inhibited the demineralization but 3W did not. 6W irradiation rehardened the demineralized enamel but 3W did partially. The color of enamel was changed to brown to black after 6W irradiation but 3W caused no color change. $CO_2$ laser irradiation showed the effects on demineralization inhibition and rehardening of human priamary tooth enamel, and the laser fluoresecence measurement technique seemed to be a valid evaluation method.

To increase the knowledge about the diet factor of the nursing bottle caries, acid production and demineralization of the primary teeth enamel by bovine milk, human milk and soya milk were studied in vitro. 12 experimental solutions were made by combination of the three kinds of milk, saliva and sucrose. Inoculum was prepared by incubation of Streptococcus sobrinus and bacteria from intraoral caries lesion. Highly polished primary teeth enamel specimens were put into the solutions and the latter were incubated at 37℃ for 24 hours. Vickers surface microhardness test and pH measurement were done before and after the incubation. Demineralization potential of soya milk was greater than that of bovine milk and human milk(P<0.05). Saliva increased demineralization more than sucrose (P>0.05). Final pH of soya milk groups was the lowest and that of bovine milk groups was the highest. There was very significant correlation between acid production and demineralization(P<0.001). The results from the present study indicate that bovine milk itself has lower cariogenic potential than human milk but it could produce enough acid to demineralize the primary teeth enamel if it were mixed with saliva and cariogenic bacteria and that the using soya milk with or instead of bovine milk in nursing would increase the risk of nursing bottle caries.
