Legg-Calve-Perthes병에 있어 자기공명 영상을 이용한 대퇴골두와 비구와의 형태학적 연구

목적: LCP병에 있어 자기공명 영상을 이용하여 고관절의 위치변화에 따른 대퇴골두와 비구의 상호관계를 연구하였다. 대상 및 방법: 35명의 LCP 환아(평균나이 7.8세; pillar B군 17예, C군 18예)를 대상으로 이환 된 고관절의 중립, 외전, 외전내회전, 외전-내회전-굴곡, 내전위에서 촬영한 관상면상의 자기공명영상을 이용하여 골두의 골단 돌출도, 비구의 골두 포함정도, 골두내측 관절 간격의 변화를 분석하고 대퇴골두와 비구의 일치도 및 주위 연부조직의 변화를 관찰하였다. 결과: 외전 및 외전-내회전위에서 골두 골단 돌출도의 감소와 비구의 골두 포함정도의 증가는 중립위에 비해 pillar B, C양군에서 통계학적 유의성을 보였으나, 골두내측 관절 간격의 감소는 pillar B군에만 유의성을 보였다(p<0.05). Pillar C군에 있어 내전위에 따른 골두내측 관절 간격의 증가는 고관절 외측의 연부조직 긴장으로 중립위에 비해 현저하지 않았다. (p<0.05). 결론: LCP병에 있어 유치 개념의 치료는 주로 pillar B군에서 적절하며, 경첩외전이 동반된 pillar C군에 있어 외반 절골술은 심한 아탈구를 유발하지 않고 관절의 일치도를 증가시키므로 안전하게 시행될 수 있다. Purpose: We used MRI to evaluate the morphological relationship of the femoral head and acetabulum with changes of hip position in LCP disease. Materials and Methods: The MRIs of 35 patients (17 pillar B, 18 pillar C; mean age: 7.8 years old) were reviewed in neutral, abduction, abduction-internal rotation, abduction-internal rotation-flexion, and adduction positions. The measurements included epiphyseal extrusion index (EEI), head coverage (HC), and medial gap ratio (MGR). The congruence of the hip joint and adjacent soft tissue changes were also studied. Results: Decreased EEI and increased HC in both pillar Band C from neutral to the abduction and abduction-internal rotation positions were statistically significant (p<0.05). However, in decreased MGR, statistical significance was seen only in pillar B. The increase of MGR in pillar C, from neutral to adduction, did not show statistical significance due to the tension of the soft tissues lying lateral to the hip joint. Conclusion: This study supports the basic concept of containment treatment in pillar B and suggests a rationale for valgus femoral osteotomy in cases of pillar C with hinge abduction.

목적: 천골 및 천장관절을 포함한 불안정성 골반골 골절 환자의 치료에 있어 해면골 유관 나사를 이용한 조기 후방 내고정술의 유용성에 대해 보고하고자 한다. 연구 대상 및 방법: 16예의 천골 및 천장 관절을 포함한 불안정성 골반 골절 환자를 대상으로 하였다. 수상기전에 따른 분류상 측면 압박이 4예, 전후면 압박이 7예, 수직 전단력이 4예, 혼합형이 1예였다. 치료는 전방 외고정술과 함께 후방 내고정술을 병행한 경우가 9예(해면골 유관 나사 7예, 경 장골 막대 2예), 전방 외고정술만을 시행한 경우가 5예, 수술적 고정 없이 보존적 요법으로 치료한 경우가 2예였다. 치료결과는 방사선학적 분석과 함께 Matta등의 기준에 따른 임상 결과를 분석하였다. 결과: 전방 외고정술과 함께 후방 내고정술을 동시에 시행한 군(만족 7예, 불만족 2예) 이 전방 외고정술만을 시행한 군(만족 1예, 불만족 4예)이나 보존적 요법으로 치료한 군(불만족 2예) 보다 우수한 임상적 결과를 보였고 방사선학적으로도 부정 유합이나 불유합의 빈도가 적었다. 결론: 해면골 유관 나사를 사용한 후방 내고정은 비 침습적이고 시술이 용이하여 조기에 시행될수 있으며 불안정성 골반골 골절 후 따를 수 있는 여러 가지 합병증들을 막을 수 있는 좋은 치료 방법이라고 생각된다. Purpose: To evaluate the usefulness of early posterior internal fixation(I/F) by cannulated cancellous screw(CCS) in unstable pelvic bone fractures involving the sacrum and sacroiliac joint. Materials and methods: Sixteen cases were evaluated retrospectively. Classification according to the injury mechanism, using Young`s criteria, was as follows: 7 anteroposterior compression, 4 lateral compression, 4 vertical shear and 1 combined mechanism. With respect to treatment, 9 cases were treated by combined anterior external fixation(E/F) and posterior I/F with CCS(7 cases) and transiliac rod(2 cases). Five cases were treated by anterior E/F only, and 2 cases were treated without surgery. We analyzed clinical results using Matta`s criteria, and radiologic abnormalities. Results: The management by posterior I/F with CCS and transiliac rod including anterior E/F has shown superior clinical results(7 satisfactory, 2 unsatisfactory) over the management by only anterior E/F(1 satisfactory, 4 unsatisfactory) or conservative methods(2 unsatisfactory). The rate of malunion and nonunion was also low in the former method in the radiographic analysis. Conclusion: Posterior I/F using CCS is an excellent surgical procedure in the treatment of unstable pelvic bone fracture due to its lessened invasiveness, early applicability, simple surgical technique and its direct effects on the prevention of several complications.

키엔벡 병에서 이차적으로 발생한 장 무지 굴곡건 병적 파열

키엔벡 병에 의한 장 무지 굴곡건의 이차적 병적 파열은 기존의 문헌에서도 3예만이 발표될 정도로 극히 드물다. 저자들은 키엔벡 병에서 이차적으로 발생한 장 무지 굴곡건의 병적 파열 1예를 경험하였고, 그 치료로 괴사된 월상골의 적출술 후 양측 장 수장건을 이용한 건구 삽입술 및 건 이식술을 통한 장 무지 굴곡건 재건술을 시행하였다. 수술 후 1년 추시 결과 환자의 좌 무지 지간 관절의 정상적인 능동적 운동의 회복과 완관절의 동통 감소 및 수술 전의 능동적 운동 범위 유지 등으로 만족할 만한 결과를 얻을 수 있었다. 저자들은 이러한 극히 드문 키엔벡 병에서 이차적으로 발생한 장무지 굴곡건의 병적 파열 1예에 대한 치료 경험을 문헌 고찰과 함께 보고하는 바이다. A pathologic rupture of the flexor pollicis longus tendon secondary to Kienbock disease is extremely rare with only three cases being reported in the literature. We encountered a case of a pathological rupture of the flexor pollicis longus tendon secondary to longstanding Kienbock disease. The treatment included tendon ball insertion after excising the collapsed lunate and a flexor pollicis longus tendon reconstruction with autogenous palmaris longus tendons. Satis-factory results with a restoration of the active motion of the interphalangeal joint of the thumb, maintenance of the preoperative active range of motion of the wrist and markedly reduced pain was achieved after a one year follow up. We describe this case with a review of the relevant literature.
