        한탄바이러스에 감염된 사람제대정맥 내피세포에서 ICAM-1 및 VCAM-1 의 발현

        배 경 : 신증후출혈열은 한탄바이러스에 의한 급성 발열성 질환으로 병인에 여러 가지 염증매개불질들이 알려져 있다. 방 법 : 본 연구에서는 한탄바이러스에 감염된 사람 제대정맥 내피세포에서 ICAM-1 및 VCAM-1의 발현 정도를 효소면역측정법을 이용하여 정량 측정하였다. 결 과 : 한탄항원에 대한 반응은 감염 후 48시간부터 중가하기 시작하였으며. ICAM-1의 발현은 감염 후 3시간이 지나면서 급격히 중가하였다. VCAM-1의 발현은 ICAM-1의 발현보다 약간 지연되었으며 감염 후 4시간이 지나면서 급격히 증가하였다. 이 같은 ICAM-1, VCAM-1 발현 증가는 희석되지 않은 바이러스를 감염 시켰을 때 더 현저하였으며 희석된 바이러스를 참염시켰을 때에는 반응이 미약하였다. 결 론 : 한탄바이러스가 사람제대정맥 내피세포에 감염되면 ICAM-1 및 VCAM-1의 발현이 증가됨을 관찰하였고 이를 효소면역측정법을 이용하여 정량격으로 비교하였다. Purpose: Hemorrhagic fever with renal syndrome (HFRS) is an acute febrile disease caused by Hantaan virus. It is believed that various kinds of inflammatory mediators and cytokines might be involved in its pathogenesis. Methods : We studied the expression of adhesion molecules, including ICAM-1 and VCAM-1 on Hantaan virus-infected human umbilical vein endothelial cells (HUVECs) with ELISA. Results : Hantaan antigen increased gradually 48 hours after virus infection, ICAM-1 increased rapidly 3 hours after virus infection and VCAM-1 increased rapidly 4 hours after infection, a little later than ICAM-1. The patterns of the change of reaction were similar between the different concentrations of virus solution that had been adsorbed on HUVECs, but the expression of ICAM--1 or VCAM-1 was greater when HUVECs were infected with stock virus rather than when infected with tenfold diluted virus. Conclusion : We report the quantification using ELISA of ICAM-1 and VCAM-1 expression of HUVECs that have been infected by Hantaan virus. Increase of their expression would play an important role in the pathogenesis of HFRS.

        성인 당뇨병 환자의 혈청내 성장호르몬과 Somatomedin-C 치에 관한 연구

        The serum levels of growth hormone and somatomedin-C were determined in g patients with insulin-dependent diabetes mellitus(IDDM), 8 patients with non-insulin-dependent diabetes mellitus (NIDDM), and 9 normal controls, who admitted at Chung-Ang University hospital from July, 1985 to March, 1986. We observed the effects of diabetes and its con trol on the levels of serum growth hormone and somatomedin-C, and the relationship between the levels of serum growth hormone and somatomedin-C, and fasting blood sugar. hemoglobin A1c(HbA1c), serum C-peptide, body mass index and diabetic retinopathy. The results were as follows; 1) Serum growth hormone levels were significantly elevated in patients with IDDM than that of controls (3. 97±1.18ng/ml versus 2. 22±1.54ng/ml, p<0.05), but serum somatomedin-C levels were not significantly different between each group-And serum growth hormone levels in diabetic patients were inversely correlated with serum somatomedin-C levels(r=0.66, p$lt;0.01). 2) Diabetic patients with ideal body weight showed significantly higher serum growth hormone- levels than patients with overweight(4.04±1.46ng/ml versus 2.86±0.41ng/ml, p$lt;0. 05) But serum somatomedin-C levels were not significantly different between each group. 3) Serum growth hormone levels were significantly decreased from 3.971±1.18ng/ml to 2.23±0.63ng/ml in patients with IDDM(p$lt;0.05) and from 3,65±0.44ng/ml to 1.78±0.80ng/ml in patients with NIDDM after the control of diabetic state(p$lt;0. 05), but serum somatomedin-C levels were significantly increased from 40. 5±17.5ng/ml to 60.8±21.7ng/ml in patients with IDDM. (p<0.001) and from 60. 5±14.7ng/ml to 78.2±18.2ng/ml in patients with NIDDM after the control of diabetic state(p$lt;0,05). 4) Serum growth hormone levels in diabetic patients were positively correlated with serum fasting blood sugar levels(r=0.80 P$lt;0.01), but inversely correlated with serum C-peptide levels (r= 0 75, p$lt;0.01). Serum somatomedin-C levels in diabetic patients were inversely correlated with serum fasting blood sugar levels(r=-0.82 p, p$lt;0.0l), but positively correlated with serum fasting C-peptide levels(r= 0.63, p$lt;0.01). But there was no significant correlation between serum somatomedin-C levels and HbA1c 1evels. 5) Serum growth hormone levels in patients with proliferative retinopathy were significantly higher than that in patients without retinopathy(3. 56±0.52ng/ml versus 2.50±0.35ng/ml, p$lt;0. 05), but the levels of somatomedin-C were- significantly lower in patients with proliferativeretinopathy than that in patients without retinopathy(34.0±12.9ng/ml versus 67.2±12.3ng/ml, p$lt;0, 001). Above results suggested that serum growth hormone and somatomedin-C levels may be closely correlated with diabetic control in adult diabetic patients. And both hormones, especially growth hormone, may be related to the development of diabetic retinopathy.

        A case of AL type primary amyloidosis is reported. Diagnosis was made by gastrofiberscopic mucosal biopsy and confirmed by polarizing, electron microscopy, serum protein electrophoresis and immune elecfrophoresis. We discussed this case of primary amyloidosis with review of literature.
