Background: Thiopental and propofol are commonly used intravenous induction agents and entropy is a new method of assessing depth of anesthesia. In this study we evaluated the entropy responses to endotracheal intubation during anesthetic induction with thiopental or propofol. Methods: Forty ASA class I and II adult female patients undergoing elective surgeries were enrolled into this randomized, prospective study. At 1 minute after fentanyl 2μg/kg, thiopental 5 mg/kg (group T) or propofol 2 mg/kg (group P) was administered for induction. And endotracheal intubation was performed following succiny choline 1 mg/kg. From pre-induction till post- intubation 10 minutes, the entropy and hemodynamic variables were measured and compared. Results: The entropy following intubation increased significantly in both groups. But entropy changes in group T were greater than those in group P remained below 65. Conclusions: Our results using the entropy to monitor anesthetic depth during induction and intubation, suggest that thiopental 5 mg/kg is more likely to be associated with lighter planes of anesthesia and consequent risk of arousal reaction following endotracheal intubation than propofol 2 mg/kg. Background: The use of tourniquet is associated with severe hemodynamic changes, particularly in elderly patients. This study examined the effects of continuously infused remifentanil or nicardipine with esmolol on the attenuation of the hemodynamic changes with enflurane inhalation during total knee arthroplasty in elderly patients. Methods: Thirty elderly female patients undergoing total knee arthroplasty were enrolled into this randomized, prospective study. Remifentanil (group R) or nicardipine with esmolol (group EN) was infused using a titration method to maintain a mean arterial pressure (MAP) of 90 mmHg to 70 mmHg during tourniquet inflation. In addition, the inhalation of enflurane was adjusted to guarantee a hypnotic state based on entropy. The hemodynamic variables, inhaled concentration of enflurane, recovery time, and postoperative pain characteristics were measured and compared. Results: Following tourniquet inflation, the MAP was increased and maintained in the target since 20 min after that in both group. Following tourniquet deflation the MAP was decreased significantly in both groups and more patients in group R required a vasopressor. Group R inhaled a lower enflurane concentration and showed more rapid recovery than group EN. There were no significant differences in the level of postoperative pain between the two groups. Conclusions: These results suggest that remifentanil is more likely to be associated with hypotension after tourniquet deflation and require more attention than nicardipine with esmolol. Background: Both ondansetron and granisetron exert their antiemetic effects via a blockade of the 5-hydroxytryptamine 3 receptor (5-HT3R). Because the 5-HT3R is a member of a superfamily of ligand-gated ion channels and has structural similarities to the nicotinic acetylcholine receptor (nAChR), a 5-HT3R antagonist may also inhibit the nAChR. This study examined the effects of 5-HT3R antagonists, ondansetron and granisetron, on rocuronium-induced neuromuscular blockade in vitro. Methods: Rat phrenic nerve-hemidiaphragm preparations were isolated and allocated randomly into seven groups (control, 1, 10, 100μg/ml of ondansetron, 0.1, 1, 10μg/ml of granisetron). Two studies were carried out using single twitch responses. In the cumulative dose-response study, rocuronium 1μg/ml and each doses of ondansetron or granisetron were administered simultaneously, and incremental 0.5μg/ml doses of rocuronium were added to obtain more than 95% neuromuscular twitch inhibition. ED5, ED50, ED90, and ED95 of rocuronium in each group were calculated using a logistic model. In the partial curarization study, the twitch heights were measured after administering ondansetron or granisetron (10 minutes after administering 3μg/ml rocuronium) and were measured 10 minutes later. The 2 twitch heights were then compared. Results: In the cumulative dose-response study, ondansetron 100μg/ml and granisetron 10μg/ml significantly reduced the ED50 of rocuronium (P < 0.05). There were no intergroup differences in the partial curarization study. Conclusions: High concentration of ondansetron and granisetron enhanced the neuromuscular blockade of rocuronium. Granisetron enhanced the neuromuscular blockade of rocuronium more potently than ondansetron. (Korean J Anesthesiol 2006; 51: 94~100)
소형선박 대상 충돌·좌초 및 추천항로 서비스 정확도 비교 방안 연구

정부는 국내 선박을 대상으로 해양사고를 줄이고 안전성을 향상시키기 위해 한국형 e-Navigation 서비스(정식명칭: 지능형 해상교통정보서비스)를 개발하여 운영하고 있다. 이 중 대형선을 항해한 선장과 항해사의 경험을 바탕으로 선박 충돌·좌초 모니터링 서비스와 추천항로 서비스가 개발되었다. 그러나 국내의 경우 이러한 서비스를 이용하는 대상은 소형선이 차지하는 비중이 크기 때문에 대형선을 기반의 서비스를 소형선에 적용하기에는 다양한 애로사항과 추가 요구사항이 나타나고 있다. 이를 개선하기 위해 해상디지털 교통정보를 활용한 데이터과학 기반 소형선용 알고리즘 모델을 개발하고 있으며, 개발된 모델의 검증 및 평가하고 기존 서비스와 정확도를 비교하는 것이 중요하다. 본연구에서는 개발된 알고리즘 모델의 성능 평가 방안을 제안하고 기존 e-Navigation 서비스와의 정확도를 비교 할 수 있는 방안을 연구하였다. To reduce maritime accidents and improve safety for domestic ships, the government has developed and is operating the Korean e-Navigation Service. The ship collision and grounding monitoring service and the route planning service were developed based on the experience of captains and navigators who navigated large ships. However, in Korea, small ships account for a large portion of the users of these services, so there are various difficulties and additional requirements for applying services based on large ships to small ships. To improve this, a data science-based algorithm model using maritime digital traffic information is being developed for small ships, and it is important to evaluate the performance of the developed model and compare its accuracy with existing services. In this study, we propose a performance evaluation method for the developed algorithm model and study how to compare and evaluate its accuracy with existing e-navigation services.
