        유전자 재조합 표피 성장인자가 백서의 피부 전층 창상치유에 미치는 영향

        The wound healing effect of topical application of the recombinant human epidermal growth factor(rhEGF) on full-thickness dermal injury was investigated. Two full-thickness excisions were made on the back of the experimental animals. The rhEGF was applied twice a day and the rate of wound closure was measured every day for 14 days. On the seventh postoperative day, the histological findings of epithelization and granulation were examined by Massons tichrome stain, and immunohistochemical staining for proliferating cell nuclear antigen (PCNA) and α-smooth muscle actin(α-SMA). The wound size was a significant reduction in the rhEGF treated groups as compared with the control group (p<0.05). However, there was no statistical difference in the wound size among the concentrations of the rhEGF treated group. Histological examination revealed that epithelization and granulation was increased significantly in the rhEGF group compared to control group (p<0.01, 0.05). PCNA and α-SMA immunoreactive cells were observed at the margin of wound and the rhEGF treatments significantly increased the number of PCNA and α-SMA immunoreactive cells as compared to those of control group (p<0.01, 0.05). Taken together, these findings suggest that rhEGF enhance the epithelial wound healing by the stimulate of cell proliferation. The wound contraction might be also affected by rhEGF application.

      • 실리콘 고무를 이용한 융비술

        실리콘 고무는 신체로부터 거부반응이 거의 없으며 간편하게 사용할 수 있고 모양과 크기에 변화가 없고 유착을 일으키지 않는 등의 장점이 있어 실리콘 고무를 사용한 융비술이 많이 시행되고 있으나 이 삽입물에 대한 모양, 길이 그리고 두께에 대하여는 구체적으로 연구가 되지 않았다. 이에 저자는 1991년 3월부터 1998년 10월까지 실리콘 고무를 이용하여 융비술을 시행받은 50명의 환자를 대상으로 하여 수술에 사용된 실리콘 고무의 모양, 길이 및 두께를 조사하였다. Augmentation rhinoplasty is one of the most popular aesthetic procedure in the orient because the typical asian nose is the characterized by a broad, relatively flat dorsum associated with a wide lobule that often exhibits alar flaring and columellar retraction. Although autogenous bone, autogenous cartilage and silicone rubbed are commonly used as nasal augmentation materials, nothing is suitable for the anatomical feature and physiologic function of the nose. An ideal implant should not be immunogenic, be easily formable, not change in size and figure and not produce adhesion for easy elimination. In augmentation rhinoplasty with silicone rubber, it is not uncommon to observe infection or hematoma as early post-operative complication and deviation or extrusion of implants as late post-operative complication. When silicone rubber was inserted through alar rim incision, the implant could be easily inserted under adequate dissecting pocket so that the infection rate was low due to smaller hematoma and edema. Silicone implant(boat and L-shape) was used in all the cases from 1991 to 1998 for the nasal augmentation. The results are as follows : 1) The postoperative follow-up ranges from 1 to 18months(mean follow-up: 5 months). 2) The patients' age ranges from 16 to 52 year-old age (mean age : 26 year-old ). 3) The mean length of silastic implant used in augmentation rhinoplasty is 4.17cm. 4) The L-shaped silastic implant is used mode frequently than boat shape(34 : 16). 5) The complications include one case of infection and one case of implant deviation.

        대구증의 수술적 교정

        Macrostomia, also called a transverse or lateral facial cleft, is a relatively rare malformation.Although many surgical procedures have been introduced, and no gold standard has yet been established. Moreover, most papers published in Korea were based on the results of the research conducted on the very limited number of patients, and for this reason its findings do not offer sufficient clinical reliability.We devised a modified commissuroplasty as follows: First, new commissure was placed 1 or 2mm inside when compared with the opposite side so that the commissure may not look longer than usual because of the scar on the side. Second, z-plasty of about 5mm was performed on the nasolabial fold to prevent the displacement of the new commissure on its lower part and avoid a continuation of a scar with the medial flap placed upward.We treated 32 cases of macrostomia from August 1, 1998 to July 1, 2002. We obtained relatively satisfactory clinical results by using this modified commissuroplasty. Based on our experience, we intend to present a clinical analysis and an operation technique of our own derived from the classic commissuroplasty, so that we may contribute to the diagnosis and treatment of the patients in the future.

        응고인자 VII 부족 환자에서의 악교정 수술: 증례보고

        Purpose: Congenital factor VII(FVII) deficiency is a rare bleeding disorder and surgery can cause excessive bleeding due to an extrinsic pathway problem. It can be diagnosed by increased PT and decreased FVII level in coagulation test. Symptom varies according to the level of FVII, but it is essential to prevent intraoperative excessive bleeding. Methods: In this report, we described the orthognatic surgery experience in a mandibular prognathism patient with congenital F'VII deficiency, in which recombinant activated factor VII(rFVIIa) was used to manage the bleeding. Rsults: We could get a successful result without any complication and there was minimal intraoperative bleeding. Conclusion: The orthognathic surgery could therefore be safely performed in patients with congenital factor VII deficiency using rFVIIa.

        백서의 전층결손 창상모델에서 Recombinant Human Epidermal Growth Factor의 Poloxamer 제형 농도에 따른 창상 치유효능의 비교

        This study was designed to investigate the optimal poloxamer concentration in the mixed solution of recombinant human epidermal growth factor and poloxamer which can be effective in the wound healing process. Two full-thickness excisions were made on the back of the experimental animals. Recombinant human epidermal growth factors(RhEGF) containing different poloxamer concentrations were applied twice a day and the rates of wound closure were measured every day for 14 days. On the 7th and 14th postoperative day, the histological analysis for epithelization and granulation were performed using computerized imaging analysis system after Masson's trichrome stains. The healing times 50% were significantly reduced in the RhEGF containing 0, 3 and 6% poloxamer treated groups as compared with both the non treated control and vehicle control group(p<0.05). However, there were no statistical differences in the healing times 50% in the RhEGF containing 10, 15 and 20% poloxamer treated groups as compared with both the non treated control and vehicle control group. Histological examinations revealed that epithelization and granulation were increased significantly in the RhEGF containing 0, 3 and 6% poloxamer treated groups as compared with control group and vehicle control group at the 7th day after operation(p<0.05).In conclusion, these findings suggest that RhEGF may enhance the epithelial wound healing process through stimulating cell proliferation. The concentration of 0, 3 and 6% of poloxamers can be applied to stabilize and enhance the wound healing effect of RhEGF for clinical application.

        언어발달시기의 구개누공으로 인한 구개인두부전: 증례보고

        Ppupose: Velopharyngeal insufficiency (VPI) during articulation development period can cause compensatory articulation like glottal stop, which can maintain VPI symptoms such as hypernasality and nasal emission despite of recovered velopharyngeal function. Methods: A girl visited our department with speech problems such as hypernasality, compensatory articulation patterns, and nasal air emission. She had history of postpalatoplasty fistula which was repaired after 1 year follow-up. Results: After treated with speech therapy without surgery, her hypernasal speech problem was corrected. From this, we could assume that the fistula during articulation development period induced VPI, which leads to glottal substitution and this compensatory articulation maintained VPI though the fistula was repaired. Conclusion: In this report, we could observe that palatal fistula in articulation development period can have detrimental effect on articulation, and also we could confirm that evaluation of soft palate mechanism with using speech sample without compensatory articulations prevent the patient from unnecessary operation.

      • del Campo 내안각 성형술과 Tutoplast^ⓡ를 사용한 전두근 현수법을 이용한 검열 축소증의 교정

        검열 축소증은 역 내안각췌피, 안검하수증, 양내안각 과다 격리 등의 특징을 나타내는 비교적 드문 선천성 기형으로서 상염색체 우성으로 유전되는 것으로 알려져 있다. 전통적으로 검열 축소증의 수술적 교정으로는 Mustarde's four flap을 이용한 내안각 성형술과 대퇴근막을 이용하여 전두근에 걸어주는 수술이 이용되어 왔어나 작도의 어려움, 과다한 반흔 형성, 재발 등의 합병증 등이 보고되었다. 이에 최근 1년간 본원을 찾아온 6예의 검열 축소증 환자들을 대상으로 del Campo법을 이용한 내안각 성형술과 Tutoplast dura를 이용한 전두근 현수법을 실시함으로써 만족할만한 결과를 얻었기에 보고하고자 한다. Blepharophimosis is a rare congenital anomaly characterized by epicanthus inversus, blepharoptosis, telecanthus and periorbital abnormality. Tradiationally, surgical correction of blepharophimosis divided two methods. First, epicanthus inversus was corrected by Mustarde's four flap technique and medial canthal tendon were fixed into the bony openings of the medial orbital wall by transnasal wiring. Second, blepharoptosis was corrected by frontalis suspension with fascia data. This methods operated one stage or two stage. Many authors were reported their clinical experiencce and surgical procedure for correction of blepharophimosis. Past 1 years, we have treated 6 cases of blepharophimosis using our procedure in which del Campo medial canthoplaty and frontalis sling with Tutoplast dura were performed in two stages. A total 6 patients were reviewed May 1987 to May 1988 with a mean follow-up period of 11 months. They were 5 females and 1 males. 4 cases were in the one family. The purpose of this study is to present a technique we use in treating this anomaly. We experienced satisfactory appearance without hypertrophic scar and other complication using our method.

        점막하 구개열 치료에 있어 Furlow 구개성형술 전후 언어 치료의 유용성

        Furlow palatoplasty has been favored by many plastic surgeons as the primary treatment for the velopharyngeal insufficiency associated with submucous cleft palate. The purpose of this article is to introduce an efficacy of Furlow palatoplasty and speech therapy performed on patients who were diagnosed belatedly as having submucous cleft palates. From 2002 to 2004, four submucous cleft palate patients over 5 years of age with velopharyngeal insufficiency received Furlow palatoplasty. The patients were evaluated through the preoperative perceptual speech assessment, nasometry, and videonasopharyngoscopy. Postoperatively, two patients achieved competent velopharyngeal function in running speech. One of the remaining two could achieve competent velopharyngeal function with visual biofeedback speech therapy and the other could not use her new velopharyngeal function in running speech because of her age. Speech therapy can correct the articulation errors and thus improve the velopharyngeal function to a certain extent by eliminating some compensatory articulations that might have an adverse influence on velopharyngeal function. This study shows that Furlow palatoplasty can successfully correct the velopharyngeal insufficiency in submucous cleft palate patients and speech therapy has a role in reinforcing surgical result. But age is still a restrictive factor even though surgery was well done.
