Objective: This case reports on the effect of traditional Korean medicine (TKM) treatment in a patient with psychogenic dystonia. Methods: A patient with generalized tremor and gait disturbance diagnosed as psychogenic dystonia was treated with Yokukan-san-gami, Jakyakgamcho-tang, Yikgeebohyul-tang, acupuncture, moxibustion, and cupping for 6 weeks. Symptom improvement was evaluated with the unified dystonia rating scale (UDRS) and the Toronto Western spasmodic torticollis rating scale (TWSTRS). Results: After 6 weeks of TKM treatment, the UDRS and TWSTRS decreased, and the patient's clinical symptoms, including generalized tremor and gait disturbance, improved. Conclusions: This case shows that TKM treatment may be effective for improving psychogenic dystonia.

Laryngeal amyloidosis is a rare benign disease. It is characterized by extracellular deposition of homogeneous and eosinophilic protein material in the form of fibrils. Diagnosis of this disease is made by histologic examination of involved tissue. It is ususlly primary or localized but rarely associated with a systemic or generalized disease. We present a case of primary laryngeal amyloidosis that was treated with KTP 532 laser vaporization.

제4차 산업혁명 시대의 범죄유형 변화와 대응 방안

제4차 산업혁명의 주요기술들인 로봇, 인공지능, 사물인터넷, 빅데이터 등을 통해 삶의 질이 향상되는 긍정적이 측면이 있다. 그러나 이런 기술들이 악용되어 새로운 유형의 범죄가 발생할 것으로 보이는 부정적인 측면도 나타날 것으로 보인다. 본 연구는 제4차 산업혁명시대의 핵심 기술들이 관련된 대표적인 범죄사례에 대하여 살펴보았고, 이를 바탕으로 향후 사이버범죄 수준을 넘어서는 새로운 범죄유형을 예상해 보면, 데이터 보안성 저하에 따른 사생활 침해와 감시위험의 증가, 민간사물 인터넷을 이용한 지능형 해킹범죄의 등장, 드론과 자율주행차를 이용한 국가중요시설의 침범과 공격, 3D 프린팅 기술을 이용한 생체조직 재생과 불법총포류의 제작, 인공지능 로봇 등 악용한 범죄, 사이버불링과 사이버테러, 비트코인 악용범죄 등이 나타날 것으로 보인다. 급속한 과학기술의 발달에 따른 여러 범죄는 정보화 사회의 본질적 부분에 까지 영향을 미칠 것으로 예상되므로 정보기술의 효과는 최대한 상승시키면서 문제점은 최소한으로 축소될 수 있도록 여러 정책을 수립하여 대응하도록 해야 한다. 시민이 참여하고 의견이 수용되는 범죄예방 체계가 구축될 수 있도록 과학기술의 연구개발과 정책연구를 통해 대응방안의 마련이 요구되는데 사이버위험과 사이버보안의 중요성에 대한 사회적 인식의 확산, 사이버보안관련법률제정 및 조직 강화, 개인정보 보호제도의 강화, 사이버위험 관리체계 구축, 형사절차의 보완, 형사사법기관의 변화, 사이버보안 국제협력체계 구축, 처벌규정의 정비, 신종범죄의 전문수사인력 양성, 비트코인을 악용한 범죄에 대한 기술적 대응방안, 사이버테러 관련 정보강화, 운영체제 다변화 및 방화벽 프로그램 권고와 같은 대응방안의 마련이 요구된다. This study aims to analyze violations of citizens privacy and increased risk of surveillance due to reduced data security, the emergence of intelligent hacking crimes using the Internet of Private Things, the invasion and attack of national critical facilities using drones and self-driving cars, the production of biocas and illegal guns using 3D printing technology, crimes using artificial intelligence robots, cyberbullying and cyber terrorism, and bitcoin abuse crimes that are all expected to emerge in the future. It is necessary to prepare countermeasures through research and development and policy research of science and technology so that a crime prevention system where citizens can participate and accept opinions can be established. It will be recommended to develop social awareness on cyber risks and the importance of cyber security, enact and strengthen cyber-security related laws and organizations, strengthen personal information protection systems, establish a cyber risk management system, supplement criminal procedures, establish a criminal judicial system, establish a cybersecurity international cooperation system, overhaul punishment regulations, train professional investigators for new crimes, strengthen cyber terrorism-related information, diversify operating systems and establish a secure firewall program.

호텔 주방의 물리적 환경요인과 조리 종사자의 조직몰입 및 직무성과 간 관계 연구: 조직몰입의 매개효과 중심으로

본 연구의 목적은 호텔 주방의 물리적 환경요인과 조직몰입, 직무성과 간 영향관계 그리고 조직몰입 정도에 따라 직무성과 간 영향관계를 검증하고, 더불어 주방의 물리적 환경요인과 직무성과 간의 영향관계에서 조직몰입이 어떠한 매개역할을 하는지 알아보기 위함이다. 가설 검증 결과, 첫째, 호텔 주방의 물리적 환경요인의 쾌적성, 편리성 요인은 조직몰입에 유의한 영향을 미치며, 둘째, 호텔 주방의 물리적 환경요인의 쾌적성, 편리성, 안전성은 직무성과에 영향을 미치는 것으로 분석되었다. 이에 호텔 주방 내 환기 및 온도, 습도 등 쾌적한 주변 요소에 대한 관리와 작업동선의 흐름을 원활하게 처리할 수 있는 주방설계가 필요하며, 주방의 조리 장비, 기기 등 안전한 관리와 시설 안전에 대한 정기적인 교육이 이루어져야 한다. 셋째, 호텔 주방 종사자의 조직몰입과 직무성과는 유의한 영향관계가 있으며, 업무 특성을 고려하여 조직몰입 강화를 위한 근무환경 개선이 요구된다. 마지막으로 호텔 주방의 물리적 환경요인과 직무성과 간 관계에서 조직몰입은 매개역할을 하며, 조리 종사자의 관점에서 능률과 생산성을 강화할 수 있는 효율적인 주방설계와 배치 등 운영체계 관리가 필요하다. The purpose of this study is to verify the influence relationship of physical environmental factors of hotel kitchen with organizational commitment, job performance and to investigate the mediating effects of organizational commitment in the influence relationship between the physical environmental elements of kitchens and job performance. Results of verifying the hypothesis are as follows. First, the pleasantness and convenience factors of physical environment had a significant impact on organizational commitment. Second, it was analyzed that pleasantness, convenience and safety of physical environment had impact on job performance. Third, organizational commitment and job performance of hotel kitchen w orkers showed significant influence relationships, and when considering work characteristics, improving work environment to strengthen organizational commitment is needed. Lastly, it is necessary to provide the operating system management that can improve efficiency and productivity from the perspective of kitchen employees while allowing organizational commitment to act as a medium in the relationship between physical environment factors of hotel kitchen and job performance.

경남지역의 돼지톡소플라즈마병 감염실태 조사

Toxoplasma gondii is a species of parasitic protozoa in the genus Toxoplasma. The definitive host of T. gondii is the cat, but the parasite can be carried by the vast majority of warm-blooded animals, including humans. It is often found in the tissues of food animals including pigs and sheep. To determine the regional prevalence of infection with T. gondii, bloods (n=300) from domestic pigs and tissues (n=200) from slaughter pigs in Gyeongnam province were tested using an enzyme-linked immunosorbent assay (ELISA) and a polymerase chain reaction (PCR) for detection of antibody and antigen. A total of 115 sero-positive pigs were identified for a prevalence rate of 38.3%. Of the 50 herds from domestic pigs tested, 34 had at least one sero-positive pig for a herd prevalence rate of 68.0%. Sero-positive rates of pigs in fattening farm were higher than that of pigs in breeding company. Sero-positive rates of sows were higher than that of growing pigs. Seasonally, sero-positive rates of pigs were highest in winter (80.0%) and lowest in spring (23.8%). According to farm size, sero-positive rates of pigs were higher in small size farms (≤2,000) than that of big size farms (>2,000). However, none of the bloods (n=300) from domestic pigs and tissues (n=200) from slaughter pigs were positive for T. gondii specific DNA by PCR.

도시 녹지경관의 경제적 가치평가 - 독립공원을 중심으로 -

이 연구의 목적은 도시 내에 존재하는 녹지경관의 경제적인 가치를 분석하는 것이다. 연구의 대상지는 서울시 서대문구 현저동에 위치한 독립공원이다. 경제적인 가치평가에 앞서, 녹지경관으로서의 양적인 특성을 파악하기 위하여 시각량 분석을 수행하였다. 시각량 분석의 결과, 공원부지 전체 면적에 대한 녹지 면적의 시각량의 비는 약 64%로 나타났다. 이 연구는 헤도닉 가격기법을 이용하여, 주변의 주택가격에 내재하고 있는 녹지경관의 가치를 평가하였다. 헤도닉 가격기법은 특정한 재화에 내재해 있는 속성의 가치를 평가하는 기법이다. 분석을 위하여 일반화된 Box-Cox 모형을 사용하였으며, $3.3m^2$당 가격을 종속변수로 하고, 주택특성과 경관특성을 독립변수로 설정하였다. 독립공원의 조망가치를 분석한 결과, $3.3m^2$당 가격에서 약 2.2% 정도를 차지하였다. 이는 이 연구에서 사용된 변수들 중에서 두 번째로 영향력이 높은 수치이다. 이러한 결과는 거주자들이 도시의 녹지경관에 대해서 거주환경을 결정하는 매우 중요한 요인 중의 하나로서 인식하고 있으며, 나아가서 도시의 녹지경관이 거주자들의 삶의 질 향상과 자산가치의 상승 등 중요한 역할을 하고 있다는 사실을 나타낸다. 그리고 이 연구의 결과는 녹지경관이 중요한 도시 기반시설로서 다루어질 수 있는 가능성을 시사한다. 또한 녹지의 가치에 대한 정량적인 평가결과는 녹지의 보존 또는 조성에 대한 타당성을 합리화할 수 있는 근거자료로서 제시될 수 있을 것으로 판단된다. The purpose of this study is to evaluate the economic value of an urban landscape. The site of this study was Independence Park in Seoul. Before measuring economic valuation, an amount of view analysis was performed to learn the visual characteristics of the landscape. As a result, the green space ratio of the park is approximately 64%. This study estimates the intrinsic value of an urban landscape that might be included in housing prices using a hedonic price model. The hedonic price model is a statistical analysis that is often used to estimate intrinsic values of certain attributes of a product. The Box-Cox model was adopted as an analysis tool while the housing price for $3.3m^2$ was used as a dependent variable and housing and landscape features as independent variables. Results show that the value of the landscape of the Independence Park is approximately 2.2% of the housing market price. The Landscape variables of the park is the second most significant of the 8 variables. This shows that residents perceived the view of the urban landscape as one of the most significant factors in their living environment. The study also indicates that urban landscapes play important roles in improving quality of life and in influencing housing prices. The implication of the study can be said to be the potential of the urban landscape as a significant urban infrastructure. These results can be used to help make policy decisions to preserve and/or develop urban landscapes.

섬망을 호소하는 비경련성 간질 중첩증 환자에 대한 복합 한의치험 1례

Nonconvulsive Status Epilepticus(NCSE) refers to continuous epileptic condition with altered mental status and behavioral symptoms, but without convulsive movement at least 10 minutes. In Salzburg criteria, well known as the diagnostic criteria of NCSE, it is diagnosed with clinical symptoms, electroencephalogram (EEG), and effects before and after the use of antiepileptic drugs (AEDs). Commonly being used to treat NCSE, AEDs are likely to have adverse effects. In the present case, a 85-year-old female NCSE patient complaining delirium underwent combined Korean medical treatment by acupuncture and herbal medicine for 13 days. The effect of treatment was assessed with delirium scales including Delirium Rating Scale-Revised-98(DRS-R-98) and Assessment Test for Delirium & Cognitive Impairment(4AT) per 2 days. After the treatment, both DRS-R-98 and 4AT scores decreased, and orientation and mental status of patient improved. This case report suggests that Korean medical treatment might be an effective option without side for those NCSE patients complaining delirium.
