        Two Cases of Acute Mastoiditis with Subperiosteal Abscess

        The incidence of mastoiditis in pediatric age has consistently increased over the last two decades due to increase of antibiotic-resistant bacteria. Compared to young patients, occurrence of acute otitis media complications such as acute mastoiditis and subperiosteal abscess is relatively low in adults. Various treatments for acute mastoiditis with subperiosteal abscess such as tympanostomy tube insertion, intravenous antibiotics, and postauricular incision and drainage have avoided the morbidity and necessity of mastoid surgery. Recently, many studies have indicated mastoidectomy only in cases of severe complications or failure of disease improvement after antibiotic treatment and myringotomy. In this report, we present two cases of successful treatment of subperiosteal abscess and discuss the management of acute mastoiditis with subperiosteal abscess in both child and adult.

        Evidence based medicine is the conscientious, explicit, and judicious use of current best evidence in making decisions about the care of individual patients. In terms of benign paroxysmal positional vertigo (BPPV), the costs to the health care system and the indirect costs of BPPV are significant. Despite its significant prevalence, and quality of life and economic impacts, considerable practice variations exist in the management of BPPV across disciplines. In this situation, to educate clinicians on evidence based approach to BPPV would be important.         Congenital Stapes Anomalies with Normal Eardrum

        Objectives. A non-progressive and conductive hearing loss with normal eardrum, but no history of trauma and infection, is highly suggestive of a congenital ossicular malformation. Among ossicular anomalies, stapes anomaly is the most common. The purpose of this study is to describe patterns of stapes anomaly and to analyze its surgical outcome with special reference to its patterns. Methods. We conducted a retrospective case review. The subjects comprised 66 patients (76 ears) who were decisively confirmed by the exploratory tympanotomy as congenital stapes anomalies without any anomalies of the tympanic membrane and external auditory canal. The preoperative and postoperative audiological findings, temporal bone computed tomography scan, and operative findings were analyzed. Results. There were 16 anomalous patterns of stapes among which footplate fixation was the most common anomaly. These 16 patterns were classified into 4 types according to the status of stapes footplate. Successful hearing gain was achieved in 51 out of 76 ears (67.1%) after surgical treatment. Conclusion. Footplate fixation was usually bilateral, whereas stapes anomalies associated with other ossicular anomaly were usually unilateral. The success of the surgical treatment of stapes anomaly might depend on its developmental status of the footplate. Stapes anomalies were detected without any fixed patterns, therefore, it is quite possible to detect a large variety of patterns in future. Objectives.         선천성 침골 기형

        Background and Objectives:A non-progressive and conductive hearing loss with normal eardrum, which has no history of trauma and infection is highly suggestive of a congenital ossicular malformation. Among ossicular anomalies, the most common anomalous ossicle is stapes. However, incus anomaly is sometimes encountered with or without stapes anomaly. The objectives of this study are to describe patterns of incus anomaly, encountered in patients who have a normal eardrum and to analyze their surgical results with special reference to the patterns of incus anomaly. Subjects and Method:We reviewed 26 patients (31 ears) with congenital incus anomaly, which were confirmed through exploratory tympanotomy at Ajou University Hospital from 1994 to 2005. We analyzed temporal bone CT, preoperative and postoperative audiometric findings, and surgical findings. Results: 10 anomalous patterns were detected out of 31 cases of congenital incus anomaly. Incus without its long process was the most common anomalous pattern. In 15 out of 26 cases (57.7%), incus anomaly was associated with stapes anomaly, which was usually unilateral. We could get hearing improvement in 26 out of 31 ears (89.1%). Conclusion:Various anomalous patterns of incus were noted in our series. Incus anomaly was often associated with stapes anomaly, which was usually unilateral. The surgical result of incus anomaly was excellent except the cases without stapes.

        평가어(Appraisal Systems) 사용에서 나타난 팬자막과 전문가 자막의 비교: 태도(Attitude) 및 강도조절(Graduation)을 중심으로

        The purpose of this study is to investigate whether differences exist between fansubs and official subtitles in preserving the factors of Attitude and Graduation, which are the two subsystems of the Appraisal systems. The source text used in this study was a script from House (Season 4: Episode 6), an American television drama series that gained much popularity worldwide. The official subtitles were provided by On Media, a Korean cable provider, and the fansubs were produced by the NSC subtitle team from an online fansubs site. The results show that some of the typical characteristics sited by the earlier studies on fansubs, such as subtitles occupying more than two lines or adding the translator’s commentary to the subtitles for better understanding, no longer apply. However, other features mentioned in previous studies such as prioritizing faithfulness over readability are still prominent as the study shows with statistical significance that fansubs tend to retain more of the Attitude and Graduation resources compared to official subtitles.

      • 落葉松 小徑木의 活用方案에 關한 硏究 : Utilization as Raw Material of Laminated Veneer Lumber(LVL) 單板積層材의 原料木으로의 利用에 關하여

        본 硏究에서는 落葉松을 製造한 후 熱水軟化處理 및 非處理한 6㎜ 두께의 落葉松單板을 메란티베니아積層板材와 混合하여 構成方法 16가지로 單板積層材를 製造하고 그에 대한 휨 强度的 性質을 조사하여 그 破壞係數, 彈性係數, 比例限界가지의 일量 및 最高荷重까지의 일 量을 구하여 相互比較하였으며, 그 결과는 다음과 같다. 즉, 落葉松單板과 메란티베니아 積層板材로 混合構成한 單板積層材가 落葉松單板만으로 構成한 單板積層材에 비해 破壞係數의 경우 최고 160%, 彈性係數는 최고 200%의 향상된 값을 보였으며, 도한 比例限界까지의 일 量은 최고 200% 정도의 향상된 값을 나타내었다. 특히 메란티베니아 積層板材가 下部에 構成된 積層材가 인장라미네이션 효과를 보여 우수한 휨 强度的 性質을 나타냈으나 中央에 構成한 경우에는 中央部位에서 발생하는 最大剪斷應力에 脆弱한 면을 보여 强度的 性質이 떨어지는 것을 알 수 있었다. 도한 熱水軟化處理 單板積層材는 非處理單板積層材에 비해 强度積 性質이 낮게 나타났는데, 이는 熱水處理에 의한 강도적 성질의 低下로 판단된다. Half of the 6㎜ thick veneers of Larch(Larix leptolepis Gordon) in this study were treated with hot water of 60℃ for 20 minutes and the other half were not treated. All of these treated and non-treated veneers were made use of into laminated veneer lumbers(LVLs) complexed with meranti veneer-laminated woods in 16manners. The manufactured laminated veneer lumbers(LVLs) were tested in bending, which included modulus of rupture(MOR), modulus of elasticity(MOE), work to proportional limit, and work to maximum load. The results were as follows; 1. The laminated veneer lumbers(LVLs) composed of the veneers of Larch and meranti-veneer-laminated woods were higher by maximum 160% in modulus of rupture than the LVL of only veneers of Larch, higher by maximum 200% in modulus of elasticity, and higher by maximum 200% in work to proportional limit. 2. The LVLs formed with meranti veneer-laminated wood in lower layer showed the tension lamination effect, and so better bending strength. 3. But the LVLs formed with meranti veneer-laminated wood in middle layer showed lower bending strength because the meranti veneer-laminated woods are weak in maximum shear stress in the mid depth of the LVL. 4. The LVLs composed of hot(60℃) water-treated veneers of Larch showed lower strength in bending than those compose of non-treated veneers.

        Power IC용 저면적 Differential Paired eFuse OTP 메모리 설계

        In this paper, a small-area 32-bit differential paired eFuse OTP memory for power ICs is designed. In case of smaller number of rows than that of columns for the OTP memory cell array, a scheme for the cell array reducing the number of SL driver circuits requiring their larger layout areas by routing the SL (source line) lines supplying programming currents for eFuse links in the row direction instead of the column direction as well as a core circuit is proposed. In addition, to solve a failure of being blown for non-blown eFuse links by the electro-migration phenomenon, a regulated voltage of V2V ($=2V{\pm}0.2V$) is used to a RWL (read word line) driver circuit and a BL (bit line) pull-up driver circuit. The layout size of the designed 32-bit eFuse OTP memory is $228.525{\mu}m{\times}105.435{\mu}m$, which is confirmed to be 20.7% smaller than that of the counterpart using the conventional cell array routing, namely $197.485{\mu}m{\times}153.715{\mu}m$. 본 논문에서는 Power IC용 저면적 32비트 differential paired eFuse OTP 메모리를 설계하였다. OTP 메모리 셀 어레이에서 행의 개수가 열의 개수보다 더 작은 경우 eFuse 링크의 프로그램 전류를 공급하는 SL (Source Line) 구동 라인을 열 방향으로 라우팅하는 대신 행 방향으로 라우팅하므로 레이아웃 면적을 많이 차지하는 SL 구동회로의 수를 줄이는 differential paired eFuse 셀 어레이 방식과 코어 회로를 제안하였다. 그리고 blowing되지 않은 eFuse 링크가 EM (Electro-Migration) 현상에 의해 blowing되는 불량을 해결하기 위해 RWL (Read Word-Line) 구동 회로와 BL (Bit-Line) 풀-업 부하회로에 V2V ($=2V{\pm}0.2V$)의 regulation된 전압을 사용하였다. 설계된 32비트 eFuse OTP 메모리의 레이아웃 면적은 $228.525{\mu}m{\times}105.435{\mu}m$으로 기존의 셀 어레이 라우팅을 이용한 IP 크기인 $197.485{\mu}m{\times}153.715{\mu}m$ 보다 20.7% 더 작은 것을 확인하였다.
