      • 어도에서의 단기간 어류 이동성 현황 및 다변수 평가모델에 대한 예비적 고찰

        본 연구의 핵심목적은 우리나라에서 어류의 어도이용 효율성을 통합적으로 평가하고, 진단할 수 있는 다변수 어도효율성평가모델(Multi-metric Fishway Efficiency Assessment Model, Mm-FEA모델)을 개발하여 현장에 적용하여 평가하는 것이다. 본 연구에서는 개발된 Mm-FEA모델은 4대강 사업의 일환으로 건설된 영산강 수계의 죽산보에 적용평가 되었다. 통합적 어도평가 모델의 변수 지표는 1) 어도의 구조적 특성, 2) 수리수문학적 특성, 및 3)생물학적 속성으로 구성되었고, 9-메트릭 모델로 확립되었다. 첫째, 어도의 구조적 특성 측면에서 M1: 어도 구조물의 경사도, M2: 어도폭/천변폭 비율, M3: 잠공부의 수류통과 용이성으로 구성되었고, 둘째, 수리수문학적 측면에서는M4: 어도 유입 월류부의 수위, M5: 어도 유출 월류부의 수위, M6: 어도의 유속상태로 구성되었으며, 셋째, 생물학적 특성 측면에서 M7: 보 상·하류 구간에서 채집된 종수대비 어도 통과 어종 비율, M8: 어도 통과 어류의 체장 분포특성 및 M9: 어도 내 회귀성/주연성 어종의 출현 빈도로구성되었다. 죽산보 어도를 평가한 결과에 따르면, Mm-FEA 모델값은 25로서 어도효율성 평가등급은 5등급시스템 기준에 의거할 때, 2등급인 “양호상태(good)"로 나타났다. 본 Mm-FEA 모델은 전체 어류를 대상으로 어도이동 효율성을 평가한 모델로서 현재, 영산강 죽산보를 대상으로 어도 이동 효율성 평가가 이루어 졌는데, 향후 전국4대강의 중·대형보에 모델을 적용 평가하기 위해서는 일부메트릭 속성 및 지표특성에 대한 메트릭 보정이 필요할 것으로 사료되며, 일부 보정 과정을 거친 후 4대강의 보에적용할 경우 좋은 핵심모델로 이용될 수 있는 것으로 사료된다.

        Carcinoids are slow growing neuroendocrine tumors (NET) originating in the enterochromaffin cells of the gastrointestinal tract. In previous studies, rectal NET comprised only about 1% of all anorectal neoplasms, however, the incidence of rectal NET has shown a recent increase. Typically, rectal NET presents as a single subepithelial nodule, and multicentricity of rectal NETs is rare, with reported incidence of 2-4.5%. Due to the rarity of multiple rectal NETs, there is no consensus or guidelines for treatment of multiple rectal NETs. However, NETs of the rectum that are less than 10 mm in diameter and do not infiltrate the muscularis propria, without distant metastasis, can be removed by endoscopy, as with solitary rectal NET. We encountered five cases of multiple rectal NETs which were treated successfully by endoscopy. (Korean J Gastroenterol 2014,64:103-109)

        승촌보의 상류구간 및 하류구간에서 어류 종 조성 및 분포특성

        Seungchon Weir was constructed in 2010 on the downstream region of Yeongsan River watershed for four major river projects. The purpose of our study was to determine whether fish compositions and distributions differ between the upper Uw and lower regions Lw of Seungchon Weir during May December 2011 along with hydroacoustic fish surveys using Eco Sounder monitoring EsM technique. Total number of fish species sampled by conventional fish sampling at all sites was 29, and 21 and 24 species occurred in the Uw and Lw, respectively. Most dominant species was Hemiculter eigenmanni 43% in the Uw and Opsaiichthys uncirostris 21% in the Lw indicating a distinct differences of the species composition. According to hydroacoustic approach of EsM, fish density was significantly higher p<0.05 in the Lw section of 250 m than any other sections, This outcome indicates that fishes moving toward the upstream were blocked by weir construction. According to analysis of tolerance guilds, tolerant species TS were made of 88% and 79% in the Uw and Lw region, respectively, while the proportions of sensitive species were < 0.1% of the total in the both regions. Trophic guild analysis showed that the proportion of omnivore species were 58% and 40% in the Uw and Lw region, respectively, while insectivores were made of 16% and 21% in the Uw and Lw region, respectively. Both analyses of tolerance guilds and trophic guilds indicate the biological degradation fo the waterbody. Fish community structure analysis, based on species diversity, dominance, and evenness indices, showed higher community stability in the Lw region than Uw region. Exotic species of largemouth bass were largely increased in this survey 14% , compared to the past <1% , indicating a modification of fish trophic structure.

        영산강수계의 죽산보에 설치된 어도에서 어류의 이동성 평가

        Fish passage asssessments were conducted in the fishway at Juksan Weir, which was constructed as a four-major rivers project in the downstream area of Youngsan-River Watershed. For the research, fish-movements/migrations were analyzed for seven times from April ~ October, 2013 using an approach of fish trap-setting. Fish fauna and compositions were analyzed in the fishway, and seasonal- and diel-movement patterns were analyzed in relation to current velocity in the fishway. Also, abundances of exotic fishes such as bluegill sunfish (Lepomis macrochirus), large-mouth bass (Micropterus salmoides), and white curcian carp (Carassius cuvieri) were monitored in the fishway. Current velocity(n = 18) in the fishway showed large variations (0.82 ± 0.63 m/s) depending on the location of the fish trap-setting and this physical factor influenced the fish movements. Fish movements, based on the CPUE of individuals, in the fishway was greater in slower velocity (mean: 0.36 m/s, range: 0.10~1.54 m/s) than faster velocity (mean: 1.51 m/s, range: 0.90~1.90 m/s). Seasonal analysis of fish movements showed that most frequent uses (8 speices and 591 individuals, 66.2% of the total) of the fishway occurred in spring period(i.e., June). Diel movement analysis, in the mean time, showed highest in the time period of 00:00 ~ 3:00 am (7 species and 281 individuals, 20.9% of the total). The efficient managements in the fishway at Juksan Weir are required in relation to the hydrological regime.

        Plummer-Vinson syndrome manifests as cervical dysphagia, iron deficiency anemia, an upper esophageal web, and atrophic glossitis. The cause of the esophageal web is thought to be iron deficiency anemia; however, the cause of Plummer-Vinson syndrome has not been established. Crohn`s disease is usually accompanied by malnutrition and iron deficiency anemia; however, no case of concomitant Crohn`s disease and Plummer-Vinson syndrome with aggravated malnutrition and anemia has been previously reported. Here, we report on a rare case of Plummer-Vinson syndrome in a Crohn`s disease patient, which caused malnutrition and constipation.

        The objectives of this study were to determine trophic sate of agricultural reservoirs within the four major watersheds and evaluate the ecosystem health using the multi- metric fish modeling approach of Lentic Ecosystem Health Assessments (LEHAs). Agricultural reservoirs, based on Trophic State Index (TSI) ranged from mesotrophic to hypertrophic depending on the intensity of Asian monsoon. Regression analysis of N:P ratios showed a negative linear function (R2 = 0.698, p < 0.0001) with TP values, but not with TN (p = 0.939). The plots of "TSI (CHL) - TSI (TP)" and "TSI (CHL) - TSI (SD)" suggest that other factors other than phosphorus limited algal biomass (CHL - TP < 0), and that non-algal particles dominated light attenuation (CHL - SD < 0). Regression model of fish trophic component against TP showed that the proportion of omnivore (OM) and insectivore fish (IS) had a positive (R2 = 0.679, p < 0.01) and negative linear function (R2 = 0.403, p < 0.05) with TP, respectively. Values of multi-metric LEHAs model averaged 26 6.5, indicating a "Fair" conditions. Overall, the ecosystem health was directly influenced by organic matter pollution and high algal production.

        Objectives This study was designed to evaluate the effect of acupuncture on peroneal nerve palsy with sensory loss and paresthesia. Methods 28-year-old peroneal nerve palsy patient was treated with acupuncture treatment. The effects of the treatment were evaluated with Visual Analog Scale (VAS), Sensory Change, and Numeral Rating Scale(NRS) before and after treatment. Results After five treatments, VAS improved by an average of 6.5 points, improving from severe to none in terms of evaluation criteria. Sensory Change showed an average improvement of 60%. NRS improved by an average of 6.625 points, improving from severe to none in terms of evaluation criteria. Conclusions Acupuncture treatment is effective for patients with sensory loss and paresthesia due to peroneal nerve paralysis.

        대구, 경북 지역의 Helicobacter pylori 제균 치료의 연도별 제균율 변화: 13년간의 다기관 연구

        Background/Aims: The eradication rates of Helicobacter pylori infection have been reported to have decreased over the years due to antibiotics resistance. The aim of this study is to investigate the trend of eradication rates of first-line triple therapy for H. pylori over the past 13 year period, and to evaluate factors affecting H. pylori eradication in Daegu and Gyeongsangbuk-do, Korea. Methods: A total of 2,982 patients with H. pylori infection who were treated with either 1 week or 2 weeks first-line therapy (proton pump inhibitor [PPI], amoxicillin, and clarithromycin) from January 1999 through December 2011 were included in this study. Data were collected by retrospectively reviewing the medical records. Results: The overall H. pylori eradication rate was 87.2%. The eradication rates from 1999 to 2011 fluctuated between 78.0% and 95.7%, but no definite evidence of a decreasing tendency was seen over the 13 year period (p=0.113). Furthermore, there was no significant difference in the eradication rate according to the duration of therapy (p=0.592). However, there was a significant difference in the eradication rate among various PPIs (p<0.01). Conclusions: There was no decreasing trend in the H. pylori eradication rate over the past 13 years in Daegu and Gyeongsangbuk-do, Korea. There also was no difference in the eradication rates depending on duration of therapy. However, a significant difference was noted among various PPIs. (Korean J Gastroenterol 2014;63:82-89)
