        Licorice is widely used as a Chinese(herbal) medicine. The glycyrrhizin, a main ingredient of the natural licorice, has a potent mineralocorticoid effect which may cause severe hypokalemia and muscle paralysis. We present a 60-year-old woman, who had been ingesting one or two spoonful of licorice powder daily for about one year, developed acute flaccied quadriparesis with high levels of serum muscle enzymes and the typical features of mineralocorticoid excess such as severe hypokalemia and metabolic alkalosis. Both plasma renin activity and serum aldosterone level were below the normal values. This case indicates that licorice-induced hypokalemic myopathy should be considered in the differential diagnosis of a patient with acute quadriparesis and hypokalemia.

      • 다발성 단신경병증의 임상적, 전기생리학적 특성

        Background : The term “mononeuropathy multiplex” means simultaneous or sequential involvement of individual noncontiguous nerve trunks, evolving over days to years. The aim of this study was to delineate the causes, clinical features, and detailed electrophysiological findings in the patients with mononeuropathy multiplex. Methods : We analyzed the medical records of 22 patients with mononeuropathy multiplex confirmed on electrophysiological studies in Inje University Seoul Paik Hospital, Seoul Municipal Boramae Hospital, and Seoul National University Hospital between 1991 to 2000. Results : The number of male and female patients was equal. The mean age was 48 years with a peak incidence in the sixth decade. The etiology could be divided into vasculitis(11 patients) or non-vasculitis group. In vasculitis group, Churg-Strauss syndrome, polyarteritis nodosa, and rheumatoid arthritis were included. The non-vasculitis group included diabetes mellitus, leprosy, and Guillain-Barre syndrome. Ulnar and median nerves were most commonly involved(91%). In descending order of frequency, peroneal, posterior tibial, sural, and radial nerves were also involved. Bilateral involvement occurred most commonly in ulnar nerve. The symptoms and signs of mononeuropathy multiplex were the initial manifestations in 12 patients(55%), which was more frequent in vasculitis group(73%). Nerve conduction abnormalities could be divided into axonal, demyelinating, or mixed type. Most(91%) of the patients in vasculitis group revealed axonal type abnormalities. The location of the nerve lesion was frequently related to potential site of entrapment in demyelinating type. Conclusions : Mononeuropathy multiplex is the presenting features of the etiological disease frequently, especially in vasculitis group. Nerve conduction studies(NCS) reveals not only axonal type but also demyelinating type abnormalities. The etiological diseases were different in each type. Therefore, NCS is very helpful for the early etiological diagnosis and therapeutic implication in the patients with mononeuropathy multiplex.

        만성 변비 환자에서 대장통과시간 검사와 배변조영술의 유용성

        Purpose: We wanted to evaluate whether both the colonic transit time (CTT) and defecography are necessary for diagnosing constipated patients, and we also wanted to assess the defecographic findings of patients with outlet obstruction on CTT. Materials and Methods: Over the recent 3 years, 26 patients (21 women and 5 men, mean age: 59 years) underwent both CTT and defecography because of their chronic constipation or defecation difficulty. The mean interval between the 2 studies was 48 days. Colonoscopy, barium enema and manometry were performed in 22, 8 and all the patients, respectively. Results: On CTT, 13 patients (50.0%) were normal and 13 patients (50.0%) were abnormal; the abnormal results were composed of outlet obstruction (n=8, 30.8%), outlet obstruction and colon inertia (n=2, 7.7%), colon inertia (n=2, 7.7%), and outlet obstruction and hindgut dysfunction (n=1, 3.8%). On defecography, 6 patients (23.1%) were normal and 20 patients (76.9%) were abnormal; the results were composed of rectocele (n=8, 30.7%), rectocele and perineal descent syndrome (PDS; n=4, 15.4%), PDS and rectal intussusception (n=3, 11.5%), spastic pelvic floor syndrome (SPFS; n=3, 11.5%), rectocele and SPFS (n=1, 3.8%), and rectal intussusception (n=1, 3.8%). Of the 11 patients with outlet obstruction on CTT, rectocele (n=4, 36.4%), SPFS (n=1, 9.1%), rectocele and PDS (n=1, 9.1%), and PDS and rectal intussusception (n=1, 9.1%) were demonstrated on defecography, except for the 4 normal cases. Conclusion: Both CTT and defecography were necessary for diagnosing the patients with chronic constipation in compensation, and 63.6% of the patients with pelvic outlet obstruction showed an abnormal pelvic defecation function. 목적: 대장통과검사와 배변조영술을 모두 시행 받은 변비 환자에서 두 검사가 모두 필요한지와 대장통과검사에서 골반출구 폐쇄 환자들이 배변조영술에서는 어떤 소견을 보이는지를 알아보고자 하였다. 대상과 방법: 대장통과검사와 배변조영술의 두 검사를 모두 시행 받은 환자 26명(남:여 = 5:21, 평균 연령 59세)을 대상으로 하였으며, 두 검사 사이의 시간 간격은 평균 48일이었다. 대장내시경이나 직장경 검사는 22명에서 시행되었고, 항문직장 압력검사는 모든 환자에서 시행되었다. 결과: 대장통과검사에서 13명(50.0%)은 정상이었고, 비정상 13명은 각각 골반출구 폐쇄가 8명(30.8%), 골반출구 폐쇄와 대장 무력증이 2명(7.7%), 대장 무력증이 2명(7.7%), 골반출구폐쇄와 후장 기능이상이 1명(3.8%)이었다. 배변조영술에서 6명(23.1%)은 정상이었고, 비정상 20명은 각각 직장류가 8명(30.7%), 직장류와 회음하수증후군이 4명(15.4%), 회음하수증후군과 직장 중첩증이 3명(11.5%), 경련성 골반저증후군이 3명(11.5%), 직장류와 경련성 골반저증후군이 1명(3.8%), 직장 중첩증이 1명(3.8%)이었다. 대장통과검사에서 골반출구 폐쇄를 보였던 11명 중 배변조영술에서 정상은 4명(36.4%)이었고, 나머지 7명 중 직장류가 4명(36.4%), 경련성 골반저증후근이 1명(9.1%), 직장류와 회음하수증후군이 1명(9.1%), 회음하수증후군과 직장 중첩증이 1명(9.1%)이 있었다. 결론: 만성 변비 환자에서 대장통과검사와 배변조영술의 두 가지 검사는 이들의 결과가 서로 보완적이어서 모두 필요하였고, 골반출구 폐쇄를 보인 환자의 63.6%가 배변조영술에서 비정상적 골반기능을 보였다.

        간내 담관암과 유사한 다발성 골수종의 간 침범: 증례 보고

        Nodular hepatic involvement of multiple myeloma is very rare. We report here on a case of nodular hepatic involvement of multiple myeloma that mimicked intrahepatic cholangiocarcinoma. In patients with multiple myeloma, hepatic involvement of the multiple myeloma might be included in the differential diagnosis of hepatic mass. 다발성 골수종 환자에서 결절형 간 침범은 매우 드물다. 저자들은 다발성 골수종 환자에서 간 내 담관암과 유사한 간 종괴가 다발성 골수종의 간 침범으로 확진된 1예를 경험하였다. 다발성 골수종 환자에서 간 내 담관암과 비슷한 간 종괴가 발견되었을 경우 다발성 골수종의 간 침범과의 감별이 필요할 것으로 생각된다.

        Cyclic Dipeptides from Bacillus vallismortis BS07 Require Key Components of Plant Immunity to Induce Disease Resistance in Arabidopsis against Pseudomonas Infection

        Cyclic dipeptides (CDPs) are one of the simplest compounds produced by living organisms. Plant-growth promoting rhizobacteria (PGPRs) also produce CDPs that can induce disease resistance. Bacillus vallismortis strain BS07 producing various CDPs has been evaluated as a potential biocontrol agent against multiple plant pathogens in chili pepper. However, plant signal pathway triggered by CDPs has not been fully elucidated yet. Here we introduce four CDPs, cyclo(Gly- L-Pro) previously identified from Aspergillus sp., and cyclo(L-Ala-L-Ile), cyclo(L-Ala-L-Leu), and cyclo(LLeu- L-Pro) identified from B. vallismortis BS07, which induce disease resistance in Arabidopsis against Pseudomonas syringae infection. The CDPs do not directly inhibit fungal and oomycete growth in vitro. These CDPs require PHYTOALEXIN DEFICIENT4, SALICYLIC ACID INDUCTION DEFICIENT2, and NONEXPRESSOR OF PATHOGENESIS-RELATED PROTEINS1 important for salicylic acid-dependent defense to induce resistance. On the other hand, regulators involved in jasmonate-dependent event, such as ETHYLENE RECEPTOR1, JASMONATE RESPONSE1, and JASMONATE INSENSITIVE1, are necessary to the CDP-induced resistance. Furthermore, treatment of these CDPs primes Arabidopsis plants to rapidly express PATHOGENESIS-RELATED PROTEIN4 at early infection phase. Taken together, we propose that these CDPs from PGPR strains accelerate activation of jasmonate-related signaling pathway during infection.
