N/A Acute leukemia after chemotherapy and/or radio- therapy for malignant disorder is called secondary acute leukemia(SAL) or therapy related leukemia. Typically, SAL is preceded by a prodrome phase or myelodysplastic syndrome (MDS) resulting in trilineage dysplasia manifested by pancytopenia, associated with cytogenetic abnormalities involving chromosome 5 and 7, and carries a poor prognosis. Although FAB M3 type(acute promyelocytic leukemia) of SAL had been rarely reported, recently there was a report on 16 cases of therapy-related acute promyelocytic leukemia. We hereby report a case of secondary acute promyelocytic leukemia after treatment of advanced breast cancer. A 62-year-old woman was admitted because of pancytopenia, purpura of legs and gingival bleeding. Since six years before admission when she was diagnosed as infiltrating ductal carcinoma of breast with metastases to lung, She had received CMF(cyclophosphamide, methotrexate, 5-fluorouracil) chemotherapy and radiotherapy, and underwent modified radical mastectomy after complete disappearance of pulmonary nodules, She had received further chemotherapy with cumulative doses of chemotherapeutic agents as following; 21g of cyclophosphamide, 410mg of doxorubicin, 21g of 5-fluorouracil, and 600 mg of methotrexate. Radiation doses to chest wall, axilla and supraclavicular area were 2880 to 3400cGy. The examination of bone marrow aspiration and biopsy showed acute promylocytic leukemia(FAR M3). Cytogenetic analysis of peripheral blood demonstrated 46XX[1]/46XX, t(7; 11) (p15; p15) [12]. The patient did not achieved a complete remission despite retinoic acid therapy and intensive chemotherapy with Ara-C and AMSA, and died 10 months after diagnosis of acute promyelocytic leukemia.
효율적인 임계 암호시스템 구현을 위한 능동적 비밀 분산에서의 빠른 공유 갱신에 관한 연구

임계 암호시스템은 현대 암호학에서 중요한 한 축을 이루는 암호학의 한 분야이다. 본 논문에서는 임계 암호시스템의 근간이 되는 비밀 분산(Secret Sharing)의 한 분야인 (k, n) threshold scheme에서 능동적 비밀 분산(Proactive Secret Sharing)을 위한 공유(share)갱신 방법을 개선한 새로운 공유 갱신 방법을 제안한다. 이전 방법은 각 참여자당 O(n²) 의 모듈라 멱승 연산을 수행하는데 비하여 제안 방법은 O(n) 의 모듈라 멱승 연산만으로 공유 갱신이 가능하다. 이와 함께 본 논문에서는 k<(1/2)n-1 인 경우에 대하여 제안 방법의 안전함을 증명한다.

Braking comfort is affected by brake judder and brake noise. Brake judder and noise phenomenon are very<br/> complex and related to subsystem or full vehicle. Brake judder is determined by the excitation source such as<br/> BTV(brake torque variation) and by their transfer to the driver’s contact points via suspension, body and steering<br/> system. In this paper, brake and suspension systems are investigated at the same time. First, optimized sutructures of<br/> both disc and drum brake systems are proposed to reduce BTV. Secondly, using the sensitivity analysis, key<br/> subsystems of the suspension system are analysed to lower the transmissibility of tactile reponses. In addition, we<br/> investigate the damping devices such hydro-bushing can improve the braking comfort.
어린이집 교사의 정서노동과 소진 간 관계에서 행복플로리시의 조절 및 매개효과

Objective: This study investigated the moderating and mediating effects of flourishing among child-care teacher``s emotional labor and burnout. Methods: The participants included 243 child-care teachers in daycare centers in Jeju. To verify moderating effect of flourishing in relation between surface acting and burnout, 2-way ANOVA was used. To verify mediating effect of flourishing in relation between deep acting and burnout, the hierarchical regression and bootstrapping analyses were used. Results: First, the surface acting of emotional labors showed significantly positive effect with burnout and the deep acting of emotional labors shows significantly negative effect with burnout. And the surface acting of emotional labors shows significantly negative effect with flourishing and the deep acting of emotional labors shows significantly positive effect with flourishing. Also the flourishing shows significantly negative effect with burnout. Second, it was revealed that the flourishing plays a role as moderator in relation between surface acting and burnout. Finally, it was revealed that the flourishing plays a role as mediator in relation between deeping acting and burnout. Conclusion: This study illustrates that flourishing acts as differently in relation to emotional labor (surface acting/deep acting) and burnout.

N/A Background: Tachyarrhythmias have various clinical features according to their tachycardia rates, systolic function of the left ventricle, the origin site and the mechanisms. Atrioventricular nodal reentrant tachycardia (AVNRT) and atrioventricular reentrant tachycardia (AVRT) might cause different hemodynamic changes due to their different mechanisms. Methods: To evaluate the hemodynamic differences of supraventricular tachycardias, atrial tachycardia (atrial pacing with AOO mode, 180/min, group I), AVRT (VA pacing interval 80 msec with DOO mode, 180/min, group II) and AVNRT (VA pacing interval 0 msec, 25 msec with DOO mode, 180/ min, group III) were simulated in eleven dogs. Hemodynamic parameters were systemic arterial pressure including systolic, mean and diastolic arterial pressure (SAP, MAP, DAP respectively), mean pulmonary arterial wedge pressure (MPAWP) and cardiac output (CO). Results: The MAP was highest in group I (87.0±20.4mmHg) and decreased with decreasing VA interval of no significance. The SAP was higher in group II (109.8±22.6mmHg) than in group III (95.3±27.1mmHg) (p <0.05). The level of MPAWP had no significant differences among three groups and showed no peculiar pattern with changes of VA interval. The CO was higher in group I (1.18 ±0.32 L/ min) than in other two groups with significant difference (p <0.01) and decreased with shortening of VA interval without significance. Conclusion: Above result s suggested that the AVRT and AVNRT have different effects on hemodynamic changes, those from different timing of atrial contraction, especially on SAP and CO. So hemodynamic compromise during tachycardia could be more severe in AVNRT than in AVRT inspite of their similar QRS morphology and tachycardia rate.(Korean J Med 62:35- 41, 2002)

Residual stress of the grey iron brake disk was studied by neutron scattering. A vacuum heat treatment schedule was employed to compare the change of residual stress in three principal orientations (i.e., normal direction, radial direction, and hoop direction) before and after stress relaxation. Dimensional variation of the disks after the heat treatment was also measured using a static disk thickness measurement device. The result shows that the residual stress in the disk was decreased after the heat treatment and the relative amounts of stress relaxation were dissimilar in three principal orientations. Permanent disk distortion was observed after dynamometer tests that simulated hot judder during brake applications, confirming the effect of residual stress on hot judder phenomena. The root cause of the residual stress induced thermal distortion was also discussed by examining the high temperature tensile strength of grey iron up to 600℃.
