N/A Objectives: A prospective study was performed to find out the correlation between neutrophil, eosinophil and eosinophilic cationic protein (ECP) the granular protein in eosinophil - and smoking history and pulmonary function change, Methods: In this study there were seventy smokers and seventy lifetime nonsmokers. None of them had the history of atopy, or the evidence of parasitic infestation. Smoking history, spirometric pulmonary function test and blood samples for CBC, serum ECP were analyzed. Results: 1) Eosinophil and neutophil count was elevated in the smokers compared with the nonsmokers. 2) Serum ECP was elevated in smokers and the magnitude of this increase was greater than that of eosinophil count. 3) In the smokers, there was a direct proportional correlation between serum ECP and FEV1, but inverse proportional correlation between neutophil count and FEV. 4) In the smokers, neutrophil count was elevated regardless of FEV value. 5) The level of serum ECP well correlated with the daily tobacco consumption but not with the duration of smoking or pack-year consumption. Conclusion: Incresed neutrophil count, decreased eosinophil count and decreased serum ECP level could be useful in the understanding of pathophysiology of lung parenchymal destruction in smokers.

Background/Aims: Gastrointestinal smooth muscle is innervated bv non-adrenergic, non-cholinergic (NANC) inhibitory neurons. These inhibitory enteric neurons play an important role in gastrointestinal motility, and are involved in receptive and adaptive relaxation of the stotnach. 5-Hydroxytryptamine(5-HT) can exert multiple actions on the mamrnalian digestive tract, including contraction or relaxation of the stomach muscle. Nevertheless, the precise role of 5-HT in the control of gastrointestinal motility is not clear. The present study was designed to investigate the effects of 5-HT and related drugs on the NANC-mediated relaxation in the circular muscle of the rat gastric fundus and to characterize the receptor subtype irvolved. Methods: We stimulated rat gastric fundus strips, contracted with 10 nM endothelin-1(ET-1), by electric field stimulation method and then all results were recorded by Polygraph. Results: 5-HT relaxed the rat gastric fundus strips precontracted with 10 nM ET-1 in a dose-dependent manner, which was affected by pretreatment with neither atropine(10 μM) nor guanethidine(50μM). Furtheimore, the 5-HT-induced relaxation was partially blocked by pretreatment with tropisetron(a 5-HT₃ antagonist) but not by methiothepin(a 5-HT₁ antagonist) or ketanserin(a 5-HT₂ antagonist). The electrical field stimulation(EFS) caused frequency-dependent relaxation of fundus strips precontracted with 10 nM ET-1. The EFS response was not affected by pretreatment with atropine or guanethidine(100 μM, each), whereas tetrodotoxin(1 μM) completely abolished the response to EFS. The relaxation elicited by EFS was significantly reduced by N^G-nitro-L-arginine(I.-NNA) and N^G-nitro-L-arginine methyl ester(L-NAME), nitric oxide synthase inhibitors, and by tropisetron but not by methiothepin or ketanserin. The concentration-dependent relaxation to 5-HT was competetively antagonized by the 5-HT3 antagonist tropisetron, but not by methiothepin or kctanserin. Conclusions: Based on these results, it is concluded that stimulation of the 5-HT3 receptois results in relaxation and 5-HT is candidated as a mediator in NANC inhibitory neurotransmission in rat gasrtic fundus. Additionally, stimulation of neuronal 5-HT3 receptors results in an inhibitory response mediated by the release of an unknown NANC neurotransmitter.

Background/Aims: To determine the usefulness of the extracorporeal shock wave lithotripsy (ESWL), we analyzed mean number of shock waves, mean number of sessions and the factors affecting the clearance rate of the ESWL in 23 patients with extrahepatic bile duct stones. We believe that this is the first report for extrahepatic bile duct stone destruction by the ESWL treatment using the piezoelectrically generated lithotriptor in Korea. Methods: Between January 1994 and August 1997, the ESWL has been performed for 23 patients with endoscopically non-extractable stones. Before treatment with the lithotriptor, the stones were identified by combination of ultrasound and C-arm methods and shock waves were produced by a piezoelectric acoustic generator. Results: The ESWL was performed successfully in 20 out of 23 patients. The clearance rate of stones was 86.9%. The clearance rate of stones and mean number of shock waves in patients with solitary stone were 87.5% (14/16) and 3,471 shock-waves, respectively. The clearance rate of stones and mean number of shock waves in patients with multiple stones were 85.7% (6/7) and 3,424 shock-waves, respectively. The clearance rate of stones and mean number of shock waves in patients with smaller than 20 mm sized stones were 87.5% (14/16) and 3,557 shock-waves, respectively. The clearanee rate of stones and mean number of shock waves in patients with bigger than 20 mm sized stanes were 85.7% (6/7) and 3,227 shock-waves, respectively. AI1 of the 3 failed lithotrypsies developed just before tranducer exchange. Conclusions: These results show that piezoelectric lithotripsy is very useful for treatment of extrahapatic bile duct stones, and that the effect in the clearance of stones is not related with the number and size of the stones. We suggest that the time of transducer exchange is closely related with the clearance rate of stones. We could clear the stones effectively by piezoelectrically generated lithotriptor with a mean number of 5 sessions and 3,500 shock waves per session.

Biliary cystadenoma is a rare biliary ductal neoplasm that usually arises in the liver and less frequently in the extrahepatic bile ducts. Clinical manifestations are non-specific; hence preoperative diagnosis depends heavily on imaging. Computed tomography, ultrasonography, angiography and cholangiogram are useful diagnostic procedures in biliary cystic tumor, but definite diagnosis depends on histologic diagnosis. Surgical resection often yields excellent results. Surgical procedures used to treat cystadenomas include: aspiration, drainage, marsupialization, and excision. If there is no evidence of metastasis, complete resection of these tumors is necessary for a complete cure. We experienced an 80-year-old man who complained of jaundice and RUQ pain. He was diagnosed as biliary mucinous cystadenoma, We confirmed our diagnosis by cholangioscopic biopsy, and performed endoscopic sphincterotomy(EST). So, we report this case and have reviewed the relevant literature as a part of our report for the subject case.
