Malignant growth of the small bowel accounts for approximately 1 to 3 percent of malignant tumors of the gastrointestinal tract. Leiomyosarcomas are the second most common primary tumor of the small bowel, their frequency being one-third to one-half of adenocarcinoma. The common clinical symptoms of leiomyosarcoma of the small intestine are hemorrhage and abdominal pain. Accurate diagnosis cannot be based on solely on the roentgenographic finding, although in certain situations the dignosis of leiomyosarcoma may be suggested strongly. We report a case of bleeding leiomyosarcoma located in the second portion of the duodenum which was first recognized by endoscopic examination and confirmed by explolaparotomy.

The birth of the WTO marks the ratification and implementation by member states of The Final Act Embodying the Results of the Uruguay Round of Multilateral Trade Negotiations. Among the 28 agreements which constitute the Final Act, the Agreement of TRIPs represents a significant development in the international protection of intellectual property. The TRIPs Agreement marks a unique amalgamation of the basic GATT principles of national treatment and most-favoured-nation treatment, with the substantive provisions of the major international agreements for the protection of industrial and intellectual property in a single, multilateral framework of rules. The adequate protection of intellectual property in overseas markets is dependent on the willingness of countries of import to recognize and enforce the intellectual property rights of foreigners. In meeting this need, the national treatment principle provides a means of ensuring not only that intellectual property rights of foreigners will be protected within competing overseas jurisdiction, but also that they will not be discriminated against in comparison with nationals. The national treatment obligation addresses the issue of internal discrimination and as a result impacts directly on domestic policies and internal politics. Consequently, during the last 50 years, as the use of non-tariff barriers has grown, as trade in high technology goods has become prevalent, and as business has become transnational on character, the national treatment principle has played an increasingly important part in ensuring trhe continued, effective harmonization of intellectual property laws.

참여정부 지방재정조정제도의 변화에 따른 형평화 효과 분석

참여정부는 2005년에 재정분권과 국가균형발전의 일환으로 대대적인 재정분권화를 단행했다. 재정분권화는 지방재정 확충뿐만 아니라 지방정부의 자율성과 책임성을 강화하는 데 목적이 있었다. 무엇보다도 지방양여금을 폐지하고 주요한 기능을 새로 신설된 국가균형특별회계(균특)에 이양하고 나머지는 지방교부세(도로분)와 국고보조금(수질개선)으로 전환하였다. 그리고 지방교부세의 재원 규모가 내국세의 15%에서 18.3%로 증대되고 기존의 사회복지 관련 국고보 조금 사업을 대상으로 하는 분권교부세와 종합부동산세를 재원으로 하는 부동산교부세세가 각각 신설되었다. 이러한 2005년의 재정분권화 前後 간의 지역적 형평화 효과를 재정자립도와 지역소득을 기준으로 하여 평가했다. 그 결과 2005년 後가 前에 비해 오히려 역진화를 초래했다는 결과가 도출되었다. 다만 균특의 형평화만이 약간 개선된 것으로 나타났지만 전체적으로 형평성을 개선하는 데 미흡한 재원규모이다. 따라서 지방교부세 배분에 있어 지역별 인구 감소율이나 지역생산의 역외 유출과 같은 지역 낙후도를 반영할 수 있는 지표를 보강하고, 국고보조금은 차등지원제도를 활성화하고 균특은 재원규모를 확대할 필요가 있다.

중위투표모형에 의하면 선거 정국에 있어 각 정당은 자신의 기존 노선과 관계없이 중도적인 선거공 약을 제시함에 따라 각 정당의 선거공약은 큰 차이가 없게 된다. 본 논문은 우리나라 19대 대통령 선거 결과를 중위투표모형에 의해 평가하고자 한다. 이를 위해 먼저, 국민의당 안철수 후보와 자유한국당 홍 준표 후보의 실패 요인과 향후 과제를 살펴보고, 다음으로 문재인 정부의 감성정치와 포퓰리즘에 대해 검토해 보고자 한다. This paper attempts to evaluate the 19th Presidential Election in Korea in terms of ‘median voter model’ which is very interested in public choice theory. Our aim is two folds. First, we analyse the failure factor of the two major political parties in the presidential election, and then suggest their future reform directions. Finally, we discuss the political sentiment and populism of the ruling government.

주민참여예산제도는 지방재정 운용의 세계적인 추세로서 재정민주주의 실현과 재원의 효율적인 운용에 목표를 두고 있다. 선진국의 경우 주민참여예산제도는 오프라인과 온라인(웹기반)에서 보완적으로 이루어지고 있으나 우리나라는 주민대표의 참여에 의한 오프라인 방식에 의존하고 있다. 국내에서 최초이면서 가장 안정된 주민예산참여제도를 운용하는 것으로 평가되고 있는 광주광역시 북구청도 오프라인에 의존함에 따라 실제운용에 있어 주민들로부터 타당하고 실현가능한 대안을 끌어내지 못하면서 주민참여의 역기능을 노출시키고 있다. 따라서 Hartford시의 사례와 같이 웹기반에 의한 실시간(real time), 유도적(instructive), 쌍방향(interactive)의 주민참여가 요구되고 있다. 이를 위해 e-Budget 콘텐츠 구축, 참여대상 예산항목의 특정화, 참여자의 대표성 확보, 복식예산 및 성과주의 예산과 병행, 웹기반과 오프라인 참여 간의 역할분담 등이 이루어져야 한다. 이러한 웹기반 주민참여는 전자정부를 표방하는 현대 정부에 있어 재정민주주의를 고양시킬 수 있을 것이다.

비침습적인 유전자 검사법으로 진단된 아벨리노 각막이상증의 임상 양상

Purpose: To introduce a new genetic method for the diagnosis of Avellino corneal dystrophy (ACD), which is non-invasive and can be easily performed on an outpatient basis, and to evaluate the relationship between the degree of corneal opacity and age or sex. Methods: A genetic study was performed on 11 patients who had a specific corneal opacity by slit-lamp examination and on four normal patients by using a specific adhesive tape to obtain epidermal keratinocytes. Corneal dystrophy was diagnosed according to the genetic study. Results: All 11 patients were confirmed as having heterozygous ACD. Heterozygous ACD patients were classified into five stages: trace, mild, moderate, severe, or very severe, based on slit-lamp photography status. Corneal stages had no relationship with sex (p=0.982), but the severity of ACD increased with age (p=0.005). Conclusions: A non-invasive sticker-type genetic study kit, the “U-gene test” is a good method to diagnose corneal dystrophy genetically. Avellino corneal dystrophy becomes more severe over time but has no relationship with sex. J Korean Ophthalmol Soc 2008;49(9):1431-1436
