        눈운동장애 없이 시력 저하를 보인 해면정맥동 내경동맥류 1예

        Purpose: Oculomotor disturbance is the common manifestation of intracavernous carotid aneurysm cases. Intracavernous carotid aneurysms causing compressive optic neuropathy with no oculomotor disturbance are relatively rare due to their anatomical characteristics. We experienced a case of intracavernous carotid aneurysm which resulted in a shifted supraclinoid segment of the internal carotid artery, presenting with visual loss and visual field defect with no oculomotor disturbance. Case summary: A 40-year-old woman presented with loss of vision in the right eye. A relative afferent pupillary defect was observed in this eye. Visual field test showed quadranopsia in the right eye. Magnetic resonance imaging revealed that the intracavernous carotid aneurysm had shifted the supraclinoid segment of the internal carotid artery to the superomedial position. The right optic nerve was directly molded by the shifted supraclinoid segment of the internal carotid artery at the point of the bifurcation between the anterior cerebral artery and the middle cerebral artery. A Guglielmi detachable coil (GDC) embolization was performed successfully with no operational complications. Six months after coiling, best corrected visual acuity of the right eye was 1.0, and the visual field defect had recovered in all except the superior temporal field. Conclusions: Oculomotor disturbance is frequently associated with intracavernous carotid aneurysms. Nevertheless, optic neuropathy without oculomotor disturbance may be the only sign in patients with an intracavernous carotid aneurysm that causes shifting of the supraclinoid segment of the internal carotid artery.         $(HF)_2(CO)$ 분자착물에 관한 Ab Initio 연구

        $6-31+G^{\ast}^{\ast}와\;6-311+G^{\ast}^{\ast}$의 기본조를 사용해서 이합체 분자착물 FH…FH, FH…CO, FH…OC 뿐만 아니라 $(HF)_2(CO)$의 형성가능한 두가지 삼합체 분자착물들에 대해서 HF-SCF, MP2 그리고 B3LYP 계산을 수행했다. 이 삼합체 분자착물들에 대한 여러 가지 바닥상태 성질들을 구하였으며, 대응하는 단위체들과 이합체들의 성질들과 비교하였다. FH…CO가 FH…OC에 비해 평균 0.85 kcal/mol 만큼 안정했고, F-H…F-H…C≡O가 F-H…F-H…O≡C 보다 1∼2 kcal/mol 만큼 안정한 것으로 계산되었다. 삼합체 분자착물 형성에 따른 C≡O stretching band 는 그들의 이합체들(FH…CO, FH…OC)에 비해 각각 24, $37\;cm^{-1}$만큼, H-F stretching band는 각각 $54, 353\; cm^{-1}$만큼 red shift 되는 것으로 평가되었다. The HF-SCF, MP2 and B3LYP calculations have been performed on the two conceivable clusters $(HF)_2(CO)$ trimers as well as their dimer complexes FH…FH, FH…CO and FH…OC applying $6-31+G^{\ast}^{\ast}$, and $6-311+G^{\ast}^{\ast}$ basis sets. A variety of ground state properties of these trimer complexes have been listed, and compared with corresponding properties of isolated monomers and combined dimers. It was calculated that FH…CO is more stable than FH…OC by average 0.85 kcal/mol and F-H…F-H…C≡O is more stable than F-H…F-H…O≡C by 1-2 kcal/mol. The C≡O stretching bands of the F-H…F-H…C≡O and F-H…F-H…O≡C were red shifted by 24, $37\;cm^{-1}$ compared with the FH…CO and FH…OC respectively. Also, H-F stretching bands were red shifted by 54 and $353\;cm^{-1}$.

        스펙트럼 영역 빛간섭단층촬영기를 이용한 근시 정도에 따른 황반 구조의 정량적 분석

        Purpose: To investigate the correlation of macular retinal thickness and refractive error using spectral-domain optical coherence tomography (SD-OCT). Methods: A total of 120 eyes with no posterior abnormalities were enrolled in the present study. Subjects were divided into 3 groups based on their spherical equivalent. Visual acuity, refraction, slit lamp examination, tonometry and fundus examination were performed. Retinal thickness between the RPE and IS/OS junction was measured at the fovea, 1 mm (inner ring) and 2 mm (outer ring) superiorly, inferiorly, nasally and temporally using SD-OCT. Overall average thickness, average foveal thickness, and the inner and outer ring macular thickness were measured. Results: The average foveal thickness was significantly greater in the high myopic eyes than in the low to moderate myopic and emmetropic eyes (p = 0.001). However, the RPE-IS/OS junction thickness of the foveola and the outer macular thickness were significantly lower (p = 0.001, p = 0.002) in the high myopic eyes. There was a weak, but significant negative correlation between refractive error and average foveal thickness (r = -0.38, p = 0.001). A positive correlation was found between refractive error and the RPE-IS/OS junction thickness (r = 0.40, p = 0.001). Conclusions: Macular retinal thickness is related to refractive error in normal subjects. Effects of eyeball elongation are more apparent in high myopic eyes than in low to moderate myopic eyes. A significant decline in the RPE-IS/OS junction thickness suggests the photoreceptor outer segments in the foveola are damaged in high myopic eyes. J Korean Ophthalmol Soc 2011;52(11):1286-1295

        류마티스 관절염 환자에서 Conserved T 세포 수용체의 CDR3 motif를 표현하는 제2형 콜라겐 특이 T세포주의 형성과 유지

        Background: To determine the molecular structure of type II collagen-specific T-cell receptors associated with rheumatoid arthritis (RA). Methods: We generated CII-specific T-cell lines of 8 RA patients by prolonged in vitro culture with bovine CII (bCII) and the immunogenic peptide (256-270) of human CII. The proliferation response towards CII stimulation was measured from the uptake of 3H-thymidine. Changes in the secretion of Th 1 and Th2 cytokines in the culture supernatent were measured by ELISA. The TCR clonotypes of these T-cells were examined by RT-PCR/SSCP analyses of all 22 $V_{\beta}$ chains. Results: T-cells from patients' tissue exhibited strong proliferation index upon CII stimulation, which was maintained up to 6 months in the culture. The secretion of INF-$\gamma$from these T-cells increased along with the duration of culture time, while the amount of IL-4 production did not show significant changes. The SSCP band patterns of patients' T-cells appear as discrete bands unlike the smeary streak produced from normal samples. Some SSCP bands, each representing selected expansion of a TCR containing certain subtype of $V_{\beta}$ peptides, appeared to be identical in more than one patients. Among these, the expansion of SSCP band representing the $V_{\beta}$ 14 CDR3 region persisted after switching the antigen to the immunogenic human peptide (256-270). Conclusion: CII-reactive T-cells expressing distinct CDR3 motifs are selectively expanded in the peripheral blood and synovial fluid of RA patients, and their persistent proliferation upon CII stimulation, as well as the production Th 1-type cytokines, may play pivotal roles in RA pathogenesis.

        마이크로파를 이용한 급속 가열 연신 공정의 동역학 모델링과 정상상태 특성 해석

        Fiber forming is an important technology for producing multi-functional goods that have a flexible and structured plane form by using raw material with special properties. This research reports on the metal fiber manufacturing technology that adopted induction heating and drawing, but without use of dies that are now installed in the conventional wire drawing processes. Heating wires allows the drawing process to build up necking and to shorten the process by reducing the process steps so that a new and efficient production method of metal fibers is to be conceived and developed. A new device that can process a single wire consists of three major parts: the drawing part comprised of capstans, that is, feeding roll and take-up roll with specially hardened circumferential surface, the change gear part with continuously controllable draw ratio, and the heating part that includes a heating source and an energy propagation channel and an electromagnetic wave absorber With this device feasibility of the induction heating method is tested and the characteristics of the metal wire drawing process is analyzed. Results show that the microwave can be applied to attenuate the metal wire as a heating source and the experimental profiles of the cross-sectional area in the heated drawing zone under a steady state condition coincide well with those from theoretical model. The diametric variation of the drawn wires increased, however, as the drawing ratio increased.

        정시안에서 스펙트럼영역 빛간섭단층촬영기를 이용한 연령과 성별에 따른 황반 두께의 변화

        Purpose: To evaluate the changes in macular thickness with regard to age and gender in normal subjects with emmetropia using spectral domain optical coherence tomography. Methods: The present study consisted of 90 healthy subjects (162 eyes) with no ophthalmic evidence of retinopathy and who had emmetropic eyes. The data from macular measurements using spectral domain optical coherence tomography was analyzed according to the groups divided by age (Group 1: 0 to 19 years of age, Group 2: 20 to 39 years of age, Group 3: 40 to 59 years of age, Group 4: 60 to 80 years of age) and gender. Results: Macular thickness of the central circle was 253.40 ± 23.03 μm in all subjects. There was no significant change with age (p > 0.05). However, the measurements at the inner (3 mm) and outer circle (6 mm) showed a reduction of macular thickness with age (p < 0.05). The macular thickness at the central and inner circle was significantly lower in the female subjects (p < 0.05). In group 3 and 4, macular thickness at the central circle in males was greater than in females. In group 3, the average inner macular thickness in males was significantly greater than in females (p < 0.05). Conclusions: In normal subjects with emmetropia, there are statistically significant differences in macular thickness between subjects of different age and gender. The results from the present study can be expected to provide a reference value for evaluating macular disease. J Korean Ophthalmol Soc 2011;52(3):299-307

        2019년 자살 관련 지표들과 추이

        This study aimed to update suicide-related indicators including suicidal ideation, suicide attempts, and the number of suicidal deaths. Based on up-to-date information, we observed the trends of suicide-related indicators. In this study, five data sources were used to observe the trends of suicide-related indicators: Statistics Korea (1983-2019), Korean National Health and Nutrition Examination (KNHANES, ‘07-13, ‘15-19), Korean Community Health Survey (KCHS, ‘08-09, ‘13, ‘17), Korean Wealth Panel Study (KOWEPS, ‘12-19), and Korea Health Panel Survey (KHP, ‘10-13, ‘16-17). The suicide rate, which peaked in 2011, declined until 2017 and then started to rise again from 2018, recording a suicide rate of 26.9 per 100,000 people in 2019. The rate of suicidal ideation estimated based on the recently available data was 4.62% (KNHANES, ‘19), 3.51% (KHP, ‘16), 2.87% (KHP, ‘17), and 1.70% (KOWEPS, ‘19). That of suicide attempt as recent year was 0.43% (KNHANES, ‘19), 0.07% (KOWEPS, ‘19). Annual percentage change of death by intentional self-harm was -2.11% (Statistics Korea), and that of suicidal ideation was -14.7% (KNHANES), -2.5% (KCHS), -10.6% (KOWEPS), and -11.3% (KHP). Annual percentage change of suicide attempt was -5.0% (KNHANES), -4.4% (KCHS), and -11.3% (KOWEPS). The lower the income level, the higher the probability of experiencing suicide ideation and suicide attempts. Considering the recent increase in suicide rate in contrast to the continuing decline in suicidal ideation and suicide attempts, continuous data observation and appropriate policies regarding suicide prevention are needed.

      • 가족복지를 위한 남북이산가족상봉

        가족은 인류역사에 있어서 모든 제도 중 가장 끈질기고 강력한 제도로서 인간의 양육 및 성장과 함께 가족 구성원의 생물학적 생존과 재생산, 사회화 기능을 담당하고 있다. 복지국가를 지향하고 있는 우리나라에서는 ‘모든 국민은 인간다운 생활을 할 권리를 가진다’라고 헌법에 명시하고 있으며, 이것은 복지사회의 기초 단위인 가족의 행복을 위해서 이산가족문제를 해결하고자 하는 이유가 되는 것이다. 그러나 남과 북의 특수한 상황을 고려하여 볼 때 이러한 노력에는 일방적인 주장보다는 당위성과 법률적 근거가 있어야 한다. 이것은 크게 직접적 근거와 간접적 근거로 대별할 수 있는데, 직접적 근거에는 남북 사이의 화해와 불가침 및 교류 · 협력에 관한 합의서와 이에 대한 부속서가 있으며 간접적 근거에는 국제연합 헌장, 세계인권선언, 제네바협약, 7 · 4남북공동성명, 국제적십자회의의 결의, 휴전협정, 남북조절위원회 합의서 등이 있다. 이러한 여러 가지의 근거에도 불구하고 이에 대하여 의무적 이행을 강제할 수 없기 때문에 이산가족상봉의 어려움이 따르고 있는 것이다. 이에 본 연구는 이산가족의 상봉이나 교류 자체를 목적으로 보기 보다는 가족 기능의 회복을 향해 가는 과정으로 파악하였다. 남북이산가족이 서로 만나고 재결합을 이루거나 또는 재결합을 이루지 못한 채 가족의 회복 또는 가족의 정상화를 위해 나가는 과정에 국민적 관심이 증대되기를 바란다. 더욱이 이러한 국민적 관심속에서 이산가족의 고통을 경감하는데 우리 민족은 물론 국제적 공감대 형성과 북한 측의 성의 있는 자세를 다시 한번 촉구하는 바이다. Family, the most persistent and steadfast system among all systems existing in human history, is playing the roles of raising and developing humans as well as preserving, reproducing and socializing the family members. In Korea as a country pursing a welfare state, the Constitution declares that 'All people reserve the right to live a decent life.' This is why the problem of separated families must be solved for the happiness of families, which are the basic unit of welfare society. Considering the special situation of South and North Korea. however, such an effort must not be an one-sided insistence but be supported by sound legal grounds. Such grounds are largely divided into direct and indirect ones. Direct grounds include the Agreement on Reconciliation, Nonaggression, Exchange and Cooperation between South and North Korea and its annexed documents, and indirect ones include the United Nations Charter, Universal Declaration of Human Rights, the Geneva Conventions, 7/4 South-North Korea Joint Declaration, the Resolution of International Conference of the Red Cross and Red Crescent, the Armistice Agreement, the Agreement of the South-North Coordinating Committee, etc. Despite these grounds, however, the reunion of separated families cannot be forced as an obligation. Thus the present study understood the reunion of separated families or exchange between them as a process toward the restoration of the family functions rather than as a goal. It is necessary for people to take more interest in the process of restoring or normalizing families with or without the reunion of separated families in South and North Korea. Thus the author urges that, in addition to our efforts, there must be international consensus and North Korea's sincere attitude to reduce separated families' pains.

      • 가습기 살균제 연구의 교훈과 의생명과학연구에서의 적용 가능성 ― 연구 관련 이해상충의 해결을 중심으로 ―

        과학 연구에서 이해상충의 문제를 보여준 황우석 박사의 줄기세포연구 사태를 경험한 지 10여 년이 지났지만 이해상충의 관리에 대한 시스템은 여전히 갖추어져 있지 않은 상태이다. 본 연구는 가습기 살균제 연구가 이해상충에 의해 연구의 왜곡이 이루어질 수 있다는 가능성을 인지하는 데에서 출발한다. 이에 본 연구에서는 가습기 살균제 연구와 관련하여 발생한 이해상충 문제를 되짚어보고 여기에서 얻을 수 있는 교훈을 의생명과학연구에 적용시킬 수 있는지 고찰하고자 하였다. 가습기 살균제 연구에서 발생한 이해상충의 문제는 연구의 다섯 단계 중 가설 설정을 제외한 모든 연구 단계(연구 설계, 수행, 결과 분석 및 결과 발표)에서 일어남을 알 수 있었다. 이러한 문제를 해결하기 위해 연구 관련 이해상충에 관하여 일찍부터 경각심을 가져 정책적으로 이해 상충을 관리해 온 미국의 법과 제도를 분석하였다. 미국은 ‘이해상충위원회’와 ‘이해상충담당자’라는 독립적인 기관을 두어 연구의 설계 단계에서 이해상충의 관리 계획을 구체적으로 수립하고 이에 따른 관리를 시행하고 있었다. 이해상충의 관리 절차에는 연구의 설계, 수행 및 보고 등 연구 전 단계에서 이해상충으로부터 발생할 수 있는 문제를 해결할 수 있도록 감독하는 것과 이해상충에 관한 규정을 어길 경우 제재할 수 있는 방법 등이 포함되어 있다. 과학 연구에 있어 이해상충 정책이 거의 없다시피 한 우리나라의 경우 미국의 정책은 참고할만한 가치가 있다고 판단된다. 한 발 더 나아가 본 연구는 우리나라의 의생명과학연구에서 이해상충이 어떻게 관리되고 있는지 「생명윤리 및 안전에 관한 법률」 및 의생명과학연구의 특징적인 기관인 기관윤리위원회의 역할을 중심으로 고찰해 보았다. 법률적으로는 이해상충에 관한 객관적인 기준과 정의 설정의 부재가 문제점으로 인식되었고, 기관윤리위원회는 부분적으로 이해상충의 관리 역할을 수행하고 있었지만 연구의 전 단계에 걸쳐 그 영향력을 행사하고 있지는 못하였다. 이에 본 연구는 이해상충의 정의에 관한 규정마련과 기관윤리위원회의 역할 강화를 통해 의생명과학연구의 피험자 보호와 신뢰성 강화를 유지하기 위한 방법을 제안한다. It has been over a decade since Hwang Woo-suk's stem cell research, which showed the problem of conflict of interest(COI) in scientific research. However, there is still no system for management of COI. This study starts from recognizing the possibility that the study of humidifier disinfectants can be distorted by COI. The purpose of this study is to review the COI related to the study of humidifier disinfectants and to examine whether the lessons learned here can be applied to biomedical research. The problem of COI in the study of humidifier disinfectants was found to occur in all the research stages (study design, performance, analysis of results, and presentation of results) except the hypothesis setting in the five stages of the study. In order to solve these problems, we analyze the laws and institutions of the United States, which have been aware of COI related to researches from the earliest awareness and have managed policy conflicts. In the United States, an independent organization called the ‘Conflict of Interest Committee’ and ‘Conflict of Interest Officers’ were established to make concrete plans for management of COI in the design phase of the study and to manage them accordingly. Procedures for managing COI include supervision to resolve issues that may arise from COI at the pre-research stage, such as design, performance and reporting of research, and ways to sanction violations of regulations regarding conflicts of interest . In the case of South Korea, where there is little conflict of interest in scientific research, the US policy is worthy of reference. In addition, this study examines how COI are managed in South Korea's biomedical research centered on the role of the Institutional Review Board(IRB), which is a characteristic institution of biomedical research and the Bioethics and Safety Act. In legal issues, the lack of objective criteria and definition of COI has been recognized as a problem, and the IRB has been partially responsible for managing conflicts of interest, but has not exercised its influence throughout the all stages of the study. Therefore, this study suggests a method to maintain the protection of the subjects and enhance the reliability of the biomedical research through the regulation of the definition of COI and the strengthening IRB’s role.
