        요통을 동반한 비만노인의 요부복합운동이 혈중지질과 혈관염증지표에 미치는 영향

        [PURPOSE] The purpose of this study was to investigate the effects of lumbar combined exercise on serum lipid levels and inflammatory factors by applying the exercise to the elderly with chronic back pain. [METHOD] The subjects include a total of 21 senior citizens (the exercise group: 10, the control group: 11) whose average age was 73.76 (±6.84) years old at the senior citizens' center at G Welfare Center in Seoul. The exercise group received lumbar combined exercise three times a week over a total of 12 weeks by using the HubEX System (Daeyang Mechanics, Seoul, KOR), which measures isometric and isotonic muscular strength. The exercise increased by 3~5% per week by the strength of 1RM 15~50% with each set containing 100 repetitions or fewer. A fasting blood test was administered to examine changes to body composition, serum lipid levels, blood glucose, and inflammatory factors before and after lumbar combined exercise. For data processing, a normality test was performed for prior values with one Kolmogorov Smirnov sample. Once normality was secured across all the variables of measurement, the investigator conducted a parametric test called repeated measures two-way ANOVA. The statistical significance level was set at .05 for all tests. [RESULT] After the lumbar combined exercise, there were significant differences in HDL in serum lipid levels between the periods (p<.05). Significant differences were also found in the inflammatory factor IL-6 between the periods (p<.01), but it is difficult to conclude that it was the effect of the exercise since the control group as well as the exercise group showed a decreasing tendency. [CONCLUSION] In short, lumbar combined exercise will have a difficult time making a significant improvement with the serum lipid level and the Vascular Inflammation Indexes. Future study needs to apply various types of exercise (time, frequency and intensity) and examine in details their effects on pain relief, serum lipid, Vascular Inflammation Indexes, and physical strength including muscular strength. [목적] 이 연구의 목적은 요통을 동반한 비만노인의 요부복합운동이 혈중지질과 혈관염증지표에 어떠한 영향을 미치는지를 규명하는데 있다. [방법] 연구대상자는 서울시에 소재한 G 복지관 노인정의 평균연령 73.76(±6.84)세의 비만노인 총 21명(운동군 10명과 통제군 11명)이며, 운동군을 대상으로 등척·등장성 근력 측정 장비(HubEX System, Daeyang Mechanics, Seoul, Korea)를 이용하여 요부복합운동을 주 3회씩 총 12주간 실시하였다. 운동은 40~60분 동안 1RM 15~50% 강도로, 매주 3~5%씩 증가시키면서, 세트 당 100회 이하로 실시하였다. 요부복합운동 프로그램 전·후에 신체구성, 혈중지질, 혈관염증지표 검사를 실시하였다. 자료처리를 위해 사전값에 대하여 One sample Kolmogorov Smirnov를 통해 정규성 검증을 실시하였고, 모든 측정변인에서 정규성이 확보되어 모수검정인 반복측정 이원변량분석(repeated measures two-way ANOVA)을 실시하였다. 모든 검정의 통계적 유의수준은 α=.05로 하였다. [결과] 요부복합운동 실시 후 혈중지질에서 HDL은 시기 간에 유의한 차이가 있었다(p<.05). 염증변인에서 IL-6 농도는 시기 간 유의한 차이가 있었으나(p<.01), 운동군 뿐만 아니라 통제군에서도 감소한 경향을 보여 운동의 효과라고 보기는 어렵다고 생각한다. [결론] 이상의 결과를 종합해 보았을 때, 요부복합운동은 요통을 동반한 비만노인의 혈중지질과 혈관염증지표에 의미 있는 영향을 보이지 못한 것으로 판단되어 향후 연구에서는 다양한 운동양식(시간, 빈도, 강도)을 적용하여 통증완화 정도와 혈중지질, 혈관염증지표, 근력 등의 체력에도 어떠한 영향을 미치는가를 상세히 검토할 필요가 있다.

        Usage of the Internet of Things in Medical Institutions and its Implications

        Objectives: The purpose of this study was to explore new ways of creating value in the medical field and to derive recommendations for the role of medical institutions and the government. Methods: In this paper, based on expert discussion, we classified Internet of Things (IoT) technologies into four categories according to the type of information they collect (location, environmental parameters, energy consumption, and biometrics), and investigated examples of application. Results: Biometric IoT diagnoses diseases accurately and offers appropriate and effective treatment. Environmental parameter measurement plays an important role in accurately identifying and controlling environmental factors that could be harmful to patients. The use of energy measurement and location tracking technology enabled optimal allocation of limited hospital resources and increased the efficiency of energy consumption. The resulting economic value has returned to patients, improving hospitals’ cost-effectiveness. Conclusions: Introducing IoT-based technology to clinical sites, including medical institutions, will enhance the quality of medical services, increase patient safety, improve management efficiency, and promote patient-centered medical services. Moreover, the IoT is expected to play an active role in the five major tasks of facility hygiene in medical fields, which are all required to deal with the COVID-19 pandemic: social distancing, contact tracking, bed occupancy control, and air quality management. Ultimately, the IoT is expected to serve as a key element for hospitals to perform their original functions more effectively. Continuing investments, deregulation policies, information protection, and IT standardization activities should be carried out more actively for the IoT to fulfill its expectations.

        헬스케어 스마트홈 운동프로그램이 비만 여성 노인의 대사증후군 위험요인에 미치는 효과

        본 연구는 유헬스 기반의 스마트홈 기술에 운동서비스의 개념을 더한 헬스케어 스마트홈 운동프로그램이 비만 여성 노인의 대사증후군 위험요인에 미치는 효과를 알아보는 데 목적이 있다. 연구대상자는 서울시 K구의 영구임대주택단지에 거주하는 체지방률 30%이상의 비만 여성 노인 중 헬스케어 스마트홈 서비스에 참여를 희망하는 자로 운동군 21명(77.19±6.94세), 통제군 13명(74.08±6.73세)으로 분류하였다. 헬스케어 스마트홈 운동프로그램은 유산소운동과 저항운동의 복합운동 형태로 12주 동안 하루 30~50분, 주당 3회로 실시하였고, 운동전후 대사증후군 위험요인을 측정하였다. 검사결과에 대한 운동효과 분석을 위해 각 변인별로 이원 반복측정 분산분석(repeated measures two-way ANOVA)을 실시하였으며 통계학적 유의수준은 α=.05로 하였다. 12주간의 헬스케어 스마트홈 운동프로그램 실시 후, 대사증후군 위험요인 중 허리둘레(p=.034)에서 사전사후 시기 및 집단 간 상호작용 효과가 유의하게 나타났다. 결론적으로 12주간의 헬스케어 스마트홈 운동프로그램은 비만 여성 노인의 대사증후군 지표인 복부비만의 개선에 효과적이었음을 확인 할 수 있었다. This study set out to investigate effects of healthcare smart-home exercise program, which adds the concept of exercise service to the healthcare-based smart home technology, on the metabolic syndrome risk factors of obese elderly women. The subjects include obese elderly women that were living in a permanent rental housing complex in K-gu, Seoul, had 30% body fat or more, and wished to participate in the healthcare smart home service. The subjects were divided into the exercise group of 21 persons(age: 77.19±6.94) and the control group of 13 persons(age: 74.08±6.73). The healthcare smart home exercise program was the combined exercise of aerobic exercise and resistance exercise and administered for 30~50 minutes per day three times a week for 12 weeks, and the metabolic syndrome risk factors of the subjects before and after exercise were measured. The author performed repeated measures two-way ANOVA for each variable at the statistical significance level of α=.05 to analyze the effects of exercise according to the test results. After 12 weeks of the healthcare smart home exercise program, there were significant interactions in waist circumference(p=.034), one of the metabolic syndrome risk factors, before and after the program and between the groups. In conclusion, the 12-week healthcare smart home exercise program was effective for improving abdominal obesity, one of the metabolic syndrome indexes, among obese elderly women.

        Digital Therapeutic Exercises Using Augmented Reality Glasses for Frailty Prevention among Older Adults

        Objectives: The objective of this study was to investigate the effects of a digital therapeutic exercise platform for pre-frail orfrail elderly individuals using augmented reality (AR) technology accessed through glasses. A tablet-based exercise programwas utilized for the control group, and a non-inferiority assessment was employed. Methods: The participants included olderadult women aged 65 years and older residing in Incheon, South Korea. A digital therapeutic exercise program involvingAR glasses or tablet-based exercise was administered twice a week for 12 weeks, with gradually increasing exercise duration. Statistical analysis was conducted using the t-test and Wilcoxon rank sum test for non-inferiority assessment. Results: In theprimary efficacy assessment, regarding the change in lower limb strength, a non-inferior result was observed for the interventiongroup (mean change, 5.46) relative to the control group (mean change, 4.83), with a mean difference of 0.63 betweengroups (95% confidence interval, –2.33 to 3.58). Changes in body composition and physical fitness-related variables differednon-significantly between the groups. However, the intervention group demonstrated a significantly greater increase in cardiorespiratoryendurance (p < 0.005) and a significantly larger decrease in the frailty index (p < 0.001). Conclusions: An ARbaseddigital therapeutic program significantly and positively contributed to the improvement of cardiovascular enduranceand the reduction of indicators of aging among older adults. These findings underscore the value of digital therapeutics inmitigating the effects of aging.
