Survey on Liver Resection Cases (2 Year Myanmar Experience)

Aims: According to the 2015 concensus (1), Myanmar population has Hepatitis C carrier of 2-4% and Hepatitis B of above 8%. Hepatocellular Carcinoma prevalence is therefore the high and it also impose significant burden to the health of our population. Our department, although founded three years ago, had to encounter with it and the resection rate is between 50-100 per year. Methods: Descriptive study based on telephone interview of resected and transplanted patients for Hepatocellular Carcinoma within two years, 2015 and 2016, focusing on recurrence and survival. Results: 57 and 95 cases of Hepatocellular Carcinoma patients underwent various types of operation from wedge resection to Transplantation. The response rate is 21% for 2015 and 56% for 2016. Sex distribution is approximately 3:1 ( Male : Female). 51 to 60 year group is the most affected age group for both years. For 2015, survival rate is only 10% and all survivor had recurrence. For 2016, survival rate is 74% and only 24% of the survivor had recurrence. Conclusions: Although the initial results of our department on treating Hepatocellular carcinoma shows improvement, it still need improvement to catch up with international figures
The Marma from Bangladesh: A “de-Arakanized” Community in Chittagong Hill Tracts

The Rakhine (Arakanese) from present-day Rakhine State (Arakan) in Western Myanmar and the Marma from the Chittagong Hill Tracts (CHT) of Southeast Bangladesh originated from the same region, share the same culture and practice the same religion. However, the people from CHT have developed a distinctive identity and are individualized by a different name “Marma”. This development raises a number of historical questions. This paper explores how the Arakanese descendants became “Marma” in Bangladesh.

Acute traumatic aortic injuries, which have substantial lethal outcomes at the time of admission, are fatal in 80% to 90% of cases. These injuries are relatively rare and have nonspecific clinical presentations. Radiologists and emergency physicians need to identify the radiological signs of acute traumatic aortic injury and differentiate them from common imaging errors to ensure accurate diagnosis and determine appropriate management protocols. In combination with image-guided interventions, advances in cross-sectional imaging have enabled nonsurgical management of acute traumatic aortic injuries. Timely and precise diagnoses of these injuries following prompt treatment are essential as up to 90% of patients presenting at the hospital can undergo early repair.
Dietary Selection of Fat by Heat-stressed Broiler Chickens

A total of 160 d-old male broiler chicks (Cobb) were brooded for three weeks and then maintained at $24{\pm}1^{\circ}C$. Commencing from d 21, chicks were assigned to one of four feeding regimens: (1) diet with 8% palm oil (PO), (2) diet with 8% soybean oil (SO), (3) diet without added fat (control), (4) a choice of PO, SO and control (CH). The diets were formulated to maintain a constant ratio of energy and protein. From d 28 to 41, all birds were exposed to $34{\pm}1^{\circ}C$. The PO, SO and CH birds had greater body weight than controls on d 42. The PO but not SO diet reduced mortality rate, body temperature and serum creatine kinase level of broiler chickens during heat exposure. Although the total intake of control, PO and SO diets was not significantly different during heat exposure, the CH birds had lower creatine kinase activity and mortality rate than those provided SO diet but not significantly different from the birds fed control and PO diets. The relative abdominal fat weight and breast intramuscular fat content percentage were significantly lower in the control birds than those of PO, SO and CH groups. There were no significant differences in both parameters among the three latter groups. These findings suggest that the uncertainty of how much dietary fat to put into diets for heat stressed broilers can be overcome by allowing them to select their own consumption.

Williams syndrome (OMIM #194050) is a rare, well-recognized, multisystemic genetic condition affecting approxi-mately 1/7,500 individuals. There are no marked regional differences in the incidence of Williams syndrome. The syndrome is caused by a hemizygous deletion of approximately 28 genes, including ELN on chromosome 7q11.2. Prenatal-onset growth retardation, distinct facial appearance, cardiovascular abnormalities, and unique hyperso-cial behavior are among the most common clinical features. Here, we report the case of a patient referred to us with distinct facial features and intellectual disability, who was diagnosed with Williams syndrome at the age of 37 years. Our aim is to increase awareness regarding the diagnostic features and complications of this recognizable syn-drome among adult health care providers. Williams syndrome is usually diagnosed during infancy or childhood, but in the absence of classical findings, such as cardiovascular anomalies, hypercalcemia, and cognitive impair-ment, the diagnosis could be delayed. Due to the multisystemic and progressive nature of the syndrome, accurate diagnosis is critical for appropriate care and screening for the associated morbidities that may affect the patient’s health and well-being.

Some years ago, analyses were performed by ITER Organization Central Team (IO-CT) to verify the structural integrity of the ITER vacuum vessel baseline design fixed in 2010 and classified as a Protection Important Component (PIC). The manufacturing phase leads the ITER Organization domestic agencies (IO-DA) and their contracted manufacturers to propose detailed design improvements to optimize the manufacturing or inspection process. These design and quality inspection changes can affect the structural margins with regards to the Codes&Standards and thus oblige to evaluate one more time the modified areas. This paper proposes an overview of the additional analyses already performed to guarantee the structural integrity of the manufacturing designs. In this way, CT and DAs have been strongly involved to keep the considerable margins obtained previously which were used to fix reasonable compensatory measures for the lack of In Service Inspections of a Nuclear Pressure Equipment (NPE).

Congenital diarrheal disorders (CDDs) with genetic etiology are uncommon hereditary intestinal diseases characterized by chronic, life-threatening, intractable watery diarrhea that starts in infancy. CDDs can be mechanistically divided into osmotic and secretory diarrhea. Congenital tufting enteropathy (CTE), also known as intestinal epithelial dysplasia, is a type of secretory CDD. CTE is a rare autosomal recessive enteropathy that presents with intractable neonatal-onset diarrhea, intestinal failure, severe malnutrition, and parenteral nutrition dependence. Villous atrophy of the intestinal epithelium, crypt hyperplasia, and irregularity of surface enterocytes are the specific pathological findings of CTE. The small intestine and occasionally the colonic mucosa include focal epithelial tufts. In 2008, Sivagnanam et al. discovered that mutations in the epithelial cell adhesion molecule (EpCAM, MIM# 185535) were the genetic cause of CTE (MIM# 613217). More than a hundred mutations have been reported to date. Furthermore, mutations in the serine peptidase inhibitor Kunitz type 2 (SPINT2, MIM# 605124) have been linked to syndromic CTE. In this study, we report the case of a 17-month-old male infant with congenital diarrhea. Despite extensive etiological workup, no etiology could be established before admission to our center. The patient died 15 hours after being admitted to our center in a metabolically decompensated state, probably due to a delay in admission and diagnosis. Molecular autopsy with exome sequencing revealed a previously reported homozygous missense variant, c.757G>A, in EpCAM, which was confirmed by histopathological examination.
Oxidative Stress and Inflammatory Biomarkers in People with Methamphetamine Use Disorder

Objective: This study aimed to investigate the blood serum levels of biomarkers specifying oxidative stress status and systemic inflammation between people using methamphetamine (METH) and the control group (CG). Serum thiol/disulfide balance and ischemia-modified albumin levels were studied to determine oxidative stress, and serum interleukin-6 (IL-6) levels and complete blood count (CBC) were to assess inflammation. Methods: Fifty patients with METH use disorder (MUD) and 36 CG participants were included in the study. Two tubes of venous blood samples were taken to measure oxidative stress, serum thiol/disulfide balance, ischemia-modified albumin, and IL-6 levels between groups. The correlation of parameters measuring oxidative stress and inflammation between groups with sociodemographic data was investigated. Results: In this study, serum total thiol, free thiol levels, disulfide/native thiol percentage ratios, and serum ischemia-modified albumin levels of the patients were statistically significantly higher than the healthy controls. No difference was observed between the groups in serum disulfide levels and serum IL-6 levels. Considering the regression analysis, only the duration of substance use was a statistically significant factor in explaining serum IL-6 levels. The parameters showing inflammation in the CBC were significantly higher in the patients than in the CG. Conclusion: CBC can be used to evaluate systemic inflammation in patients with MUD. Parameters measuring thiol/disulfide homeostasis and ischemia-modified albumin can be, also, used to assess oxidative stress.
Experimental and theoretical analyses of a double-cylindrical trough solar concentrator

Parabolic trough, Fresnel, heliostat mirror, or dish-type collectors are used to obtain high temperature in solar thermal applications. The operation of this system is similar to that of a Fresnel collector, except for its reflective mirror, which is cylindrical and not flat. The experiments were carried out in the solar energy system manufactured for producing hot water, superheated water, and steam in the cylindrical trough solar concentrator. According to the test results, the thermal power of the system and the average thermal efficiency of the collector were a maximum of 15 kW and approximately 35 %, respectively.
