Perceived Risk of Re-Identification in OMOP-CDM Database: A Cross-Sectional Survey

Background: The advancement of information technology has immensely increased the quality and volume of health data. This has led to an increase in observational study, as well as to the threat of privacy invasion. Recently, a distributed research network based on the common data model (CDM) has emerged, enabling collaborative international medical research without sharing patient-level data. Although the CDM database for each institution is built inside a firewall, the risk of re-identification requires management. Hence, this study aims to elucidate the perceptions CDM users have towards CDM and risk management for re-identification. Methods: The survey, targeted to answer specific in-depth questions on CDM, was conducted from October to November 2020. We targeted well-experienced researchers who actively use CDM. Basic statistics (total number and percent) were computed for all covariates. Results: There were 33 valid respondents. Of these, 43.8% suggested additional anonymization was unnecessary beyond, “minimum cell count” policy, which obscures a cell with a value lower than certain number (usually 5) in shared results to minimize the liability of re-identification due to rare conditions. During extract-transform-load processes, 81.8% of respondents assumed structured data is under control from the risk of re-identification. However, respondents noted that date of birth and death were highly re-identifiable information. The majority of respondents (n = 22, 66.7%) conceded the possibility of identifier-contained unstructured data in the NOTE table. Conclusion: Overall, CDM users generally attributed high reliability for privacy protection to the intrinsic nature of CDM. There was little demand for additional de-identification methods. However, unstructured data in the CDM were suspected to have risks. The necessity for a coordinating consortium to define and manage the re-identification risk of CDM was urged.

N/A Background : Androgenetic alopecia is considered to be a genetically determind disorder influenced by age and androgen. The proportion of patients with androgenetic alopecia among the total number of patients with alopecia seems to be gradually increasing. Objective : The purpose of this study was to find the family history, clinical and endocrine status of the patients with androgenetic alopecia. Method : 736 patients with androgenetic alopecia who had visited the Department of Dermatology, Yongsan and Pil-dong Hospital, College of Medicine, Chung-Ang University during the last 2 years(1999.1-2000.12) were examined. Results : 1) Incidence of androgenetic alopecia among the total number of alopecia patients was 75.8%. 2) There were 571 male and 165 female patients being most prevalent in the 3rd decade in male patients and in the 4th decade in female patients. 3) In male androgenetic alopecia, Norwood-Hamilton`s type Ⅱ was the most common and following Ⅲv, Ⅱ, Ⅳ, Ⅲa, and in female patients, Ludwig`s type Ⅰ was most common. 4) There was a family history of baldness in 59.5% of first degree relatives in male patients and 30.9% in female patients. 5) The most common associated disorder is seborrheic dermatitis. 6) Serum testosterone levels were increased in 78 males(13.7%) and 23 females(13.9%). Conclusion : Based on these results, the number of young patients who visit dermatologists tends to be increasing and genetic factor, ages and abnormal androgen metabolism in scalp are affecting the development of androgenetic alopecia.

본 연구는 요양시설에 입소한 노인 가족의 가족참여에 대한 현상학적 연구이다. 연구 참여자는 총 11명으로 2016년에 요양시설 노인 가족을 심층 면접함으로 자료 수집이 진행되었다. 모든 면접 자료는 녹음되고 필사되어 자료 분석이 이루어졌다. 자료의 분석은 Colaizzi"s 방법을 사용하였다. 요양시설에서의 가족참여는 3개의 범주와 5개의 주제모음, 14개의 주제로 이루어져 있다. 3개의 핵심 범주에는 ‘버팀목으로 남은 도리 다하기’, ‘생소함에 익숙해지기’, ‘풀리지 않는 악순환의 고리’로 구성되어 있다. 본 연구를 통하여 요양시설 입소 노인 가족의 가족참여에 대한 깊은 이해와 통찰이 가능하였다. 그리고 본 연구 결과를 통하여 요양시설 노인 가족의 특수성이 반영된 도구개발의 기초자료가 되며 요양시설에서 역할을 담당하는데 도움을 제공할 뿐만 아니라 궁극적으로 안녕과 삶의 질에도 긍정적인 영향이 나타날 것으로 기대된다. This study was to explore and describe the experience of family involvement of family caregivers of elders in nursing home. Participants were eleven family caregivers of elders in nursing home. Individual in-depth interviews were used to collect the data in 2016. All interviews were audio-taped and verbatim transcripts were made for the analysis. Data were analyzed using Colaizzi"s phenomenological method. Family involvement in nursing home was organized into three categories, five theme-clusters, and 14 themes. Three essential themes emerged from the analysis ‘do one"s duty as a buttress to the elders’, ‘accustomed to unfamiliarity’, ‘Unbroken vicious circle’. The findings in this study provide deep understanding and insight of family involvement of family caregivers of elders in nursing home. And it should help in the development of interventions tailored specifically of family caregivers in nursing home to support their role adaptation and ultimately improving their well-being and quality of life.
