        Skeletal cryptococcosis is an uncommon infection. Cryptococcus is a common cause of meningitis and infects 7∼10% of patients with AIDS. As well as AIDS, the infection may be seen in association with leukemia, lymphoma, Hodgkin's disease, sarcoidosis, tuberculosis and diabetes, also in patients on steroid medication. But there is no case report of skeletal cryptococcosis following allogeneic hematopoietic stem cell transplantation. A 40-year-old woman was admitted to the hospital because of low back pain. She had chronic myelogenous leukemia for 2 years and underwent allogeneic hematopoietic stem cell transplantation 8 months ago. She have been treated with steroid and cyclosporine orally because of chronic graft versus host disease. On examination she was afebrile and had posterior lower lumbar tenderness. But, she had no reduced strength of low extremities. Open biopsy was underwent. Histology demonstrated budding, round-to-oval, refractile yeast-like organisms within debris. The results of a lumbar puncture were unremarkable and cerebrospinal fluid culture failed to grow bacteria and yeast. The patient was treated with amphotericin B (1 gram) and AmBisome□ (2.8 gram) over 6 weeks. Three months after cessation of therapy, the patient was doing well.(Korean J Infect Dis 33:298∼301, 2001)

        Shear reinforcement has an important role to strengthen shear capacity of reinforced concrete beam. Although the flexural design corresponding to design code does not consider the effects of the shear reinforcement, its utilization in flexural region leads to the improved ductility and the enhanced strength by confining concrete. It allows to design the over-reinforcement beam. In this study, we considered the effects of the shear reinforcement on the flexural ductility and strength of reinforced concrete beam according to the spacing of shear reinforcement, steel ratio, and shear span ratio. The flexural capacity depends on the amount of longitudinal bars and the ductility the confinement degree by the shear reinforcement. The flexural analysis varies with the stress-strain relation of confined concrete. The analytical results based on the stress-strain relation presented by Chung almost corresponded to the experimental ones. It was observed that the proper design of shear reinforcement leads to utilize the over-reinforcement concrete beam.

        시각장애학교 교사의 학교도서관 이용 및 활용수업 실태 : A Study on Teachers' Use of the School Library and School Library-Assisted Instruction at School for the Visually Impaired

        연구목적: 본 연구는 시각장애학교 학교도서관 이용 및 활용수업 실태를 알아보고자 하였다. 연구방법: 시각장애 중·고등학교 교과 담당 교사 86명을 대상으로 한 조사연구를 실시하였다. 연구결과: 과반수정도가 도서관 활용수업을 진행한 적이 있다고 하였다. 국어교사는 대부분 경험이 있는 반면 다수의 수학교사는 경험이 없었으며, 활용수업을 실시하지 않는 이유로는 과목의 특성을 언급하였다. 도서관 활용수업은 교과수업의 질을 높이기 위해 실시한다고 하였고, 대부분의 교사가 시각장애학생에게 도서관 활용수업이 필요하며 교수학습 면에서도 중요하다고 하였다. 또한 학교도서관 활용수업을 통해 자기주도 학습능력, 정보 활용능력, 수업에 대한 관심과 흥미도가 모두 향상된다고 인식하고 있었다. 도서관 활용수업을 활성화시키기 위해서는 대체자료 및 보조공학기기의 보완, 편의시설이 필요하다고 하였다. 결론: 전담 사서교사의 배치를 통해 체계적인 지원 체계와 전문성을 확보하고 지속적인 연수를 통하여 학교도서관 활용수업에 대한 홍보, 교육과정과의 연계 및 다양한 프로그램과 교수매체의 개발이 필요하다. Purpose: This research examines teachers’ use of school library and their implementation of school library-assisted instruction at middle and high schools for the visually impaired. Method: A questionnaire was distributed to 86 middle and high school teachers for the visually impaired. Results: First, more than a half of the teachers had implemented school library-assisted instruction. While most teachers of Korean had taught library-assisted classes, most math teachers responded that they had not used school libraries because of the nature of the subject matter. Regarding the purpose of school library-assisted instruction, a majority of teachers mentioned the improvement of the quality of teaching and learning. Second, most teachers acknowledged the benefits of library-assisted instruction for visually impaired students and its importance in teaching and learning. The teachers expected that, by utilizing school libraries, they could enhance the visually impaired students’ self-initiated learning ability, information utilization ability, attention and interest in class. Lastly, teachers suggested that alternative materials and assistive technology should be made available for more effective utilization of school libraries. Conclusion: The research indicates that support systems are also necessary, including professional librarians, continual education on school library-assisted instruction, development of connected curriculum, and development of instructional media.

      • 항결핵 치료과정에서 호전되었다가 악화된 뇌 결핵종 1예 보고

        In spite of nationmide tuberculosis control program, tuberculosis is still prevalent in Korea. Accordingly, tuberculous meningitis also poses one of the main trouble some problems. Recently, We experienced a perplexing case of tuberculous menin-gitis and tubercdoma in a 24-year old female patient. She was admitted to our hospital owing to meningeal irritation sign, aphasia, VI nerve palsy, and ,fever. CSF analysis showed a finding consistent with that of presumptive tuberculous meningitis.. Initial MRI of the brain revealed a large mass(tuhercu1oma) on left parieto-temporal area. Antitubeculous regimen(1soniazid (INH), rifampin(RMP), ethambutol(EMB), pyrazinamide(PZA)} of meningeal dose was given immediately, and her symptoms and signs were dramatically disappeared with a few days after treatment. She was discharged with improved condition and under maintenance treatment of tuberculosis. Follow-up one year after treatment. revealed that the previous lesion was nearly diminished. She showed no symptom at all until she recognized a total blindness of sudden onset on her left eye a t the 18th month of treatment. Brain MRI checked at that time showed multiple masses of tuberculoma scattered over the brain base and one of them invaded left optic nerve. In our opinion, the cause of relapse in this patient might he explained by the selection and ultimate replication of a minor population of resistant strain. Although such relapse is an extremely rare event, we think that we should always pay attention to this possible resistance problem every time we meet patients with tuberculous meningitis.

      • 국내산 과일 및 채소 추출물을 이용한 체중 조절 관련 기능성 식품소재 탐색

        Obesity can be defined as a disorder of lipid metabolism and the control of this process is regulated by enzymes involved in lipid metabolic pathway. Obesity is also characterized increases in the number and size of adipocyte differentiated from fibroblastic pre-adipocytes in adipose tissue. The aim of this study was to screen ethanol extracts of 21 fruits and vegetables cultivated in Republic of Korea for identifying natural sources of lipase inhibitor and to investigate the effects of several extracts resulting in suppression of triglyceride (TG) accumulation in 3T3-L1 cells. Among the tested extracts, Diospyros kaki L. showed the highest contents of total sugar and the content of total phenolic compounds was detected 17 species while there was not detected in Ixeris dentate root and Allium fistulosum C. etc. We were screened 26 extracts of fruits and vegetables for pancreatic lipase inhibitory activity compared with orlistat. The strongest lipase inhibitory activities were shown by extract of Allium sativum L. stem, Allium sativum L. bulb, Apteryx australis, Diospyros kaki L. Malus Pumila, Perilla frutescens and Prunus mume regardless of concentrations treated. Eight extracts including Brassica juncea var. integrilolia had no effect on lipase inhibition. During the differentiation of 3T3-L1 preadipocyte to mature adipocyte, the cells were treated with carrier or 7 extracts(1, 5 and 10 ㎍/ml), which was shown to highest lipase inhibitory effects. We were quantified lipid contents secreted mature adipocytes and stained with Oil Red O. All cells treated with the Apteryx australis and Prunus mume significantly decreased TG contents than the control. These findings suggested that the screened fruit and vegetable could be a promising source as the effective and safe lipase inhibitors for isolating bioactive ingredients and developing functional foods.

        Roles of Aluminum Hydroxide and Monophosphoryl Lipid A Adjuvants in Overcoming CD4<sup>+</sup> T Cell Deficiency To Induce Isotype-Switched IgG Antibody Responses and Protection by T-Dependent Influenza Vaccine

        <P>Vaccine adjuvant effects in the CD4-deficient condition largely remain unknown. We investigated the roles of combined monophosphoryl lipid A (MPL) and aluminum hydroxide (Alum) adjuvant (MPL+Alum) in inducing immunity after immunization of CD4 knockout (CD4KO) and wild-type (WT) mice with T-dependent influenza vaccine. MPL+ Alum adjuvant mediated IgG isotypeswitched Abs, IgG-secreting cell responses, and protection in CD4KO mice, which were comparable to those in WT mice. In contrast, Alum adjuvant effects were dependent on CD4(+) T cells. MPL+ Alum adjuvant was effective in recruiting monocytes and neutrophils as well as in protecting macrophages from Alum-mediated cell loss at the injection site in CD4KO mice. MPL+Alum appeared to attenuate MPL-induced inflammatory responses in WT mice, likely improving the safety. Additional studies in CD4-depleted WT mice and MHC class II KO mice suggest that MHC class II+ APCs contribute to providing alternative B cell help in the CD4-deficient condition in the context of MPL+Alum-adjuvanted vaccination.</P>

        Fermented Ginseng with Bifidobacterium Inhibits Angiogenesis of Human Umbilical Endothelial Cells in vitro and in vivo

        Ginseng is a widely-used alternative medicine for the treatment of cancer, diabetes, and cardiovascular diseases. Active components of P. ginseng, absorbed through gastrointestinal tract are the fermented gin-senosides by intestinal microorganisms. In the present study, we investigated the inhibitory effects of fermented ginseng with bifidobacterium (FGb) on the angiogenesis by analyzing in vitro tube formation and invasion assay using human umbilical vein endothelial cells (HUVECs), and in vivo angiogenesis using chick chorioallantoic membrane (CAM) assay. Treatment with FGb inhibited tube-like structure formation in a concentration-dependent manner. In addition, FGb significantly suppressed HUVEC invasion through Matrigel. Moreover, FGb dose-dependently inhibited VEGF-induced angiogenesis in a CAM assay. These results suggest that FGb is a valuable anti-angiogenic remedy.

        Intercultural Intermediality: The Unspoken Text in Intercultural Film Adaptations of Shakespeare

        In this paper I wade tangentially into the on-going debate as to whether, in global productions of Shakespeare, Shakespeare acts as a vehicle of reciprocal cultural exchange or of continuing Western cultural hegemony. I emphasize what Li Lan Yong likes to refer to as the positionality of the audience member—specifically, how familiar the individual is with the language of the source play—and from there focus on two film adaptations of Shakespeare that are self-consciously intermedial in nature: Akira Kurosawa’s Throne of Blood (1957) and a Korean film that is a very loose adaptation of Hamlet, The King and the Clown (Lee Joon-ik, dir., 2005). I explore in particular how the directors draw upon the liveness of theatre to transform famous metaphors of theatricality in the source texts for moral and political commentary, while indirectly intervening in the question of Shakespeare’s status in the global cultural market place.

        하동지역 매실 당절임의 표준화 방안 연구

        본 연구에서는 하동 지역에서 제조되고 있는 13종의 매실 당절임 제품을 수집하여 외관, 조직감, 신맛, 단맛 및 전체적인 기호도 등 관능평가 결과 우수한 제품 5종을 선별하여 이화학적 및 제조 특성을 조사하였다. 5종의 매실 당절임 제품의 수분함량은 45.2~53.3%였으며 pH는 약 2.8 내외로 모든 실험구가 유사하게 나타났다. 산도는 0.4% 내외로 모든 당절임 제품에서 유사하였고 당도는 13번 제품이 다른 실험구들에 비해 다소 낮게 나타나 당과 산의 함량이 낮은 제품의 관능적 기호도가 높을 것으로 생각된다. 또한 조단백질, 조지방 함량도 모든 실험구들에서 각각 약 0.4, 0.8% 내외로 유사하게 나타났다. 관능평가시 조직감이 가장 우수하다고 선별된 13번 제품의 기계적 경도 값이 다른 구에 비해 높게 나타났으며 색도는 밝은 색에 황색과 청색에 가까운 제품이 소비자의 관능에 우수할 것으로 생각된다. 5종의 우수한 매실 당절임 제품의 주요 유기산은 수산, 사과산, 구연산 및 푸마르산으로 나타났으며 구연산 함량은 전체 유기산 함량의 60% 이상 수준이었다. 또한 주요 유리당은 glucose, fructose, rhamnose 및 sucrose로 나타났으며 glucose 함량은 전체 유리당 함량에 65% 이상 검출되어 당도 결과와 유사한 경향을 나타내었다. 하동지역에서 제조되고 있는 매실 당절임 제품 13종의 관능검사 결과 기호도가 우수한 5종의 제품 중 2, 10 및 11번은 하동지역에서 재배되는 품종을 알 수 없는 재래종이었고 12번 제품은 남고(南高), 13번 제품은 백가하(百加賀) 품종으로 제조된 것이었다. 또한 매실 과육과 자당의 배합비율은 5.5 : 4.5로 제조된 13번 제품을 제외하고 4종의 제품에서 동일한 비율로 제조하였으며 7개월 이상 저온에서 숙성시킨 것으로 조사되었다. 따라서 하동지역에서 주로 생산되고 지역적 상품화가 될 수 있는 매실 당절임을 제조하기 위해서는 백가하 품종으로 과육과 자당을 5.5 : 4.5 비율로 배합하여 5oC의 저온에서 약 7개월 이상 숙성시켜 제조하는 것이 가장 적합 할 것으로 생각된다. Prunus mume is rich in organic acid, and excels in antioxidant activity. However, development of processed foods using Prunus mume is very limited, and characteristics of its products are different according to the region they are processed in. In this study, we collected 13 kinds of sugar-preserved Prunus mume home-made products in Hadong, and investigated their physical and manufacturing characteristics to select a superior product. 5 kinds of sugar-preserved Prunus mume products were selected according to results of sensory evaluation, and the overall acceptability of product No.13 was superior. Moisture contents of the 5 samples ranged from 45.2~53.3%, and pH and acidity showed similar values at about 2.8 and 0.4%, respectively. Sugar content of product No.13 were slightly lower than other products. Crude protein and lipid contents were similar at about 0.4 and 0.8%, respectively. Hardness value of product No.13, selected by its texture in sensory evaluation, was higher than others. The main components of organic acids were oxalic, malic, citric and fumaric acid, and citric acid content occupied more than 60% of total organic acid content. Also, the main components of free sugar were glucose and fructose.

        장류 및 젓갈 분리 균주 추출물의 비만세포 매개 항염증효과

        본 연구에서 장류 및 젓갈 유래 균체를 각각 sonication, 100℃에서 30 min, 125℃에서 15 min의 조건으로 추출하여 HMC-1에 처리하였을 때, 이 추출방법에 따른 HMC-1매개 염증반응 억제효과를 조사하였다. 추출방법을 달리하여 추출한 장류 및 젓갈 유래 균체 J80, G147이 HMC-1생존율에 미치는 영향은 대조구 B (Not treated)와 비교하였을 때 86~96%로 유의적 차이를 보이지 않아 장류 및 젓갈 유래 균체에 의한 세포 독성은 나타나지 않았다. 장류 및 젓갈 유래 균체의 모든 전처리구에서 histamine 유리 억제효과를 나타내었으며 특히, 100℃에서 30min 동안 가열하였을 때 histamine 억제율이 J80에서 28.86%, G147에서 41.14%으로 가장 높게 나타냈다. IL-8분비량은 J80, G147의 모든 전처리구에서 IL-8 분비억제효과가 나타났으며, J80의 경우 100℃, 30 min에서 처리한구에서 분비량이 0.78 ng/㎖로 대조구 B (Not treated)에 비하여 약 28.45%의 분비억제효과로 가장 높게 나타내었다. G147의 경우 125℃, 15 min에서 처리한 구에서 분비량이 0.73 ng/㎖로 약 36.52%의 분비억제효과로 가장 높게 나타내었다. IL-6 분비량을 측정한 결과 J80, G147의 모든 실험구에서 IL-6 분비억제효과가 나타났으며, J80에서는 100℃, 30 min한 D구의 분비량이 0.40 ng/㎖으로 대조구 B (Not treated)에 비하여 약 38.46%의 분비억제효과로 가장 높았으며, G147 역시 100℃, 30 min 동안 처리한 D구의 분비량이 0.27 ng/㎖으로 대조구 B (Not treated)에 비하여 56.45%의 분비억제효과로 가장 높게 나타냈다. TNF-α 분비량 역시 IL-6와 마찬가지로 J80, G147에서 대조구 B보다 낮은 분비량으로 TNF-α 분비 억제효과가 나타났으며 J80의 경우 100℃, 30 min 처리한 D의 TNF-α 분비량이 0.052 ng/㎖으로 66.67%의 분비억제효과로 가장 높았으며, G147의 경우 sonication을 처리한 C구의 TNF-α 분비량이 0.086 ng/㎖으로 41.1%의 분비억제효과로 다른 구에 비해 높은 억제효과를 나타내었다. 위의 결과를 통해서 장류 및 젓갈 유래 균체의 3가지 전처리 방법 중 100℃, 30 min 처리 시 염증 반응 억제 효과가 가장 높을 것으로 예상된다. 결과를 미루어보아 장류를 섭취하는 대표적인 방법인 가열하여 찌개로 조리과정 중장류 및 젓갈 속의 균체가 열에 가열되면서 염증억제효과 높일 수 있는 가장 좋은 조리 방법으로 사료되며, 이는 대중들이 가장 가깝게 염증반응 억제효과를 볼 수 있는 식품일 것이다. 또한 염장처리과정만을 거친 젓갈을 섭취하는 것보다 열처리과정을 거친 젓갈을 섭취할 경우 염증억제효과를 더 높일 수 있을 것으로 예상된다. The mast cell is one of the major effector cells in inflammatory reactions and can be found in most tissues throughout the body. Activated mast cells can produce histamine, as well as a wide variety of other inflammatory mediators such as eicosanoids, proteoglycans, proteases, and several pro-inflammatory cytokines, such as tumor necrosis factor (TNF)-α, and interleukins (IL-6), IL-8, IL-4, IL-13. In the present study, we isolated two bacterial strains (J80 and G147) from fermented soybean and Jeotgal,and investigated the inhibitory effects of their extracts which were prepared by several pretreatment methods (sonication for 20 min, heating at 100℃ for 30 min, autoclaving at 121℃ for 15 min) on the mast cell-mediated inflammatory response. The pretreated bacterial extracts had no cytotoxicity against Human Mast Cell (HMC-1). Among various pretreatments, the extracts treated at 100℃ showed highest inhibition of histamine release (J80, 28.46%; G147, 41.14%). The J80 and G147 extracts treated at 100℃ resulted in the inhibition of IL-6 secretion by 38.46% and 56.45%, respectively. The J80 extract treated at 100℃ resulted in the inhibition of TNF-α secretion by 66.67%, but G147 extract showed the highest inhibition effect by 41.1% when treated with sonication. These results suggest that bacterial extracts treated at 100℃ have a higher level of anti-inflammatory effects than other treatments such as sonication or autoclaving.
