        Exercise-induced anaphylaxis(EIA) is a medical emergency in that it derives from a physical allergy. We reported a case of food-independent EIA based on history and laboratory data. A 40-year-old male patient was presented with generalized urticaria, angioedema, and syncope after jogging. He had suffered from allergic rhinitis for 25 years. Increased level of serum tryptase was noted. Mast cell activation might be suggested in the pathophysiologic mechanism of EIA. (J Asthma Allergy Clin Immunol 24 : 152-5, 2004)

        오스템퍼링한 구상흑연주철(ADI)의 드릴 가공시 경도 및 현미경조직변화에 관한 연구

        This paper was carried out to know the influence of advanced austempered ductile cast iron (ADI) on the tool life and mechanical properties of drilling machinability. For manufactured method of ADI, the spheroidal graphite cast iron were austenized at 900℃ for 1 hour and then austempered for 2 hour at 370℃ in the salt bath. And interrelationship has been investigated between tool life and mechanical characteristics of specimen material on drilling condition when the ordinary and step-feed drilling are carried out to drill holes of specimens. Tensile strength and hardness of ADI decrease and elongation of ADI increases with the increase austempered temperature. It is known that about 2 times of tool life in the case of step-feed decreases compared with ordinary feed due to the high hardness of ADI and hardness ascribed to the fact that retained austenite beame to martensite state due to cutting heat in drilling. Under the constant feed rate 0.1㎜/rev relation between hardness and length of end tip after drilling can be formularized to Hv=788.46L^-0.096 for the cutting speed 6.1m/min.

      • Selective hydrophobic derivatization on the surface of helical silica nanotubes

        <P><B>Abstract</B></P> <P>The chiral 1,2-diphenylethylenediamine derivative that is capable of spontaneous self-assembly was employed as an organogel template to produce a helical mesoporous silica nanotube containing gelators therein by following sol-gel polycondensation of TEOS. The synthesis enabled the successful introduction of the hydrocarbon of octyl silane (hydrophobic functional group) onto the outer surface of the silica nanotube while preserving the hydrophilic silanol (SiOH) group on internal surface of silica nanotube free from the gelators. This synthetic condition consists of a pre-stage of the introduction of a hydrophobic hydrocarbon functional group onto the outer surface of the silica nanotube selectively, and the post-stage washing of the gelators was presented together with a method analyzing the actions of organogels in the respective experimental processes.</P> <P><B>Highlights</B></P> <P> <UL> <LI> A hydrophobic octyl groups were introduced on the surface of silica nanotube with organogels. </LI> <LI> The hydrocarbons were derivatized on the outer surface of silica nanotube via TEM image mapping and TOF-SIMS spectroscopy mode. </LI> <LI> A synthesis process of derivatizing hydrocarbons on the outer surface of the silica nanotubes has been proposed. </LI> <LI> EA, LC and TOF-SIMS analyses were performed to confirm that the organogels remained in the silica nanotubes during the washing process. </LI> </UL> </P> <P><B>Graphical abstract</B></P> <P>[DISPLAY OMISSION]</P>

      • Utility of GRACE and ACUITY-HORIZONS risk scores to guide dual antiplatelet therapy in Korean patients with acute myocardial infarction undergoing drug-eluting stenting

        <P><B>Abstract</B></P> <P><B>Background</B></P> <P>Dual antiplatelet therapy (DAPT) is recommended in patients receiving drug-eluting stents (DES). However, bleeding risk should be weighed against ischemic risk. Utility of GRACE risk score and ACUITY-HORIZONS bleeding risk score was assessed in patients with acute myocardial infarction (MI) according to use of P2Y12 blocker.</P> <P><B>Methods</B></P> <P>From the Korea Acute Myocardial Infarction Registry-National Institute of Health database, 7791 patients with acute MI receiving DES were divided into ticagrelor (<I>n</I> =1554) and clopidogrel (<I>n</I> =6237) groups. Propensity-matched 12-month mortality and bleeding event rates were compared according to GRACE and ACUITY-HORIZONS scores. Patients who received thrombolysis, prasugrel or anticoagulants, or who discontinued or switched DAPT were excluded.</P> <P><B>Results</B></P> <P>In all patients, high-risk patients more often received clopidogrel. After propensity score matching (<I>n</I> =1553 in each group), 12-month mortality was not different, but TIMI major bleeding rate was higher with ticagrelor (2.8% vs. 1.4%, <I>p</I> =0.007). On subgroup analysis, 12-month mortality was lower with ticagrelor in patients with high (>140) compared to low-to-moderate risk GRACE score (5.1% vs. 7.9%, <I>p</I> =0.04). When combined with ACUITY-HORIZONS bleeding score, 12-month mortality was lower with ticagrelor in patients with high GRACE score but without very high (≥20) ACUITY-HORIZONS score (2.4% vs. 5.3%, <I>p</I> =0.03).</P> <P><B>Conclusions</B></P> <P>In patients with acute MI receiving DES, GRACE and ACUITY-HORIZONS scores may help guide DAPT.         <P>The development of a water oxidation catalyst has been a demanding challenge in realizing water splitting systems. The asymmetric geometry and flexible ligation of the biological Mn<SUB>4</SUB>CaO<SUB>5</SUB> cluster are important properties for the function of photosystem II, and these properties can be applied to the design of new inorganic water oxidation catalysts. We identified a new crystal structure, Mn<SUB>3</SUB>(PO<SUB>4</SUB>)<SUB>2</SUB>·3H<SUB>2</SUB>O, that precipitates spontaneously in aqueous solution at room temperature and demonstrated its high catalytic performance under neutral conditions. The bulky phosphate polyhedron induces a less-ordered Mn geometry in Mn<SUB>3</SUB>(PO<SUB>4</SUB>)<SUB>2</SUB>·3H<SUB>2</SUB>O. Computational analysis indicated that the structural flexibility in Mn<SUB>3</SUB>(PO<SUB>4</SUB>)<SUB>2</SUB>·3H<SUB>2</SUB>O could stabilize the Jahn–Teller-distorted Mn(III) and thus facilitate Mn(II) oxidation.         RTDS-based Model Component Development of a Tri-axial HTS Power Cable and Transient Characteristic Analysis

        The transient characteristics of the tri-axial High Temperature Superconducting (HTS) power cable are different from those of a conventional power cable depending on whether the cable is under a steady or transient state due to the quench. Verification using simulation tools is required to confirm both the characteristics of the cable and the effect of the cable when it is applied to a real utility. However, a component for the cable has not been provided in simulation tools; thus the RTDSbased model component of the tri-axial HTS power cable was developed, and a simulation was performed under the transient state. The considered properties of model component include resistance, reactance and temperature. Simulation results indicate the variation of HTS power cable condition. The results are used for the transient characteristic analysis and stability verification of the tri-axial HTS power cable. In the future, the RTDS-based model component of the cable will be used to implement the hardware-in-the-loop simulation with a protection device.

        Effectiveness of Drug-Eluting Stents versus Bare-Metal Stents in Large Coronary Arteries in Patients with Acute Myocardial Infarction

        <P>This study compared clinical outcomes of drug-eluting stents (DES) versus bare-metal stents (BMS) in large coronary arteries in patients with acute myocardial infarction (MI). A total of 985 patients who underwent single-vessel percutaneous coronary intervention (PCI) in large coronary arteries (≥ 3.5 mm) in lesions < 25 mm were divided into DES group (n = 841) and BMS group (n = 144). Clinical outcomes during 12 months were compared. In-hospital outcome was similar between the groups. At six months, death/MI rate was not different. However, DES group had significantly lower rates of target-lesion revascularization (TLR) (1.7% vs 5.6%, <I>P</I> = 0.021), target-vessel revascularization (TVR) (2.2% vs 5.6%, <I>P</I> = 0.032), and total major adverse cardiac events (MACE) (3.4% vs 11.9%, <I>P</I> = 0.025). At 12 months, the rates of TLR and TVR remained lower in the DES group (2.5% vs 5.9%, <I>P</I> = 0.032 and 5.9% vs 3.1%, <I>P</I> = 0.041), but the rates of death/MI and total MACE were not statistically different. The use of DES in large vessels in the setting of acute MI is associated with lower need for repeat revascularization compared to BMS without compromising the overall safety over the course of one-year follow-up.</P>

        Cystic Hemangiopericytoma in the Third Ventricle

        Primary intracranial hemangiopericytoma is rare and resemble meningioma on imaging study. It shows meningeal attachment, and is usually isointense with gray matter on T1-weighted MR image with heterogeneous enhancement and prominent vascular flow voids on T2-weighted image. Cystic type of hemangiopericytoma is very rare and only 3 cases have been reported in the literature which arised in the middle fossa, cerebellum, and occipital area. Ventricular hemangiopericytomas were reported in 9 cases, and all of them were solid type. Authors experienced a peculiar case of cystic hemangiopericytoma in the 3rd ventricle and report it with review of the literature.

        Comparative Analysis of Clinical Outcomes Using Propensity Score Matching: Coronavirus Disease 2019 vs. Seasonal Influenza in Korea

        Background: The advent of the omicron variant and the formulation of diverse therapeutic strategies marked a new epoch in the realm of coronavirus disease 2019 (COVID-19). Studies have compared the clinical outcomes between COVID-19 and seasonal influenza, but such studies were conducted during the early stages of the pandemic when effective treatment strategies had not yet been developed, which limits the generalizability of the findings. Therefore, an updated evaluation of the comparative analysis of clinical outcomes between COVID-19 and seasonal influenza is requisite. Methods: This study used data from the severe acute respiratory infection surveillance system of South Korea. We extracted data for influenza patients who were infected between 2018 and 2019 and COVID-19 patients who were infected in 2021 (pre-omicron period) and 2022 (omicron period). Comparisons of outcomes were conducted among the pre-omicron, omicron, and influenza cohorts utilizing propensity score matching. The adjusted covariates in the propensity score matching included age, sex, smoking, and comorbidities. Results: The study incorporated 1,227 patients in the pre-omicron cohort, 1,948 patients in the omicron cohort, and 920 patients in the influenza cohort. Following propensity score matching, 491 patients were included in each respective group. Clinical presentations exhibited similarities between the pre-omicron and omicron cohorts; however, COVID-19 patients demonstrated a higher prevalence of dyspnea and pulmonary infiltrates compared to their influenza counterparts. Both COVID-19 groups exhibited higher in-hospital mortality and longer hospital length of stay than the influenza group. The omicron group showed no significant improvement in clinical outcomes compared to the pre-omicron group. Conclusion: The omicron group did not demonstrate better clinical outcomes than the pre-omicron group, and exhibited significant disease severity compared to the influenza group. Considering the likely persistence of COVID-19 infections, it is imperative to sustain comprehensive studies and ongoing policy support for the virus to enhance the prognosis for individuals affected by COVID-19.
