        Comparison of Liver Transplantation and Liver Resection for Hepatocellular Carcinoma Patients with Portal Vein Tumor Thrombus Type I and Type II

        Purpose: The aim of this study was to compare the efficacy of liver transplantation (LT) and liver resection (LR) for hepatocellularcarcinoma (HCC) patients with portal vein tumor thrombus (PVTT) and to investigate risk factors affecting prognosis. Materials and Methods: A total of 94 HCC patients with PVTT type I (segmental PVTT) and PVTT type II (lobar PVTT) were involvedand divided into LR (n=47) and LT groups (n=47). Recurrence-free survival (RFS) and overall survival (OS) were comparedbefore and after inverse probability of treatment weighting (IPTW). Prognostic factors for RFS and OS were explored. Results: Two treatment groups were well-balanced using IPTW. In the entire cohort, LT provided a better prognosis than LR. Among patients with PVTT type I, RFS was better with LT (p=0.039); OS was not different significantly between LT and LR(p=0.093). In subgroup analysis of PVTT type I patients with α-fetoprotein (AFP) levels >200 ng/mL, LT elicited significantly longermedian RFS (18.0 months vs. 2.1 months, p=0.022) and relatively longer median OS time (23.6 months vs. 9.8 months, p=0.065). Among patients with PVTT type II, no significant differences in RFS and OS were found between LT and LR (p=0.115 and 0.335,respectively). Multivariate analyses showed treatment allocation (LR), tumor size (>5 cm), AFP and aspartate aminotransferase(AST) levels to be risk factors of RFS and treatment allocation (LR), AFP and AST as risk factors for OS. Conclusion: LT appeared to afford a better prognosis for HCC with PVTT type I than LR, especially in patients with AFP levels>200 ng/mL.

        Prediction and Monitoring of the Construction Vibration Effect on an Adjacent Old Long Span Double-Convex Arch Bridge

        Ground vibrations caused by the construction of impact drilling piles may affect adjacent bridges, so corresponding prediction and monitoring are necessary to ensure the safety of bridges. In this study, the construction near an existing long-span double-convex arch bridge is taken as a case study to investigate different layouts of construction machines emanating harmful ground vibrations to the existing adjacent bridge by numerical simulations and monitoring. The peak acceleration and velocity acquired from numerical simulations, and monitoring were successfully used to implement impact drilling pile construction near the existing bridge. The results show that there are good consistencies between the monitoring and numerical simulation results, and the vibration acceleration of the foundation of abutments and piers are mainly derived from the vibration caused by the construction within a distance of 50 m, while the construction beyond 50 m has little effect on the old bridge. The numbers and locations of machines near the same foundation may have a weakening or strengthening effect on the peak acceleration and velocity due to destructive and constructive interference of waves, which can help to control the effects of the vibrations of the existing adjacent bridge.

        Synthesis of Core−Shell Au@TiO<sub>2</sub> Nanoparticles with Truncated Wedge-Shaped Morphology and Their Photocatalytic Properties

        <P>Core−shell Au@TiO<SUB>2</SUB> nanoparticles with truncated wedge-shaped TiO<SUB>2</SUB> morphology have been synthesized successfully by a simple and flexible hydrothermal route. Morphological evolution of TiO<SUB>2</SUB> shells was investigated by transmission electron microscopy (TEM), high-resolution transmission electron microscopy (HRTEM), and X-ray diffraction technique. It has been revealed that the truncated wedge-shaped TiO<SUB>2</SUB> shells experience an epitaxially segmented orientation growth. Also, the (101) crystal planes of TiO<SUB>2</SUB> crystals grow preferentially on the surface of gold nanocrystals stabilizing the heterointerfaces, then faster [001] growth results in the “budding” process occurs, producing growth sites on the initial deposition TiO<SUB>2</SUB> layers, where the TiO<SUB>2</SUB> crystals grow up into truncated wedge-shaped morphologies. It is also found that morphological evolution of TiO<SUB>2</SUB> shells is dependent on the produced F<SUP>−</SUP> ion concentration from hydrolyzed TiF<SUB>4</SUB> precursors. The produced F<SUP>−</SUP> ions not only facilitate the formation of well-defined wedge-like TiO<SUB>2</SUB> shells, but also contribute to the externally exposed truncated crystal {004} facets. As the representative photocatalyst, the catalytic activities of the resultant core−shell Au@TiO<SUB>2</SUB> nanoparticles were investigated by photoinitiated oxidation degradation of gaseous acetaldehyde. It has been indicated that the nanostructured core−shell Au@TiO<SUB>2</SUB> photocatalyst represents high photocatalytic activity when exposed to UV or visible light irradiation.         The outcomes of first-generation EGFR-TKIs (Gefitnib and Erlotinib) have shown great advantages over traditional treatment strategies in patients with non-small cell lung cancer (NSCLC), but unfortunately we have to face the situation that most patients still fail to respond in the long term despite initially good control. Up to now, the mechanism of acquired resistance to EGFR-TKIs has not been fully clarified. Herein, we sought to compile the available clinical reports in the hope to better understanding the subsequent treatment choices, particularly on whether restoring after a drug holiday or switching to another EGFR-TKI is the better option after failure of one kind of EGFR-TKI.

        Studies of the Electronic and the Optical Properties of the Ordered Ternary Alloys X0.5Y0.5O2(X, Y = Si, Ge, Sn) by Using First-principles Calculations

        In this paper, the electronic and the optical properties of oxides for group-IV elements in an inverse Ag2O structure, including Si, Ge, and Sn, and their ternary alloys, were studied with the density function theory of first principles. The results show that they have a high-density phase and a high dielectric constant. Moreover, the lattice constants of Si0.5Ge0.5O2 and SiO2 are really close to those of Si(100) surfaces, suggesting the possibility of forming a defect-free superlattice. With changing element, the calculated results show that the lattice constants of the ternary alloys tend to be bigger while the energy gaps show a different trend. The optical properties of these compounds as functions of physical quantities, such as the density of states, the complex dielectric function,and the static dielectric constant, are given to support the potential applications of the compounds in the future.

        Outcome analysis in patients with uterine sarcoma

        Purpose: To analyze the prognostic factors for survivals and to evaluate the impact of postoperative whole pelvic radiotherapy (WPRT) on pelvic failure in patients with uterine sarcoma treated with radical surgery. Materials and Methods: We retrospectively analyzed 75 patients with uterine sarcoma who underwent radical surgery with (n = 22) or without (n = 53) radiotherapy between 1990 and 2010. There were 23 and 52 patients with carcinosarcoma and non-carcinosarcoma (leiomyosarcoma, 22; endometrial stromal sarcoma, 25; others, 5), respectively. The median follow-up period was 64 months (range, 17 to 269 months). Results: The 5-year overall survival (OS) and pelvic failure-free survival (PFFS) of total patients was 64.2% and 83.4%, respectively. Multivariate analysis revealed that mitotic count (p = 0.006) was a significant predictor of OS. However, factors were not found to be associated with PFFS. On analyzing each of the histologic subtypes separately, postoperative WPRT significantly reduced pelvic failure in patients with carcinosarcoma (10.0% vs. 53.7%; p = 0.046), but not in patients with non-carcinosarcoma (12.5% vs. 9.9%; p = 0.866). Among the patients with carcinosarcoma, 4 patients (17%) had recurrence within the pelvis and 3 patients (13%) had recurrence in other sites as an initial failure, whereas among the patients with non-carcinosarcoma, 3 patients (6%) experienced pelvic failure and 13 patients (25%) experienced distant failure. Conclusion: The most significant predictor of OS was mitotic count. Based on the improved PFFS after postoperative WPRT only in patients with carcinosarcoma and the difference in patterns of failure between histologic subtypes, optimal adjuvant treatment options should be offered to patients based on the risk of recurrence patterns.

      • The Clinicopathological and Prognostic Impact of 14-3-3 Protein Isoforms Expression in Human Cholangiocarcinoma by Immunohistochemistry

        The 14-3-3 proteins are highly conserved, ubiquitous molecules involved in a variety of biologic phenomena, such as cell cycle control, and apoptosis. However, their expression in cholangiocarcinoma has not been previously characterized. In this paper, immunohistochemistry using specific anti-14-3-3 monoclonal antibodies was performed on formalin-fixed;, paraffin embedded archival tissue from 86 patients of cholangiocarcinoma. We also examined the correlation between expression and survival rate and clinicopathologic factors such as tumor location, tumor size, pathologic differentiation, lymphatic permeation, lymph node metastasis, and tumor stage. Positive 14-3-3 proteins expression was observed for 6 isoforms (${\beta}$, ${\sigma}$, ${\gamma}$, ${\theta}$, ${\delta}$, ${\eta}$) of these proteins in 86 patients of cholangiocarcinoma. ${\beta}$ and ${\sigma}$ isoform immunoreactivity was correlated with lymph node metastasis, tumor stage and patients' survival rate. In addition, ${\delta}$ isoform immunoreactivity showed trends with tumor location, tumor size, pathologic differentiation and tumor stage, while the ${\theta}$ isoform was correlated with pathologic differentiation. These results indicated that upregulated expression of some isoforms of 14-3-3 may be a common mechanism for evading apoptosis in cholangiocarcinoma, so that targeting 14-3-3 may be a novel promising strategy for the treatment of this tumor.

        Theoretical Study on the Point Defects in N-Doped Anatase TiO2

        In this work, first-principles calculations for the electronic and the optical properties of titanium dioxide (TiO2) with point defects are performed by using a plane wave pseudopotential method in the framework of the density functional theory and the generalized gradient approximation. The point defects, substitutional and interstitial nitrogen, in anatase TiO2 are investigated to understand the origin of the visible-light sensitivity of nitrogen (N)-doped TiO2. The results show that bands originating from substitutional N 2p states appear above the top of the valence band. These states tend to be delocalized and to mix with the valence band as the nitrogen concentration is increased. The optical absorption in the range between 400 and 700 nm is enhanced by the substitutional N impurities. The eect of interstitial N on the electronic and the optical properties of TiO2 depends on the interaction between the nitrogen dopants and the surrounding oxygen. The bonding between N and O introduces a series of localized occupied states under and above the upper valence band, and the electron transitions from these states improve the visible-light absorption of TiO2.
