Purpose: Strokes are challenging to diagnose in pediatric emergency departments (EDs) as level of suspicion is low and atypical presentations are common. We analyzed clinical features, epidemiology and factors of delayed identification in arterial ischemic strokes (AIS) and hemorrhagic strokes (HS). Methods: Single-center retrospective cohort study of children aged between 29 days and 18 years old diagnosed with stroke between July 2016 to June 2021. Results: Among 36 children, 11 (30.5%) had AIS, 25 (69.4%) had HS. Median age for AIS was 9 years (interquartile range [IQR], 2 to 9) and HS 9 years (IQR, 1 to 11.5) (P=0.715). Focal neurological deficit was seen in 72.7% of AIS and 20% of HS (P=0.006). Only 18.2% of AIS and 52.0% of HS presented within 6 hours of symptoms. Median time from symptom onset to ED presentation was 24 hours (IQR, 12 to 28) for AIS and 7 hours (IQR, 1.8 to 48) for HS (P=0.595). Most (85.6%) arrived by own transport. Median time from presentation to neuroimaging was 7 hours (IQR, 0.9 to 7) for AIS and 4.8 (IQR, 1.3 to 16.8) hours for HS (P=0.376). Eleven patients, 9/25 (36.0%) HS and 2/11 (18.2%) AIS, did not have stroke as differential diagnosis at ED (P=0.714). Common initial diagnoses were viral illness or headaches. On univariate analysis, age <1 (odds ratio [OR], 17.5; 95% confidence interval [CI], 1.2 to 250.4; P=0.035) and absence of focal neurological deficit (OR, 13.091; 95% CI, 1.5 to 117.9; P=0.022) were significant factors for delayed identification. Conclusion: Index of suspicion for pediatric strokes among caregivers and clinicians should be increased. Public awareness campaigns are recommended.
Examination of Chinese homeroom teachers' performance of professional school counselors' activities
Qi Shi,Wade C. Leuwerke 서울대학교 교육연구소 2010 Asia Pacific Education Review Vol.11 No.4
Chinese homeroom teachers' performance of professional school counselor activities was explored. A total of 109 homeroom teachers in Beijing and Harbin, China reported their performance of 68 different school counseling activities as part of their regular actions as a homeroom teacher. Results found that on average homeroom teachers performed a wide variety of counseling tasks and together almost all tasks were performed by homeroom teachers. Further, participants reported frequently providing individual counseling to students on a range of concerns. Findings in this study support calls for the continued development of school counseling in China, hiring more school counselors, and additional training for homeroom teachers. Limitations, recommendations, and avenues for future research are described.

Background: PTSD (Post-traumatic stress disorder, PTSD) had a great impact on health care workers during the COVID-19 (Corona Virus Disease 2019, COVID-19). Better knowledge of the prevalence of PTSD and its risk factors is a major public health problem. This study was conducted to assess the prevalence and important risk factors of PTSD among medical staff during the COVID-19. Methods: The databases were searched for studies published during the COVID-19, and a PRISMA (preferred reporting items for systematic review and meta-analysis) compliant systematic review (PROSPERO-CRD 42021278970) was carried out to identify articles from multiple databases reporting the prevalence of PTSD outcomes among medical staff. Proportion random effect analysis, I2 statistic, quality assessment, subgroup analysis, and sensitivity analysis were carried out. Results: A total of 28 cross-sectional studies and the PTSD results of doctors and nurses were summarized from 14 and 27 studies: the prevalences were 31% (95% CI [confidence interval, CI]: 21%-40%) and 38% (95% CI: 30%-45%) in doctors and nurses, respectively. The results also showed seven risks (p < 0.05): long working hours, isolation wards, COVID-19 symptoms, nurses, women, fear of infection, and pre-existing mental illness. Two factors were of borderline significance: higher professional titles and married. Conclusion: Health care workers have a higher prevalence of PTSD during COVID-19. Health departments should provide targeted preventive measures for medical staff away from PTSD.

B, N-codoped carbon nanofibers were massively prepared by heat treatment of electrospun carbon nanofibers with the mixture of boric acid/urea in N2 (BNCf-N) and subsequently activated in NH3 (BNCf- NA). The directly electrospun self-standing 3D non woven structure with void spaces between each fiber facilitates the mass transport of reactant and resulted molecules. Further NH3 activation gives BNCf-NA a high surface area of 306.3 m2 g-1 with micro/mesoporous structure, providing abundant passageway for proton transfer. Simultaneously, NH3 activation also realizes the optimization of surface functionalities, such as more BeNeC and pyridinic-N. These intriguing features render BNCf-NA excellent catalytic behavior with nearly four-electron oxygen reduction reaction (ORR) process in alkaline media, especially much better stability and methanol tolerance than the commercial Pt/C catalyst. Our work provides a large-scale preparation method for efficient metal-free catalysts toward ORR, thus further intensifying the commercial application of fuel cells.

BACKGROUND: Osteoporosis is a systemic bone disease characterized by decreased bone density and deterioration of bone microstructure, leading to an increased probability of fragility fractures. Once segmental bone defect occurs, it is easy to cause delayed union and nonunion. METHODS: The aim of this study is to investigate the efficacy of extracorporeal shock wave (ESW) and teriparatide-loaded hydrogel (T-Gel) combined strategy on the cell activity and differentiation of osteoporosis derived bone marrow mesenchymal stem cells (OP-BMSCs) in vitro and bone regeneration in osteoporotic segmental bone defects in vivo. RESULTS: In vitro, the strategy of combining ESW and T-Gel significantly enhanced OP-BMSCs proliferation, survival, migration, and osteogenic differentiation by up-regulating the alkaline phosphatase activity, mineralization, and expression of runt-related transcription factor-2, type I collagen, osteocalcin, and osteopontin. In the segmental bone defect models of osteoporotic rabbits, Micro-CT evaluation and histological observation demonstrated this ESW-combined with T-Gel injection significantly induced bone healing by enhancing the osteogenic activity of the local microenvironment in osteoporotic defects. CONCLUSION: In conclusion, ESW-combined with T-Gel injection could regulate the poor osteogenic microenvironment in osteoporotic defects and show potential for enhancing fragility fractures healing. This paper proposes an Image Texture Median Filter (ITMF) to analyze and detect Android malware on Drebin datasetsMedian Filter (MF) to reflect the similarity of the malware binary file block. At the same time, using the MAEVS (Malware Activity Embedding in Vector Space) to reflect the potential dynamic activity of malware. In order to ensure the improvement of the classification accuracy, the above-mentioned features(ITMF feature and MAEVS feature)are studied to train Restricted Boltzmann Machine (RBM) and Back Propagation (BP). The experimental results show that the model has an average accuracy rate of 95.43% 1. We design a model of “ITMF” combined with Image Processing of with few false alarms. to Android malicious code, which is significantly higher than 95.2% of without ITMF, 93.8% of shallow machine learning model SVM, 94.8% of KNN, 94.6% of ANN.
