      • Degradation mechanisms in gate-all-around silicon Nanowire field effect transistor under electrostatic discharge stress – a modeling approach

        The failure and degradation mechanisms of gate-all-around silicon nanowire FET subjected to electrostatic discharge (ESD) are investigated through device modeling. Transmission line pulse stress test is simulated and device degradation physics is modeled. The device degradation level, interface state concentration and hard breakdown are shown and analyzed. From the model, we found that ESD stress can induce severe performance degradation or even hard breakdown of gate-all-around nanowire device, and the interface traps due to hot carrier injection is responsible for the device degradation.

        Propofol suppresses non-small cell lung cancer progression by modulating circ_0001727/miR-516b-5p/LRRC1 axis

        Propofol plays an anti-cancer role in diverse cancers, including non-small cell lung cancer (NSCLC). We aimed to study the function and underlying mechanism of propofol in NSCLC.Cell Counting Kit-8 (CCK-8) and colony formation assays were used to detect cell proliferation. The expression of circ_0001727, microRNA-516b-5p (miR-516b-5p) and leucine-rich repeat-containing protein 1 (LRRC1) mRNA was tested via quantitative real-time polymerase chain reaction (qRT-PCR). Cell migration and invasion were assessed by transwell assay. Angiogenesis and cell apoptosis were determined by tube formation assay and flow cytometry, respectively. Western blot (WB) assay was performed to measure all protein levels. In vivo experiments were conducted via establishing mice xenograft model. Dual-luciferase reporter and RNA Immunoprecipitation (RIP) assays were carried out to verify the relationship between miR-516b-5p and circ_0001727 or LRRC1.Circ_0001727 was overexpressed in NSCLC, and propofol treatment reduced circ_0001727 level in NSCLC cells. Propofol could repress proliferation, migration, invasion, and angiogenesis while accelerated apoptosis of NSCLC cells, while these effects were augmented by circ_0001727 knockdown. Moreover, circ_0001727 depletion in combined with propofol also inhibited tumorigenesis in vivo. MiR-516b-5p was targeted by circ_0001727, and miR-516b-5p downregulation counteracted the suppressive influence of circ_0001727 deficiency on the malignant behaviors of NSCLC cells. LRRC1 was targeted by miR-516b-5p, and miR-516b-5p exerted its anti-tumor function in NSCLC cells by targeting LRRC1. Additionally, circ_0001727 regulated LRRC1 expression via sponging miR-516b-5p.Propofol inhibited NSCLC progression by regulation of circ_0001727/miR-516b-5p/LRRC1 axis, which might offer an effective therapeutic target for NSCLC therapy.

      • Multilateral Engagement of North Korea: An Assessment of the Six-Party Talks and the ASEAN Regional Forum

        North Korea’s unchecked missile and nuclear program is one of the most pressing global security concerns. This article evaluates the multilateral engagement efforts that have been pursued by regional stakeholders, specifically assessing the Six-Party Talks vis-a-vis the ASEAN Regional Forum (ARF), and explaining why these multilateral efforts have failed to resolve the nuclear crisis. Given the poor performances of these two multilateral platforms, this article seeks to assess the feasibility and policy implications of defusing the longstanding nuclear crisis through multilateral engagement. Despite stalling and a myriad of obstacles, the Six-Party Talks has a better chance than the ARF at curbing the nuclear crisis. At best, the ARF can contribute by playing a complementary role by helping deescalate tensions or cultivating better diplomatic ties.

        Laparoscopic splenectomy for splenic rupture secondary to metastatic choriocarcinoma

        Gestational choriocarcinoma is a rare and aggressive type of gestational trophoblastic neoplasia, which is characterized by early vascular invasion and widespread metastases. Choriocarcinoma metastasizes hematogenously, and bleeding from metastases is common. Splenic rupture from a metastatic tumour is exceedingly rare, with only a few reports. We report a case of a 41-year-old female presenting with acute abdomen and haemorrhagic shock secondary to splenic rupture from metastatic choriocarcinoma, which was managed with emergency laparoscopic splenectomy.

        Synthesis of stable hollow silica nanospheres

        Hollow silica nanospheres with their size distribution ranging from 350 nm to 450 nm are synthesized by using polystyrene (PS) templates in the present study. On the basis of PS templates, silica, the hydrolyzate of TEOS(tetraethyl orthosilicate) under moist alkaline condition at ambient temperatures and atmospheric pressures, it is set to be coated on the surface of the PS spheres. Since the size of PS sphere core can be easily controlled, it is expected to serve various needs of different sized hollow silica nanospheres in industrial applications. It is proposed that the PS cores be removed by either thermal pyrogenation or solvent dissolved. Morphology of the hollow silica nanospheres is characterized by scanning electron microscopy (SEM).

        Effective online large-group teaching in health professions education

        Online large-group teaching (OLGT), employed to reach a large group of learners in separate physical locations, allows asynchronous learning and facilitates social distancing. While online large-groups can be a powerful and resource-lean means of health professions education, it has challenges and potential pitfalls that may affect the learning process and outcomes. Through a sociomateriality framework, this article describes strategies for effective online large-group teaching in health professions education in three key strands. Firstly, to optimize learning, OLGT sessions should match learning needs with appropriate OLGT platforms, incorporate strategies to sustain learner attention, and accommodate learners of different abilities. Secondly, to develop a learning culture, OLGT must not only focus on cognitive aspects of learning but also build a community of practice, nurture digital professionalism and professional identity. Thirdly, we discuss the avoidance of pitfalls such as cognitive overload of both tutors and learners, technical issues and security risks, mitigating inequities in access to online learning, and the use of program evaluation to plan for sustained improvements. We conclude with a case vignette that discusses the challenges of OLGT and the application of the above strategies in a teaching scenario.

        Extended Whole-body Ga-68 DOTATATE PET-CT in evaluating Tumour-Induced Osteomalacia: Case report and review of literature

        Tumour-induced osteomalacia is a rare paraneoplastic syndrome that manifests as chronic hypophosphataemia, non-specific bone pain and muscle weakness. It is generally caused by phosphaturic mesenchymal tumour (PMT), which is uncommonly associated with synchronous tumours. However, diagnosis is often delayed for several years due to the rarity, indolent growing nature and non-specific symptoms of the disease, often resulting in an overlook by clinicians during assessments. The patient initially presented with hypophosphataemia and generalised skeletal pain with multiple atraumatic fractures. Blood tests revealed serum calcium levels at the upper limit and extremely low inorganic phosphate levels. Herein, we report a case where two synchronous PMTs from two different sites were detected by ‘extended’ whole-body Ga-68 DOTATATE PET-CT, leading to remission of the disease after complete surgical removal. Early detection and diagnosis of PMT neoplasm is crucial, as complete surgical resection of this tumour is the only definitive treatment currently known. Upon excision, this curable disease will result in complete resolution of symptoms and blood parameters, leading to remission of the disease which significantly improves the patient’s quality of life. PMT often over-expresses somatostatin receptors (SSTR), predominantly subtype 2A, and Ga-68 DOTATATE PET-CT is a selective SSTR imaging that targets this characteristic over-expression in these tumours. The high diagnostic accuracy of Ga-68 DOTATATE PET-CT should be the primary imaging modality for full evaluation of this disease.

        Global prevalence of depression and anxiety in patients with hepatocellular carcinoma: Systematic review and meta-analysis

        Background/Aims: Depression and anxiety are associated with poorer outcomes in patients with hepatocellular carcinoma (HCC). However, the prevalence of depression and anxiety in HCC are unclear. We aimed to establish the prevalence of depression and anxiety in patients with HCC. Methods: MEDLINE and Embase were searched and original articles reporting prevalence of anxiety or depression in patients with HCC were included. A generalized linear mixed model with Clopper-Pearson intervals was used to obtain the pooled prevalence of depression and anxiety in patients with HCC. Risk factors were analyzed via a fractional-logistic regression model. Results: Seventeen articles involving 64,247 patients with HCC were included. The pooled prevalence of depression and anxiety in patients with HCC was 24.04% (95% confidence interval [CI], 13.99–38.11%) and 22.20% (95% CI, 10.07–42.09%) respectively. Subgroup analysis determined that the prevalence of depression was lowest in studies where depression was diagnosed via clinician-administered scales (16.07%;95% CI, 4.42–44.20%) and highest in self-reported scales (30.03%; 95% CI, 17.19–47.01%). Depression in patients with HCC was lowest in the Americas (16.44%; 95% CI, 6.37–36.27%) and highest in South-East Asia (66.67%; 95% CI, 56.68–75.35%). Alcohol consumption, cirrhosis, and college education significantly increased risk of depression in patients with HCC. Conclusions: One in four patients with HCC have depression, while one in five have anxiety. Further studies are required to validate these findings, as seen from the wide CIs in certain subgroup analyses. Screening strategies for depression and anxiety should also be developed for patients with HCC.

        Perceptions of a night float system for intern doctors in an internal medicine program: an Asian perspective

        Long duty hours have been associated with significant medical errors, adverse events, and physician “burn-out”. An innovative night float (NF) system has been implemented in our internal medicine program to reduce the negative effects of long duty hours associated with conventional full-call systems. However, concerns remain if this would result in inadequate training for interns. We developed a structured questionnaire to assess junior doctors’ perceptions of the NF system compared to full calls, in areas of patient safety, medical training, and well-being. Ninety-seven (71%) of the 137 doctors polled responded. Ninety-one (94%) felt the NF system was superior to the full call system. A strong majority felt NF was beneficial for patient safety compared to full call (94% vs. 2%, p<0.001). The NF system was also perceived to reduce medical errors (94% vs. 2%, p<0.001) and reduce physician “burn-out” (95% vs. 5%, p<0.001). Beyond being a practical solution to duty-hour limitations, there was a significant perceived benefit of the NF system compared to the full call in terms of overall satisfaction, patient safety, reducing medical errors and physician “burn-out”.
