Background/Aims: We evaluated the clinical features of ciprofloxacin-resistant Proteus mirabilis bacteremia and risk factors for ciprofloxacin resistance. Methods: From October 2000 to July 2009, 37 patients with clinically significant P. mirabilis bacteremia were identified and data from patients with ciprofloxacin-resistant and ciprofloxacin-susceptible P. mirabilis bacteremia were compared. Results: The most common underlying diseases were neurologic disease (37.8%) and solid tumors (29.7%). The most common site of infection was the urinary tract (35.1%). Ten of the 37 patients (27.0%) were infected with ciprofloxacin-resistant isolates, and univariate analysis revealed a significant relationship between ciprofloxacinresistant P. mirabilis bacteremia and neurologic disease, recent operation, L-tube insertion, percutaneous tube use, and extended-spectrum β-lactamase (ESBL) production (all p < 0.05). ESBL was detected in six of 10 (60%) ciprofloxacin-resistant isolates, while only three of 27 (11%) ciprofloxacin-susceptible isolates produced ESBL (p = 0.005). In a logistic regression analysis, ESBL production remained a significant factor associated with ciprofloxacin resistance, after adjusting for other variables. Conclusions: These data indicate a close association between ciprofloxacin resistance and ESBL-production in P. mirabilis bacteremia. This association is particularly troublesome because the therapeutic options for serious infections caused by ESBL-producing P. mirabilis are severely restricted. (Korean J Intern Med 2011;26:89-93)
벼 Callus 繼代培養이 植物體 再分化에 미치는 影響

벼 完熟玄米의 胚로부터 誘起된 callus의 繼代培養에 影響을 미치는 繼代培養 回數, 期間 및 培地內에 添加되는 生長調整劑의 組成 等과 같은 몇가지 要因에 대한 實驗을 遂行하여 얻어진 結果는 다음과 같다. 벼 callus의 繼代培養은 4週間隔 보다는 2週間隔으로 培養하는 것이 效果的이었고 植物體 再分化率은 品種과 繼代培養 期間에 따라 크게 相異한 것으로 나타났다. "盈德벼"의 경우는 2週間隔으로 4回 繼代培養된 callus에서 72.5%의 가장 높은 再分化率을 보였으며, 4週間隔 2回 繼代培養된 callus에서도 58.3%의 再分化率을 보였다. Callus誘起 培地에 CH(2g/1), MI(200㎎/1), TH(2㎎/1) 等을 添加하면 callus生長量도 많았고 植物體 再分化率도 向上되었으며, 植物體 再分化에는 kinetin 單用보다는 kinetin과 NAA를 混用하는 것이 效果的인 것으로 나타났다. Callus 繼代培養에 CH, MI, TH 等을 添加한 培地를 利用하면 embryogenic callus의 發生量이 增加하는 경향이었고 品種에 따라서는 植物體 再分化率도 높게 維持되는 것으로 나타났다. The effect of subculture intervals and passages on plant regeneration from seed derived callus was determined. Regeneration capacity of callus varied with rice cultivars and subculture intervars tested. The callus subcultured every 2 weeks produced more plants than that of 4 weeks. The calli from a Tongil-type rice cultivar, Milyang 23, lost easily their regeneration ability when the calli were subcultured every 2 weeks and 4 weeks. The callus induced from a japonica cultivar, "Yeongdeogbyeo", showed to maintain high frequency(>70%) of plant regeneration when it was subcultured every 2-week intervals. Casein hydrolysate supplemented in callus induction medium enhanced callus growth and its regeneration. High frequency of plant regeneration was obtained from the calli transferred on ?? medium supplemented with kinetin (2㎎/1) and NAA(1㎎/1). The subcultured calli in the medium supplemented with casein hydrolysate(2g/1), myo-inositol(200mg/1) and thiamine-HCL(2㎎/1) increased the frequency of embryogenic callus formation and plant regeneration.

This study was made to identify compliance in self-medication, and factors influencing the self-medication of pulmonary TB patients. Self-medication of pulmonary TB patients is a very important factor for the cure of the patients. In this study, variables were used from three theories of health behavior. These were the Health Belief Model, Health Locus of Control, Theory of Planned Behavior. These were included to examine their effect on self-medication. Data were collected during the period from July 1 to August 20, 1994 using a structured questionnare. And they were analyzed by mean, standard deviation. ANOVA, Pearson Correlation Coefficient, and Multiple Regression analysis using the SAS program. The result were as follows : 1. The mean on the self-medication scores ranging from 6.0 to 12.0 was 10.93. The mean for the self-evaluation scores of the self-medication ranging from 50.0 to 100.0, was 86.51. 2. There were significant associations between the scores on self-medication and age(F=2.34, P=0.033), and method of treatment(F=4.65, P=0.018). And there were significant associations between the self-evaluation scores of self-medication and age(F=3.79,P=0.000), marital status(F=7.91, P=0.000), and presence of TB patients among family(F=4.92, P=0.000). 3. (a) the relationship between the scores on self-medication and perceived barrier in health belief revealed a significant correlation(r=-0.2046, p=0.0082). (b) The relationship between the scores on self-medication and other-dependency in LOC revealed a significant correlation(r=0.2322, p=0.0018). (c) The relationship between the self-evaluation score of self-medication and other-dependency in LOC revealed a significant correlation(r=0.1946, p=0.0122). (d) The relationship between the attitude in self-medication of the subjects and the self-evaluation score of self-medication revealed a significant correlation(r=0.2102, p=0.0066). 4. (a) 14.8% of the score of compliance in self-medication of the subjects was explained by five variables ; Behavioral Intention, Duration of Treatment, Age, Perceived Sensitivity and Perceived Barrier. (b) 8.7% of the score of self-evaluation of self-medication was explained by three variables : Perceived Control, Perceived Sensitivity, and Age. In conclusion : This study provides insights and information which may be valuable for motivation and instruction to improve compliance in self-medication among pulmonary TB patients.

In order to calculate the daily intake amount of NO_3 for Korean, it is necessary to evaluate the basic information on average NO_3 content in beverage & drinks like another major sources for NO_3 such as vegetables, milk, grain, fruit and water. This experiment was aimed to find out the average content of NO_3 in favorite drinks for Koreans, the content of NO_3 in 5 sorts of tea, 7 coffees, 5 beers and 14 Korean traditional teas were investigated by 3 manufacturers in 5 duplications. The NO_3 content in Korean traditional tea show between 15 and 49ppm, but Brown rice-Green tea and Green tea have lower value than Red tea, Duchung tea and Jasmine tea. The lowest concentration among the Korean traditional tea shows in the Tea water boiled with burned rice(Sungryung tea). Chrysanthemum tea has the highest value among the imported teas, but Yongjeongmyeong tea has the lowest one. Red teas imported from Sri Lanka. USA show 30-33ppm which is similar to Korean red tea. The NO_3 content in de-caffeined coffee has the lowest value ever, but instant coffee such as Maxim coffee and Choice coffee show 44ppm and 47ppm respectively, whereas Raspberry coffee and Hazelunt coffee show relatively low concentration between 20 and 24ppm. But beers have lower value in NO_3 content compared to another beverage such as tea, coffee and Korean traditional tea. There is no difference between Korean produced or imported beer in terms of NO_3 content, but the lowest value was shown in Heinecken beer.

<P>Autophagy is an important antimicrobial effector process that defends against Mycobacterium tuberculosis (Mtb), the human pathogen causing tuberculosis (TB). MicroRNAs (miRNAs), endogenous noncoding RNAs, are involved in various biological functions and act as post-transcriptional regulators to target mRNAs. The process by which miRNAs affect antibacterial autophagy and host defense mechanisms against Mtb infections in human monocytes and macrophages is largely uncharacterized. In this study, we show that Mtb significantly induces the expression of MIR144*/hsa-miR-144-5p, which targets the 3-untranslated region of DRAM2 (DNA damage regulated autophagy modulator 2) in human monocytes and macrophages. Mtb infection downregulated, whereas the autophagy activators upregulated, DRAM2 expression in human monocytes and macrophages by activating AMP-activated protein kinase. In addition, overexpression of MIR144* decreased DRAM2 expression and formation of autophagosomes in human monocytes, whereas inhibition of MIR144* had the opposite effect. Moreover, the levels of MIR144* were elevated, whereas DRAM2 levels were reduced, in human peripheral blood cells and tissues in TB patients, indicating the clinical significance of MIR144* and DRAM2 in human TB. Notably, DRAM2 interacted with BECN1 and UVRAG, essential components of the autophagic machinery, leading to displacement of RUBCN from the BECN1 complex and enhancement of Ptdlns3K activity. Furthermore, MIR144* and DRAM2 were critically involved in phagosomal maturation and enhanced antimicrobial effects against Mtb. Our findings identify a previously unrecognized role of human MIR144* in the inhibition of antibacterial autophagy and the innate host immune response to Mtb. Additionally, these data reveal that DRAM2 is a key coordinator of autophagy activation that enhances antimicrobial activity against Mtb.</P>

Melioidosis, an infectious disease caused by the facultative intracellular gram-negative bacillus Burkholderia pseudomallei, is geographically restricted to Southeast Asia and Northern Australia. It commonly affects the patients with underlying medical diseases, such as diabetes mellitus or renal failure. In South Korea, only six cases of melioidosis have been reported in literature. However, no patients had chronic kidney disease. We report a case of melioidosis presenting as septicemic pneumonia in a 48-year old man undergoing hemodialysis. He was a returning traveler from Thailand.
