      • 顯忠祠의 植栽樹種에 관한 硏究

        One of the major reasons for what Hyonch'ungsa was built is handing down the patriotic spirit of General Lee, Sun-sin from generation to generation. It was designated as a historic site (National No. 155) by our government and has been visited by many people including students and tourists. This paper is to explore some problems revealed against the spirit of Chungmugong (General Lee's added name) with realtion to the plants in the sacred area of Hyonch'ungsa. 1. 21.33% of the plants came from foreign countries. They are all considered unproper as historic site plants. 2. Above all, 26.83% of plnats which were from Japan could have diluted the symbolic meaning of Chungmugong. 3. Most of plants around the main building and the relic pavilion which are from abroad including Japan could be come into serious question. 4. Large clusters of exotic plants have tendency of swallowing up the magnificient appearance of the precinct buildings. 5. The growth of the plants close to the buildings keeps the dignified appearance shrunk. 6. The high ratio of planing evergreen trees may deprive us a strong sence of a season. 7. Some kinds of shade trees could be recommended around the thru-way so as to provide us with comfortableness and agreeableness especially in summer times. It is suggested that the plants of foreign origin have to be substituted for those of our own in order to keep the proper meaningness of Hyonch'ungsa.

        용접공들의 면역능에 관한 연구

        The authors examined human immunity indicators such as CD4, CD8, Natural Killer(NK) cell, IgG, IgA, and IgM, and heavy metals such as Pb, Cr, Cd to evaluate the health hazard of welders. We selected total 126 male subjects, 60 welders who had more than 5 year working history as an exposure group, and 66 office workers and medical college students as a control group. This immunity evaluation approach is the first newly designed study that has never been tried in past studies on welder's heath evaluation. And the results were as follows. 1. Total lymphocyte count of peripheral blood was significantly higher in welder group, 2,615±650 ea/㎕ compared with 2,368±681 ea/㎕ in control group(p>0.05). 2. The proportion of CD4, CD8, NK cell in welder group was 37.5±9.4%, 24.0±8.8%, and 21.0±9.5%, respectively, and 35.5±6.5%, 25.0±6.9%, and 19.6±8.8% in control group. CD4 and NK cell were slightly higher, and CD8 was slightly lower in welder group. but there was no statistical significance between two groups(p≥0.05). 3. IgG, IgA, and IgM in welder group were 15.9㎎/㎗, 2.7㎎/㎗ and 1.5㎎/㎗, respectively, and 15.6㎎/㎗, 2.9㎎/㎗ and 1.7㎎/㎗ in control group. There was no statistical significance between two groups(p≥0.05). 4. Blood lead, cadmium and chromium concentrations were 4.2±1.8㎍/㎗, 0.4±0.38㎍/㎗, 1.2±0.6㎍/㎗ in welder group and 3.2±1.7㎍/㎗, 0.44±0.29㎍/㎗, 1.1±0.6㎍/㎗ in control group, respectively, and these value were within normal ranges.

        Case Reports : Nicolau Syndrome in Patient Following Diclofenac Administration: A Case Report

        Nicolau syndrome is a rare adverse reaction to a variety of intra-muscular drug preparations. The typical presentation is pain around the injection site soon after injection, followed by erythema, livedoid patch, hemorrhagic patch, and finally, necrosis of skin, subcutaneous fat, and muscle tissue. The phenomenon has been related to the administration of a variety of drugs, including non-steroidal anti-inflammatory drugs, corticosteroids, and penicillin. We report a case with typical features associated with diclofenac injection for pain control in a patient who had undergone bilateral total knee arthroplasty. (Ann Dermatol 23(4) 501~503, 2011)

        Navigation-Assisted Total Knee Arthroplasty for the Knee Retaining Femoral Intramedullary Nail, and Distal Femoral Plate and Screws

        Proper ligament balancing, restoration of the mechanical axis and component alignment are essential for the success and longevityof a prosthesis. In conventional total knee arthroplasty (TKA), an intramedullary guide is used to improve the alignment. Anextramedullary guide can be used in cases of severe femoral bowing or intramedullary nailing but its use is more subjective andrelies on the surgeon's experience. This paper reports two successful cases of navigation-assisted TKA for severe right knee osteoarthritisretaining a femoral intrameullary nail, and left knee osteoarthritis retaining a distal femoral plate.

        Accuracy of Patient-Specific Instrument for Cylindrical Axis Implementation in Kinematically Aligned Total Knee Arthroplasty

        Background: In kinematically aligned total knee arthroplasty (KA-TKA), the cylindrical axis (CA) is very important in restoring the native joint line and kinematics of the pre-arthritic knee. This study aimed to determine the accuracy of patient-specific instrument (PSI) for restoring the CA for femoral bone resection in KA-TKA. Methods: Thirty KA-TKAs were performed using a computed tomography (CT)-based PSI system. Data from preoperative CT were reconstructed into three-dimensional (3D) models using 3D-planning software. The CA was created by connecting the centers of each virtual sphere to the medial and lateral femoral condyles using computer software. Femoral bone resection of the distal and posterior condyles was performed parallel to the sagittal planes of the CA. The thickness of the CA-referenced bone resection was determined based on the thickness necessary for the respective regions of the femoral component. The PSI was manufactured to locate the guide pin for a conventional cutting block. The accuracy of PSI for KA-TKA was evaluated as the absolute error between the preoperatively predicted thickness and the intraoperative measurements in each of the four regions, as well as the difference in error between distal-medial (DM) and posterior-medial (PM) and between distal-lateral (DL) and posterior-lateral (PL). Results: The differences in thickness of bone cut in the DM, DL, PM, and PL were 0.79 ± 0.39 mm (range, –1.20 to 1.50), 0.70 ± 0.42 mm (range, –1.50 to 1.50), 0.80 ± 0.46 mm (range, –0.80 to 1.50), and 0.75 ± 0.47 mm (range, –2.10 to 1.40), respectively. There was no significant difference in the thickness error between DM and PM (p = 0.959) and between DL and PL (p = 0.812). Conclusions: In KA-TKA, PSI was effective for accurate femoral bone resection based on virtually planned thickness.

        Clinical Results of Complex Subtrochanteric Femoral Fractures with Long Cephalomedullary Hip Nail

        Purpose: Good results of the cephalomedullary nails have been reported in proximal femoral fractures recently. Based on length of nails and shape of screws fixed in a femoral head for proximal fragment fixation, the proper nail length was in dispute. The purpose of this study was to evaluate the clinical and radiological results of a long cephalomedullary hip nail for the treatment of comminuted subtrochanteric femoral fractures. Materials and Methods: Twenty-one consecutive patients with severe subtrochanteric femoral fractures who had undergone intramedullary fixation using long-PFNA II between March 2010 and March 2013 were followed-up for over 12 months. Their mean age was 64.8 years old (range, 43-85 years). Sixteen of 22 cases were high energy trauma. According to Seinsheimer`s classification, 5 cases were type IV and 16 cases were type V. For radiological assessment, time to union, change of neck-shaft angle, sliding length, tip-apex distance (TAD) and leg length discrepancy (LLD) were measured. For clinical evaluation, a modified Koval index was investigated. Results: Mean operation time was 96 minutes. An average decrease of neck-shaft angle was 4.5˚. The average sliding length of the helical blade was 4.2 mm. Average LLD was 3.0 mm, and TAD was 23.0 mm. Mean modified Koval index score at final follow-up was 4.6 points. All the 21 subtrochanteric fractures healed uneventfully on an average of 24.2 weeks (range, 18-30 weeks). Conclusion: Long cephalomedullary hip nail provides excellent clinical and radiological outcomes in the comminuted subtrochanteric fracture.

        Hip&Pelvis : CASE REPORT : Focal Myositis around Hip Joint: 3 Cases Report

        Focal myositis, a benign myositis which mostly occurs at lower extremity, is a disease that is spontaneously improved by conservative treatments such as bed rest and administration of nonsteroidal anti-inflammatory drug. Focal myositis is known to occur mostly at lower extremity, but we could not find a report of occurrence around hip. Therefore, authors attempt to report clinical progression along with the literature review.

        Acute Gluteal Compartment Syndrome without Gluteal Hematoma -A Case Report-

        Acute gluteal compartment syndrome (AGCS) is a rare condition associated with trauma, drug abuse, alcohol intoxication, prolonged immobilization, hip arthroplasty and epidural anesthesia. We report the case of a 42-yearold woman presenting severe buttock pain following decreased lower extremity motor function after an incident whereby she rolled down a flight of stairs. We performed fasciotomy of the gluteal fascia in order to provide relief from acute gluteal compartment syndrome. At the 2 month follow up visit her sensory and motor function had improved. Acute gluteal compartment syndrome is a rare condition which can result in misdiagnosis or delayed diagnosis. Careful consideration is needed for patients suffering severe buttock pain.

      • A retrospective study on the management of osteoporosis in Parkinson's disease e A single institution, preliminary study

        Purpose: The aim of this study is to evaluate clinical characteristics of osteoporosis management in patients with Parkinson's disease. Materials and methods: Data on patients with Parkinson's disease was collected from March, 2000 to March, 2013 at single institution. We investigated portion of BMD test, portion of diagnosed osteoporosis, portion of pharmacological treatment, prevalence and location of primary fractures and presence of second-fractures after primary fracture on in patients with idiopathic Parkinson's disease. Results: Total of 865 patients were included. Among the 865 subjects, the number of patients who underwent diagnostic studies was 189 (21.8%) and the number of patients who was diagnosed with osteoporosis with BMD test was 72 (38.1%), which was 8.3% of the whole patients. Among the patients diagnosed with osteoporosis, the number of patients who were administered with osteoporosis medication for more than 1 year was 49 (68.0%), which was 5.6% of the whole patients. One hundred thirty (15.0%) patients suffered from low energy fractures. Of the 130 fracture sites, 57(43.3%) occurred in proximal femur, 47 (36.1%) of in vertebra, 8 (6.1%) in wrist, 4 (3.0%) in proximal humerus and 14 (10.7%) elsewhere. Twenty two (16.9%) patients suffered from second fracture after the primary fracture. Conclusion: In Parkinson's disease patients, the diagnosis and pharmacologic treatment of osteoporosis were not done adequately, and thus both medical staffs and patients should take this matter into consideration.

        Is Full-Length Intramedullary Nail Necessary for Atypical Subtrochanteric Femoral Fracture Associated with Bisphosphonate?

        Background: American Society for Bone and Mineral Research recommend the use of intramedullary reconstruction of full length-nail for atypical subtrochanteric femoral fracture (ASFF). However, there is no study on the incidence of the ipsilateral femoral fracture after index operation of ASFF, and full-length nail has disadvantage as iatrogenic fracture and leg length discrepancy (LLD). The aim of this study was to investigate the incidence of ipsilateral secondary fracture after using partial-length nail, and to compare the outcomes on surgery of ASFF between partial length-nails and full length-nails. Methods: Forty-five consecutive fractures with ASFFs which had undergone intramedullary fixation using cephalomedullary nail between 2011 and 2018 were enrolled. The 45 cases were grouped based on nail length into the partial-length nail group (n=26) and the full-length nail group (n=19). Ipsilateral secondary fracture, time to union, intra-operative iatrogenic fracture, metal failure, LLD, operative duration, and post-operative 24-hr blood loss were investigated. Results: There was no ipsilateral secondary fracture after index operation. There were no statistically significant differences between the partial-length nail and full-length nail groups in the time to union, LLD, and post-operative 24-hr blood loss (P=0.427, 0.478, and 0.228, respectively). Operative duration showed statistically significant difference between 2 groups (P=0.034). Metal failure were occurred in 1 (3%) case of the partial-length nail group and 2 (10%) cases of the full-length nail group. Iatrogenic fractures during nail insertion occurred in 2 (7%) cases of the partial-length-nail group and 3 cases (15%) of the full-length nail group. Conclusions: Although large scale studies are required, our study indicate that full-length nails are not usually required for the treatment of ASFF.
