With the widespread use of fertility medications and techniques of in vitro fertilization, multiple gestation has become epidemic in modern obstetric practice. Delivery of the initial fetus in a multiple gestation usually is followed by delivery of the subsequent fetus or fetuses shortly thereafter. However in rare circumstances, the delivery is delayed for days due to disappearance of uterine contraction after delivery of the first fetus. When uterine activity ceases after the delivery of the first neonate, a policy of nonintervention may be considered in case of an immature or very premature delivery, in order to achieve a gestational age for the remaining fetus(es) more compatible with neonatal survival. We report a case of twin gestation with prolongation of the delivery interval between the twins for 46 days.

<P><B>Abstract</B></P> <P>H5Nx clade 2.3.4.4 highly pathogenic avian influenza viruses (HPAIVs) have been disseminated to wide geographic regions since 2014. In 2016, five distinct genotypes (C-1 to C-5) of clade 2.3.4.4c H5N6 HPAIVs were detected in South Korea. In this study, we evaluated the pathogenicity, susceptibility to infection, and transmissibility of the two strains representing the C-1 and C-4 genotypes of the H5N6 viruses, which have different PA and NS gene, in domestic ducks. Although the susceptibility to infection of domestic ducks to the two strains was similar, the C-4 genotype virus induced higher mortality in ducks than C-1 genotype virus. A higher titer of viral shedding were detected in ducks challenged with the C-4 genotype virus compared with the C-1 genotype virus. These results indicated that the reassortment of HPAIVs with prevailing low pathogenic avian influenza viruses could effect on the pathogenicity in ducks.</P> <P><B>Highlights</B></P> <P> <UL> <LI> The two genotypes of H5N6 HPAI viruses showed different pathogenicity in ducks. </LI> <LI> The C-4 genotype virus induced higher mortality in ducks than C-1 genotype virus. </LI> <LI> The reassortment of HPAIVs could effect on the pathogenicity in ducks. </LI> </UL> </P>

<P><B>Abstract</B></P> <P>Wild birds play a major role in the evolution, maintenance, and dissemination of highly pathogenic avian influenza viruses (HPAIV). Sub-clinical infection with HPAI in resident wild birds could be a source of dissemination of HPAIV and continuous outbreaks. In this study, the pathogenicity and infectivity of two strains of H5N8 clade 2.3.4.4 virus were evaluated in the Mandarin duck (<I>Aix galericulata</I>) and domestic pigeon (<I>Columba livia domestica</I>). None of the birds experimentally infected with H5N8 viruses showed clinical signs or mortality. The H5N8 viruses efficiently replicated in the virus-inoculated Mandarin ducks and transmitted to co-housed Mandarin ducks. Although relatively high levels of viral shedding were noted in pigeons, viral shedding was not detected in some of the pigeons and the shedding period was relatively short. Furthermore, the infection was not transmitted to co-housed pigeons. Immunohistochemical examination revealed the presence of HPAIV in multiple organs of the infected birds. Histopathological evaluation showed the presence of inflammatory responses primarily in HPAIV-positive organs. Our results indicate that Mandarin ducks and pigeons can be infected with H5N8 HPAIV without exhibiting clinical signs; thus, they may be potential healthy reservoirs of the H5N8 HPAIV.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Mandarin ducks and pigeons can be infected with H5N8 HPAI viruses without exhibiting clinical signs. </LI> <LI> The titer of excreted virus was relatively high in a Mandarin duck and was detected in multiple organs. </LI> <LI> Mandarin ducks and pigeons may be potential healthy reservoirs of the H5N8 HPAI virus. </LI> </UL> </P>
Structure and Expression Analyses of SVA Elements in Relation to Functional Genes

SINE-VNTR-Alu (SVA) elements are present in hominoid primates and are divided into 6 subfamilies (SVA-A to SVA-F) and active in the human population. Using a bioinformatic tool, 22 SVA element-associated genes are identified in the human genome. In an analysis of genomic structure, SVA elements are detected in the 5′ untranslated region (UTR) of HGSNAT (SVA-B), MRGPRX3 (SVA-D), HYAL1 (SVA-F), TCHH (SVA-F), and ATXN2L (SVA-F) genes, while some elements are observed in the 3′UTR of SPICE1 (SVA-B), TDRKH (SVA-C), GOSR1 (SVA-D), BBS5 (SVA-D), NEK5 (SVA-D), ABHD2 (SVA-F), C1QTNF7 (SVA-F), ORC6L (SVA-F), TMEM69 (SVA-F), and CCDC137 (SVA-F) genes. They could contribute to exon extension or supplying poly A signals. LEPR (SVA-C), ALOX5 (SVA-D), PDS5B (SVA-D), and ABCA10 (SVA-F) genes also showed alternative transcripts by SVA exonization events. Dominant expression of HYAL1_SVA appeared in lung tissues, while HYAL1_noSVA showed ubiquitous expression in various human tissues. Expression of both transcripts (TDRKH_SVA and TDRKH_noSVA) of the TDRKH gene appeared to be ubiquitous. Taken together, these data suggest that SVA elements cause transcript isoforms that contribute to modulation of gene regulation in various human tissues.

Demand for in vitro pharmacological evaluation and toxicity test using human hepatocytes has been increasing. In USA and Europe, human hepatocytes obtained from donated whole liver unsuitable for transplantation were distributed to researchers and deposited in cell bank facility as cryopreserved vial. In Korea, however, incidence of transplantation-inappropriate whole liver has been quite low and the whole livers almost have so severe liver disease such as fatty or fibrotic liver that cannot meet the demand. In this study we aimed to isolate human hepatocytes from liver resection surgery-originated partial liver, and assure the isolated human hepatocytes and its cryopreserved hepatocytes to be qualified for the in vitro pharmacological evaluation and drug toxicity tests. We compared those with commercially available human hepatocyte, BD GenTest<SUP>TM</SUP> by cell morphology, hepatic gene expression, urea synthesis, albumin secretion, ammonia removal, and cytochrome P450 induction activities. Changes in hepatotoxic gene expression after cryopreservation are evaluated with a typical hepatotoxic drug, acetaminophen. Consequently, the fresh hepatocytes from the partial liver and its cryopreserved hepatocytes expressed their intrinsic hepatic functions well and showed equal hepatotoxicity gene expression trend regardless to cryopreservation. Therefore, liver resection surgery-originated partial liver can be used as a useful source of human hepatocytes for various pharmacological and hepatotoxicity test.
A Strong Binding Encryption Scheme from Lattices for Secret Broadcast

<P>In 2009, Jeong et al. suggested a secret broadcast scheme in broadcasting networks using binding encryption. With a binding encryption scheme, a ciphertext is made for a group of receivers, and we can assure that all of the receivers will extract the exact same plaintext from the ciphertext. However, Wu et al. pointed out that Jeong et al.'s binding encryption scheme provides only weak decryption consistency. In this paper, we suggest a binding encryption scheme that provides strong decryption consistency without random oracles. Our binding encryption scheme uses the unique properties of lattices to provide strong decryption consistency.</P>

Purpose: The purpose of this study was to identify the effects of hyperventilation and hyperoxygenation before and after endotracheal suctioning using closed-suction system on mechanically ventilated patients. Method: We selected 20 patients who were mechanically ventilated and randomly assigned into 4 groups according to the type of treatments using repeated measure. We applied 3 types of treatment, hyperventilation, hyperoxygenation, hyperventilation+hyperoxygenation, and no treatment and measured physiologic effects-SpO_(2), HR and, mena BP. Result: There were no significant differences between the group with no treatment and the group with hyperventilation, before and after endotracheal suctioning using closed-suction system(SpO_(2)(p=.693), HR(p=.970) and mean BP(p=.638)). There were no significant differences between the group with no treatment and the group with hyperoxygenation before and after endotracheal suctioning using closed-suction system(SpO_(2)(p=.693), HR(p=.970) and mean BP(p=.638)). There were no significant differences between the group with no treatment and the group with hyperventilation hyperoxygenation before and after endotracheal suctioning using closed-suction system(SpO_(2)(p=.693), HR(P=.970) and mean BP(P=.638)). Conclusion: As the results, all of the methods, hyperventilation, hyperoxygenation, and hyperventilation hyperoxygenation can be used when nurses apply endotracheal suctioning using closed-suction systems. In case of hypoxemic patients, hyperoxygenation is recommended for prevention of deoxygenation due to endotracheal suctioning using closed-suction systems.
기관내삽관시 전기 화상 환자에 대한 Vecuronium의 근이완 효과

Background : In inducing anesthesia for burn patients, nondepolarizing muscle relaxant (NDMR) is usually used, because succinylcholine, a widely used muscle relaxant may cause hyperkalemia. It is well known that because burn patients show resistance to NDMR, a high dose of NDMR is needed for them. In this study, we wanted to know whether there is significant difference of the relaxation effect between 0.1 mg/Kg and 0.15 mg/Kg dose's of vecuronium, and between burn and unburn patients. Methods : Subjects are 40 male patients having 1 or 2 ASA physical status (20 are burn patients and the other 20 are unburn patients). We divided them into 4 groups; 1) Group BI (burn patients, vecuronium 0.1 mg/Kg) 2) Group BII (burn Pts, vecuronium 0.15 mg/Kg) 3) Group UBI (unburn Pts, vecuronium 0.1 mg/Kg) 4) Group UBII (unburn Pts, vecuronium 0.15 mg/Kg). Average onset times (time from injection of vecuronium to zero first twitch height (T1)) were measured and intubating condition were scored on 0 to 4 scale. Results : The onset time of vecuronium and distribution of intubation scores didn't show statistical differences among 4 groups. Conclusion : The onset time of vecuronium and intubating condition in burn patients dosen't show a difference from unburn patient. (Korean J Anesthesiol 1999; 36: 21∼26)

<P>Highly pathogenic avian influenza A(H5N8) viruses were isolated from migratory waterfowl in South Korea during fall 2014–winter 2015, a recurrence after initial introduction in winter 2014. These reappeared viruses were phylogenetically distinct from isolates circulating in poultry farms in South Korea.</P>
