      • 항암화학요법을 받는 암 환자에 대한 Nucare^R의 영양지지효과

        Purpose : The purpose of this study was to evaluate the effect of nutritional support with enteral liquid supplement in cancer patients receving chemotherapy for possible benefit in nutritional, immunologic and golbal function of patients. Patients and Methods : From October 1995 to February 1997, 30 advanced cancer patients receving chemotherapy were divided two roups. The Nycare group, in addition to normal diet, Nucare^R enteral supplement was given for 1week right after chemotherapy for the duration of 2 chemotherapy cycles. Control group received only normal diet without parenteral fluid supplement for 2 chemotherapy cycles. Results : Median ages were 53 end 56 years for Nucare group and Contrl group respectively. Performance scores was less than 2 by ECOG scale. All patients were stage Ⅲ or Ⅳ. The physical parameters, such as weight, arm muscle circumference(AMC) and triceps skin fold(TSF) were decreased in both groups after 2 cycles of anticancer therapy. but it was less severe in Nucare group(p<0.05). Serum transferrin was maintained in mild deficit state in Nucare group, whereas it aggravated form mild to moderate deficit in Control group(p<0.05). Serum albumin level increased in Nucare group without statistical significance. but it decreased from normal to mild deficit in Control group. Serum total protein did not change significantly in Nucare group. but in Control group, serum total protein was decreased from 7.24±0.9 to 6.52±0.5(P<0.05). Total lymphocyte count did not change significantly in both groups. Conclusion : This study shows that the nutritional support with Nucare^R was effective in the prevention of nutritional deficit status in patients receving a nticancer chemotherapy.

      • 저항운동프로그램이 뇌졸중 환자의 활동체력 증진에 미치는 영향

        The purpose of this study was to compare activity fitness of daily living(AFDL) in elderly stroke patients. Six older stroke patients(M: 2, F:4, 74.5±9.0yr) were participated in the AFDL test before and after 12weeks exercise programs(resistance exercise by Thera-band, therapeutic exercise, two times per week, twenty-sixty minute per day, performing one to three sets of 4-12 repetitions). Six items representing various parts of the body(i.e., arm, leg, shoulder) and categories of movement(i.e., manipulation, locomotion, and stability) were selected, and defined as the human movement abilities. Six performance test(grip strength, arm curl, catching a dropped bar, carrying beans using chopsticks, manipulating pegs in a pegboard, stepping) items related to the AFDL were measured in test before and after by the same investigators, using the same instruments. The results of the comparison with the 3 items between the test before and after showed that the test after were significantly(P<0.05) superior in catching a dropped bar, carrying beans using chopsticks, manipulating pegs in a pegboard. No difference existed in grip strength, arm curl, stepping. These results suggest that resistance training programs utilizing elastic tubing can serve as practical and effective means of eliciting strength gains in stroke patients. From these results, it can be concluded that the higher activity fitness of daily living(AFDL) level would contribute to a lifestyle of more engagement in exercise or physical activity.

      • 단시간의 최대운동에 의한 산화적 스트레스와 회복에 관한 연구

        The purpose of this study was to investigate that the oxidative stress would be occured by acute maximal exercise, and the stress would be recovered during the recovery. To examine this hypothesis, Forty middle school students(M, 14.7±0.5yr, l71㎝/58㎏) were participated in maximal exercise test. Venous blood samples were drawn at the forearm antecubital vein immediately before and after the test, and after 90 minutes of recovery. The concentration of Taurine, TEARS, GSH, GSSG and the activities SOD, GPX, GR of the blood samples were measured as the oxidative and antioxidative indicators values. The results of this study support that 1. the oxidative stress is occurred immediately after the acute maximal exercise, and the increases of the concentrations and the activities of the antioxidative indicators mean that endogenous antioxidative defense system functions to maintain the homeostasis of body against the oxidative stress during the acute exercise. 2. And these indicators recovered to the rest level after 90 minutes of recovery also mean that the antioxidative system disturbed by the acute exercise could be recovered within 90 minutes.

      • 악성종양에서 혈청 Intercellular Adhesion Molecule-1의 혈청

        Intercellular adhesion molecule-1 (ICAM-1) is a glycoprotein serving as ligands for leukocyte intergrin receptors, i.e. LFA-1, MAC-l. It has been suggested that the expression of ICAM-1 and the levels of circulating ICAM-1 were increased in several malignancies. We measured serum ICAM-1 by ELISA in patients with stomach cancer (n=25), acute lymphoblastic leukemia (n=7), non-Hodgkin's lymphoma (n=14), and metastasis of unknown origin (n=5). Serum levels of circulating ICAM-1 in all malignancies were significantly higer than in normal controls. And serum level of ICAM-1 in stomach cancer ptients having metastatic disease was higher than in patients with localized disease (p=0.027). These results suggest that elevated serum ICAM-1 is a rather common feature for malignancies, not unique for certain types of cancer such as malignant melanoma. It remains to be clalified whether marked increase of serum ICAM-1 in metastatic stomach cancers simply reflect tumor burden or this molecule plays a role in progression in stomach cancer.

        점핑활동 프로그램이 다운증후군 아동의 하지 근력 및 자세조절 전략에 미치는 영향

        The purpose of this study was to suggest jumping activity program for increasing the leg strength and postural control strategy and to identify the effects of jumping activity program. Participants were twelve male elementary school students with Down syndrome(exercise group: n=6, non-exercise group: n=6). The jumping activity program administrated during 8 weeks, 3 times a week, for 70 minutes at a time. The isokinetic strength variables and postural control strategy data were analyzed using t-tests(a=.05) to identify difference of means within each group. In conclusion, most of all the isokinetic strength and postural control strategy variables of participants in the exercise group were increased. Therefore, this study suggests that through the participation of jumping activity program children with Down syndrome at the age of 7~13 will stabilize their joints surrounded by muscles, increase the strength of their lower limb, and maintain the posture by increasing muscle reaction speed between lower limb and trunk.

      • 급성골수백혈병에 대한 관해유도화학요법 후의 Granulocyte Colony-stimulating Factor의 효과

        Granulocyte colony-stimulating factor(G-CSF) have been shown to hasten the recovery of neutropenia following anti-cancer chemotherapy. There are controversial opinions on the use of G-CSF in acute myelogenous leukemia(AML) because clonogenic studies have shown that G-CSF stimulates leukemic colonies as well as granulocyte colonies. In this study, we evaluated the effectiveness and safety of recombinant human G-CSF after induction chemotherapy with DAV regimen(Ara-C 100mg/㎡ day 1-8, Doxorubicin 45mg/㎡ day 3-5, VP-16 100mg/㎡ day 6-8) in 9 patients with AML. G-CSF therapy(200 ㎍/㎡/day) was begun 2 days after the end of chemotherapy and continued for 10 days. 17 AML patients who recieved the same chemotherapy before the onset of this study were used as historical control. G-CSF shortened the duration of granulocytopenia (less than 500/㎣) significantly (13 vs 23 days, p<0.001), but it had no effect on platelet recovery. Although the incidence of febrile episodes was almost the same, the duration of febrile episodes was shorter in the group treated with G-CSF( 5 vs 12 days, p=0.03). There was no evidence that G-CSF accelerated the regrowth of leukemic cells and the complete remission rates between the 2 groups were not different. These results show that G-CSF accelerates the recovery of granulocytopenia and shortens the febrile days after chemotherpy in patients with AML, without affecting the regrowth of leukemic cells.

      • 혈관중심성 T세포/자연살상세포 림프종의 임상적 고찰

        Although the Working Formulation is commonly used to classify NHL in Korea, it has been recognized as imperfect for primary extranodal lymphoma, expecially for patients with nasal, paranasal disease because of their histological characteristics. Angiocentric T/NK cell lymphoma remains rare clinical presentation in North America and Europe but is more common in Asia and Latin America. The optimal mode of treatment has also not been decided on, probably because of their rarity and limited understanding of its natural course. The aim of this study was to analyze our experience with 14 patients with angiocentric lymphoma seen at the Chungnam Natioanl University Hospital over the last 5 years. We reviewed the records of 14 patients who were treated at the Chungnam National University Hospital between January, 1996 and December, 2000. The angiocentric T/NK cell lymphoma accounted for about 8.2% of NHL. The median age was 52.5 (range 36-71) years. Sex ratio was equal. According to the Ann Arbor staging system, disease was classified as stage Ⅰin 3 patients, stage Ⅱ in 8 patients and stage Ⅳ in 3 patients. Systemic B symptoms were present in only four patients in only four patients. All patients has extranodal involvement. The most frequently involved extranodal sites were nasal cavity followed by paranasal sinuses and orpharynx. The most common presenting symptoms were nasal obstruction with purupent rhinorrhea followed by dysphagia. According to the International prognostic index (IPI), most patients was in low-risk group (11 patients). Nine patients responded completely. Seven patients were received combination chemotherapy and 2 patients had radiation therapy as a initial treatment. Five patients were relapsed. After a median follow-up of 660.5 days, the overall survival and disease-free survival rates at 3 years was 56.8% and 42.9% respectively. The median survival time was not yet reached. The median overall survival time was 1338.5 (range 1212-1564) days for non-relapsed and 733.4 (range 302-1008) days for relapsed patients. The median time from CR to relapse was 100.5 (range, 39-159) days. There were limitations to know enough about the characteristics of angiocentric T/NK cell lymphoma because of a small number of patients. In the future, multicenter trials will be neccesary to identify clinical characteristics, optimal treatment mordalities and their responses, and prognostic factors.

        Seoul National University About 34% of the adult population paticipated in sport and physical activities at least one or two times per average week. Many more than women took part in sports and physical activities. They badly want sports complex center and exercise programs. They thought that sports complex center is the basic constructions of university as library or the statistic center. They exercised for health or fitness development in sports complex center of around the house. they enjoyed playing sports activities such as tennis, swimming for loe or two hours per average week. The men and women responded paticipating in moderately active level of physical activity. The younger age group prefer tennis, swimming, bowling, calisthenic exercise, while the older age group is more likely to mount climbling, working, golf. The social economic status of populer player estimated high relative to the common peoples. Professional sports have pogitive view point in paticipation in sports for all the people or leisure satisfaction. They thought that theexercise was the best weight control methods and Obesity was caused by the lack of exercise.

        Performance of strenuous physical activity can increase oxygen consumption by 10 to 15-fold over rest to meet energy demands. The resulting elevated oxygen consumption produces an "oxidative stress" that leads to the generation of free radicals and lipid peroxidation. A defense system of free radical scavengers minimizes these dangerous radicals. Indirect measurements of free radicals generated during exercise include assessing products of lipid peroxidation that appear in the blood (e.g., malondialdehyde and conjugated dienes) or expired in the breath (pentane). Changes in antioxidant scavengers and associated enzymes (e.g., glutathione, tocopherol, glutathione peroxidase) also provide clues about demands on the defense system. Physical training has been shown to result in an augmented antioxidant system and a reduction in lipid peroxidation. In recent years, much attention has been paid to the role antioxidants play in physical activity and physical performance. This attention is largely focused on the increase in free radicals generated during strenuous exercise. As oxygen consumption, and hence oxidative stress, is increased, free radicals are produced. Studies have reported that antioxidant supplementation can reduce evidence of exercise induced lipid perozidation. It has been suggested that those who engage in chronic physical activity, placing a constant oxidative stress on the muscles and other cells, may require additional antioxidants to help them recover from exercise. Other than their role in reducing tissue damage, the data are equivocal with respect to whether antioxidants will directly enhance physical performance. Antioxidant supplementation may be prepared to handle an sudden increase in oxidative stress. Studies have shown increases in oxidative stress during performance of strenuous exercise and then supplementation would reduce this stress. The effects of antioxidant supplementation on physical performance are very affirmative.

      • 최대하 운동시 온도·습도 변화에 따른 피부온도와 발한량의 변화

        The study was to investigate the difference of the total sweat rate, local sweat and skin temperature according to environments during submaximal exercise. For this, eight male field hockey athletes were made to take bicycle exercise of 40% Vo₂max for 60min. The environmental conditions were divided into four groups ; Environment Ⅰ (Temperature 18℃, Relative Humidity 40%). Environment Ⅱ (Temperature 18℃, Relative Humidity 70%). Environment Ⅲ (Temperature 30℃, Relative humidity 40%). Environment Ⅳ (Temperature 30℃, Relative Humidity 70%). Local sweat rate, rectal temperature and main skin temperature were recorded every 10 minutes by thermister, total sweat rate measured by the weight loss during the exercise. Local sweat rate according to environments during submaximal exercise increased mainly by relative humidity. The other local skin temperature and the rising-rate of body temperature had an effect on it. Total sweat rate according to environments during submaximal exercise was more influenced by hot-high humidity rather than by temperature especially led to the 1,400g's weight loss during the one hour exercise. Local skin temperature according to environments during submaximal exercise was significantly differentiated by temperature and humidity as well as by time. Mean skin temperature according to environments during submaximal exercise was varied, up-and-down repeatedly. No relationships between rectal temperature or local skin temperature and total sweat rate or local sweat rate but these factors were partly interrelated. To conclude, total sweat rate was influenced most by humidity and local sweat rate differed according to local skin temperature and the rising-rate of body temperature.
