골반 골절 환자에 있어서 골절 위치에 따른 하부 요로 손상의 연관성

Background and Objectives: Lower urinary tract injuries in pelvic bone fracture can lead to significant complication. We tried to find out the relationship between the fracture location and the bladder, urethra injury in patients who sustaining pelvic bone fracture. We thought that the fracture locations could serves as markers for injury. Materials and Methods: A clinical study was made on 43 patients who sustaining pelvic bone fracture with bladder or urethra injury in the Chosun University hospital during the period of 5 years from January, 2002 to December 2006. Diagnosis was done by retrograde cystography, retrograde urethragraphy, abdomenpelvic computed tomography. Results: Of the 43 patients reviewed, 27 patients had intraperitoneal type bladder rupture, and 12 patients had extra-peritoneal type bladder rupture. 9 patients had urethral rupture. Intraperitoneal type bladder rupture cases are most commonly associated with infrapubic ramus, suprapubic ramus and acetabulum. but the statistically significance is abscent (p= 0.642). Also extraperitoneal type bladder rupture cases are most commonly associated with infrapubic ramus, suprapubic ramus and acetabulum. but the statistically significance is abscent (p= 0.548). And all type bladder rupture cases are most commonly associated with infrapubic ramus, suprapubic ramus and acetabulum. but the statistically significance is abscent (p= 0.775). And urethral rupture cases are most commonly associated with infrapubic ramus, suprapubic ramus and acetabulum but the statistically significance is abscent (p= 0.528). Lastly, Combineded bladder rupture and urethral rupture cases are most commonly associated with infrapubic ramus, llium, Symphysis pubis. But the statistically significance is abscent (p= 0.462). Conclusions: In our study when there is a pelvic bone fracture, the fractured site of pelvic bone have no statistically association with lower urinary tract injury. But most common pelvic bone fracture site of all lower urinary tract injury cases is infrapubic ramus. Therefore, if infrapubic ramus fracture is found in the condition of pelvic bone fracture, it is highly likely to have lower urinary tract injury. More careful observation and diagnosis is necessary for this case.
항생제 다제내성균 Staphylococcus aureus SA2로부터 분리한 테트라사이클린 내성 플라스미드 pKH6의 염기서열

한국에서 임산균주로부터 분리한 테트라사이클린(Tc) 내성 플라스미드 pKH6의 전체 염기서열을 결정하여 가장 대표적인 Tc 내성 플라스미드인 pT181과 비교하였다. pKH6의 전체 길이는 4439bp로 pT181과 동일하게 나타났으며 염기서열은 7개의 염기에서 차이를 보였다. 전부 다 염기쌍이 치환된 형태로 나타났으며 그 중 3개는 coding 부위에서 일어났다. coding 부위에서 일어난 경우에도 1개는 동일한 아미노산을 coding 하였기 때문에 pT181과 차이가 없었으며 RepC와 Pre 단백질에 있어 각각 1개의 아미노산에서 차이를 나타내었다. The complete nucleotide sequence of pKH6, a tetracycline-resistance (Tc^r) plasmid isolated from multi-drug resistant Staphlococcus aureus SA2, has been determined and compared with that of the staphylococcal Tc^r plasmid pT181. The nucleotide sequences of the two plasmids are in agreement except for 7 nucleotides. All differences are caused by base pair substitutions. Among 6 substitutions, 3 occurred in coding regions. However, only tow base substitutions in coding regions resulted in changes of amino acid sequences in two different ORFs of repC and Pre proteins.
음낭외상시 고환파열에 대한 초음파 검사의 진단적 유용성

Purpose: We determined the use of scrotal ultrasonography in the initial diagnosis and management of testicular injuries due to blunt scrotal trauma. Mateiials and Methods: We reviewed 54 patients of scrotal trauma in the last 8 years. Forty patients were evaluated by ultrasonography preoperatively. A heterogeneous echo pattern of the testicular parenchyma with loss of oval shape, contour definition was the basis for diagnosis of testicular rupture. Among them, 33 cases underwent scrotal exploration and 7 cases were managed conservatively. The remaining 14 patients were evaluated only by physical examination, 8 patients of them were explored immediately because of suspicious testicular mpture. The accuracy of ultrasonography were determined by comparing radiographic interpretations with intraoperative findings. Results: Among the 33 patients who were evaluated by ultrasonography and explored immediately, there were 26 suspected testicular ruptures on ultrasonography. Surgical exploration revealed testicular rupture in 23 and simple hematoma in 10. The testicular rupture was correctly diagnosed by ultrasonography in the 22 cases, and there were 4 false-positive and 1 false-negative diagnoses of testicular rupture. The sensivity and specificity for the diagnosis of testicular rupture are 96% and 60%, and the positive and negative predictive values are 85% and 86%, respectively, Conclusions: Scrotal ultrasonography, with the main radiographic finding of a heterogeneous echo pattern of the testicular parenchyma with loss of oval shape, contour definition, is highly sensitive on the diagnosis of testicular rupture. And it increases the salvage rate for testicular rupture with preservation of the testicular function and prevents delayed complications of chronic pain, atrophy and orchiectomy associated with missed testicular rupture.

In this study, Song-Cheon river Basin located at Dae-kwan-lyong Meyn in Kang-Won Province was monitored to analyze characteristics of non-point source pollution during the snowmelt period. The characteristics of NPS pollution such as runoff, change in water quality and pollutant loads by snowmelt were investigated from 23th February 2010 to 27th April 2010. The results showed that The climate change effects flow, turbidity and concentration of SS and COD with showing similar trend of fluctuation. Daily pollutant loads per unit area were SS 74.43 kg/ha/day, COD 1.25 kg/ha/day, BOD 0.21 kg/ha/day, TN 0.717 kg/ha/day, TP 0.141 kg/ha/day and the flow weight mean concentration has been SS 985.97 mg/L, COD 16.28 mg/L, BOD 2.67 mg/L, TN 9.302 mg/L, TP 1.834 mg/L, respectively. The flow and pollutant loads both of SS and of COD significantly increased during the snowmelt period. Because structurally loosen soil by freezing and thawing deteriorates water quality, research and management measures will be needed.
선속과 해양기상이 선박 연료소모량에 미치는 영향에 대한 실선 실험 해석

Ship fuel consumption is affected by ship speed and ocean environmental factors, which are resistance to ships. There have been not only mainly theoretical studies due to the difficulty of obtaining real ship experiment data, but also insufficient studies on correlation analysis between ship speed and ocean environment factors. In this study, we performed data pre-processing work to interpolate and integrate multi-type and shape data acquired by real ship experiments. Using the integrated data, we analyzed an empirical correlation between FOC and ship speed, ocean environmental factors (wind, current, wave). Navigation experiments for collecting data were performed on five voyages, starting at Busan Port, Ulsan Port, and Ulleungdo Island. FOC data was extracted from the electronic flow meter pre-installed for the research on ship, and ship position (Lon, Lat), speed (SOG) data were acquired from AIS and mobile phone, and ocean environmental data was used grid-type data generated by Korea Operational Oceanographic System (KOOS). As a result of daily correlation analysis, SOG was the greatest influence on FOC. Current velocity had the greatest effect on FOC according to the stronger external force by correlation analysis of average filtering condition. When receiving external force from the ship s heading, Wind speed was derived the highest rate of change in FOC compared to the maximum correlation coefficient of daily analysis. Current velocity was analyzed to have a high effect on FOC at low ship speed (6~7kts), and wind velocity was analyzed to be correlated with FOC at high ship speed (10kts), and wave affected FOC at all ship speed. wind velocity was found to have the greatest influence on the FOC during Maneuvering, and the correlation of wave force was analyzed to be the highest correlation during Cruising. In addition, we found to save the FOC with increasing rate of SOG change to 11% according to a force of 65kg⋅f acting on ship stern. This study has a meaning in the aspect of using the results of the real ship experiment because the results were derived with the limited condition and ship type, it is necessary that supplementation research.
