<P><B>Background</B></P><P>Nanoparticles fabricated from the biodegradable and biocompatible polymer, polylactic-co-glycolic acid (PLGA), are the most intensively investigated polymers for drug delivery systems. The objective of this study was to explore fully the development of a PLGA nanoparticle drug delivery system for alternative preparation of a commercial formulation. In our nanoparticle fabrication, our purpose was to compare various preparation parameters.</P><P><B>Methods</B></P><P>Docetaxel-loaded PLGA nanoparticles were prepared by a single emulsion technique and solvent evaporation. The nanoparticles were characterized by various techniques, including scanning electron microscopy for surface morphology, dynamic light scattering for size and zeta potential, x-ray photoelectron spectroscopy for surface chemistry, and high-performance liquid chromatography for in vitro drug release kinetics. To obtain a smaller particle, 0.2% polyvinyl alcohol, 0.03% D-α-tocopheryl polyethylene glycol 1000 succinate (TPGS), 2% Poloxamer 188, a five-minute sonication time, 130 W sonication power, evaporation with magnetic stirring, and centrifugation at 8000 rpm were selected. To increase encapsulation efficiency in the nanoparticles, certain factors were varied, ie, 2–5 minutes of sonication time, 70–130 W sonication power, and 5–25 mg drug loading.</P><P><B>Results</B></P><P>A five-minute sonication time, 130 W sonication power, and a 10 mg drug loading amount were selected. Under these conditions, the nanoparticles reached over 90% encapsulation efficiency. Release kinetics showed that 20.83%, 40.07%, and 51.5% of the docetaxel was released in 28 days from nanoparticles containing Poloxamer 188, TPGS, or polyvinyl alcohol, respectively. TPGS and Poloxamer 188 had slower release kinetics than polyvinyl alcohol. It was predicted that there was residual drug remaining on the surface from x-ray photoelectron spectroscopy.</P><P><B>Conclusion</B></P><P>Our research shows that the choice of surfactant is important for controlled release of docetaxel.</P>

Paclitaxel (PTX) is an anti-cancer agent extracted from the bark of the Pacific yew (Taxus brevifolia). In clinical trials, PTX has demonstrated significant activity against a variety of tumors such as drug-resistant ovarian cancer, metastatic breast cancer, non-small cell lung cancer and AIDS related Kaposi's sarcoma. However, the bioavailability of PTX is mainly limited by cytochrome P450 activity in gut wall and liver, and drug transporters, such as P-gp in gut wall and liver. As well as, PTX is very poor soluble in water (<0.5 mg/L) and other pharmaceutically acceptable solvents. The commercial formulation of PTX consists of micellar solution of the drug in cremophor EL (polyoxyethylated castor oil) containing 50% absolute ethanol. However, it is well known that the use of cremophor EL is associated with hypersensitivity reactions, neurotoxicity and nephrotoxicity and also the solubility of PTX is increasing with the addition of citric acid. Therefore, this study is to optimize the mixing order of individual components for stabilized paclitaxel injection.

Intravenous immunoglobulin (IVIG) therapy has been introduced to idiopathic dilated cardiomyopathy due to their antiviral and anti-inflammatory effects. But each study reported conflicting result and treatment regimen has not been clearly established. We experienced a case of 28-year-old woman with idiopathic dilated cardiomyopathy with severely depressed cardiac function. Its onset time was obvious within 1 month. Despite of conservative treatment of heart failure, sudden cardiac arrest was developed. We tried IVIG therapy, and her symptoms and cardiac function were improved after IVIG treatment.

This study was conducted an experiment for the thermal flow mechanism of the surrounding butane gas can and pressure change in butane can with oversized cookware. And numerical analysis was performed to analyze the process of heat transfer around butane can. Effect of radiant heat from small size cookware is very small. Thus small size cookware does not have a significant impact on the butane gas range safety. But radiant heat of oversized cookware is larger than small size cookware. Therefore it gives an adverse effect on the butane can. And when internal pressure of butane can is greater than about 406.8~447.2 kPa, a safety device of portable butane gas range were working. The causes of safety device working is because of trivet height. Trivet height is lowered, the radiant heat is increased. This radiant heat is to raise pressure of inside butane can. Experimental and numerical analysis results, the lower thermal conductivity of the cookware is greater the effects of radiant heat.
Quantitative Analysis and Preformulation of Extracts from Alnus Japonica

Alnus japonica has been known to exert antioxidative, anti-inflammatory, anti-cancer and immune response inhibitory effects. The aim of this study was to figure out the characteristics of extracts obtained with different extraction solvent such as water, 100% ethanol, 70% ethanol or 70% methanol because characteristic components such as oregonin and hirsutanone extracted from Alnus japonica might be essential for the biological activity. For this purpose, oregonin and hirsutanon of four extracts, index ingredient of Alnus japonica, were analyzed with HPLC and physicochemical studies such as SEM, particle size and zeta potential were conducted. In cell cytotoxicity study, extract of water showed the highest cytotoxicity among four extracts. In case of oregonin, 70% MeOH and water extracts showed high contents and in case of hirsutanone, all extracts showed similar contents except 70% EtOH extracts. The extract of 70% MeOH from Alnus japonica for both oregonin and hirsutanone appeared to have the highest content. Both oregonin and hirsutanone extracted from Alnus japonica using 70% methanol showed stability in pH 1.2.

The experiment was conducted to measure temperature and pressure around operating portable butane gas ranges with oversized cookwares. In this experiment, portable butane gas range with different safety equipment and two kinds of oversized cookware, each of which is made of aluminum alloy and rock, was used. As a result, temperature of the bottom of the butane and the pressure of butane barrel tend to aligned with each other. Through this result, it is reasonable that a safety criteria can be decided based on the temperature of the bottom of butane. Especially, portable butane gas ranges are safely conducted under the condition of the temperature is under 50 degree and pressure is under 500 kPa, respectively, at the bottom of the butane barrel.

Intestinal hemorrhage, perforation, obstruction, and fistula formation are the common complications associated with intestinal tuberculosis. However, these complications usually occurr in active stage of intestinal tuberculosis. A 45-year-old man was diagnosed as intestinal tuberculosis and received anti-tuberculosis medications for 9 months. After the end of treatment, intestinal lesion was cured. However a deformed appendiceal orifice due to hypertrophic sear resulting in symptomatic appendictis was noted. We report a case of acute appendicitis due to intestinal stricture after the successful treatment of intestinal tuberculosis. (Korean J Gastroenterol 2007;50:277-279)
